Person-centered endoscopy safety checklist: development,implementation,and evaluation

AIM to describe the development and implementation of a person-centered endoscopy safety checklist and to evaluate the effects of a "checklist intervention".MEtHODS the checklist,based on previously published safety checklists,was developed and locally adapted,taking patient safety aspects into consideration and using a person-centered approach. this novel checklist was introduced to the staff of an endoscopy unit at a Stockholm University Hospital during half-day seminars and team training sessions. Structured observations of the endoscopy team's performance were conducted before and after the introduction of the checklist. In addition,questionnaires focusing on patient participation,collaboration climate,and patient safety issues were collected from patients and staff. RESULtS A person-centered safety checklist was developed and introduced by a multi-professional group in the endoscopy unit. A statistically significant increase in accurate patient identity verification by the physicians was noted(from 0% at baseline to 87% after 10 mo,P t baseline vs 96% after 10 mo,P = nonsignificant). Observations indicated that the professional staff made frequent attempts to use the checklist,but compliance was suboptimal: All items in the observed nurse-led "summaries" were included in 56% of these interactions,and physicians participated by directly facing the patient in 50% of the interactions. On the questionnaires administered to the staff,items regarding collaboration and the importance of patient participation were rated more highly after the introduction of the checklist,but this did not result in statistical significance(P = 0.07/P = 0.08). the patients rated almost all items as very high both before and after the introduction of the checklist;hence,no statistical difference was *** the intervention led to increased patient identity verification by physicians-a patient safety improvement. Rectal nitric oxide as biomarker in the treatment of inflammatory bowel disease: Responders versus nonresponders

AIM： to explore rectal nitric oxide （NO） as biomarker of treatment response in ulcerative colitis （UC） and Crohn＇s disease （CD）, and examine relationships between rectal NO, mucosal expression of NO synthases （NOS）, and pro-inflammatory cytokines. MEtHODS： twenty-two patients with UC and 24 with CD were monitored during steroid treatment. Rectal NO levels were measured and clinical activities were assessed on days 1, 3, 7 and 28. Mucosal presence of NOS and pro-inflammatory cytokines were analyzed by immunohistochemistry and Rt-PCR. RESULtS： Active UC and CD displayed markedly increased rectal NO levels （10950 ± 7610 and 5 040 ± 1 280 parts per billion （ppb）, respectively） as compared with the controls （154 ± 71 ppb, P 〈 0.001）. Rectal NO correlated weakly with disease activity in both UC and CD （r = 0.34 for UC and r = 0.48 for CD, P 〈 0.01）. In 12 patients, a steroid-refractory course led to colectomy. these patients had only slightly increased NO levels （UC： 620 ± 270 ppb; CD： 1260 ± 550 ppb） compared to those with a therapeutic response （UC： 18860 ± 530 ppb, P 〈 0.001, CD： 10060 ± 3200 ppb, P 〈 0.05）. CONCLUSION： Rectal NO level is a useful biomarker of treatment response in IBD as low NO levels predicts a poor clinical response to steroid treatment.

Background:Due to the wide variety of morphology,size,and dynamics,selecting an optimal valve size and location poses great difficulty in percutaneous pulmonary valve implantation(PPVI).this study aimed to report our experience with in vitro bench testing using patient-specific three-dimensional(3D)-printed models for planning PPVI with the Venus ***:Patient-specific 3D soft models were generated using PolyJet printing with a compliant synthetic material in 15 patients scheduled to undergo PPVI between July 2018 and July 2020 in Central China Fuwai Hospital of Zhengzhou ***:3D model bench testing altered treatment strategy in all patients(100%).One patient was referred for surgery because testing revealed that even the largest Venus P-valve would not anchor *** the remaining 14 patients,valve size and/or implantation location was altered to avoid valve migration and/or compression coronary *** four patients,it was decided to change the point anchoring because of inverted cone-shaped right ventricular outflow tract(RVOt)(n=2)or risk of compression coronary artery(n=2).Concerning sizing,we found that an oversize of 2-5 mm *** of the valve was dictated by the flaring of the in-and outflow portion in the pulmonary *** was successful in all 14 patients(absence of valve migration,no coronary compression,and none-to-mild residual pulmonary regurgitation[PR]).the diameter of the Venus P-valve in the 3D simulation group was significantly smaller than that of the conventional planning group(36[2]vs.32[4],Z=-3.77,Ptro testing indicated no need to oversize the Venus P-valve to the degree recommended by the balloon-sizing technique,as 2-5 mm sufficed.

Rehabilitation of Semi-Arid Grasslands through the Perennialization of Lots by Implementing Perennial Forage Exotic Grass

Argentina is the country with the highest proportion of arid and semi-arid ecosystems in Latin America. In the rangelands of Southwestern Buenos Aires (Patagones Department), there is a clear advancement of the agricultural frontier to the detriment of the native forest in this region. Due to rainfall variation and seed acquisition, thinopyrum ponticum is cultivated as a forage perennial crop in this region. Our objective was to evaluate the performance of t. ponticum as a facilitating crop for the medium-term rehabilitation of natural grasslands in semi-arid areas. the working hypotheses were that: 1) native perennial grass cover increases over the years and 2) diversity and specific fi c richness of the vegetation are enhanced by the duration of tall Wheatgrass implantation. Data were collected from commercial plots where t. ponticum was shown: recent implantation (5-8 - 8 years, RI); medium implantation (13-15 - 15 years, MI); and old implantation (20-22 - 22 years, OI). thirty-four species were identified fi ed and classified fi ed into seven functional groups: Annual grasses, annual herbs, perennial herbs, exotic perennial herbs, perennial forage grasses, exotic perennial forage grasses, perennial forage exotic grasses, and nonforage perennial grasses. thinopyrum ponticum's ' s total cover was between RI and OI. total cover, species richness, and Shannon-Weaver diversity index showed no differences among treatments. Perennial forage grasses exhibited higher cover values in sites with greater implantation age and annual grasses showed the opposite response. Our results indicated that t. ponticum does not invade the sampled plots and enhances the colonization of the planted plots by perennial forage native species. However, even though the herbaceous cover had been recovered, the woody layer which could provide environmental services and specific fi c values for conservation was not.

BACKGROUND Prevalence of hepatocellular carcinoma(HCC)is increasing,especially in patients with metabolic dysfunctionassociated steatotic liver disease(MASLD).AIM to investigate rifaximin(RIF)effects on epigenetic/autophagy markers in *** Adult Sprague-Dawley rats were randomly assigned(n=8,each)and treated from 5-16 wk:Control[standard diet,water plus gavage with vehicle(Veh)],HCC[high-fat choline deficient diet(HFCD),diethylnitrosamine(DEN)in drinking water and Veh gavage],and RIF[HFCD,DEN and RIF(50 mg/kg/d)gavage].Gene expression of epigenetic/autophagy markers and circulating miRNAs were *** All HCC and RIF animals developed metabolic-dysfunction associated steatohepatitis fibrosis,and cirrhosis,but three RIF-group did not develop *** animals who developed HCC with those who did not,miR-122,miR-34a,tubulin alpha-1c(tuba-1c),metalloproteinases-2(Mmp2),and metalloproteinases-9(Mmp9)were significantly higher in the *** opposite occurred with Becn1,coactivator associated arginine methyltransferase-1(Carm1),enhancer of zeste homolog-2(Ezh2),autophagy-related factor LC3A/B(Map1 Lc3b),and p62/sequestosome-1(p62/SQStM1)-*** with controls,Map1 Lc3b,Becn1 and Ezh2 were lower in HCC and RIF-groups(Pto RIF(Ptic expression of Mmp9 was higher in HCC in relation to the control;the opposite was observed for p62/Sqstm1(PtM1 protein was lower in the RIF-group compared to the control(P=0.024).there was no difference among groups for tuba-1c,Aldolase-B,alpha-fetoprotein,and Mmp2(P>0.05).miR-122 was higher in HCC,and miR-34a in RIF compared to controls(Pto RIF,and the inverse was observed for miR-224(Pthere was no difference among groups regarding miR-33a,miR-143,miR-155,miR-375 and miR-21(P>0.05).CONCLUSION RIF might have a possible beneficial effect on preventing/delaying liver carcinogenesis throu

Extending the ionic conductivity is the pre-requisite of electrolytes in fuel cell technology for high-electrochemical *** this regard,the introduction of semiconductor-oxide materials and the approach of heterostructure formation by modulating energy bands to enhance ionic conduction acting as an electrolyte in fuel ***（n-type; SnO2） plays a key role by introducing into p-type SrFe0.2ti0.8O3-δ（SFt） semiconductor perovskite materials to construct p-n heterojunction for high ionic ***,two different composites of SFt and SnO2are constructed by gluing p-and n-type SFt-SnO2,where the optimal composition of SFt-SnO2（6:4） heterostructure electrolyte-based fuel cell achieved excellent ionic conductivity 0.24 S cm-1with power-output of1004 mW cm-2and high OCV 1.12 V at a low operational temperature of500℃.the high power-output and significant ionic conductivity with durable operation of 54 h are accredited to SFt-SnO2heterojunction formation including interfacial conduction assisted by a built-in electric field in fuel cell ***,the fuel conversion efficiency and considerable Faradaic efficiency reveal the compatibility of SFt-SnO2heterostructure electrolyte and ruled-out short-circuiting ***,the first principle calculation provides sufficient information on structure optimization and energy-band structure modulation of *** strategy will provide new insight into semiconductor-based fuel cell technology to design novel electrolytes.
