Role of digital single-operator cholangioscopy in the diagnosis and treatment of biliary disorders

Due to the need for improvement in the diagnosis and minimally invasive therapy of the bile duct disorders new technologies for cholangioscopy have been recently developed. Per-oral cholangioscopy has become an important diagnostic and therapeutic tool leading to avoidance of aggressive and unnecessary surgery in many clinical scenarios. This paper focuses on the newly developed SpyGlass DS technology, its advantages, and the technique of single-operator cholangioscopy(SOC), biliary indications and possible adverse events. We also review the available literature; discuss the limitations and future *** SOC(D-SOC) is a useful technique, which provides endoscopic imaging of the biliary tree, optical diagnosis, biopsy under direct vision and therapeutic interventions. The implementations are diagnostic and therapeutic. Diagnostic indications are indeterminate biliary strictures, unclear filling defects, staging of cholangiocarcinoma, staging of ampullary tumors(extension into the common bile duct), unclear bile duct dilation, exploring cystic lesions of the biliary tree,unexplained hemobilia, posttransplant biliary complications. Therapeutic indications are lithotripsy of difficult stones, retrieval of migrated stents, foreign body removal, guide wire placement, transpapillary gallbladder drainage and endoscopic tumor ablative therapy. Most studied and established indications are the diagnosis of indeterminate biliary stricture and intraductal lithotripsy of difficult stones. The adverse events are not different and more common compared to those of Endoscopic retrograde cholangiopancreatography(ERCP)alone. D-SOC is a safe and effective procedure, adjunct to the standard ERCP and the newly available digital technology overcomes many of the limitations of the previous generations of cholangioscopes.

Endoscopic retrograde cholangiopancreatography(ERCP)with stenting is the treatment modality of choice for patients with benign and malignant bile duct *** could fail in cases of duodenal obstruction,duodenal diverticulum,ampullary neoplastic infiltration or surgically altered *** these cases percutaneous biliary drainage(PTBD)is traditionally used as a rescue procedure but is related to high morbidity and mortality and lower quality of *** ultrasound-guided biliary drainage(EUS-BD)is a relatively new interventional procedure that arose due to the development of curvilinear echoendoscope and the various endoscopic devices.A large amount of data is already collected that proves its efficacy,safety and ability to replace PTBD in cases of ERCP *** is also possible that EUS-BD could be chosen as a first-line treatment option in some clinical scenarios in the near *** EUS-BD techniques are developed EUS-guided transmural stenting,antegrade stenting and rendezvous technique and can be personalized depending on the individual ***-BD is normally performed in the same session from the same endoscopist in case of ERCP *** lack of training,absence of enough dedicated devices and lack of standardization still makes EUS-BD a difficult and not very popular procedure,which is related to life-threatening adverse *** training models,dedicated devices and guidelines hopefully will make EUS-BD easier,safer and well accepted in the *** paper focuses on the technical aspects of the different EUS-BD procedures,available literature data,advantages,negative aspects and the future perspectives of these modalities.

Cryptocurrency, as an emerging social phenomenon, has attracted great research attentions and starts to form a new research area in the past decade. A thorough review of the existing cryptocurrency literatures helps us to understand the status-quo and the promising research future of this emerging field. This paper provides a systematic review of the cryptocurrency literatures by employing bibliometrics and network analysis methods. A total of 833 publications from Web of Science Core Collection and Scopus have been collected in the time period of January 2008 to June 2018. We examine the descriptive features, countries’ contributions and collaboration networks, author collaboration networks and reference co-citation networks to identify actors with significant influence in this field. The results show that cryptocurrency research is experiencing exponential growth, but scientific collaborations between different countries and authors are still insufficient. The intellectual bases of the cryptocurrency domain rest on the early-year literatures that provide fundamental understandings for cryptocurrency, and that bridge different research fields. The analysis indicates that the current research trend focuses on cryptocurrency market analysis incorporating market behaviors and trading characteristics.

This paper introduces a technique for searching for bright massive stars in galaxies beyond the Local *** search for massive stars,we processed the results of stellar photometry from the Hubble Space Telescope(HST)images using the DAOPHOT and DOLPHOT *** results of such searches are demonstrated with examples of the galaxies DDO 68,M94 and NGC *** the galaxy DDO 68,the LBV star changes its brightness,and massive stars in M94 can be identified by excess in the Ha *** the galaxy NGC 1672,we measure the distance for the first time by the TRGB method,which enabled determining the luminosities of the brightest stars,likely hypergiants,in the young star formation *** far,we have performed stellar photometry on HST images of 320 northern sky galaxies located at a distance less than 12 *** allowed us to identify 53 galaxies with probable *** photometric and spectral observations of these galaxies are planned to search for massive stars.

The interaction of the structure-directing agent with the zeolite framework determines germanium distribution in SCM-15 germanosilicate

We report results from computational modeling of the relative stability of germanosilicate SCM-15 structure due to different distribution of germanium heteroatoms in the double four-member rings(D4Rs)of the framework and the orientation of the structure directing agent(SDA)molecules in the as-synthesized *** calculated relative energies of the bare zeolite framework suggest that structures with germanium ions clustered in the same D4Rs,e.g.,with large number of Ge–O-Ge contacts,are the most *** simulations of various orientations of the SDA in the pores of the germanosilicate zeolite show different stability order-the most stable models are the structures with germanium spread among all ***,for SCM-15 the stabilization due to the presence of the SDA and their orientation,is thermodynamic factor directing both the formation of specific framework type and Ge distribution in the framework during the *** relative stability of bare structures with different germanium distribution is of minor *** differs from SCM-14 germanosilicate,reported earlier,for which the stability order is preserved in presence of ***,even for zeolites with the same chemical composition and SDA,the characteristics of their framework lead to different energetic preference for germanium distribution.

Paravertebral Block in Thoracotomy—Comparison between Single-Shot and Catheter Methods

Aim: Thoracic paravertebral block is increasingly used as a method of postoperative analgesia in thoracic surgery. We aimed to compare two different methods of implementation: paravertebral thoracic block with a catheter placed in the epidural space, and a single injection of anesthetic at the end of surgery. Methods: We randomized 60 patients undergoing VATS, in 17 (group A) single-dose paravertebral block, and in another 21 (group B) catheter technique and postoperative infusion of local anesthetic were used, and in the latter, the control group (C) of 22 patients, analgesia was performed according to a standardized protocol with non-steroidal analgesics and opioids as needed. In all patients, in the early postoperative 48 hours, we measured the intensity of pain and opioid consumption. Results: There were no statistically significant differences between the collected preoperative data in the three groups of patients. There were no significant differences in subjective pain sensations in the early postoperative period (up to 12 hours). However, the number of opioids used was significantly lower in groups A and B compared to the control group, and in group B (with catheter technique) after the 12th to 48th hour after surgery there was almost no need for strong analgesics. Conclusion: Continuous infusion of local anesthetic in the area of the surgical incision provides much better pain relief than a single paravertebral block in the area of the incision.

The flexibility of metal-organic frameworks(MOFs)featuring stimuli-responsive structural transitions is often governed not only by the chemical composition and topology but also by orthogonal factors such as particle size,desolvation method,and history of the sample.A precise understanding of the mechanism behind such observations has been lacking up to now,and there are still substantial open questions concerning the impact of sample treatment *** DUT-8(M)family([M_(2)(2,6-ndc)2(dabco)]n,2,6-ndc=2,6-naphthalene dicarboxylate,dabco=1,4-diazabicyclo-[2.2.2]-octane),encompasses isostructural compounds based on Ni,Zn,Co,and Cu in the cluster node and is representative of pillared layer MOFs,often showing flexible *** this contribution,we discuss a possible explanation for the differences in flexibility observed in desolvated phases of DUT-8(Cu).Theoretical calculations and crystallographic data shed light on the preferred formation of interpenetrated confined closed pore phases in DUT-8(Cu)in contrast to DUT-8(Ni,Co,Zn)where the closed pore phases are formed.
