Neonatal Intestinal Obstruction in Acute Renal Failure in Premature Infant: A Case Report

Introduction: Intestinal obstruction is a common cause of abdominal surgery in the neonate. Diagnosis is straightforward using standard radiology, and surgical technique depends on the underlying anatomical lesion. Peritoneal dialysis (PD) is an effective, albeit invasive, therapy for neonatal renal failure. We report a case of neonatal obstruction with severe renal failure treated by PD to highlight our hospital practice and possible remedies in a context of limited resources. Case Presentation: This was a female neonate of moderate prematurity admitted on day 4 of life for management of a flat neonatal obstruction. Radiological diagnosis suggested small bowel atresia. Biological tests revealed severe renal failure with creatinine levels of 416 micromoles per liter and blood urea of 27.1 micromoles per liter. Management consisted of preoperative peritoneal dialysis for 48 hours followed by laparotomy. The intraoperative diagnosis was GROSFELD type IIIa digestive atresia. The postoperative course was favourable, transit was resumed on day 5 and the patient returned home on day 12. Progress at 3 months was satisfactory. Conclusion: Neonatal intestinal obstruction with renal failure in premature infants is associated with a poor prognosis, even more so if there is a delay in treatment. Peritoneal dialysis seems to be a suitable alternative for this management in our working conditions with limited resources.

Factors of Adherence to Concurrent Tuberculosis Treatment and Antiretroviral Therapy among HIV-TB Co-Infected Individuals in the East Region, Cameroon in the COVID-19 Era: A Retrospective Cohort Study

Context/Objectives: Tuberculosis (TB) and HIV co-infection is a serious health problem in Cameroon. The problems associated with poor adherence to treatment are on the increase worldwide. This problem can be observed in all situations where patients are required to administer their own medication, whatever the type of illness. The general objective of this study was to assess the factors affecting adherence to treatment among HIV-TB co-infected patients in health facilities in the East Region in the COVID context. Method: A retrospective cohort study before and during COVID-19 was conducted in HIV care units in 13 health districts in the East Region of Cameroon. Data were collected using a questionnaire recorded in the Kobo Collect android application, analyzed using SPSS version 25 software and plotted using Excel. Results: The pre-COVID-19 cohort compared to the during-COVID-19 cohort had a 1.90 risk of not adhering to treatment (OR: 1.90, CI {1.90 - 3.37}) and the difference was statistically significant at the 5% level (p-value = 0.029). Frequency of adherence was 65.4% (140/214). Adherence before COVID-19 was 56.9% whereas during COVID-19, it was 74.3%. Conclusion: The implementation of targeted interventions in the COVID-19 context, using evidence-based data and integrating the individual needs of HIV-TB co-infected patients, improved adherence to concurrent anti-tuberculosis treatment and antiretroviral therapy during the COVID-19 Era.

Bacterial cellulose (BC) is a versatile biomaterial with unique properties that make it promising for various biomedical applications, including wound healing. The extracellular matrix (ECM) plays a crucial role in wound healing, providing a structural scaffold and signaling cues for cell migration and proliferation. This study investigates the potential of BC as a scaffold for ECM production and its effect on in vivo wound healing. In this work, the bacterial cellulose fermentation process is modified by the addition of Green Propolis and Usnic acid to the culture medium and natural materials before the bacteria are inoculated. In vivo behaviour using natural membranes for regenerative medicine is presented and it is in edit. Overall, our findings demonstrate the potential of BC as a scaffold for ECM production and its beneficial effects on in vivo wound healing. BC-based dressings may offer a novel approach to promoting wound healing and tissue regeneration in clinical settings. Further studies are warranted to optimize BC-based therapies and explore their full potential in regenerative medicine.

The synthesis of new 4-imino-4H-chromeno[2,3-d]pyrimidin-3(5H)-amine in four steps including one step under microwave dielectric heating is reported. The structural identity of the synthesized compounds was established according to their spectroscopic analysis, such as FT-IR, NMR and mass spectroscopy. These new compounds were tested for their antiproliferative activities on seven representative human tumoral cell lines (Huh7 D12, Caco2, MDA-MB231, MDA-MB468, HCT116, PC3 and MCF7) and also on fibroblasts. Among them, only the compounds 6c showed micromolar cytotoxic activity on tumor cell lines (1.8 50 50 > 25 μM). Finally, in silico ADMET studies ware performed to investigate the possibility of using of the identified compound 6c as potential anti-tumor compound.

Man＇s innate clocks functioning allows to understand WHEN and WHY therapies are efficient. Vigil chronotypes determination and respect allows to avoid scholars＇ failure. Performances depend on chronotypes and time changes. Both minimal and maxima/durations of night sleep cycles result from interactions between endogenous and exogenous clocks. Our ecoexotope is structured by solar, lunar and terrestrial rhythms which are synchronisers for endophysiotope clocks. Man night sleep changes depend on lunar cycles entrainment. Sleep analyses point to circa-annual solar rhythms used as controls to evidence circa-monthly lunar ones. To evidence physiological responses individual longitudinal records are used. To evidence lithotherapeutic effects, stimuli responses are tested according to a double-blind placebo-controlled survey. WHAT mineral to chose？, WHY？, HOW to treat？, WHEN？ Compared with controls, jadeite or nephrite enhances night sleep quality with a 15 fold decrease of awakenings and urinations. The highest placebo effect was below 4 fold increase. Depending on minerals and trace elements, properties change. The mineral crystal structure is evidenced to have an action. The contact area with the skin is a limiting factor. Placebo effects are greater during the day phase. Red jasper treatment enhances the number and intensity of diurnal physical working. Minerals act in a dose-dependent manner and in synergy. Hematite sole gives a placebo effect, but increases the effect of serpentinite by a 15 fold value. Within a clocks network, the latency phase of the whole is shorter than the shortest latency phase of each clock, enhancing the system reactivity.

Farmers’ Knowledge and Perceptions of the Effects of Climate Variability and Pollution on Crop Production and Their Varying Adaptation Strategies in The Gambia

Crop and livestock production is critical to food security in The Gambia. Over the years, the country has experienced a reduced yield due to perceived climate change events with limited studies on how climate change and pollution affect crop production. This study assesses farmers’ knowledge and perceptions of the effects of climate variability and pollution on crop production and their varying adaptation strategies in The Gambia. Both quantitative and qualitative methods were used in this study. The sample size for quantitative data collection was calculated as 432 while the qualitative data involves both the focus group discussions and key informant interviews. The focus group discussions comprised two districts in each of the six agricultural regions and two farming communities engaged in crop production were chosen from each district. Furthermore, eight key informant interviews from relevant institutions were conducted. The study shows that The Gambia is highly vulnerable to extreme climatic events. Although most farmers opined that agricultural land contamination emanates from farm runoff and indiscriminate waste dumping, they had little knowledge of heavy metal pollution and bioremediation. The results showed that farmers experienced constraints such as inadequate access to credit, water, and irrigation facilities, insufficient access to efficient inputs, salt intrusion, etc. which threatened food security. The study concludes that crop farmers acknowledged the existence and impacts of climate change, and therefore recommend the availability and affordability of climate change resilient crops and promote variability awareness campaigns to address climate change impacts in The Gambia.

Study of Hemorrhoid Disease in the Department of General Surgery of the Cs Ref of the Commune I Bamako

This work was a retrospective prospective study carried out in the general surgery department of the Cs ref of commune I from January 2009 to December 2012. Our study aimed to determine the hospital frequency of hemorrhoidal disease, to identify the contributing factors, describe the clinical and therapeutic aspects and analyze the surgical consequences. We collected 73 patients with an average age of 37.8 years (range: 19 years and 77 years) with a sex ratio of 2.5 in favor of men. We operated on 49 patients or 67.1% of cases. 71.2% of our patients were married and lived in Bamako. Schoolchildren and middle managers made up 39.8% of cases. 82.2% of patients were seen in ordinary consultation and 69.9% complained of progressive anal pain in 66.7% of cases. This pain was triggered by defecation in 88.2% of our patients. The contributing factors were dominated by constipation (53.4%), working in a seated position (41.1%) and a sedentary lifestyle (16.4%) of cases. Stage IV external hemorrhoids and hemorrhoidal thrombosis represented 53.8% of surgical indications. The most used surgical technique was that of Milligan-Morgan, i.e. 51.9% of cases and 45.2% of patients treated medically received a combination of transit regulator, venotonics and analgesics. The surgical aftermath was marked by hemorrhage (3 cases), delayed healing (5 cases), urinary retention (1 case) and scarring anal stenosis (1 case). The morbidity rate was 5.48% of cases and a zero mortality rate. The average length of hospitalization was 2 days with extremes of 1 and 5 days.

Modeling and optimization of rotary kiln treating EAF dust

Electric arc furnace （EAF） dust from steel industries is listed by the United Sates EPA as a hazardous waste under the regulations of the Resource Conservation and Recovery Act due to the presence of lead, cadmium and chlorine. The disposal of the approximately 650000 t of EAF dust per year in the U.S. and Canada is an expensive and unresolved problem for the majority of steel companies. The waelz process has been considered as the best process for treating the EAF dust. A process model, combined thermodynamic modeling with heat transfer calculations, has been developed to simulate the chemical reactions, mass and heat trans- fer and heat balance in the kiln. The injection of air into the slag and the temperature profile along the kiln have been modeled. The effect of （CaO＋MgO）/SiO2 on the solidus temperature of slag has also been predicted and discussed. Some optimized results have been presented.

Candida auris since it discovery in 2009 is becoming a severe threat to human health due to its very quickly spread, its worldwide high resistance to systemic antifungal drugs. In resource-constrained settings where several conditions are met for its emergence and spread, this worrisome fungus could cause large hospital and/or community-based outbreaks. This review aimed to summarize the available data on C. auris in Africa focusing on its epidemiology and antifungal resistance profile. Major databases were searched for articles on the epidemiology and antifungal resistance profile of C. auris in Africa. Out of 2,521 articles identified 22 met the inclusion criteria. In Africa, nearly 89% of African countries have no published data on C. auris. The prevalence of C. auris in Africa was 8.74%. The case fatality rate of C. auris infection in Africa was 39.46%. The main C. auris risk factors reported in Africa were cardiovascular disease, renal failure, diabetes, HIV, recent intake of antimicrobial drugs, ICU admissions, surgery, hemodialysis, parenteral nutrition and indwelling devices. Four phylogenetic clades were reported in Africa, namely clades I, II, III and IV. Candida auris showed a pan-African very high resistance rate to fluconazole, moderate resistance to amphotericin B, and high susceptibility to echinocandins. Finally, C. auris clade-specific mutations were observed within the ERG2, ERG3, ERG9, ERG11, FKS1, TAC1b and MRR1 genes in Africa. This systematic review showed the presence of C. auris in the African continent and a worrying unavailability of data on this resilient fungus in most African countries.
