Cytogenetic and molecular aberrations of multiple myeloma patients： a single-center study in Singapore

Background Much is known about the cytogenetic lesions that characterize multiple myeloma （MM） patients from the USA, Europe, and East Asia. However, little has been published about the disease among Southeast Asians. The aim of this study was to determine the chromosomal abnormalities of MM patients in our Singapore population. Methods Forty-five newly-diagnosed, morphologically confirmed patients comprising 18 males and 27 females, aged 46-84 years （median 65 years） were investigated by karyotyping and fluorescence in situ hybridization （FISH）. FISH employing standard panel probes and lp36/lq21 and 6q21/15q22 probes was performed on diagnostic bone marrow samples. Results Thirty-four cases （75.6%） had karyotypic abnormalities. Including FISH, a total detection rate of 91.1% was at- tained. Numerical and complex structural aberrations were common to both hyperdiploid and non-hyperdiploid patients. Numerical gains of several recurring chromosomes were frequent among hyperdiploid patients while structural rear- rangements of several chromosomes including 8q24.1 and 14q32 characterized non-hyperdiploid patients. With FISH, immunoglobulin heavy chain （IGH） gene rearrangements, especially fibroblast growth factor receptor 3 （FGFR3）/IGH and RB1 deletion/monosomy 13 were the most common abnormalities （43.4%）. Amplification lq21 was 10 times more frequent （42.5%） than del（1p36） and del（6q21）. Conclusions We have successfully reported the comprehensive cytogenetic profiling of a cohort of newly-diagnosed myeloma patients in our population. This study indicates that the genetic and cytogenetic abnormalities, and their frequencies, in our study group are generally similar to other populations.

CXC ligand 17(CXCL17)is a novel CXC chemokine whose clinical significance remains largely *** the present study,we characterized the prognostic value of CXCL17 in patients with hepatocellular carcinoma(HCC)and evaluated the association of CXCL17 with immune *** examined CXCL17 expression in 227 HCC tissue specimens by immunohistochemical staining,and correlated CXCL17 expression patterns with clinicopathological features,prognosis,and immune infiltrate density(CD4 T cells,CD8 T cells,B cells,natural killer cells,neutrophils,macrophages).CXCL17-producing cells accumulated in peritumoral stroma and intratumoral tissues compared with the nontumor region of ***-Meier survival analysis showed that both increased intratumoral CXCL17(P=0.015for overall survival[OS],P=0.003 for recurrence-free survival[RFS])and peritumoral CXCL17(P=0.002 for OS,P<0.001 for RFS)were associated with shorter OS and *** in the CXCL171ow group had significantly lower5-year recurrence rate compared with patients in the CXCL17high group(peritumoral:53.1%vs.77.7%,P<0.001,intratumoral:58.6%vs.73.0%,P=0.001,respectively).Multivariate Cox proportional hazards analysis identified peritumoral CXCL17 as an independent prognostic factor for both OS(hazard ratio[HR]=2.066,95%confidence interval[CI]=1.296-3.292,P=0.002)and RFS(HR=1.844,95%CI=1.218-2.793,P=0.004).Moreover,CXCL17 expression was associated with more CD68 and less CD4 cell infiltration(both P<0.05).The combination of CXCL17 density and immune infiltration could be used to further classify patients into subsets with different prognosis for *** results provide the first evidence that tumor-infiltrating CXCL17+cell density is an independent prognostic factor that predicts both OS and RFS in ***17 production correlated with adverse immune infiltration and might be an important target for anti-HCC therapies.

Hepatocellular carcinoma(HCC) is the sixth most common cancer and the third most common cause of cancer-related deaths worldwide. The Barcelona Clinic Liver Cancer(BCLC) classification has been endorsed as the optimal staging system and treatment algorithm for HCC by the European Association for the Study of Liver Disease and the American Association for the Study of Liver Disease. However, in real life, the majority of patients who are not considered ideal candidates based on the BCLC guideline still were performed hepatic resection nowadays, which means many hepatic surgeons all around the world do not follow the BCLC guidelines. The accuracy and application of the BCLC classification has constantly been challenged by many clinicians. From the surgeons' perspectives, we herein put forward some comments on the BCLC classification concerning subjectivity of the assessment criteria, comprehensiveness of the staging definition and accuracy of the therapeutic recommendations. We hope to further discuss with peers and colleagues with the aim to make the BCLC classification more applicable to clinical practice in the future.

探讨我国外科医师诊断与治疗原发性肝细胞癌(以下简称肝癌)是否已到达外科4.0。外科4.0是Hooshair A于2018年提出,主要是描述基于不同时期科学发现和应用推动外科发展的4个阶段:外科1.0(18―20世纪,外科科学期);外科2.0(1950―1980年,超大型手术和器官移植期);外科3.0(1980年至21世纪初,微创外科普及期)和外科4.0(21世纪初至今,数字外科期)。在医疗卫生领域中,数字科学的应用带来翻天覆地的改变。外科4.0主要是由大数据、人工智能、自动化、现代机器人和5G通讯五大科学推动,目前在外科领域初探成功,但在肝癌的诊断与治疗领域中,尚有很大发展空间。

“道”与“术”是中国传统文化中的两个概念。“道”指基本概念、基本原则、准则和规则，“术”则是“道”在日常生活中的应用。若将这两个概念应用于临床医学，“道”就是指每一位优秀临床医师都应遵循的基本原则和准则，即医德优良、医学知识丰富、医技精湛，以及将其应用于治病救人的能力。此外，每一位临床医师都应致力于终生医学继续教育和培训，扩展医学知识。对于一位优秀的外科医师而言，一个额外的要求就是拥有一双妙手。“术”指外科医师要通过自己的双手来运用医学知识和技能。“道”与“术”平衡是成为一位优秀外科医师的基本要求，也是其未来发展的基石。外科临床研究是基于良好的临床工作和结果，而好的临床研究能够促进临床工作和结果，两者相辅相成，形成良性循环。换言之，外科临床研究与创新是“道”与“术”平衡的最终成果。

A six-year descriptive epidemiological study of human coronavirus infections in hospitalized patients in Hong Kong

We conducted a six-year epidemiological study on human coronaviruses(HCoVs) circulating in Hong Kong, using 8275 nasopharyngeal samples from patients with acute respiratory tract infections. HCoVs were detected in 77(0.93%) of the samples by a pan-HCoV RT-PCR assay. The most frequently detected HCoV species was HCoV-OC43(0.58%), followed by HCoV-229E(0.15%),HCoV-HKU1(0.13%) and HCoV-NL63(0.07%). HCoVs were detected throughout the study period(September 2008–August 2014), with the highest detection rate from September 2010 to August2011(22/1500, 1.47%). Different seasonal patterns of each HCoV species in Hong Kong were ***-OC43 was predominant in the fall and winter, whereas HCoV-HKU1 showed peak activity in winter, with a few cases occurred in spring and summer. HCoV-229 E mainly occurred in winter and spring, while HCoV-NL63 was predominant in summer and autumn. HCoVs most commonly infect the elderly and young children, with median age of 79.5 years(range, 22 days to 95 years).Intriguingly, the detection rate of HCoV-OC43 in the age group of > 80 years(26/2380, 1.09%) was significantly higher than that in the age group of 0–10 years(12/2529, 0.47%)(P < 0.05). These data provides new insight into the epidemiology of coronaviruses.

Ever since its earliest reports, portal venous gas (PVG) has been associated with numerous intraabdominal catastrophes and has served as an indication for urgent surgical exploration. It is traditionally regarded to be an ominous finding of impending death, with highest mortality reported in patients with underlying bowel ischemia. Today, computed tomography has demonstrated a wider range of clinical conditions associated with PVG, some of which are ‘benign’ and do not necessarily require surgery, unless when there are signs of intraabdominal catastrophe or systemic toxicity. One of these ‘benign’ conditions is Crohn’s disease. The present report describes a 19-year-old Chinese boy with Crohn’s pancolitis who presented with septic shock associated with PVG and portal vein thrombosis, and was successfully managed surgically. To our knowledge, this is the fi rst report of PVG and portal vein thrombosis associated with Crohn’s disease in a Chinese patient. In addition, we have also reviewed the reports of another 18 Crohn’s patients with PVG previously described in the English literature. Specifi c predisposing factors for PVG were identified in 8 patients, including barium enema, colonoscopy, blunt abdominal trauma, and enterovenous f istula. The patients who developed PVG following barium enema and blunt trauma were all asymptomatic and no specifi c treatment was necessary. Eleven patients (58%) who presented with signs of intraabdominal catastrophe or systemic toxicity required either immediate or eventual surgery. The overall mortality rate among the 19 patients was only 11%. The present literature review has shown that the f inding of PVG associated with Crohn’ s disease does not always mandate surgical intervention. It is the clinical features and the related complications that ultimately determine the treatment approaches. The overall outcome of PVG associated with Crohn’s disease has been favourable.
