BACKGROUND Inflammatory bowel disease(IBD) is a chronic, inflammatory disorder characterised by both intestinal and extra-intestinal pathology. Patients may receive both emergency and elective care from several providers, often in different hospital settings. Poorly managed transitions of care between providers can lead to inefficiencies in care and patient safety issues. To ensure that the sharing of patient information between providers is appropriate, timely, accurate and secure, effective data-sharing infrastructure needs to be developed. To optimise inter-hospital data-sharing for IBD patients, we need to better understand patterns of hospital encounters in this *** To determine the type and location of hospital services accessed by IBD patients in *** was a retrospective observational study using Hospital Episode Statistics, a large administrative patient data set from the National Health Service in *** patients with a diagnosis of IBD following admission to hospital were followed over a 2-year period to determine the proportion of care accessed at the same hospital providing their outpatient IBD care, defined as their ‘home provider'. Secondary outcome measures included the geographic distribution of patient-sharing, regional and age-related differences in accessing services, and type and frequency of outpatient ***95055 patients accessed hospital services on 1760156 occasions over a 2-year follow-up period. The proportion of these encounters with their identified IBD‘home provider' was 73.3%, 87.8% and 83.1% for accident and emergency,inpatient and outpatient encounters respectively. 目的:应用质量源于设计（QbD）实验设计方案开发一种分析万古霉素中杂质的优化方法。方法:将Fusion Method Development软件、Empower 2色谱数据软件（CDS）和配备PDA、色谱柱管理器及溶剂选择阀的ACQUITY UPLC系统相结合,开发一种可分离万古霉素中杂质的UPLC方法。结果:应用QbD的Fusion软件方法有效改善了万古霉素中杂质的分离效果,之前手动开发的UPLC方法只能检出26种杂质,而本研究开发的方法将观测到的杂质种类增加到了39种。结论:将专业软件与UPLC技术相结合,快速的开发出了一种用于分析万古霉素及其杂质的优化方法,该方法在方法验证和转换过程中可保持稳定。

Antimicrobial delivery has been advocated for guided tissue regeneration (GTR) or guided bone regeneration (GBR) therapies involving patients with aggressive or unresolved periodontitis/peri-implantitis. Electrospun chitosan membranes demonstrate several advantages over traditional GTR barrier membranes because they stimulate healing, mimic the topology of the extracellular matrix, and allow for diffusion of nutrients and wastes into/out of the graft site, and were shown to stimulate bone formation in a rabbit calvarial criticalsize defect model. Previously, we have shown improvements in mechanical properties and degradation kinetics by crosslinking electrospun membranes with 5 mM or 10 mM genipin. We have also demonstrated the ability of elecrospun chitosan membranes to inhibit lippopolysaccharide (LPS)-induced monocyte activation. In this study, minocycline was incorporated into the chitosan membrane by passive absorption at 5 or 10 mg/mL. The minocycline-loaded membranes and control membranes (carrier only) were tested against Porphyromonas gingivalis (P. gingivalis) by repeated zone of inhibition (ZOI) measurements. Testing showed that uncrosslinked and genipin-crosslinked membranes have similar capacity to absorb aqueous solutions (swelling ratio 1.7 - 2.2). Minocycline loading resulted in bacterial inhibition for up to 8 days from crosslinked membranes (with 11 mm initial ZOI) whereas uncrosslinked membranes loaded with minocycline only inhibited bacteria for 4 days (with 8 mm initial ZOI). These in vitro results suggest that genipin-crosslinked electrospun chitosan membranes loaded with minocycline may be able to reduce early bacterial contamination of GTR graft sites.

BACKGROUND:This prospective,randomized trial was undertaken to evaluate the utility of adding end-tidal capnometry(ETC)to pulse oximetry(PO)in patients undergoing procedural sedation and analgesia(PSA)in the emergency department(ED).METHODS:The patients were randomized to monitoring with or without ETC in addition to the current standard of *** endpoints included respiratory adverse events,with secondary endpoints of level of sedation,hypotension,other PSA-related adverse events and patient ***:Of 986 patients,501 were randomized to usual care and 485 to additional ETC *** this series,48%of the patients were female,with a mean age of 46 *** manipulations(71%),cardioversion(12%)and abscess incision and drainage(12%)were the most common procedures,and propofol and fentanyl were the sedative/analgesic combination used for most *** was no difference in patients experiencing de-saturation(Sa O2<90%)between the two groups;however,patients in the ETC group were more likely to require airway repositioning(12.9%vs.9.3%,P=0.003).Hypotension(SBP<100 mm Hg or<85 mm Hg if baseline<100 mm Hg)was observed in 16(3.3%)patients in the ETC group and 7(1.4%)in the control group(P=0.048).CONCLUSIONS:The addition of ETC does not appear to change any clinically significant *** found an increased incidence of the use of airway repositioning maneuvers and hypotension in cases where ETC was *** do not believe that ETC should be recommended as a standard of care for the monitoring of patients undergoing PSA.

The Tribbles(TRIB) family of pseudokinase proteins has been shown to play key roles in cell cycle, metabolic diseases, chronic inflammatory disease, and cancer development. A better understanding of the mechanisms of TRIB pseudokinases could provide new insights for disease development and help promote TRIB proteins as novel therapeutic targets for drug discovery. At the 2 nd International Symposium on Tribbles and Diseases held on May 7–9, 2018 in Beijing, China, a group of leading Tribbles scientists reported their findings and ongoing studies about the effects of the different TRIB proteins in the areas of immunity, metabolism, fundamental cell biology and cancer. Here, we summarize important and insightful overviews from 4 keynote lectures, 13 plenary lectures and 8 short talks that took place during this meeting. These findings may offer new insights for the understanding of the roles of TRIB pseudokinases in the development of various diseases.

世界野生虎(Pantheratigris)传统上被划分为8个亚种,其中3个亚种已于20世纪灭绝,而剩余种群的生存仍然受到偷猎、栖息地丧失和片断化的威胁。作为唯一栖息着4个现存虎亚种的国家,中国在世界虎的保护事业中负有重要责任,然而其野生和圈养虎的分类地位却仍然不确定。最近一项研究(Luoetal,2004)从所有现存野生虎分布地区(包括中国)采集了134份“基准样品”(即原产野外或有确定地域起源的个体生物样品),对虎的系统地理学、种群结构以及遗传起源进行了全面分析。所用的分子标记包括四千碱基对的线粒体DNA、30个核基因组微卫星位点,以及MHC-DRB基因。研究结果表明,虽然虎的整体遗传多态性较低,但是种群分化程度很高,它们被划分为6个,而不是5个现存亚种(1)西伯利亚虎(***);(2)苏门答腊虎(***);(3)孟加拉虎(***);(4)华南虎(***);(5)印支虎(***);(6)新定义的亚种马来虎,暂命名为***。由于所研究样本量有限,目前暂定的华南虎亚种还需进一步确定。现有华南虎圈养种群包括遗传关系相距较远的两支一支与印支虎(***)无异;而另一支则与其他种群均相距甚远,可能代表了真正的华南虎(***)。利用分子生物学方法对中国动物园中圈养虎的遗传起源调查亟待进行,以确认该圈养种群整体的遗传独特性或者非独特性。换言之,这将是确认华南虎是否仍然存在的关键。
