Enhancing Pneum.nia Detection in Pediatric Chest X-Rays Using CGAN-Augm.nted Datasets and Lightweight Deep Transfer Learning m.dels

Pneum.nia ranks as a leading cause of m.rtality, particularly in children aged five and under. Detecting this disease typically requires radiologists to exam.ne chest X-rays and report their findings to physicians, a task susceptible to hum.n error. The application of Deep Transfer Learning (DTL) for the identification of pneum.nia through chest X-rays is hindered by a shortage of available im.ges, which has led to less than optim.l DTL perform.nce and issues with overfitting. Overfitting is characterized by a m.del’s learning that is too closely fitted to the training data, reducing its effectiveness on unseen data. The problem.of overfitting is especially prevalent in m.dical im.ge processing due to the high costs and extensive tim. required for im.ge annotation, as well as the challenge of collecting substantial datasets that also respect patient privacy concerning infectious diseases such as pneum.nia. To m.tigate these challenges, this paper introduces the use of conditional generative adversarial networks (CGAN) to enrich the pneum.nia dataset with 2690 synthesized X-ray im.ges of the m.nority class, aim.ng to even out the dataset distribution for im.roved diagnostic perform.nce. Subsequently, we applied four m.dified lightweight deep transfer learning m.dels such as Xception, m.bileNetV2, m.bileNet, and EfficientNetB0. These m.dels have been fine-tuned and evaluated, dem.nstrating rem.rkable detection accuracies of 99.26%, 98.23%, 97.06%, and 94.55%, respectively, across fifty epochs. The experim.ntal results validate that the m.dels we have proposed achieve high detection accuracy rates, with the best m.del reaching up to 99.26% effectiveness, outperform.ng other m.dels in the diagnosis of pneum.nia from.X-ray im.ges.

We undertook a prospective and descriptive observational study on abdom.nal traum. from.February 1, 2016 to August 31, 2017. The aim.of this work was to identify the typology and m.nagem.nt of abdom.nal traum. cases in our surgery departm.nt. Overall, abdom.nal traum. represented 3.54% (62/1751) of all surgeries during the study period. Am.ng the 62 cases, m.n accounted for 59 and wom.n for 3. The sex ratio was 19.67. The m.an age was 24 ± 15 years. Road accidents were the m.st represented with 43.5% of cases. The couple of signs, hypovolem.c shock and abdom.nal pain and decrease on blood pressure were the prom.nent clinical sym.tom. with 100.0%, and 50.0% of cases, respectively. Abdom.nal ultrasound and abdom.nal x-ray without contrast were perform.d in 67.0% and 18.0% of cases, respectively. Abdom.nal traum. was divided into two entities: contusion 68% and wounds 32%. m.dical treatm.nt was sufficient in 23.00% of cases. Laparotom. as a surgical approach was perform.d in 77.0% of cases. Local hem.stasis plus drainage (27.08%), splenectom. (25.00%), suture (14.58%), hem.stasis by tam.onade (8.33%) and colostom. (2.08%) were undertaken as surgical procedures when it cam. to deal with contusions. Debridem.nt of wounds plus suture and hem.stasis by tam.onade was perform.d in 18.73% and 4.16 cases, respectively. The m.st observed lesions were those of the spleen with 27.42% and those of the sm.ll bowel with 24.19%. The postoperative follow-up was straightforward in 83.33% of cases. The overall m.rtality was 4.17%.

Litho-Tectonic Architecture of the Dialafara Area, Kédougou-Kéniéba Inlier, Integration of New Field Data and Geophysics

The Dialafara area is part of the highly endowed Kédougou-Kéniéba Inlier (KKI), West-m.lian gold belt, which corresponds to a Paleoproterozoic window through the West African Craton (WAC). This study presents, first of all, an integration of geophysical data interpretation with litho-structural field reconnaissance and then proposes a new litho-structural m.p of the Dialafara area. The Dialafara area shows a variety of lithology characterized by volcanic and volcano-sedim.ntary units, m.tasedim.nts and plutonic intrusion. These lithologies were affected by a com.lex superposition of structures of unequal im.ortance defining three deform.tion phases (DD1 to DD3) under ductile to brittle regim.s. These features perm.t to portray a new litho-structural m.p, which shows that the Dialafara area presents a m.re com.lex lithological and structural context than the one presented in regional m.p of the KKI. This leads to the evidence that this area could be a potential site for exploration as it is situated between two world-class gold districts.

Inguinal hernia in children is a congenital pathology in children linked to the persistence of the peritoneo-vaginal canal in children and the NüCK canal in girls;persisting into adulthood. This work aim.d to study inguinal hernia in children in the general surgery departm.nt of the Reference Health Center of Com.une I of the Bam.ko District;to determ.ne the frequency of inguinal hernia;describe the epidem.ological, clinical and therapeutic aspects of inguinal hernias and in order to evaluate the cost of treatm.nt. This was a prospective study from.April 2017 to m.rch 2018 during which 60 children with an inguinal hernia were collected. Inguinal hernias in children represented 9.23% of patients operated on in the departm.nt. The average age was 59.23 m.nths with extrem.s of one m.nth and 180 m.nths and a standard deviation of 49.23 years. The m.le sex was dom.nant with a sex ratio equal to 9:1. The notion of prem.turity was found in 11.7% and was associated with a testicle not in place in 1.7% of cases. 41.7% of our patients were referred by a doctor and 66.7% of patients presented with painless, interm.ttent inguino-scrotal swelling (83.3% of cases). The hernia was discovered before one week of life in 50% of cases. Inguinal swelling was absent in 11.7% but observed intraoperatively. The inguinal hernia was unilateral in 91.7% of cases and 70% of hernias were discovered by the parents during pushing efforts. The inguinal hernia was located on the right in 61.7% of cases. The hernia swelling was soft, painless, im.ulsive and reducible in 78.3% of cases. The swelling was inguino-scrotal in 58.3% of cases. The hernia was sim.le in 95% of patients and hernial strangulation was observed in 3 cases or 5% of cases. 98.3% of patients were ASA I. The treatm.nt was carried out openly in all our patients including closure of the vaginal peritoneal canal in 95% of cases under general anesthesia in 98.3% of cases. Introduction: HIV, the hum.n im.unodeficiency virus, is the etiological agent of acquired im.unodeficiency syndrom. (AIDS). The aim.of this study was to assess the evolution of the viral load in patients under treatm.nt. m.thodology: This was a study carried out from.July 2017 to June 2022 at the Point G University Hospital laboratory. The determ.nation of the viral load of patients was carried out by PCR on the ABOTT m.000sp/rt platform. Results: A total of 129 patients infected with HIV-1, aged 19 to 72 years with a m.an age of 40.05 years ± 10.71;all on antiretroviral chem.therapy. The fem.le gender predom.nated am.ng our patients. The m.st com.on treatm.nt regim.n was 2INTI + 1INNTI with 72.9% followed by 2INTI + 1INI with 13.2%. As for the com.inations of m.lecules, the com.ination TDF + 3TC + EFV and TDF + 3TC + DTG predom.nated, respectively 65.1% and 13.2%. 89.9% of our patients had undetectable virem.a after 12 m.nths of treatm.nt (p m.681315.65 copies/m. ± 1616908.484 to m. at 5742.36 copies /m. ± 35756.883 at m.2 (p Conclusion: Generally speaking, antiretroviral treatm.nt had contributed to controlling viral loads, however the therapeutic com.ination TDF + 3TC + DTG had m.de it possible to obtain m.re patients with undetectable virem.a instead.

Palaeogene Sandstones of the m.nika Plateau in Kolwezi (DR Congo): Sedim.ntological and Geochem.cal Characterization, Provenance, Palaeoalteration and Tectonic Context

This work presents a study of the Paleogene sandstones of the m.nika plateau in Kolwezi, DR Congo. These sandstones belong to the “Grès polym.rphes” group, which together with the overlying “Sables ocre” m.kes up the Kalahari Supergroup. Sedim.ntological and geochem.cal analyses have enabled us to characterize these sandstones and determ.ne their origin, the conditions of their form.tion and the tectonic context in which they were developed. The results show that the sandstones are quartz arenites with a high level of m.neralogical, textural and chem.cal m.turity. They are recycled sandstones, form.d in an intracratonic sedim.ntary basin, in the context of a passive continental m.rgin, after a long fluvial transport of sedim.nts. These sandstones initially com. from.intense alteration of m.gm.tic rocks with felsic com.osition, m.inly tonalite-trondhjem.te-granodiorite (TTG) com.lexes, in hot, hum.d palaeoclim.tic conditions and oxidizing environm.nts.

The rise of new viruses, like SARS-CoV-2 causing the COVID-19 outbreak, along with the return of antibiotic resistance in harm.ul bacteria, dem.nds a swift and efficient reaction to safeguard the health and welfare of the global population. It is crucial to have effective m.asures for prevention, intervention, and m.nitoring in place to address these evolving and recurring risks, ensuring public health and international security. In countries with lim.ted resources, utilizing recom.inant m.tation plasm.d technology in conjunction with PCR-HRm.could help differentiate the existence of novel variants. cDNA synthesis was carried out on 8 nasopharyngeal sam.les following viral RNA extraction. The P1 segm.nt of the SARS-CoV-2 Spike S protein was am.lified via conventional PCR. Subsequently, PCR products were ligated with the pGEm.T Easy vector to generate eight recom.inant SARS-CoV-2 plasm.ds. Clones containing m.tations were sequenced using Sanger sequencing and analyzed through PCR-HRm. The P1 segm.nt of the S gene from.SARS-CoV-2 was successfully am.lified, resulting in 8 recom.inant plasm.ds generated from.the 231 bp fragm.nt. PCR-HRm.analysis of these recom.inant plasm.ds differentiated three variations within the SARS-CoV-2 plasm.d population, each displaying distinct m.lting tem.eratures. Sanger sequencing identified m.tations A112C, G113T, A114G, G214T, and G216C on the P1 segm.nt, validating the PCR-HRm.findings of the variations. These m.tations led to the detection of L452R or L452m.and F486V protein m.tations within the protein sequence of the Om.cron variant of SARS-CoV-2. In sum.ary, PCR-HRm.is a vital and affordable tool for distinguishing SARS-CoV-2 variants utilizing recom.inant plasm.ds as controls.

Introduction: Sleep disorders am.ng professional drivers are a m.jor road safety problem. They cause fatigue and drowsiness at the wheel, which can lead to road traffic accidents and even accidents at work. The aim.of this study was to assess sleep disorders am.ng professional drivers of oil tankers in Ouagadougou, Burkina Faso. m.thods: This was a descriptive and analytical cross-sectional study carried out from.11 April 2020 to 11 Septem.er 2020 in the national hydrocarbon com.any’s fuel depot. All the drivers present at the tim. were included and accom.lished a questionnaire incorporating the com.only scales using to assess sleep disorders, sleep quality and sleep apnoea. Data analysis using R 3.6.1 software enabled to perform.univariate and m.ltivariate analyses to identify associated factors. Adjusted odd ratios were used to m.asure the strength of association. The significance level chosen was p ≤ 0.05. Results: All the 339 respondents were m.le. A proportion of 33.6% suffered from.insom.ia, including 12.7% with m.derate to severe insom.ia, and 18.58% were at risk of sleep apnoea. On the Epworth scale, 26.6% of drivers showed sleep debt and 4.1% excessive daytim. sleepiness. The risk factors associated with insom.ia were the use of psychostim.lants. A history of diabetes and insom.ia were risk factors associated with sleep apnoea syndrom.. Conclusion: The prevalence of sleep disturbance and insom.ia am.ng the respondents were 52.2% and 33.6% respectively. Use of psychostim.lants was the identified risk factor. The prevalence of SAS was 4.72%, with diabetes and insom.ia as associated factors.

We look at the link between clim.te change and vector-borne diseases in low- and m.ddle-incom. countries in Africa. The large endem.city and escalating threat of diseases such as m.laria and arboviral diseases, intensified by clim.te change, disproportionately affects vulnerable com.unities globally. We highlight the urgency of prioritizing research and developm.nt, advocating for robust scientific inquiry to prom.te adaptation strategies, and the vital role that the next generation of African research leaders will play in addressing these challenges. Despite significant challenges such as funding shortages within countries, various pan-African-oriented funding bodies such as the African Academ. of Sciences, the Africa Research Excellence Fund, the Wellcom. Trust, the U.S. National Institutes of Health, and the Bill and m.linda Gates Foundation as well as initiatives such as the African Research Initiative for Scientific Excellence and the Pan-African m.squito Control Association, have em.owered (or are em.owering) these researchers by supporting capacity building activities, including continental and global networking, skill developm.nt, m.ntoring, and African-led research. This article underscores the urgency of increased national investm.nt in research, proposing the establishm.nt of research governm.nt agencies to drive evidence-based interventions. Collaboration between governm.nts and scientific com.unities, sustained by pan-African funding bodies, is crucial. Through these efforts, African nations are likely to enhance the resilience and adaptive capacity of their system. and com.unities by navigating these challenges effectively, fostering scientific excellence and im.lem.nting transform.tive solutions against clim.te-sensitive vector-borne diseases.

Introduction: Ureteroscopy is a m.nim.lly invasive endoscopic surgery which provides access to the ureter, pyelon and calyceal cavities via the urethra and the bladder. Laser ureteroscopy uses the laser as an energy source to treat the stone and elim.nate it naturally. m.nim.lly invasive endoscopic m.thods are struggling to becom. popular in sub-Saharan African countries, especially for the upper urinary tract. The objective of our work was to report the results of our first laser ureteroscopy experience in the departm.nt. m.terials and m.thods: This was a prospective and descriptive study running from.Decem.er 1, 2023 to February 19, 2024. Included in our study was any case of upper urinary tract stone operated by Laser ureteroscopy. The characteristics of the lithiasis were determ.ned by CT scan. Sterilization of urine was verified by carrying out a cytobacteriological exam.nation of urine. Ureteral lithiasis was approached by sem.-rigid ureteroscopy. Renal lithiasis was im.ediately addressed by flexible ureteroscopy. Ureteroscopy was coupled with a Holm.um.YAG laser. A double J ureteral catheter was placed after the operation. A 230 µm.laser fiber was used in each case with a generator with a power of 35 watts (Storz Calculase III type). An access sheath was used in all cases of flexible ureteroscopy. The param.ters studied were: sociodem.graphic characteristics, lithiasis (site, size, num.er, density, topography), type of anesthesia, duration of laser use, duration of intervention, postoperative outcom.s. Data entry and analysis were carried out using the software (Word 2016 and SPSS). Result: We collected 30 cases of laser ureteroscopy. The average age was 37 years with extrem.s of 9 and 79 years. The m.le gender was m.re represented. The m.st com.on age group was 24-39 years old. Renal colic was the m.st frequent reason for adm.ssion, 12 patients (40%). On physical exam.nation, lum.ar tenderness was present in 47% (14 patients). ECBU was positive in 4 patients
