This palaeontological study focuses on the sedimentology and palaeofauna of Areia do Mastro locale in Cabo Espichel (Sesimbra, south of Lisbon). Two main geological formations are identified in Areia do Mastro: the Areia do Mastro Formation and the Papo-Seco Formation. We report herein the fossilized vertebrate and invertebrate remains from the basal deposits (marls, sands and gravels) of Papo-Seco Formation assigned to Lower Barremian (Lower Cretaceous). The sedimentological analysis and identified palaeofauna indicated a changing environment with important biological and ecological aspects. Extensive periods of sea level fluctuations and subsequent transgressive marine episodes had as a result the continental infralittoral internal platform to be gradually replaced by a lagoon, as revealed from the fossil remains embodied in the sediments during the formation of the afore mentioned sedimentary sequences.

Totally robotic vs 3D laparoscopic colectomy: A single centers preliminary experience

AIM: to compare robotic and three-dimensional(3D) laparoscopic colectomy based on the literature and our preliminary ***: This retrospective observational study compared operative measures and postoperative outcomes between laparoscopic 3D and robotic colectomy for cancer. From September 2013 to September 2014,24 robotic colectomies and 23 3D laparoscopic colectomy were performed at our Department. Data were analyzed and reported both by approach and by colectomy side. Robotic left colectomy(RL) vs laparoscopic 3D left colectomy(LL 3D) and Robotic right colectomy(RR) vs laparoscopic 3D(LR 3D). Rectal cancer procedures were not ***: There were 18 RR and 11 LR 3D,6 RL and 12 LL 3D. As regards LR 3D,extracorporeal anastomosis(EA) was performed in 7 patients and intracorporeal anastomosis(IA) in 4; the RR group included 14 IA and 4 EA. There was no mortality. Median operative time was higher for the robotic group while conversion rate(12.5% vs 13%) and lymph nodes removed(14 vs 13) were similar for both. First flatus time was 1 d for RR and 2 d the other patient groups. Oral intake was resumed in 1 d by LR and in 2 d by the other patients(P = 0.012). Overall cost was €4950 and €1950 for RL and LL 3D,and €4450 and €1450 for RR and LR 3D,respectively. CONCLUSION: There were no differences betweenRR and LR 3D,except that IA was easier with RR,and probably contributed with the learning curve to the longer operative time recorded. Both techniques offer similar advantages for the patient with significantly different costs. In left colectomies robotic colectomy provided better outcomes,especially in resections approaching the rectum.

Physical,Biochemical and Biological Characterization of Two Opposite Areas in the Southern Adriatic Sea(Mediterranean Sea)

The Drini and ManfredoniaGulfswere investigated in May 2008 and April 2009, respectively. The gulfs are located in the South Eastern (GulfofDrini) and South Western (GulfofManfredonia)Adriatic Sea. The areas are partially influenced by two main Adriatic surface currents: the Eastern Adriatic Current-EAC that flows north-westward along the eastern side, and the Western Adriatic Current—WAC that flows south-eastward along the western side of the basin. The spatial variations of temperature, salinity, fluorescence, dissolved oxygen concentration, Coloured Dissolved Organic Matter—CDOM, nutrients, chlorophyll a and phytoplankton composition parameters in the two areas were observed and compared. CDOM regulates the penetration of UV light into the sea and plays an important role in many hydrological and biogeochemical processes on the sea surface layer including primary productivity. The phytoplankton specific diversity of the Gulf of Manfredonia showed a spring community with dinoflagellates (21 taxa) as the main important fraction, coccolithophorales (6 taxa) and diatoms with 10 identified taxa. The phytoplankton distribution along the eastern coast showed a different biodiversity: a prevalence of dinoflagellates (58 taxa) included harmful microalgae such as Alexandrium, Dinophysis and Lingulodinium genus. Diatoms were less abundant, among them Pseudo-nitzschia was also reported which could include some potentially toxic species. Nanoplankton are ever abundant in offshore waters and an exceptional bloom of cyanobacteria was registered in Buna-Boyana estuary due to strong industrial impact. The gulfs showed similar physical and biochemical characteristics despite the WAC carried out along the westernAdriatic Seathe water rich in nutrients from the major northern Italian rivers. No correlations were found between CDOM and chlorophyll a in the two areas and this implied that, probably, the primary source of CDOM might come from terrestrial input rather than the biological

Excess body weight, liver steatosis, and early fibrosis progression due to hepatitis C recurrence after liver transplantation

AIM: To investigate how weight gain after OLT affects the speed of fibrosis progression (SFP) during recurrent hepatitis C virus (HCV) infection of the ***: Ninety consecutive patients (63 males,median age 53 years; 55 with HCV-related liver disease),transplanted at a single institution, were studied. All were followed for at least 2 years after OLT and had at least one follow-up graft biopsy, performed not earlier than 1 year after the transplant operation. For each biopsy, a single,experienced pathologist gave an estimate of both the staging according to Ishak and the degree of hepatic *** SFP was quantified in fibrosis units/month (FU/mo).The lipid metabolism status of patients was summarized by the plasma triglycerides/cholesterol (T/C) ratio. Body mass index (BMI) was measured before OLT, and 1 and 2 years after ***: In the HCV positive group, the highest SFP was observed in the first post-OLT year. At that time point,a SFP ≤0.100 FU/mo was observed more frequently among recipients who had received their graft from a young donor and had a pre-transplant BMI value ＞26.0 kg/m2. At completion of the first post-transplant year, a BMI value ＞26.5 kg/m2 was associated with a T/C ratio ≤1. The proportion of patients with SFP ＞0.100 FU/mo descended in the following order: female recipients with a high T/C ratio, male recipients with high T/C ratio, and recipients of either gender with low T/C ratio. Hepatic steatosis was observed more frequently in recipients who, in the first post-transplant year, had increased their BMI ≥1.5 kg/m2 in comparison to the pre-transplant value. Hepatic steatosis was inversely associated with the staging ***: Among HCV positive recipients, excessweight gain post-OLT does not represent a factor favoring early liver fibrosis development and might even be protective against it.

Novel endoscopic management for pancreatic pseudocyst with fistula to the common bile duct

Pancreatic pseudocyst formation is a well-known complication of pancreatitis. It represents about 75% of the cystic lesions of the pancreas and might be located within or surrounding the pancreatic tissue. Sixty percent of the occurrences resolve spontaneously and only persistent, symptomatic or complicated cysts need to be treated. Complications include infection, hemorrhage, gastric outlet obstruction, splenic infarction and rupture. The formation of fistulas to other viscera is rare and most commonly occurs within the stomach, duodenum or colon. We report a case of a patient with a pancreatic pseudocyst in communication with the common bile duct. There have been only few cases reported in the literature. We successfully managed our case by performing an endoscopicultrasound-guided drainage of the pancreatic collection and a contemporaneous stenting of the common bile duct. Performed independently, both drainages are effective, safe and well-coded and the expertise on these procedures is widespread. By our knowledge this therapeutic approach was never reported in literature but we retain this is the most correct treatment for this very rare condition.
