AIM: To investigate the age differences in the risk factors, clinicopathological characteristics and patterns of treatment of female breast cancer patients. METHODS: Seven thousand one hundred and fiftytwo women with primary breast cancer from the Hong Kong Breast Cancer Registry were recruited after receiving patients' consent, they were asked to complete standardized questionnaires which captured their sociodemographic characteristics and risk factors associated with breast cancer development. Among them, clinicopathological data and patterns of treatment were further collected from medical records of 5523 patients with invasive breast cancers. Patients were divided into two groups according to the age at diagnosis: younger(< 40 years old) vs older patients(≥ 40 years old) for subsequent analyses.RESULTS: Analysis on the sociodemographic characteristics and exposure to risk factors were performed on 7152 women with primary breast cancer and the results revealed that younger patients were more likely to have unhealthy lifestyles; these include a lack of exercise(85.4% vs 73.2%, P < 0.001), having high stress in life(46.1% vs 35.5%, P < 0.001), having dairy/meat-rich diets(20.2% vs 12.9%, P < 0.001),having alcohol drinking habit(7.7% vs 5.2%, P = 0.002). Younger patients were also more likely to have hormone-related risk factors including nulliparity(43.3% vs 17.8%, P < 0.001) and an early age at menarche(20.7% vs 13.2%, P < 0.001). Analyses on clinicopathological characteristics and patterns of treatment were performed on 5523 women diagnosed with invasive breast cancer. The invasive tumours in younger patients showed more aggressive pathological features such as having a higher percentage of grade 3 histology(45.7% vs 36.5%, P < 0.001), having a higher proportion of tumours with lymphovascular invasion(39.6% vs 33.2%, P = 0.003), and having multifocal disease(15.7% vs 10.3%, P < 0.001); they received different patterns of treatment than their older counterparts.CONCLUSION: Younger patients in Hong Kong are more

BACKGROUND There are limited data on the use of glucose transport protein 1(GLUT-1)expre-ssion as a biomarker for predicting lymph node metastasis in patients with colorectal cancer.GLUT-1 and GLUT-3,hexokinase(hk)-II,and hypoxia-induced factor(HIF)-1 expressions may be useful biomarkers for detecting primary tumors and lymph node metastasis when combined with fluorodeoxyglucose(FDG)uptake on positron emission tomography/computed tomography(PET/CT).AIM To evaluate GLUT-1,GLUT-3,hk-II,and HIF-1 expressions as biomarkers for detecting primary tumors and lymph node metastasis with 18F-FDG-PET/CT.METHODS This retrospective study included 169 patients with colorectal cancer who underwent colectomy and preoperative 18F-FDG-PET/CT at Chungbuk National University Hospital between January 2009 and May 2012.Two tissue cores from the central and peripheral areas of the tumors were obtained and were examined by a dedicated pathologist,and the expressions of GLUT-1,GLUT-3,hk-II,and HIF-1 were determined using immunohisto-chemical staining.We analyzed the correlations among their expressions,various clinicopathological factors,and the maximum standardized uptake value(SUVmax)of PET/CT.RESULTS GLUT-1 was found at the center or periphery of the tumors in 109(64.5%)of the 169 patients.GLUT-1 positivity was significantly correlated with the SUVmax of the primary tumor and lymph nodes,regardless of the biopsy site(tumor center,Phk-II,and HIF-1 expressions were not significantly correlated with the SUVmax of the primary tumor and lymph nodes.CONCLUSION GLUT-1 expression was significantly correlated with the SUVmax of 18F-FDG-PET/CT for primary tumors and lymph nodes.Clinicians should consider GLUT-1 expression in preoperative endoscopic biopsy in interpreting PET/CT findings.

High mobility group box 1 in the central nervous system:regeneration hidden beneath inflammation

High-mobility group box 1 was first discovered in the calf thymus as a DNA-binding nuclear protein and has been widely studied in diverse fields,including neurology and neuroscience.High-mobility group box 1 in the extracellular space functions as a pro-inflammatory damage-associated molecular pattern,which has been proven to play an important role in a wide variety of central nervous system disorders such as ischemic stroke,Alzheimer’s disease,frontotemporal dementia,Parkinson’s disease,multiple sclerosis,epilepsy,and traumatic brain injury.Several drugs that inhibit high-mobility group box 1 as a damage-associated molecular pattern,such as glycyrrhizin,ethyl pyruvate,and neutralizing anti-high-mobility group box 1 antibodies,are commonly used to target high-mobility group box 1 activity in central nervous system disorders.Although it is commonly known for its detrimental inflammatory effect,high-mobility group box 1 has also been shown to have beneficial pro-regenerative roles in central nervous system disorders.In this narrative review,we provide a brief summary of the history of high-mobility group box 1 research and its characterization as a damage-associated molecular pattern,its downstream receptors,and intracellular signaling pathways,how high-mobility group box 1 exerts the repair-favoring roles in general and in the central nervous system,and clues on how to differentiate the pro-regenerative from the pro-inflammatory role.Research targeting high-mobility group box 1 in the central nervous system may benefit from differentiating between the two functions rather than overall suppression of high-mobility group box 1.

We report on the design and characterization of the plasma mirror system installed on the J-KAREN-P laser at the Kansai Photon Science Institute,National Institutes for Quantum Science and Technology.The reflectivity of the single plasma mirror system exceeded 80%.In addition,the temporal contrast was improved by two orders of magnitude at 1 ps before the main pulse.Furthermore,the laser near-field spatial distribution after the plasma mirror was kept constant at plasma mirror fluence of less than 100 kJ/cm^(2).We also present the results of investigating the difference and the fluctuation in energy,pulse width and pointing stability with and without the plasma mirror system.

3D Nanoprinting with Charged Aerosol Particles-An Overview

CONSPECTUS:Three-dimensional(3D)printing is a revolutionary technology allowing rapid,cost-effectively,and flexible design of desired 3D products.In this regard,various techniques for downscaling 3D printing have been developed based on several methodologies in the past decades to exploit the advantages of 3D printing at the micro/nanoscale.The development of 3D nanoprinting techniques has provided a method to design unique and sophisticated nanoscale 3D objects enabling functional applications that are thus far constrained with conventional planar nanomanufacturing processes such as photolithography and electron beam lithography.However,the versatile and practical realization of 3D nanoprinting in a broad science and engineering field remains challenging due to limitations such as feature size resolution,suitable materials,and design flexibility.Therefore,innovative 3D nanoprinting techniques are required to overcome current limitations.In this Account,we describe our achievements in developing 3D nanoprinting with charged aerosols relying on the physics behind counteracting electric fields.We introduce the generation of charged aerosols,the most feasible in the broad range of fundamental building blocks(mainly all types of metals and alloys,their oxides,polymers,and organics)for 3D nanoprinting.Moreover,charged aerosols can be processed at ambient conditions easily.The aerosols could be precisely controlled and delivered for the assembly,using electric fields of complicated configurations despite the Brownian diffusion and other chaotic processes.The converging electric fields are formed around openings by the interactions of two electric fields.One of the electric fields comes from a negatively biased substrate.In contrast,the counteracting electric field comes from the positive ions distributed on a prepatterned dielectric layer over the substrate.As a result,the positively charged aerosols are focused through these fields to grow with nanoscale resolution only in the openings of the layer.Furthermore,a growi

Association between smoking,pancreatic insulin secretion and insulin resistance in Chinese subjects with or without glucose intolerance

Background There are studies suggesting smoking may increase the risk of type 2 diabetes. Effects of smoking on insulin secretion and insulin resistance （IR） are, however, controversial. Methods This is a cross-sectional study. Since there were very few smokers among Hong Kong Chinese women, only men （n=-1068） were analyzed in this report. Fasting and 2-hour plasma glucose and insulin were measured. Insulinogenic index as well as beta-cell function and IR based on homeostatic model assessment （HOMA） by computer model （HOMA Calculator v2.2） were calculated. Results Of the 1068 men, 147 had newly diagnosed diabetes, 131 newly diagnosed impaired glucose tolerance （IGT） and 790 were non-diabetic normal controls. Smokers had similar fasting and 2-hour insulin levels, insulinogenic index and HOMA derived beta-cell function as compared to non-smokers in the groups with diabetes, IGT or normal oral glucose tolerance test （OGTT）. IR was also similar between smokers, ex-smokers and non-smokers in those with normal OGTT. In men with IGT or diabetes, after adjustment for age and body mass index, smokers were more insulin resistant as compared to non-smokers （IR, IGT： 1.59±1.07 vs 1.03±0.54, P〈0.05; diabetes： 1.96±1.36 vs 1.06±0.45, P〈0.01）. With Logistic regression analysis, comparing smokers and non-smokers, IR was independently associated with smoking （odds ratio （95% C/）, IGT： 2.23 （1.05, 4.71）; diabetes： 3.92 （1.22, 12.58））. None of the other insulin parameters enter into the model among those with normal OGTT or comparing ex-smokers and non-smoker or smokers and ex-smokers. Conclusions In Chinese men, smoking did not show any direct association with insulin levels and pancreatic insulin secretion. Smoking men with IGT or diabetes appeared more insulin resistant than their non-smoking counterparts.

Rationale Haematoma growth is common early after intracerebral haemorrhage(ICH),and is a key determinant of outcome.Tranexamic acid,a widely available antifibrinolytic agent with an excellent safety profile,may reduce haematoma growth.Methods and design Stopping intracerebral haemorrhage with tranexamic acid for hyperacute onset presentation including mobile stroke units(STOP-MSU)is a phase Ⅱ double-blind,randomised,placebo-controlled,multicentre,international investigator-led clinical trial,conducted within the estimand statistical framework.Hypothesis In patients with spontaneous ICH,treatment with tranexamic acid within 2 hours of onset will reduce haematoma expansion compared with placebo.Sample size estimates A sample size of 180 patients(90 in each arm)would be required to detect an absolute difference in the primary outcome of 20%(placebo 39%vs treatment 19%)under a two-tailed significance level of 0.05.An adaptive sample size re-estimation based on the outcomes of 144 patients will allow a possible increase to a prespecified maximum of 326 patients.Intervention Participants will receive 1 g intravenous tranexamic acid over 10 min,followed by 1 g intravenous tranexamic acid over 8 hours;or matching placebo.Primary efficacy measure The primary efficacy measure is the proportion of patients with haematoma growth by 24±6 hours,defined as either≥33%relative increase or≥6 mL absolute increase in haematoma volume between baseline and follow-up CT scan.Discussion We describe the rationale and protocol of STOP-MSU,a phase Ⅱ trial of tranexamic acid in patients with ICH within 2 hours from onset,based in participating mobile stroke units and emergency departments.

Integrated Design of a Compact and Lightweight S-Band Traveling-Wave Tube Amplifier for a New Class of Microwave Power Module

A new class of compact and lightweight S-band 1 kW traveling-wave tube (TWT) is being developed for a microwave power module (MPM) that will be used for phased antenna array radar applications. The proposed S-band MPM provides a tenfold peak power increase compared to state-of-the-art S-band MPMs. In this paper, the design of the vacuum power booster TWT part of the MPM is presented. The compact and lightweight S-band TWT is driven by a 6 kV, 0.9 A electron beam. The amplifier is predicted by large-signal simulations to generate over 1 kW at S-band with 25 dB saturated gain and over 40% efficiency. The stability from unwanted oscillations has been investigated. To suppress the oscillations, the helix circuit has been coated with carbon composite material. The coaxial input and output antennas have been fabricated. For efficiency enhancement, a multi-stage depressed collector (MDC) has been designed using a 3D particle-in-cell (PIC) simulator, VORPAL. The collector design makes use of a current loop based on a feedback mechanism for effective design process. The integrated designs of a helix circuit, an electron gun, a periodic permanent magnet (PPM), antennas, and a collector are presented.
