Deepening trochleoplasty combined with balanced medial patellofemoral ligament reconstruction for an adequate graft tensioning

AIM To evaluate our modified deepening trochleoplasty com-bined with a balanced medial patellofemoral ligament(MPFL) reconstruction for soft tissue alignement. METHODS Thirty-three knees with with recurrent patellar dislocations and a trochlear dysplasia in 30 patients(m/f = 12/21, mean age 24 ± 9 years) underwent a combination of a modified deepening trochleoplasty and a balanced MPFL reconstruction for a medial soft tissue alignement. After a mean follow-up period of 29 ± 23 mo, patients' return to sports, possible complications as well as the clinical outcomes using the Kujala, International Knee Documentation Committee(IKDC) and Lysholm scoring were evaluated. Moreover, patients' satisfaction with the general outcome, the cosmetic outcome, the pre-and postoperative pain and a potential avoidance behaviour were assessed with additional standardized questionnaires which also included different visual analog scales. RESULTS There were no signs of a persistent instability. The Kujala score improved from a mean of 64 ± 16 points to 94 ± 9 points, the Lysholm score improved from a mean of 63 ± 17 to 95 ± 6 points and the IKDC score from 58 ± 11 to 85 ± 12 points, P < 0.0001, respectively. The assessment of pain using a visual analog scale showed a significant pain reduction from a mean of 4.8 ± 2.0 to 1.3 ± 3.4 points(P < 0.0001). Two of 26 cases(92%) who were engaged in regular physical activity before surgery did not return to full sporting activities. One patient felt that his sport was too risky for his knee and reported an ongoing avoidance behaviour. The other patient preferred to wait for surgery of her contralateral knee. Of the eight patients who were not engaged in sporting activities before surgery, three started regular sporting activities after surgery. In 31 of the 33 cases(94%), the patients were very satisfied with the clinical outcome of the surgery. BACKGROUND The optimal way to home-monitor patients with inflammatory bowel disease(IBD)for disease progression or relapse remains to be *** To determine whether an electronic health(eHealth)screening procedure for disease activity in IBD should be implemented in clinical practice,scheduled every third month(3M)or according to patient own decision,on demand(OD).METHODS Adult IBD patients were consecutively randomized to 1-year open-label eHealth interventions(3M vs OD).Both intervention arms were screening for disease activity,quality of life and fatigue and were measuring medical compliance with the constant care web-application according to the screening interventions OD or *** activity was assessed using home measured fecal calprotectin(FC)and a disease activity *** In total,102 patients were randomized(n=52/503M/OD)at baseline,and 88 patients completed the 1-year study(n=433M;n=45 OD).No difference in the two screening procedures could be found regarding medical compliance(P=0.58),fatigue(P=0.86),quality of life(P=0.17),mean time spent in remission(P>0.32),overall FC relapse rates(P=0.49),FC disease courses(P=0.61),FC time to a severe relapse(P=0.69)and remission(P=0.88)during 1 ***(interquartile range)numbers of FC home-monitoring test-kits used per patient were significantly different,3M:6.0(5.0-8.0)and OD:4.0(2.0-9.0),P=*** The two eHealth screening procedures are equally good in capturing a relapse and bringing about ***,the OD group used fewer FC home testkits per *** screening procedures can be recommended for adult IBD patients in clinical web-practice.

AIM： Growth hormone （GH） directly interacts with the enterocyte stimulating ion absorption and reducing ion secretion induced by agonists of cAMR Since nitric oxide （NO） is involved in the regulation of transepithelial ion transport and acts as a second messenger for GH hemodynamic effects, we tested the hypothesis that NO may be involved in the resulting effects of GH on intestinal ion transport. METHODS： Electrical parameters reflecting transepithelial ion transport were measured in Caco-2 cell monolayers mounted in Ussing chambers and exposed to GH and cholera toxin （CT） alone or in combination, in the presence or absence of the NO synthase （NOS） inhibitor, Nω-nitro-L-arginine methyl ester （L-NAME）. Similar experiments were conducted to determine cAMP and nitrite/nitrate concentrations. NOS expression was assayed by Western blot analysis. RESULTS： L-NAME causes total abrogation of absorptive and antisecretory effects by GH on intestinal ion transport, In addition, L-NAME was able to inhibit the GH-effects on intracellular cAMP concentration under basal conditions and in response to CT, GH induced a Ca^2＋ -dependent increase of nitrites/nitrates production, indicating the involvement of the constitutive rather than the inducible NOS isoform, which was directly confirmed by Western blot analysis. CONCLUSION： These results suggest that the GH effects on intestinal ion transport, either under basal conditions or in the presence of cAMP-stimulated ion secretion, are mediated at an intracellular level by the activity of cNOS.

Soy(Glycine max)and oats(Avena sativa)are plant sources used in milk-alternative ***,protein and lipid constituents of these food matrices can undergo alterations during the *** this work,a commercial formulation of soy and oat-based beverages were comparatively *** the 12 months of shelf life and two following months,their phenolic content,antioxidant capacity,lipid peroxidation,protein carbonyl formation and protein breakdown were *** phenolic content and antioxidant capacity of soy and oat-based beverages were maintained during the entire period of 14 *** beverages did not show any increase in spontaneous lipid peroxidation beyond the basal level,however,due to the different content of unsaturated fats,when lipid peroxidation was stimulated,soy exhibited a major peroxidizability with respect to oat *** alteration of proteins,estimated as carbonyl group formation,presented no increase with respect to the basal levels both in soy and oat beverages for all 14 ***,soy proteins showed a gradual increase of proteolytic activity up until half of the shelf life,while oat did not show significant changes in protein *** conclusion,both soy and oat beverages resulted oxidatively stable throughout their *** suggest that phytochemicals might guarantee the oxidative stability of the product,possibly in combination with antioxidant bioactive peptides,which already have well-known benefits on human health.

The bee genus Thrinchostoma Saussure, 1890 is reported from China with a new species and a new record species, T. （Thrinchostoma） yunnanense Niu ＆ Zhu, sp. nov. and T（T.） sladeni Cockerell, 1913. An illustrated key to the two Chinese species of the subgenus Thrinchostoma （T.） is provided. The type specimens are deposited in Institute of Zoology, Chinese Academy of Sciences, Beijing, China.

Infectious endometritis is one of the main causes of subfertility/infertility in the mare. In this report, we present the first case of endometritis in mare associated with a strain of Enterococcus casseliflavus, an unusual gram-positive bacterium which can also be a zoonotic agent. Furthermore, the isolated strain showed a worrying multidrug-resistant profile. The accurate finding of a successful antimicrobial treatment and consequently,the pregnancy diagnosis indicate the importance to isolate, identify and define the antibiotic resistance profile of bacteria associated with endometritis.
