Prevalence of Coinfection Malaria-Covid-19 at the International Hospital Center of Kinshasa during the 3^rd Wave of the Pandemic

Background: Since 2019, Covid-19 pandemic has afflicted the world and countries of Africa. Despite the limited resources, these countries already disturbed by multiple diseases that have not yet been controlled such as malaria, must face this pandemic whose success in the management depends on the early detection of the disease. The objective of this study was to determine the prevalence of Malaria-Covid-19 coinfection in our environment. Methods: This was a retrospective analysis of patients’ data with Covid-19 infection from May to July 2021 at the International Hospital center of Kinshasa “CHIK”. We collected data and analysis was performed on the sociodemographic parameters, the notion of anticovid-19 vaccination as well as the duration of the symptomatology before the consultation, the clinical manifestations and the laboratory data available while including the data of the thick drop. Results: A total of 84 patients were registered with an average age of 35.23 ± 12.74 years. The male sex was predominant (82.1%). The Indian community was the most affected (44.2%). The average of days elapsed before the consultation of 3.63 days. The anti-Covid-19 vaccination rate was 20.3%. The prevalence of Malaria-Covid-19 coinfection was 29.76%. In coinfected patients, fever and cough were more reported (64%). Regarding biological and inflammatory parameters, 31.8% of coinfected patients had a platelet count less than 150,000 elements/mm3 compared to 11.6% in non-Co-infected (p = 0.046). Conclusion: The Malaria-Covid-19 comorbidity prevalence is high in Malaria endemic country like Democratic Republic of Congo (DRC). It is necessary to make better distinction, to detect early the comorbidity in order to better guide care and not be limited to treating malaria, letting the Covid-19 evolve.

Introduction: The DRC had planned 23 mass distribution campaigns distribution of long-lasting insecticidal nets (LLIN) for the years 2018-2020, the implementation of which spanned from 2018 to July 2021. This article reviews the campaign’s planning process, the results, challenges, and lessons learned. Methods: A descriptive method was used to postpone the planning and implementation process according to the National Malaria Control Program (NMCP) standards adapted to the COVID-19 context. The changes and adaptations implemented as well as the challenges encountered are described. Results: Between January 2018 to June 2021, 23 LLIN mass distribution campaigns were organized in the DRC with the financial support of The Global Fund to Fight against Tuberculosis, AIDS and Malaria (GFTAM) and Against Malaria Foundation (AMF) allowing the distribution of 55,273,473 LLINs to 19,048,372 households at risk of malaria transmission with an average of 2.9 LLINs per household. The enumerated population (111,081,191) exceeded 7% of the micro plans projected population (102,790,391) while the number of households enumerated (19,311,629) was 3% lower compared to the micro planning projection households (19,862,417). Compared to a reported household coverage of 96% of households achieved over the expected households, the independent monitoring carried out revealed 91% of households served in the intervention areas. The main reasons for not reaching households mentioned by the respondents were absence at the time of distribution (26%) followed by the loss of vouchers (16%). Several communication channels were used among which, community workers were the most frequently mentioned (63.1%), followed by radios (18.5%) and churches (12.4%). Conclusion: Good planning, effective coordination of stakeholders, and revision of the implementing campaigns methods following the COVID-19 were factors in the success of this campaign. Community-Based Pregnant Women Initiative to Support Emergency Obstetric Care in Kimpese Health Zone, Province of Kogo Central, the Democratic Republic of the Congo

Background: Maternal mortality remains high in low- and middle-income countries. Many maternal deaths occur within hospitals often due to a lack of access to basic emergency obstetric care (Emoc). Aim: The study aimed to assess the effectiveness of a community-based pregnant women initiative to support emergency obstetric care in a Kimpese Health Zone (HZ). Setting: Kimpese Health Zone, Province of Central Kongo, in the South Western of the Democratic Republic of the Congo. Results: Four hundred and thirty-five women were received for Emoc between January 2008 and June 2011, 75% were aged between 18 - 34 years old. Mechanical dystocia was the first cause of emergencies (34%). The diagnosis concordance between the transfers and the reasons for the transfer was weak and not statistically significant (Kappa = 0.350, p = 0.405). After the évaluation of the newborns, a favorable outcome was noted after the 10th day of de follow-up (95.4%). In multivariate analysis, the lack of ANC visits, the waiting time > 1 hour before the visit at the referral hospital, and assisted birth were the factors associated with the unfavorable obstetric outcomes. Conclusion: Improvement in maternal health involves better preparation of the community and the healthcare system for Emergency Obstetric Care. Research is needed for innovative and effective interventions in resource constraints settings.

Family Planning: Choosing Contraceptive Methods in Kongo Central Province, the Democratic Republic of the Congo

Background: In developing countries, access to modern contraceptive methods remains a concern, particularly in Sub-Saharan Africa (SSA). Although satisfactory progress in increasing family planning use has been observed worldwide, demand and unmet need continue to grow. This study describes the different reasons that push women in rural and urban areas of Kongo Central province in the Democratic Republic of Congo (DRC) to choose one of the modern contraceptive methods. Methods: A cross-sectional, analytical study on the 705 clients recruited during the family planning mini-campaigns organized from January to March 2021 in 8 health zones of the Kongo Central Province in DRC. Community Contraceptive Methods Distributors (CCMD) collected the data, community health workers (CHW) trained in clinical providers’ community family planning service. The Pearson’s Chi2 test was used to compare the proportions of the methods chosen in the different study variables categories, and logistic regression was done to analyze the strength of association between the uses of long-acting reversible contraceptives (LARC) with the study variables. Results: The mean age (SD) of the clients was 25.7 years. The median number of living children was 2 children and that of pregnancies carried per client was 3. Three (3) clients under the age of 20 out of 126 (2.4%) have 3 to 4 living children and one a up to 6 living children. Out of a total of 705 clients, 11.1% aged under 20 and 39.1% of clients aged 20 to 24 requested LARC. Divorced and unmarried women chose SAC and married and common-law women chose LARC more, compared to other categories of women. Unemployed women prefer SACs more than other professional categories, and women farmers prefer LARC. The level of education has no influence on the choice of method. The use of LARC increased with the number of pregnancies carried, children alive, and children who died. The age and number of children who died did not influence the assoc

Epidemiology of Cancer in Rural Congo: Case of IME Kimpese Hospital, Democratic Republic of Congo

Background and Aim: Cancer is a public health problem in developing countries. It is aggravated by diagnostic confirmation difficulties. The aim of this study was to determine the epidemiological profile of cancers in Congolese rural areas. Methods: A historical cohort study was conducted with 914 histopathological protocols at the IME/Kimpese hospital from January 2008 to December 2013. Results: The mean age of patients was 55.2 ± 14.5 years, and 61.9% were women with a sex ratio of 2W/M. Ductal carcinoma (23.6%), squamous cell carcinoma (23.5%) and adenocarcinoma (23.1%) were the most common cancers. The most affected organs were breast (26.3%) and prostate (14.3%). 41.5% of cancers had an undifferentiated character and more than half (51.1%) a differentiated character. The majority of this cancer infiltrated other organs (57.9%). Among cancers where grade was found, 16% and 10.8% were respectively grades 1 and 4, and the presence of metastases was observed in 10.9% of cancers. Conclusion: Cancer is more frequent in elderly women with a predominance of squamous and ductal carcinoma. The presence of a national cancer registry is necessary in the country for a good follow-up of cancer cases.
