Using adherence to diabetes management guidelines as a case study,this paper applied a novel geospatial hot-spot and cold-spot methodology to identify priority counties to target *** for this study were obtained from the Dartmouth Atlas of Healthcare,the United States Census Bureau’s American Community Survey and the University of Wisconsin County Health Rankings.A geospatial approach was used to identify four tiers of priority counties for diabetes preventive and management services:diabetes management cold-spots,clusters of counties with low rates of adherence to diabetes preventive and management services(Tier D);medicare spending hot-spots,clusters of counties with high rates of spending and were diabetes management cold-spots(Tier C);preventable hospitalisation hot-spots,clusters of counties with high rates of spending and are diabetes management cold-spots(Tier B);and counties that were located in a diabetes management cold-spot cluster,preventable hospitalisation hot-spot cluster and medicare spending hot-spot cluster(Tier A).The four tiers of priority counties were geographically concentrated in Texas and Oklahoma,the Southeast and central *** these tiers,there were 62 Tier A *** of preventable hospitalisations and medicare spending were higher in Tier A counties compared with national *** same counties had much lower rates of adherence to diabetes preventive and management *** novel geospatial mapping approach used in this study may allow practitioners and policy makers to target interventions in areas that have the highest *** refinement of this approach is necessary before making policy recommendations.

AIm: To investigate the relation of patient characteristics and procedural parameters to the endoscopic detection rate of colonic adenomas. Further to study,which factors may be capable to predict the localization of adenomatous ***: We used the data base of a prospective randomized colonoscopy study(The Colo Cap trial) to identify patients being diagnosed with colon adenoma. Logistic regression analysis was conducted to reveal predictors for adenoma detection in the entire colon and also with respect to the proximal and distal part. Covariates including age, gender, duration of colonoscopy and comorbidities were defined to determine association between predictors and adenoma ***: Equal numbers of adenomas were detected in the proximal and distal side of the splenic flexure [126(57%) vs 94(43%), P = 0.104]. Simultaneous occurrence of adenomas in both sides of the colon was rare. The appearance of both proximal and distal adenoma was associated with increasing age(P = 0.008 and P = 0.024) and increasing duration of colonoscopy(P male gender was a predictor for adenoma detection in the proximal colon(P = 0.008) but statistical significance was slightly missed with respect to the distal colon(P = 0.089). Alcohol abuse was found to be a predictor for the detection of distal adenoma(P = 0.041). CONCLUSION: Increasing age and longer duration of colonoscopy are factors with a strong impact on adenoma detection both in the proximal and distal colon. Since proximal adenomas occurred in absence of distal adenomas, complete colonoscopy should be performed for screening.

Sand wave migration near the southeastern corner of martha's Vineyard, massachusetts, USA

Sand waves of approximately 2 m in height were observed to migrate nearly 40 m with counterclockwise rotation between two bathymetric surveys performed three months apart near the southeastern corner of martha's Vineyard, massachusetts. The region is characterized by strong tidal currents, intermittent energetic surface wave events, and shallow water with local depth ranging from 2 to 7 m. This study uses the process-based model, Delft3D, with a three-dimensional approach to examine the sand wave dynamics by incorporating surface waves, winds, currents, and bathymetric observations. The model successfully simulates sand wave migration in comparisons to observations. model sensitivity analyses show that the sand wave migration reduces by 65% with the absence of the surface waves. The modeled sand wave migration speed is correlated with the tidal current Shields parameter, and sharp increases in migration speed occur when the wave-driven Shields parameter increases in response to energetic surface wave events. The combined effect of tides, surface waves, and bathymetry is the origin of the rotational aspect of the sand wave, using the Shields parameter as an indicator of tidal currents and surface wave influence on sand wave dynamics.

Steve Pennycook is a pioneer in the application of high-resolution scanning transmission electron microscopy(STEm)and in particular the use of annular dark-field(ADF)*** we show how a general framework for 4D STEm allows clear links to be made between ADF imaging and the emerging methods for reconstructing images from 4D STEm data *** show that both ADF imaging and ptychographical reconstruction can be thought of in terms of integrating over the overlap regions of diffracted discs in the detector *** approach allows the similarities in parts of their transfer functions to be understood,though we note that the transfer functions for ptychographic imaging cannot be used as a measure of information *** also show that conditions of partial spatial and temporal coherence affect ADF imaging and ptychography similarly,showing that achromatic interference can always contribute to the image in both cases,leading to a robustness to partial temporal coherence that has enabled high-resolution imaging.
