Aims and background: we evaluate CT-3Tesla MRI fusion in conformal radiotherapy for localized prostate cancer.Methods: 18 consecutive patients underwent a 3T MRI scan under radiotherapy planning conditions, after the CT scan. Bowel and bladder preparation were prescribed. CT and MR images were automatically fused;prostate and seminal vesicles were contoured on CT and on MRI, organs at risk were defined on CT-MRI fusion. Late rectal and sexual toxicity, differences in target volume between MRI and CT and differences in rectal and penile bulb dose distribution based on CT only or on CT-MRI fusion were evaluated.Results: one patient experienced a late rectal toxicity;no patient had sexual toxicity. The difference between the mean MRI and CT target volumes was statistically significant (p = 0.0001 paired Student's t-test). The dose-volume histogram (DVH) analysis shows a significant reduction of the dose received by the rectum and the penile bulb in MRI-plans compared to CT-plans.Conclusions: 3 Tesla MRI scan under radiotherapy planning conditions along with bowel preparation significantly improves the definition of the target volume sparing normal tissue irradiation.

Life sciences such as medical, nutritional, and those related to agrarian biotechnologies are experiencing a constant increase. Therefore, food consumption is today related to health state. Aiming to create an innovative spinneret system that defines the tight correlation between high nutritional and nutraceutical value of foods and their beneficial effects on consumer’s health state, we tested the effects related to the administration of 40 g of the Tonda Gentile Romana Italian hazelnut cultivar (Viterbo, Italy). To this end, we enrolled 24 subjects for a period of 14 weeks, based on a crossover design. Although several papers and reviews on hazelnut intake benefits are available, few studies have investigated the effects of hazelnut-enriched diet on plasma key parameters commonly used to ascertain the individual’s well-being status. The primary endpoint of the trial was to assess LDL change related to hazelnut administration, which resulted in significant beneficial effects on cholesterol parameters (LDL dropped from 133.09 mg/dl to 119.18 mg/dl). Interestingly, despite within the normal ranges, uric acid and serum creatinine mean values significantly decreased after hazelnut intake. A significant increment was also observed for ALT (from 30.9 U/L to 35.22 U/L, P = 0.01), while GGT significantly decreased from 38.04 mg/dl at T0 to 35.27 mg/dl at T1 (P = 0.001). No significant changes were also observed in azotemia, serum iron, glycemia, AST, total bilirubin, Hb, WBCs, RBCs, platelet count, and total plasma proteins. All the blood parameters analyzed appeared to be reversible after 6 weeks from the end of the treatment. Mann-Whitney U test did not show any significant differences in T0 - T1 Δ% LDL considering gender, age and baseline LDL. These findings may be of help in the development of strategies for personalized nutrition and diseases management.

Efficacy, risk factors and complications of endoscopic polypectomy: Ten year experience at a single center

AIM: To examine the efficacy and complications of colonoscopic resection of colorectal polypoid lesions. METHODS: We retrospectively reviewed 1354 polypectomies performed on 1038 patients over a ten- year period. One hundred and sixty of these were performed for large polyps, those measuring ≥ 20 mm. Size, shape, location, histology, the technique of polypectomy used, complications, drugs assumption and associated intestinal or extra intestinal diseases were analyzed. For statistical analysis, the Pearson χ2 test, NPC test and a Binary Logistic Regression were used. RESULTS: The mean patient age was 65.9 ± 12.4 years, with 671 men and 367 women. The mean size of polyps removed was 9.45 ± 9.56 mm while the size of large polyps was 31.5 ± 10.8 mm. There were 388 pedunculated and 966 sessile polyps and the most common location was the sigmoid colon (41.3%). The most frequent histology was tubular adenoma (55.9%) while for the large polyps was villous (92/160 -57.5%). Coexistent malignancy was observed in 28 polyps (2.1%) and of these, 20 were large polyps. There were 17 procedural bleeding (1.3%) and one perforation. The statistical analysis showed that cancer is correlated to polyp size (P < 0.0001); sessile shape (P < 0.0001) and bleeding are correlated to cardiac disease (P = 0.034), tubular adenoma (P = 0.016) and polyp size.CONCLUSION: The endoscopic resection is a simple and safe procedure for removing colon rectal neoplastic lesions and should be considered the treatment of choice for large colorectal polyps. The polyp size is an important risk factor for malignancy and for bleeding.

Painful scar neuropathy:principles of diagnosis and treatment

Nerve-tissue interactions are critical.Peripheral nerve injuries may involve intraneural and extraneural scar formation and affect nerve gliding planes,sometimes leading to complex clinical presentations.All of these pathological entities involve pain as the main clinical symptom and can be subsumed under the term“painful scar neuropathy”.The authors review the literature on treatment approaches to peripheral nerve scar neuropathy and the outcomes of neurolysis-associated procedures and propose a simple classification and a therapeutic approach to scar neuropathy.The search retrieved twenty-one papers,twenty of which reported pain reduction or resolution with various techniques.There is no consensus on the best therapeutic approach to neuropathic pain due to scar tethering.Most authors report good or excellent results with different techniques,from nerve wrapping with anti-adhesion devices to nerve coverage or wrapping with vascularized tissue.The authors’classification of and therapeutic approach to peripheral nerve scar lesions aims at promoting a logical approach based on the analysis of lesion type(perineural,or endoneural and perineural),pain type(due to traction or external trauma,pain at rest),and number of previous operations.Patients need to be informed that multiple procedures may be required,that outcomes may be partial,and that surgery can potentially worsen preoperative conditions.The review found no evidence for the best therapeutic approach to scar neuropathy,but there is consensus on a multidisciplinary approach.

Two-dimension(2D)magnets have recently developed into a class of stoichiometric materials with prospective applications in ultra-compact spintronics and quantum computing.Their functionality is particularly rich when different magnetic orders are competing in the same material.Metalloxenes REX2(RE=Eu,Gd;X=Si,Ge),silicene or germanene—heavy counterparts of graphene—coupled with a layer of rare-earth metals,evolve from three-dimension(3D)antiferromagnets in multilayer structures to 2D ferromagnets in a few monolayers.This evolution,however,does not lead to fully saturated 2D ferromagnetism,pointing at a possibility of coexisting/competing magnetic states.Here,REX2 magnetism is explored with element-selective X-ray magnetic circular dichroism(XMCD).The measurements are carried out for GdSi2,EuSi2,GdGe2,and EuGe2 of different thicknesses down to 1 monolayer employing K absorption edges of Si and Ge as well as M and L edges of the rare-earths.They access the magnetic state in REX2 and determine the seat of magnetism,orbital,and spin contributions to the magnetic moment.High-field measurements probe remnants of the bulk antiferromagnetism in 2D REX2.The results provide a new platform for studies of complex magnetic structures in 2D materials.

This palaeontological study focuses on the sedimentology and palaeofauna of Areia do Mastro locale in Cabo Espichel (Sesimbra, south of Lisbon). Two main geological formations are identified in Areia do Mastro: the Areia do Mastro Formation and the Papo-Seco Formation. We report herein the fossilized vertebrate and invertebrate remains from the basal deposits (marls, sands and gravels) of Papo-Seco Formation assigned to Lower Barremian (Lower Cretaceous). The sedimentological analysis and identified palaeofauna indicated a changing environment with important biological and ecological aspects. Extensive periods of sea level fluctuations and subsequent transgressive marine episodes had as a result the continental infralittoral internal platform to be gradually replaced by a lagoon, as revealed from the fossil remains embodied in the sediments during the formation of the afore mentioned sedimentary sequences.

Physical,Biochemical and Biological Characterization of Two Opposite Areas in the Southern Adriatic Sea(Mediterranean Sea)

The Drini and ManfredoniaGulfswere investigated in May 2008 and April 2009, respectively. The gulfs are located in the South Eastern (GulfofDrini) and South Western (GulfofManfredonia)Adriatic Sea. The areas are partially influenced by two main Adriatic surface currents: the Eastern Adriatic Current-EAC that flows north-westward along the eastern side, and the Western Adriatic Current—WAC that flows south-eastward along the western side of the basin. The spatial variations of temperature, salinity, fluorescence, dissolved oxygen concentration, Coloured Dissolved Organic Matter—CDOM, nutrients, chlorophyll a and phytoplankton composition parameters in the two areas were observed and compared. CDOM regulates the penetration of UV light into the sea and plays an important role in many hydrological and biogeochemical processes on the sea surface layer including primary productivity. The phytoplankton specific diversity of the Gulf of Manfredonia showed a spring community with dinoflagellates (21 taxa) as the main important fraction, coccolithophorales (6 taxa) and diatoms with 10 identified taxa. The phytoplankton distribution along the eastern coast showed a different biodiversity: a prevalence of dinoflagellates (58 taxa) included harmful microalgae such as Alexandrium, Dinophysis and Lingulodinium genus. Diatoms were less abundant, among them Pseudo-nitzschia was also reported which could include some potentially toxic species. Nanoplankton are ever abundant in offshore waters and an exceptional bloom of cyanobacteria was registered in Buna-Boyana estuary due to strong industrial impact. The gulfs showed similar physical and biochemical characteristics despite the WAC carried out along the westernAdriatic Seathe water rich in nutrients from the major northern Italian rivers. No correlations were found between CDOM and chlorophyll a in the two areas and this implied that, probably, the primary source of CDOM might come from terrestrial input rather than the biological production from phytoplankton.
