Ruptured intracranial aneurysms(IAs)affect a small proportion of the population;however,the morbidity and mortality is disproportionally high.Although little is known about IA formation,progression,and rupture,mounting evidence suggests that inflammation may play an important role in IA pathogenesis.There is emerging evidence to suggest that leukocytes play a key role in generating and maintaining a pathologic inflammatory response that leads to aneurysm formation and rupture.We present the current literature pertaining to the role of leukocytes in aneurysm formation,progression,and rupture.The contributions of individual cell types are detailed,with special attention paid to the cytokine and molecular profiles.The role of magnetic resonance imaging as a means by which to evaluate aneurysm‑associated inflammation is reviewed.Finally,we discuss leukocytes as potential targets of pharmacologic intervention.

Helicobacter pylori(H.pylori)infection is well known to be associated with the development of precancerous lesions such as chronic atrophic gastritis(AG),or gastric intestinal metaplasia(GIM),and cancer.Various molecular alterations are identified not only in gastric cancer(GC)but also in precancerous lesions.H.pylori treatment seems to improve AG and GIM,but still remains controversial.In contrast,many studies,including meta-analysis,show that H.pylori eradication reduces GC.Molecular markers detected by genetic and epigenetic alterations related to carcinogenesis reverse following H.pylori eradication.This indicates that these changes may be an important factor in the identification of high risk patients for cancer development.Patients who underwent endoscopic treatment of GC are at high risk for development of metachronous GC.A randomized controlled trial from Japan concluded that prophylactic eradication of H.pylori after endoscopic resection should be used to prevent the development of metachronous GC,but recent retrospective studies did not show the tendency.Patients with precancerous lesions(molecular alterations)that do not reverse after H.pylori treatment,represent the"point of no return"and may be at high risk for the development of GC.Therefore,earlier H.pylori eradication should be considered for preventing GC development prior to the appearance of precancerous lesions.

Objective:To evaluate the perioperative as well as early oncological outcomes of patients undergoing robotic retroperitoneal lymph node dissection for treatment of testicular cancer.Methods:We conducted a prospective consecutive case series of patients undergoing robotic assisted retroperitoneal lymph node dissection for metastatic testicular cancer between May 2018 and July 2021 at our institution.Data were collected on patient and tumour characteristics,intraoperative and postoperative parameters,and functional and oncological outcomes.Descriptive statistics are presented.Results:Nineteen patients were identified;18(94.7%)completed the procedure robotically and one was converted to open surgery;78.9%of patients had stage≥IIB and 12(63.2%)patients had undergone prior chemotherapy.The median operative time was 300(interquartile range[IQR]240-315)min.Median blood loss was 100(IQR 50-175)mL.Median length of stay was 2(range 1-11)days.All robotically completed patients commenced diet and passed flatus on Day 1 and were discharged by Day 3.The median lymph node yield was 40.5(IQR 38-51)nodes.All patients undergoing nerve-sparing procedures recovered antegrade ejaculatory function.One patient had a Clavien-Dindo III complication(chylous ascites requiring drainage).At a median follow-up of 22.3(IQR 16.3-24.9)months,one patient developed retroperitoneal recurrence,which was successfully treated with second-line chemotherapy;no other patients have had recurrences.Conclusion:Robotic retroperitoneal lymph node dissection is a safe and feasible alternative to open surgery in appropriately selected patients,offering low morbidity.Early oncological outcomes are promising.Larger cohorts and longer follow-ups are required to validate our institution's findings.

The presence of the debris in the Earth’s orbit poses a significant risk to human activity in outer space.This debris population continues to grow due to ground launches,the loss of external parts from space ships,and uncontrollable collisions between objects.A computationally feasible continuum model for the growth of the debris population and its spatial distribution is therefore critical.Here we propose a diffusion-collision model for the evolution of the debris density in the low-Earth orbit and its dependence on the ground-launch policy.We parametrize this model and test it against data from publicly available object catalogs to examine timescales for the uncontrolled growth.Finally,we consider sensible launch policies and cleanup strategies and how they reduce the future risk of collisions with active satellites or space ships.

Ever since the introduction of Western biomedicine into China, there has been friction between proponents of this new approach, and proponents of the traditional medical practices that had evolved over thousands of years in China. For the most part, this friction has been couched in the unexamined assumption that Western medicine is based on science, while Chinese medicine is not. This article will examine that assumption, which in this author’s opinion is unjustified and incorrect. Having practiced acupuncture for the past 50 years, after receiving my doctoral degrees in medicine and physiology, my thinking on this topic has also evolved over time. I will begin this article with some historical information that bears on the topic under discussion, followed by my personal ideas about how to resolve the confiicts that have arisen.

Visual simultaneous localisation and mapping(vsLAM)finds applications for indoor and outdoor navigation that routinely subjects it to visual complexities,particularly mirror reflections.The effect of mirror presence(time visible and its average size in the frame)was hypothesised to impact localisation and mapping performance,with systems using direct techniques expected to perform worse.Thus,a dataset,MirrEnv,of image sequences recorded in mirror environments,was collected,and used to evaluate the performance of existing representative methods.RGBD ORB-sLAM3 and BundleFusion appear to show moderate degradation of absolute trajectory error with increasing mirror duration,whilst the remaining results did not show significantly degraded localisation performance.The mesh maps generated proved to be very inaccurate,with real and virtual reflections colliding in the reconstructions.A discussion is given of the likely sources of error and robustness in mirror environments,outlining future directions for validating and improving vsLAM performance in the presence of planar mirrors.The MirrEnv dataset is available at https://***/10.17035/d.2023.0292477898.

Metastasis is the leading cause of cancer-related deaths.Transforming growth factor beta(TGF-β)signaling drives metastasis and is strongly enhanced during cancer progression.Yet,the use of on-target TGF-βsignaling inhibitors in the treatment of cancer patients remains unsuccessful,highlighting a gap in the understanding of TGF-βbiology that limits the establishment of efficient anti-metastatic therapies.Here,we show that TGF-βsignaling hyperactivation in breast cancer cells is required for metastasis and relies on increased small extracellular vesicle(sEV)secretion.Demonstrating sEV’s unique role,TGF-βsignaling levels induced by sEVs exceed the activity of matching concentrations of soluble ligand TGF-β.Further,genetic disruption of sEV secretion in highlymetastatic breast cancer cells impairs cancer cell aggressiveness by reducing TGF-βsignaling to nearly-normal levels.Otherwise,TGF-βsignaling activity in non-invasive breast cancer cells is inherently low,but can be amplified by sEVs,enabling invasion and metastasis of poorly-metastatic breast cancer cells.Underscoring the translational potential of inhibiting sEV trafficking in advanced breast cancers,treatment with dimethyl amiloride(DMA)decreases sEV secretion,TGF-βsignaling activity,and breast cancer progression in vivo.Targeting both the sEV trafficking and TGF-βsignaling by combining DMA and sB431542 at suboptimal doses potentiated this effect,normalizing the TGF-βsignaling in primary tumors to potently reduce circulating tumor cells,metastasis,and tumor self-seeding.Collectively,this study establishes sEVs as critical elements in TGF-βbiology,demonstrating the feasibility of inhibiting sEV trafficking as a new therapeutic approach to impair metastasis by normalizing TGF-βsignaling levels in breast cancer cells.

Purpose:The growing literature on private supplementary tutoring,widely known as shadow education,addresses multiple dimensions and roles.However,many studies inadequately capture the diversity and implications of different forms of tutoring.This paper examines these matters in Myanmar,highlighting complexities and ambiguities in descriptors and in the nature of provision.Design/Approach/Methods:Using a mixed-methods design,this study was conducted in Mandalay Region and Chin state.The data were obtained from interviews with I10 Grade II students,34 high school teachers,30 parents,29 private tutoring providers,and two private school owners.Findings:The article categorizes tutoring forms based on their styles and orientations,particularly lecture-type teaching and homework supervision by guides,class sizes,durations of fee-charging(e.g,annually),the number of subjects taught per tutor,and tutoring locations.The study brings the topic out of the shadows for clearer analysis of the phenomenon and thus for assessment of its implications.OriginalityNalue:The study makes a methodological contribution to the wider picture by demonstrating the need for greater clarity and precision in the national and international comparative literatures.It also underscores shortcomings of predetermined categories in questionnaires and the value of qualitative interviews.
