We present a device for passive unidirectional liquid transport. The capillary channels used are bioinspired by the shape of the spermathecae （receptaculum seminis） of rabbit fleas （Spilopsyllus cuniculi） and rat fleas （Xenopsytla cheopis）. The spermatheca is an organ of female fleas that stores sperm until suitable conditions to lay eggs are found. We translated and multiplied the natural form and function of a spermatheca to create a continuous capillary system from which we designed our microfluidic device based directly on the model from nature. Applying the Young-Laplace equation, we derived a theoretical description of local liquid transport, which enables model-guided design. We arranged the bioinspired capillaries in parallel and engraved them in poly（methyl methacrylate）（PMMA） plates by CO2 laser ablation. The fabricated structures transport soapy water passively （i.e., without external energy input） in the forward direction at velocities of about 1 mm＇s i while halting the liquid fronts completely in the backward direction. The bio- inspired capillary channels are capable of unidirectional liquid transport against gravity. Distance and velocity measurements prove the feasibility of the concept. Unidirectional passive liquid transport might be advantageous in technical surfaces for liquid man- agement, for instance, in biomedical microfluidics, lab-on-chip, lubrication, electronics cooling and in micro-analysis devices.

BACKGROUND The introduction of fine needle biopsies(FNB)to clinical practice presents a changing trend towards histology in the endoscopic ultrasound-guided tissue acquisition(EUS-TA).AIM To evaluate the clinical performance of a new FNB needle,the 22-gauge(22G)Franseen needle,when sampling pancreatic solid lesions.METHODS Consecutive patients with an indication for EUS-TA for the assessment of pancreatic solid lesions were included in this prospective,single-center,singlearm trial.Each patient underwent a puncture of the lesion two times using the 22G Franseen needle and the obtained samples were directly placed into formalin for histological analysis.The primary study endpoint was the rate of high-quality obtained specimen.Secondary endpoints included the length and diameter of the core specimen,the diagnostic accuracy and the complication rate.RESULTS From June 2017 to December 2018,forty patients with pancreatic solid lesions(22 females;mean age 67.2 years)were enrolled.Tissue acquisition was achieved in all cases.High-quality histology,rated with Payne score 3,was obtained in 37/40 cases(92.5%)after two needle passes.The mean size of the acquired histological core tissue was 1.54 mm×0.39 mm.The diagnostic accuracy for the correct diagnosis was 85%(34/40).Only one adverse event was occurred,consisting of a self-limiting bleeding in the puncture site.CONCLUSION The 22G Franseen needle achieved according to our standardized protocol a high rate of histological core procurement,and a high diagnostic accuracy,with one minor adverse event reported.

BACKGROUND Liver transplantation(LT)presents a curative treatment option in patients with early stage hepatocellular carcinoma(HCC)who are not eligible for resection or ablation therapy.Due to a risk of up 30%for waitlist drop-out upon tumor progression,bridging therapies are used to halt tumor growth.Transarterial chemoembolization(TACE)and less commonly stereotactic body radiation therapy(SBRT)or a combination of TACE and SBRT,are used as bridging therapies in LT.However,it remains unclear if one of those treatment options is superior.The analysis of explant livers after transplantation provides the unique opportunity to investigate treatment response by histopathology.AIM To analyze histopathological response to a combination of TACE and SBRT in HCC in comparison to TACE or SBRT alone.METHODS In this multicenter retrospective study,27 patients who received liver transplantation for HCC were analyzed.Patients received either TACE or SBRT alone,or a combination of TACE and SBRT as bridging therapy to liver transplantation.Liver explants of all patients who received at least one TACE and/or SBRT were analyzed for the presence of residual vital tumor tissue by histopathology to assess differences in treatment response to bridging therapies.Statistical analysis was performed using Fisher-Freeman-Halton exact test,Kruskal-Wallis and Mann-Whitney-U tests.RESULTS Fourteen patients received TACE only,four patients SBRT only,and nine patients a combination therapy of TACE and SBRT.There were no significant differences between groups regarding age,sex,etiology of underlying liver disease or number and size of tumor lesions.Strikingly,analysis of liver explants revealed that almost all patients in the TACE and SBRT combination group(8/9,89%)showed no residual vital tumor tissue by histopathology,whereas TACE or SBRT alone resulted in significantly lower rates of complete histopathological response(0/14,0%and 1/4,25%,respectively,P value<0.001).CONCLUSION Our data suggests that a combination of TACE and SBRT increases the

Metal oxide thin films for soft and flexible electronics require low cost,room temperature fabrication,and structuring processes.We here introduce an anodic printing process to realize the essential building blocks of electronic circuitry,including resistors,capacitors,field-effect transistors,diodes,rectifiers,and memristors directly on imperceptible plastic substrates.Largely independent on surface properties,we achieve high-quality,few nanometer thin dielectric and semiconducting films even on rough substrates via localized anodisation of valve metals using a scanning droplet cell microscope.We demonstrate printing-like fabrication of 3D multilayer solid-state capacitors with a record-high areal capacity of 4μF cm−2.Applicable to the whole class of valve metals and their alloys,our method provides a versatile fabrication technique for the circuits that empower the flexible and stretchable electronics of tomorrow.

拉科酰胺(Lacosamide,LCM)的静脉制剂具有良好的耐受性和安全性,促使其在癫痫持续状态(Status Epilepticus,SE)中得到应用。本项系统评价的目的是确定和评估LCM在SE中的使用情况。在电子数据库中用联合检索方式对2008年—2016年10月的文献进行了系统检索,并使用标准化的评估表格从各项研究中将有关研究设计、方法框架、数据源、有效性和LCM引起的不良反应的信息进行提取并系统地报告。研究共评估了522次SE发作,其中包括486例成人和36例儿童及青少年发作,女性占51.7%。LCM的总体效力为57%。LCM对非惊厥性(57%,82/145)和全面痉挛性(61%,30/49,P=0.68)SE的效力相似,但对局灶运动性SE的总体有效率更高(92%,34/39,P<0.001)。而LCM在用药次序靠后时其疗效从100%下降到20%。治疗期间的主要不良反应是眩晕,视力异常,复视和共济失调。总体而言,LCM具有良好的耐受性,并且没有具临床意义的药物相互作用。现有数据表明LCM在SE中的使用是有前景的,有效率为57%。LCM的优势在于没有潜在的药物相互作用,并且在紧急情况需迅速增加剂量时可静脉使用。
