To assess the peripheral refractive changes that occur when eye growth decelerates in *** We measured peripheral refraction and on-axis vitreous chamber depth on a total of 53 marmosets;36 treated monocularly for 12 weeks 9 hrs/day(26 with-5 D contact lenses,10 with+5 D)and 17 untreated *** the 26 marmosets treated with-5 D,10 wore contact lenses 9 hrs/day without interruptions and were mea-sured at the end of 10 weeks of treatment(T10)and after 4 weeks of recovery(R14).Eight had both contact lenses removed for 30 mins twice/day(mid-morning and mid-afternoon)during the first four weeks of treatment(early interruption group)and eight had the lenses removed during the second four weeks of treatment(late interruption group).The two interruption groups were measured after 4,8 and 12 weeks of treatment only(T4,T8,T12).

Influence of hospital volume and outcomes of adult structural heart procedures

Hospital volume is regarded amongst many in the medical community as an important quality metric. This is especially true in more complicated and less commonly performed procedures such as structural heart disease interventions. Seminal work on hospital volume relationships was done by Luft et al more than 4 decades ago, when they demonstrated that hospitals performing > 200 surgical procedures a year had 25%-41% lower mortality than those performing fewer procedures. Numerous volume-outcome studies have since been done for varied surgical procedures. An old adage "practice makes perfect" indicating superior operator and institutional experience at higher volume hospitals is believed to primarily contribute to the volume outcome relationship. Compelling evidence from a slew of recent publications has also highlighted the role of hospital volume in predicting superior post-procedural outcomes following structural heart disease interventions. These included transcatheter aortic valve repair, transcatheter mitral valve repair, septal ablation and septal myectomy for hypertrophic obstructive cardiomyopathy, left atrial appendage closure and atrial septal defect/patent foramen ovale closure. This is especially important since these structural heart interventions are relatively complex with evolving technology and a steep learning curve. The benefit was demonstrated both in lower mortality and complications as well as better economics in terms of lower length of stay and hospitalization costs seen at high volume centers. We present an overview of the available literature that underscores the importance of hospital volume in complex structural heart disease interventions.

The high effectiveness of the third dose of BNT162b2 in healthy adolescents against Omicron BA.1 has been reported in some studies,but immune responses conferring this protection are not yet *** this analysis,our study(NCT04800133)aims to evaluate the humoral and cellular responses against wild-type and Omicron(BA.1,BA.2 and/or BA.5)SARS-CoV-2 before and after a third dose of BNT162b2 in healthy *** 5 months after 2 doses,S IgG,S IgG Fc receptor-binding,and neutralising antibody responses waned significantly,yet neutralising antibodies remained detectable in all tested adolescents and S IgG avidity increased from 1 month after 2 *** antibody responses and S-specific IFN-γ^(+)and IL-2^(+)CD8^(+)T cell responses were significantly boosted in healthy adolescents after a homologous third dose of *** to adults,humoral responses for the third dose were non-inferior or superior in *** S-specific IFN-γ^(+)and IL-2^(+)CD4^(+)and CD8^(+)T cell responses in adolescents and adults were comparable or ***,after 3 doses,adolescents had preserved S IgG,S IgG avidity,S IgG FcγRIIIa-binding,against Omicron BA.2,as well as preserved cellular responses against BA.1 S and moderate neutralisation levels against BA.1,BA.2 and *** from 100 and 96%of adolescents tested at 1 and 5 months after two doses could also neutralise *** study found high antibody and T cell responses,including potent cross-variant reactivity,after three doses of BNT162b2 vaccine in adolescents in its current formulation,suggesting that current vaccines can be protective against symptomatic Omicron disease.

Background. One of the most active chemotherapy combinations in advanced or recurrent cervical cancer is cis-platin-paclitaxel. However, this palliative regimen is associated with significant toxicity. Carboplatin-paclitaxel is thus an attractive option. Methods. Patients with advanced or recurrent carcinoma of the cervix treated with carboplatin-paclitaxel from April 2000 were included in the study. Starting doses of carboplatin-paclitaxel were: AUC 5-6 and 155-175 mg/m2, respectively, repeated every 28 days. Results. Twenty-five women treated with this combination were identified. Twenty-three women (92% ) had prior treatment with pelvic radiotherapy and 14 (56% ) had had concurrent radio-sensitizing cisplatin. There was a 20% PR and a 20% CR rate (10/25). The median progression-free survival for the entire group was 3 months. Responders had a median PFS of 16 months. Fourteen patients (56% ) had died of disease progression. The median overall survival (OS) was 21 months. Common toxicities included: grade 1 or 2 anemia, 68% ; grade 3 or 4 anemia, 32% ; grade 3 or 4 neutropenia, 32% ; and grade 1 or 2 peripheral neuropathy, 24% . ECOG PS did not change significantly while on treatment. Eighty-four percent of treatments were delivered on time, and 96% at full dose. Conclusions. Carboplatinpaclitaxel is an active combination in advanced and recurrent cervical cancer. In this predominantly pre-irradiated group, the combination was deliverable, well tolerated, and the most commonly observed toxicity was anemia.1078. Background: Primary neuritic leprosy (PNL) is a rare form of leprosy where the characteristic skin lesions are absent. Investigations of apparently normal skin from the areas of sensory change have revealed microscopic evidence of nerve involvement. Clinical studies have found that a proportion of patients develop visible skin lesions during follow- up. The aim of the study was to perform a clinical and histological analysis of PNL patients who developed visible skin lesions during treatment and follow- up, to gain insight into the pathogenesis of the disease. Methods: Twenty- nine individuals in a series of 182 PNL patients developed visible skin lesions during follow- up. Analysis of the number, location, histology and time of onset of the new skin lesions in relation to the type and regularity of the treatment regimen were noted. A biopsy from the new skin lesion when available was compared with the nerve biopsy findings at the time of initial diagnosis. Results: Thirty- eight per cent of patients developed a single patch and 28% developed two patches. Over three- quarters of these were on the lower limb (47% ) or the upper limb (29% ). Sixty- two per cent of patients developed the lesions within 2 years of the onset of symptoms. Patients on regular treatment developed patches earlier than those on irregular treatment or no treatment. A skin biopsy from the new patch revealed borderline tuberculoid leprosy histology in 47% of the patients. Conclusions: The findings suggest that leprosy primarily affects the nerve and that a neuritic phase precedes the development of visible cutaneous lesions.

背景与目的本科学声明提供了血管性因素对认知损害（co,airiveimpairment）和痴呆促成作用证据的纵览。血管性因素在老年认知损害和痴呆中的作用十分普遍。本文对血管性认知损害（vascularcognitiveimpairment，VCI）的定义、神经病理学、基础科学和病理生理学研究、神经影像学的作用、血管性和其他相关危险因素的作用以及潜在的防治方法进行了回顾，旨在为临床医生提供一份总体指导，以进一步加深对VCI和痴呆及其防治的理解。方法写作组成员由写作组联合主席基于先前在相关领域的研究成果提名，并经美国心脏协会（AmericanHeartAssociation，AHA）卒中委员会科学声明监督委员会、流行病学和预防委员会以及文稿监督委员会批准。写作组采用系统文献回顾（主要涵盖1990年至2010年5月期间的文献）、以往公布的指南、个人资料以及专家观点总结现有证据，提出当前知识的不足，并对适当的问题根据AHA标准制定推荐意见。所有写作组成员均有机会对推荐意见进行评论并认可了最终版本。经AHA的同行评议以及卒中委员会领导阶层、流行病学和预防委员会以及科学声明监督委员会审阅后，最终获得AHA科学咨询和协调委员会批准。结果VCI概念的提出涵盖了与所有类型脑血管性脑损害（而不仅仅是卒中）相关的整个认知障碍（cognitivedisorder）谱，其范围从轻度认知损害到完全痴呆。神经血管单元和脑血流调节机制障碍可能是VCI的潜在病理生理学过程的重要组成部分。脑淀粉样血管病已成为阿尔茨海默病（Alzheimerdisease，AD）、微梗死、微出血、脑出血以及VCI风险的一个重要标志。老年认知损害的神经病理学改变常常是AD与微血管性脑损害的混合状态，二者可叠加和协同增高认知损害风险。就此而言，MRI和其他神经影像学技术在VCI的确定和识别中起着重要作用，并提供证据说明以白质高信号和小的深部梗死为特征的皮质下型VCI很常见。在许多情况下，VCI的风险标志同样也是卒中的传统危险因素。这些危险因素包括但不仅限于心房颤动、高血压、糖尿病和高胆固醇血症。而且，这些血管危险因素也可能是AD的风险标志。颈动脉内膜一中膜厚度和动脉僵硬度是动脉老化的标记物，也可能成为VCI的风险标志。目前，美国食品药物管理局尚未批准任何VCI的特异性治疗方法。不过，对卒中和心血管病传统危险因素的评价和控制，可能对包括老年人群在内的VCI的预防具有作用。结论血管性因素在认知损害和痴呆中的促成作用很重要。近年来，在临床前、神经病理学、神经影像学、生理学和流行病学研究基础上，对VCI的认识得到了不断提高。推荐使用跨学科的、转化的以及交互的方法进一步加深对此领域的了解，并更好地掌握其神经心理学特征。需要开展前瞻性定量临床一病理学一神经影像学研究，以提高对神经影像学改变的病理学基础，以及血管病变和AD病变在VCI和AD临床演变中的复杂的丰互作用的认识。可能需要在中年期尽早肝始对血管危险标志进行长期干预，以预防或延迟VCI或AD发病的研究。住高危人群I}1强化控制血管危险因素则是另一个重要的研究方向。
