Background: Post-hepatectomy liver failure(PHLF) is a leading cause of postoperative mortality after liver surgery. Due to its significant impact, it is imperative to understand the risk stratification and preventative strategies for PHLF. The main objective of this review is to highlight the role of these strategies in a timeline centered way around curative resection. Data sources: This review includes studies on both humans and animals, where they addressed PHLF. A literature search was conducted across the Cochrane Library, Embase, MEDLINE/Pub Med, and Web of Knowledge electronic databases for English language studies published between July 1997 and June 2020. Studies presented in other languages were equally considered. The quality of included publications was assessed using Downs and Black’s checklist. The results were presented in qualitative summaries owing to the lack of studies qualifying for quantitative analysis. Results: This systematic review with 245 studies, provides insight into the current prediction, prevention, diagnosis, and management options for PHLF. This review highlighted that liver volume manipulation is the most frequently studied preventive measure against PHLF in clinical practice, with modest improvement in the treatment strategies over the past decade. Conclusions: Remnant liver volume manipulation is the most consistent preventive measure against PHLF.

Background:Parents of children with complex right ventricular outflow tract(RVOT)anomalies are confronted with their child’s need for heart surgery early in life and repeated reoperations later on.Preoperative assessment needs to be performed whenever an indication for reoperation is suspected.The aim was to illuminate the experiences of parents of children diagnosed with RVOT anomalies,in particular,how they experience their child’s heart disease and everyday life during the assessment and after the decision on whether to perform a reoperation.Method:Individual interviews(n=27)were conducted with nine parents on three occasions between 2014 and 2016 and analyzed using reflexive thematic analysis.Results:The analysis resulted in the following five main coexisting themes:The heart surgery keeps my child alive illuminates parents’experiences during and after the assessment and emphasizes that heart surgery,although dreaded,is central for their child’s survival;Everyday struggles illuminates the different struggles parents had to face to ensure that their child would be in the best possible condition;the remaining three themes,Unconditional love,Trust in life,and Togetherness,illuminate the ways in which the parents gained inner strength and confidence in their everyday lives.Conclusion:Although the parents were grateful for the assessment and had learned to navigate among the fears it aroused,they experienced several distressing situations during the assessment process that should be addressed.By inviting both the parents and their child to participate in the child’s care,individualized support can take into account the needs of both parents and child.

A New Fuzzy Adaptive Algorithm to Classify Imbalanced Data

Classification of imbalanced data is a well explored issue in the data mining and machine learning community where one class representation is overwhelmed by other classes.The Imbalanced distribution of data is a natural occurrence in real world datasets,so needed to be dealt with carefully to get important insights.In case of imbalance in data sets,traditional classifiers have to sacrifice their performances,therefore lead to misclassifications.This paper suggests a weighted nearest neighbor approach in a fuzzy manner to deal with this issue.We have adapted the‘existing algorithm modification solution’to learn from imbalanced datasets that classify data without manipulating the natural distribution of data unlike the other popular data balancing methods.The K nearest neighbor is a non-parametric classification method that is mostly used in machine learning problems.Fuzzy classification with the nearest neighbor clears the belonging of an instance to classes and optimal weights with improved nearest neighbor concept helping to correctly classify imbalanced data.The proposed hybrid approach takes care of imbalance nature of data and reduces the inaccuracies appear in applications of original and traditional classifiers.Results show that it performs well over the existing fuzzy nearest neighbor and weighted neighbor strategies for imbalanced learning.

Classification of Soil Types Using Geographic Object-Based Image Analysis and Random Forests

There is increasing interest in developing automatic procedures to segment landscapes into soil spatial entities that replace conventional, expensive manual procedures for delineating and classifying soils. Geographic object-based image analysis(GEOBIA)partitions remote sensing imagery or digital elevation models into homogeneous image objects based on image segmentation. We used an object-based methodology for the detailed delineation and classification of soil types using digital maps of topography and vegetation as soil covariates, based on the Random Forests(RF) classifier. We compared the object-based method's results with those of a pixel-based classification using the same classifier. We used 18 digital elevation model derivatives and 5 remote sensing indices that were related to vegetation cover and soil. Using 171 soil profiles with their associated environmental variable values,the RF method was used to identify the most important soil type predictors for use in the segmentation process. A stack of rastergeodatasets corresponding to the selected predictors was segmented using a multi-resolution segmentation algorithm, which resulted in homogeneous objects related to soil types. These objects were further classified as soil types using the same method, RF. We also conducted a pixel-based classification using the same classifier and soil profiles, and the resulting maps were assessed in terms of their accuracy using 30% of the soil profiles for validation. We found that GEOBIA was an effective method for soil type mapping, and was superior to the pixel-based approach. The optimized object-based soil map had an overall accuracy of 58%, which was 10% higher than that of the optimized pixel-based map.

The term"periodontal disease"refers to a group of chronic inflammatory illnesses caused by specific microorganisms from subgingival biofilm,that affect the toothsupporting tissues.Recent research has also shown that periodontal infection plays a role in aggravating systemic disease states at distal sites,reinforcing the significance of the oral cavity for general health.Additionally,it has been suggested that gastroenterological malignancies may be promoted by hematogenous,enteral or lymphatic translocation of periopathogens.In the past 25 years,the global burden of pancreatic cancer(PC)has more than doubled,making it one of the major causes of cancer-related mortality.Periodontitis has been linked to at least 50%increased risk of PC and it could be considered a risk factor for this malignancy.A recent study performed on 59000 African American women with a follow up of 21 years showed that participants who had poor dental health had higher chances of PC.The findings,according to researchers,might be related to the inflammation that some oral bacteria trigger.Regarding the mortality of PC,periodontitis considerably raises the chance of dying from PC.Microbiome alterations in the gut,oral cavity and pancreatic tissues of PC patients occur when compared to healthy flora,demonstrating a link between PC and microecology.Inflammation may also contribute to PC development,although the underlying pathway is not yet known.The function of the microbiome in PC risk has drawn more focus over the last decade.Future risk of PC has been linked to the oral microbiome,specifically increased levels of Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans and decreased relative abundance of Leptotrichia and Fusobacteria,suggesting that it may have an impact on the inflammatory condition by expanding,altering,and regulating the commensal microbiome.Patients who received periodontal treatment had significantly decreased incidence rate ratios for PC.By analyzing patterns in the microbiome composition throughout PC develop

In the past years smart homes solutions have become more and more popular with the introduction of a high number of both Internet of Things(IoT)applications and smart devices.Home automation and security systems market is in a continuous growth,traditional security systems are evolving fast,and more and more people choose Smart home solutions.In this paper,we propose two IoT based systems in the context of Smart homes:qToggle for multiple home automation,and MotionEyeOS,a video surveillance OS for single-board computers.Most qToggle devices are based on ESP8266/ESP8285 chips or on Raspberry Pi boards and smart sensors,while MotionEye uses Raspberry Pi boards.

Test Time Minimization for Hybrid BIST of Core-Based Systems

This paper presents a solution to the test time minimization problem for core-based systems. We assume a hybrid BIST approach, where a test set is assembled, for each core, from pseudorandom test patterns that are generated online, and deterministic test patterns that are generated off-line and stored in the system. In this paper we propose an iterative algorithm to find the optimal combination of pseudorandom and deterministic test sets of the whole system, consisting of multiple cores, under given memory constraints, so that the total test time is minimized. Our approach cmploys a fast estimation methodology in order to avoid exhaustive search and to speed-up the calculation process. Experimental results have shown the efficiency of the algorithm to find near optimal solutions.

We read with great interest the recently published article of Wong et al.(1).Through their study,the authors sought to evaluate the efficacy and safety of stereotactic body radiation therapy(SBRT)in patients presenting with hepatocellular carcinoma(HCC),awaiting liver transplantation(LT).This is the first prospective study to analyze SBRT as a bridging treatment before LT and compare it to other bridging therapies such as transarterial chemoembolization(TACE)and high intensity focused ultrasound(HIFU).

New adso rbent material was obtained by modification of commercial Amberlite XAD7 with thiourea that represents a non-toxic,cheap and environmentally friendly extractant.Prepared adsorbent was used for removal of neodymium ions from aqueous solutions.Thiourea modified Amberlite involved in this study shows good adsorption capacities(74.3 mg/g) and excellent efficiency during Nd removal process.In order to elucidate the mechanism of the Nd adsorption process kinetic,thermodynamic and equilibrium studies were performed,establishing this way which kinetic model better describes the Nd adsorption process.Moreover the thermodynamic studies prove that the Nd adsorption on thiourea modified Amberlite XAD7 is an endothermic and spontaneous process.
