Viewpoints of the target population regarding barriers and facilitators of colorectal cancer screening in the Czech Republic

BACKGROUND Public awareness of colorectal cancer(CRC) and uptake of CRC screening remain challenges. The viewpoints of the target population(asymptomatic individuals older than 50) regarding CRC screening information sources and the reasons for and against participation in CRC screening are not well known in the Czech Republic. This study aimed to acquire independent opinions from the target population independently on the health *** To investigate the viewpoints of the target population regarding the source of information for and barriers and facilitators of CRC *** A survey among relatives(aged 50 and older) of university students was conducted. Participants answered a questionnaire about sources of awareness regarding CRC screening, reasons for and against participation, and suggestions for improvements in CRC screening. The effect of certain variables on participation in CRC screening was *** Of 498 participants, 478(96%) respondents had some information about CRC screening and 375(75.3%) had participated in a CRC screening test. General practitioners(GPs)(n = 319, 64.1%) and traditional media(n = 166, 33.3%) were the most common information sources regarding CRC screening. A lack of interest or time and a fear of colonoscopy or positive results were reported as reasons for non-participation. Individuals aged > 60 years [adjusted odds ratio(aOR) = 2.30, 95% confidence interval(CI)(1.42-3.71), P = 0.001], females(aOR =1.95, 95%CI(1.26-3.01) P = 0.003), and relatives of CRC patients(aOR = 4.17,95%CI(1.82-9.58) P = 0.001) were more likely to participate in *** regarding screening provided by physicians-GPs:(aOR = 8.11,95%CI(4.90-13.41), P < 0.001) and other specialists(aOR = 4.19, 95%CI(1.87-9.38),P = 0.001) increased participation in screening. Three major cardiovascular outcome trials(CVOTs)with a new class of antidiabetic drugs-sodium-glucose cotransporter 2(SGLT2)inhibitors(EMPAREG OUTCOME trial with empagliflozin,CANVAS Program with canagliflozin,DECLARE-TIMI 58 with dapagliflozin)unexpectedly showed that cardiovascular outcomes could be improved possibly due to a reduction in heart failure risk,which seems to be the most sensitive outcome of SGLT2 *** other CVOT to date has shown any significant benefit on heart failure *** more impressive findings came recently from the DAPA-HF trial in patients with confirmed and well-treated heart failure:Dapagliflozin was shown to reduce heart failure risk for patients with heart failure with reduced ejection fraction regardless of diabetes ***,despite their possible wide clinical implications,there is much doubt about the mechanisms of action and a lot of questions to unravel,especially now when their benefits translated to nondiabetic patients,rising doubts about the validity of some current mechanistic *** time frame of their cardiovascular benefits excludes glucoselowering and antiatherosclerotic-mediated effects and multiple other mechanisms,direct cardiac as well as systemic,are suggested to explain their early cardiorenal *** are:Anti-inflammatory,antifibrotic,antioxidative,antiapoptotic properties,then renoprotective and hemodynamic effects,attenuation of glucotoxicity,reduction of uric acid levels and epicardial adipose tissue,modification of neurohumoral system and cardiac fuel energetics,sodiumhydrogen exchange *** most logic explanation seems that SGLT2 inhibitors timely target various mechanisms underpinning heart failure *** the proposed mechanisms of their action could interfere with evolution of heart failure and are discussed separately within the main text.

Influence of COM-peptides/proteins on the properties of flocs formed at different shear rates

Coagulation followed by floc separation is a key process for the removal of algal organic matter(AOM) in water treatment. Besides optimizing coagulation parameters,knowledge of the properties of AOM-flocs is essential to maximizing AOM ***, the impact of AOM on the floc properties remains unclear. This study investigated how peptides/proteins derived from the cellular organic matter(COM) of the cyanobacterium Microcystis aeruginosa influenced the size, structure, and shape of flocs formed at different shear rates(G). Flocs formed by kaolinite, COM-peptides/proteins and a mixture of the same were studied, and the effect of intermolecular interactions between floc components on floc properties was assessed. The coagulation experiments were performed in a Taylor–Couette reactor, with aluminum(Al) or ferric sulphate(Fe) utilized as coagulants. Image analysis was performed to gauge floc size and obtain data on fractal dimension. It was found that floc properties were affected by the presence of the COM-peptides/proteins and the coagulant used. COM-peptides/proteins increased floc size and porosity and widened floc size distributions. The Fe coagulant produced larger and less compact flocs than Al coagulant. Moreover, the decrease in floc size that occurred in parallel with increase in shear rate was not smooth in progress. A rapid change for the kaolinite-coagulant suspension and two rapid changes for the suspensions containing COM were observed. These were attributed to various intermolecular interactions between floc components participating in coagulation at different G. Based on the results obtained, shear rates suitable for efficient separation of flocs containing COM were suggested.

AIM To determine factors correlated with postoperative radial shortening in patients with distal radius fractures treated with volar locking distal radius *** A total of 250 patients with a distal radius fracture stabilised with volar locking plates between January 2010 and December 2014 were included in a multicentre retrospective cohort study. We measured the distance of the distal locking screws to the joint line immediately postoperatively and then measured radial shortening after six to eight weeks using the change in ulnar *** Multivariate linear regression analysis showed that there was a significant linear association between the distance of the screws from the joint line and radial shortening. No other patient, injury, or treatment-related characteristic significantly influenced radial shortening in multivariate *** Distal locking screws should be placed as close as possible to the subchondral joint line to prevent postoperative loss of reduction.

BACKGROUND Muscle growth promoters are being developed for the treatment of diseaseinduced loss of muscle *** and ostarine are selective androgen receptor modulators(SARMs)with a non-steroidal structure and a presumably more favorable side effect *** recent years,these substances with or without“post-cycle therapy”(PCT)are often misused by amateur athletes aiming to promote muscle *** the same time,reports on their toxic effects on organ systems are *** SUMMARY We report two cases of liver injury in young men who used ligandrol and/or ostarine for a few weeks followed by the use of substances for *** liver injury occurred in both cases after stopping SARMs while on *** clinical picture was dominated by jaundice and *** biochemical pattern showed a mixed type of injury with normal alkaline phosphatase and high concentrations of bilirubin and serum bile acids. Histological evidence showed predominantlycholestatic injury with canalicular bile plugs, ductopenia, and mild hepatocellulardamage without significant fibrosis. The patients recovered from the conditionafter 3 mo. The off target effects of SARMs were likely idiosyncratic, but ourreport highlights the yet unrecognized effects of other toxic substances used forPCT, supra-therapeutic doses, and the complete absence of monitoring foradverse *** muscle-building amateur athletes, SARMs (ligandrol or ostarine) and/orsubstances in PCT may cause cholestatic liver injury with prolonged recovery.

Ustekinumab: “Real-world” outcomes and potential predictors of nonresponse in treatment-refractory Crohn’s disease

BACKGROUND Ustekinumab was approved in Europe for the treatment of adults with moderate to severe Crohn's disease(CD)in 2016,and there is an urgent need for data on its everyday *** To obtain data on the daily use of *** This is a retrospective monocentric *** with moderate to severe CD who began ustekinumab therapy at the inflammatory bowel diseases outpatient clinic of the Heidelberg University Hospital between December 2016 and March 2018 were selected based on electronic patient *** primary study endpoint was combined steroid-free clinical remission or steroid-free clinical response at 24±6 wk of ustekinumab *** study endpoints were:achievement of mucosal healing,sonographic and magnetic resonance imaging response,biochemical response,the need for intestinal surgery within 24±6 wk after treatment initiation,the occurrence of adverse events,treatment discontinuation due to nonresponse or adverse events,improvement of extraintestinal manifestations,clinical response at 48±6 wk of therapy,and association of response with nucleotid oligodimerisation domain 2 *** Fifty-seven patients with CD(5.3%anti-tumour necrosis factorαnaive,63.2%having undergone at least one intestinal surgery)were included in the *** patients(35.1%)achieved steroid-free clinical remission,6(10.5%)steroid-free clinical response and 31(54.4%)were *** discontinuation due to adverse events occurred in two patients(3.5%).Male sex,the presence of extraintestinal manifestations and the use of steroids at baseline were predictors of nonresponse to ustekinumab *** In a“real-world”treatment-refractory cohort of patients with CD,ustekinumab appeared efficacious and safe.

H pylori and gastric cancer: Shifting the global burden

Infection with H pylori leads to a persistent chronic inflammation of the gastric mucosa, thereby increasing the risk of distal gastric adenocarcinoma. Numerous studies have determined a clear correlation between Hpylori infection and the risk of gastric cancer; however, general eradication is not recommended as cancer prophylaxis and time points for treatment remain controversial in different areas of the world. Prevalence rates in Western countries are decreasing, especially in younger people （〈 10%）; and a decline in distal gastric adenocarcinoma has been observed. Risk groups in Western countries still show considerably higher risk of developing cancer, especially in patients infected with cagA^＋ strains and in persons harboring genetic polymorphism of the IL-1B promoter （-511T/T） and the corresponding IL-1 receptor antagonist （IL-1RN^＊2）. Thus, general eradication of all infected persons in Western countries not recommended and is limited to risk groups in order to achieve a risk reduction. In contrast, infection rates and cancer prevalence are still high in East Asian countries. A prevention strategy to treat infected persons may avoid the development of gastric cancer to a large extent and with enormous clinical importance. However, studies in China and Japan indicate that prevention of gastric cancer is effective only in those patients that do not display severe histological changes such as atrophy and intestinal metaplasia. Thus, prophylactic strategies to prevent gastric cancer in high risk populations such as China should therefore especially aim at individuals now at younger age when the histological alterations caused by the bacterial infection was still reversible. In countries with a low prevalence of gastric cancer, risk groups carrying cagA^＋ strains and IL-1 genetic polymorphisms should be identified and treated.

Design of molecule-based magnetic conductors

Enabling the use of rationally designed thin films in technological devices is a recognized goal in materials science. However, constructing such thin films using highly ordered supramolecular architectures with well-controlled size and growth direction has remained an elusive target. Here, we introduce a layer-by- layer protocol to grow hybrid thin films of molecule-based magnetic conductors comprising arachidic acid and donor bis（ethylenedioxy）tetrathiafulvalene （BEDO-TTF） as the organic component and Cu/Gd complexes as the inorganic component. The construction of layered hybrid thin films was achieved at ambient conditions by employing the Langmuir-Blodgett method, which provides good control over film thickness and packing of molecules in the monolayer. As demonstrated by X-ray diffraction, these films are crystalline with distinct organic and inorganic sublattices, where the BEDO-TTF molecular layer is interfaced with the inorganic layer. Due to the flexibility of the Langmuir-Blodgett deposition technique, this result indicates a route toward the preparation of well-ordered films with various functionalities, determined by the choice of the inorganic compound that is combined with the π-unit of BEDO-TFF. Moreover, the ability to deposit films on a variety of substrates establishes the potential for lower-cost device fabrication on inexpensive substrates.
