AIM: To assess the relationship between vitamin D re-ceptor (VDR) gene polymorphisms and the presence of hepatocellular carcinoma (HCC). METHODS: Two-hundred forty patients who underwent liver transplantation were studied. The etiologies of liver disease were hepatitis C (100 patients), hepatitis B (37) and alcoholic liver disease (103). A group of 236 healthy subjects served as controls. HCC in the explanted liver was detected in 80 patients. The following single nucle-otide gene polymorphisms of the VDR were investigatedby polymerase chain reaction and restriction fragment length polymorphism: FokI C>T (F/f), BsmI A>G (B/b), ApaI T>G (A/a) and TaqI T>C (T/t) (BAT). RESULTS: The frequencies of genotypes in patients without and with HCC were for FokI F/F = 69, F/f = 73, f/f = 18 and F/F = 36, F/f = 36, f/f = 8; BsmI b/b = 45, B/b = 87, B/B = 28 and b/b = 33, B/b = 35, B/B = 12; for ApaI A/A = 53, A/a = 85, a/a = 22 and A/A = 27, A/a = 38, a/a = 15; for TaqI T/T = 44, T/t = 88, t/t = 28 and T/T = 32, T/t = 38, t/t = 10. Carriage of the b/b genotype of BsmI and the T/T genotype of TaqI was signif icantly associated with HCC (45/160 vs 33/80, P < 0.05 and 44/160 vs 32/80, P < 0.05, respectively). The absence of the A-T-C protective allele of BAT was signif i-cantly associated with the presence of HCC (46/80 vs 68/160, P < 0.05). A strong association was observed between carriage of the BAT A-T-C and G-T-T haplotypes and HCC only in alcoholic liver disease (7/46 vs 12/36 vs 11/21, P < 0.002, respectively).CONCLUSION: VDR genetic polymorphisms are sig-nificantly associated with the occurrence of HCC in patients with liver cirrhosis. This relationship is more specific for patients with an alcoholic etiology.

BACKGROUND In paediatric patients with complicated nephrotic syndrome(NS), rituximab(RTX) administration can induce persistent IgG hypogammaglobulinemia among subjects showing low basal immunoglobulin G(IgG) *** To evaluate the effect of RTX on IgG levels and infections in patients with complicated NS and normal basal IgG *** We consecutively enrolled all patients with complicated NS and normal basal IgG levels undergoing the first RTX infusion from January 2008 to January 2016. Basal IgG levels were dosed after 6 wk of absent proteinuria and with a maximal interval of 3 mo before RTX infusion. The primary outcome was the onset of IgG hypogammaglobulinemia during the follow-up according to the IgG normal values for age [mean ± standard deviation(SD)].RESULTS We enrolled 20 patients with mean age at NS diagnosis of 4.2 ± 3.3 years. The mean age at the first RTX infusion was 10.9 ± 3.5 years. Eleven out of twenty patients(55%) developed IgG hypogammaglobulinemia. None of these patients showed severe or recurrent infections. Only one patient suffered from recurrent acute otitis media and underwent substitutive IgG infusion. Three patients undergoing only the two "starting doses" experienced normalization of IgG levels. Using Kaplan-Meier analysis, the cumulative proportion of patients free of IgG hypogammaglobulinemia was 57.8% after the first RTX dose, 51.5% after the third dose, 44.1% after the fourth dose, and 35.5% after the fifth *** RTX can induce IgG hypogammaglobulinemia in patients with pre-RTX IgG normal values. None of the treated patients showed severe infections.

Metabolic Requirements during Six Minutes Walking Tests in Patients Affected by Chronic Obstructive Pulmonary Disease in Different Stages

Backgrounds: In Chronic Obstructive Pulmonary Disease (COPD) a multi factorial effort limitation becomes pro- gressively relevant as the disease progresses in the consecutive stages. It is measured by both six minutes walking test (6MWT) and maximal cardiopulmonary incremental test (CPET). Aim: It is important to assess in each stage of dis-ease the metabolic load during 6MWT referring to the outcome of CPET and to ascertain whether there is a significant rela- tionship between the measures obtained by CPET and 6MWT. Methods: Four group of fifteen patients affected by COPD in stage I to IV underwent 6MWT and maximal CPET in the same day and results were compared to a group of healthy people. Airflow obstruction was measured by whole body plethysmography, blood gases by gas analysis, maxi- mal oxygen consumption and metabolic parameters by ergometer, lactic acid levels by analyzer. Results: Maximal oxygen consumption (V’O2max) and 6MWT are progressively impaired and related (V’O2max = 1.25 ? 0.26, 1.152 ? 0.4, 1.03 ? 0.44, .85 ? 0.2 l/m;6MWD = 452 ? 84, 446 ? 82, 381 ? 165, 200 ? 100 respectively in GOLD I to IV stage). Oxygen consumption (V’O2) during 6MWT becomes stable after 3 - 4 minutes and reached at the end of the test close to those measured at de-compensated metabolic acidosis anaerobic threshold (TDMA) (85 ± 0.4 l/m vs. 9 ? 0.4 l/m) in stage I to III, while in COPD there is no difference between V’O2max and V’O2 during 6MWT (0.85 ? 0.2 vs. 0.8 ? 0.23 l/m). 6MWT is more suitable to determine oxygen desaturation than CPET (dSaO2 ?4 ± 2% vs. ?2 ± 1%). 6MWD, the workload performed in 6MWT and V’O2max are significantly related. Conclusion: 6MWT looks as a suitable sub maximal test related CPET. Metabolic requirements under 6MWT are close to TDMA and are obtained in a suitable, self paced, usual exercise, close to everyday experience and thus related to activity daily levels. As the disease worsens the differences between V’O2 during 6MWT and V’O2max wane.

Objectives:To test hypothesized associations between the ABO blood group(ABO-bg)system and the pathological features of pros-tate cancer(PCa).Material and methods:Between January 2013 and September 2019,1173 patients underwent radical *** between ABO-bg levels and pathological features were evaluated using statistical methods.Results:Overall,1149 consecutive patients were evaluated using the ABO-bg system,which was represented by O-bg(42.8％)and A-bg(41.3％),followed by B-bg(11.1％)and AB-bg(4.8％).Only positive surgical margins(PSMs)was correlated with ABO-bg(Pear-son correlation coefficient,r=0.071;p=0.017),and the risk was increased in group-O(odds ratio[OR],1.497;95％confidence interval,1.149-1.950;p=0.003)versus *** clinical and pathological models,O-bg was at increased risk of PSM after the adjustment for prostate-specific antigen,percentage of biopsy-positive cores,and high surgical volume(adjusted OR,1.546;95％con-fidence interval,1.180-2.026;p=0.002);however,the adjusted OR did not change after the adjustment for tumor load and stage as well as high surgical volume.Conclusions:In clinical PCa,the risk of PSM was higher in O-bg versus non-O-bg patients after the adjustment for standard *** studies are needed to confirm the association between ABO-bg and unfavorable PCa features.

Nonlinear uncertainty propagation is of critical importance in many application fields of astrodynamics. In this article, a framework combining the differential algebra technique and the Gaussian mixture model method is presented to accurately propagate the state uncertainty of a nonlinear system. A high-order Taylor expansion of the final state with respect to the initial deviations is firstly computed with the differential algebra technique. Then the initial uncertainty is split to a Gaussian mixture *** the high-order state transition polynomial, each Gaussian mixture element is propagated to the final time, forming the final Gaussian mixture model. Through this framework, the final Gaussian mixture model can include the effects of high-order terms during propagation and capture the non-Gaussianity of the uncertainty, which enables a precise propagation of probability density. Moreover, the manual derivation and integration of the high-order variational equations is avoided, which makes the method versatile. The method can handle both the application of nonlinear analytical maps on any domain of interest and the propagation of initial uncertainties through the numerical integration of ordinary differential equation. The performance of the resulting tool is assessed on some typical orbital dynamic models, including the analytical Keplerian motion, the numerical J_2 perturbed motion,and a nonlinear relative motion.
