Sporadic versus hereditary gastrinomas of the duodenum and pancreas: Distinct clinico-pathological and epidemiological features

Gastrinomas are defined as gastrin secreting tumors that are associated with Zollinger-Ellison syndrome （ZES）. ZES is characterized by elevated fasting gastrin serum levels, positive secretin stimulation test and clinical symptoms such as recurrent peptic ulcer disease, gastroesophageal reflux disease and occasional diarrhea. Genetically, nonhereditary （sporadic） gastrinomas are distinguished from hereditary gastrinomas, which are associated with multiple endocrine neoplasia type 1 （MEN1） syndrome. In general, duodenal gastrinomas are small and solitary if they are sporadic and multiple as well as hereditary. The sporadic gastrinomas occur in the duodenum or in the pancreas while the hereditary gastrinomas almost all occur in the duodenum. Our series of 77 sporadic duodenal neuroendocrine tumors （NETs） includes 18 patients （23.4%） with gastrinomas and ZES. Of 535 sporadic NETs in the pancreas collected from the NET archives of the departments of pathology in Zürich, Switzerland, and Kiel, Germany, 24 patients （4.5%） suffered from sporadic pancreatic gastrinomas and ZES. These NETs have to be distinguished from tumors with immunohistochemical positivity for gastrin but without evidence of ZES. An additional 19 patients suffered from MEN1 and ZES. These patients showed exclusively duodenal gastrinomas, but not pancreatic gastrinomas. The prognosis of sporadic and MEN1- associated duodenal gastrinomas is better than that of pancreatic gastrinomas, since they progress slowly to liver metastasis. In summary, sporadic and MEN1- associated gastrinomas in the duodenum and pancreas show different clinico-pathological and genetic features. The incidence of sporadic duodenal gastrin-producing tumors is increasing, possibly due to optimized diagnostic procedures. In contrast, pancreatic MEN1- associated gastrinomas seem to be extremely rare. Early Identification of Stable Transformation Events by Combined use of Antibiotic Selection and Vital Detection of Green Fluorescent Protein (GFP) in Carrot (Daucus carota L.) Callus

Genetic transformation is a useful technique to complement conventional breeding in crop improvement. Although carrot has been a model organism for in vitro embryogenesis study, genetic transformation of carrot is still lengthy and labor intensive. An efficient transformation and detection system is desirable. Direct infection of Agrobacterium to carrot calli has provided an easy way for carrot genetic transformation. To improve the efficiency of antibiotic selection in this method, we report the combined use of an improved green-fluorescent protein, referred to as smGFP, to establish a versatile selection method for carrot callus transformation system. By combining antibiotic selection with the bright fluorescence observed in the callus tissue, we were able to easily identify stable transformants in early stage of the transformation process. In addition to the GFP expression of the callus cells, the transgenic nature of callus cells was confirmed with Southern and Western analysis. We found we can link the simplicity of carrot-callus-cell transformation, early detection of stable transformants with antibiotic selection, visualization of GFP fluorescence, and molecular analysis （Southern and Western） of callus tissue （non-photosynthetic tissue） to provide a more efficient way in identifying stable transformants at early stage of carrot transformation.

Clinical outcome and predictors of survival after TIPS insertion in patients with liver cirrhosis

AIM： To determine the clinical outcome and predictors of survival after transjugular intrahepatic portosystemic stent shunt （TIPS） implantation in cirrhotic patients. METHODS： Eighty-one patients with liver cirrhosis and consequential portal hypertension had TIPS implanta- tion （bare metal） for either refractory ascites （RA）（n = 27） or variceal bleeding （VB）（n = 54）. Endpoints for the study were： technical success, stent occlusion and stent stenosis, rebleeding, RA and mortality. Clini- cal records of patients were collected and analysed. Baseline characteristics [e.g., age, sex, CHILD score and the model for end-stage liver disease score （MELD score）, underlying disease] were retrieved. The Kaplan- Meier method was employed to calculate survival from the time of TIPS implantation and comparisons were made by log rank test. A multivariate analysis of factors influencing survival was carried out using the Cox pro- portional hazards regression model. Results were ex- pressed as medians and ranges. Comparisons between groups were performed by using the Mann-Whitney u-test and the4,2 test as appropriate. RESuLTS： No difference could be seen in terms of age, sex, underlying disease or degree of portal pressure gradient （PPG） reduction between the ascites and the bleeding group. The PPG significantly decreased from 23.4 ± 5.3 mmHg （VB） vs 22.1± 5.5 mmHg （RA） be- fore TIPS to 11.8 ±4.0 vs 11.7 ± 4.2 after TIPS im- plantation （P = 0.001 within each group）. There was a tendency towards more patients with stage CHILD A in the bleeding group compared to the ascites group （24 vs 6, P = 0.052）. The median survival for the ascites group was 29 mo compared to 〉 60 mo for the bleed- ing group （P = 0.009）. The number of radiological con- trols for stent patency was 6.3 for bleeders and 3.8 for ascites patients （P = 0.029）. Prediction of indoor airflow distribution often relies on high-fidelity,computationally intensive computational fluid dynamics(CFD)*** intelligence(Al)models trained by CFD data can be used for fast and accurate prediction of indoor airflow,but current methods have limitations,such as only predicting limited outputs rather than the entire flow ***,conventional Al models are not always designed to predict different outputs based on a continuous input range,and instead make predictions for one or a few discrete *** work addresses these gaps using a conditional generative adversarial network(CGAN)model approach,which is inspired by current state-of-the-art Al for synthetic image *** create a new Boundary Condition CGAN(BC-CGAN)model by extending the original CGAN model to generate 2D airflow distribution images based on a continuous input parameter,such as a boundary ***,we design a novel feature-driven algorithm to strategically generate training data,with the goal of minimizing the amount of computationally expensive data while ensuring training quality of the Al *** BC-CGAN model is evaluated for two benchmark airflow cases:an isothermal lid-driven cavity flow and a non-isothermal mixed convection flow with a heated *** also investigate the performance of the BC-CGAN models when training is stopped based on different levels of validation error *** results show that the trained BC-CGAN model can predict the 2D distribution of velocity and temperature with less than 5%relative error and up to about 75,ooo times faster when compared to reference CFD *** proposed feature-driven algorithm shows potential for reducing the amount of data and epochs required to train the Al models while maintaining prediction accuracy,particularly when the flow changes non-linearlywith respectto an input.

Impact of field strength and RF excitation on abdominal diffusion-weighted magnetic resonance imaging

AIM:To retrospectively and prospectively compare diffusion-weighted(DW)images in the abdomen in a1.5T system and 3.0T systems with and without twochannel functionality for ***:DW images of the abdomen were obtained on 1.5T and 3.0T(with and without two-channel functionality for B1shimming)scanners on 150 patients(retrospective study population)and 10 volunteers(prospective study population).Eight regions were selected for clinical significance or artifact susceptibility(at higher field strengths).Objective grading quantified signal-to-noise ratio(SNR),and subjective evaluation qualified image quality,ghosting artifacts,anddiagnostic *** significance was calculated usingχ2tests(categorical variables)and independent two-sided t tests or Mann-Whitney u tests(continuous variables).RESuLTS:The 3.0T using dual-source parallel transmit(dpTX 3.0T)provided the significantly highest SNRs in nearly all *** regions susceptible to artifacts at higher field strengths(left lobe of liver,head of pancreas),the SNR was better or similar to the 1.5T ***,both dpTX 3.0T and 1.5T systems provided higher image quality,diagnostic value,and less ghosting artifact(Pues)compared to the 3.0T system without dual-source parallel transmit(non-dpTX 3.0T).CONCLuSION:The dpTX 3.0T scanner provided the highest *** image quality,lack of ghosting,and diagnostic value were equal to or outperformed most currently used systems.
