Background：Musculoskeletal disorders（MSD） comprise a wide range of conditions,associated with an enormous pain and impaired mobility,and are affecting people＇s lives and *** of musculoskeletal disorders typically involves a multidisciplinary team *** findings have been found in previous studies evaluating the effectiveness of complementary therapies,though little attention has been paid to evaluating of the effectiveness of integrated packages of care combining conventional and complementary approaches for musculoskeletal conditions in a National Health Service（NHS） ***：To determine the feasibility of all aspects of a pragmatic observational study designed：（1） to evaluate the effectiveness and cost effectiveness of integrated treatments for MSDs in an integrated NHS hospital in the UK;（2） to determine the acceptability of the study design and research process to patients;（3） to explore patients＇ expectation and experience of receiving integrated ***：This is an observational feasibility study,with 1-year recruitment and 1-year follow-up,conducted in Royal London Hospital for Integrated Medicine,University College London Hospital Trust,*** eligible patients with MSDs newly referred to the hospital were included in the *** are integrated packages of care（conventional and complementary） as currently provided in the ***-36 Health Survey,short form Brief Pain Inventory,Visual Analogue Scale,and modified Client Service Receipt Inventory will be assessed at 4/5 time ***-structured interview/focus group will be carried out before treatment,and 1 year after commence of ***：We intend to conduct a pragmatic observational study of integrated medical treatment of MSDs at a public sector *** will inform the design of a future trial including recruitment,retention,suitability of the outcome measures and patients experiences.

目的与“不用针灸”对比,确定“使用针灸”的治疗方法对慢性头痛患者的头痛、健康状况及无病时间的影响。设计随机对照试验。试验场所英格兰和威尔士的开业医生诊所。受试对象 401位慢性头痛患者,主要是偏头痛患者。干预患者被随机分配进入针灸治疗组(在 3个月内接受12次针灸治疗)和对照组(接受常规治疗)。主要结果测定指标治疗前、治疗后3个月及12个月对头痛积分,SF-36健康状况及用药量进行评估。每3个月对治疗方法作1次评估。头痛积分即判定疗效的主要终点指标。结果针灸组治疗后12个月的头痛积分(针灸组161例患者,平均头痛积分为16．2,标准差 13．7,比治疗前减少34％)比对照组低(对照组140 例患者,平均头痛积分为22．3,标准差17．0,比治疗前减少16％)。两种方法调整后的差异为4．6 (95％可信区间为2．2-7．0;P=0．0002)。对于包含缺失数据的灵敏度分析,该结果也是肯定的。针灸组的患者每年少头痛22(8-38)天。尽管达到统计学差异的项目仅有生理功能、精力和健康改变等,SF-36的资料也有利于针灸组。与对照组相比,针灸组的患者减少了15％的服药量(P=0．02), 减少了25％的就诊量(P=0．10),同时减少15％的病休时间(P=0．2)。结论针灸在初级保健中用于治疗慢性头痛, 特别是偏头痛,可以产生与临床相关的持久益处。应当考虑扩大英国国家医疗保险体系中针灸治疗的范围。

Open-Source Hardware Is a Low-Cost Alternative for Scientific Instrumentation and Research

Scientific research requires the collection of data in order to study, monitor, analyze, describe, or understand a particular process or event. Data collection efforts are often a compromise: manual measurements can be time-consuming and labor-intensive, resulting in data being collected at a low frequency, while automating the data-collection process can reduce labor requirements and increase the frequency of measurements, but at the cost of added expense of electronic data-collecting instrumentation. Rapid advances in electronic technologies have resulted in a variety of new and inexpensive sensing, monitoring, and control capabilities which offer opportunities for implementation in agricultural and natural-resource research applications. An Open Source Hardware project called Arduino consists of a programmable microcontroller development platform, expansion capability through add-on boards, and a programming development environment for creating custom microcontroller software. All circuit-board and electronic component specifications, as well as the programming software, are open-source and freely available for anyone to use or modify. Inexpensive sensors and the Arduino development platform were used to develop several inexpensive, automated sensing and datalogging systems for use in agricultural and natural-resources related research projects. Systems were developed and implemented to monitor soil-moisture status of field crops for irrigation scheduling and crop-water use studies, to measure daily evaporation-pan water levels for quantifying evaporative demand, and to monitor environmental parameters under forested conditions. These studies demonstrate the usefulness of automated measurements, and offer guidance for other researchers in developing inexpensive sensing and monitoring systems to further their research.

Growth factor receptor-bound protein 2 （Grb2） is an essential intracellular adapter protein, consisting of one SH2 domain flanked by two SH3 domains. Grb2-SH2 domain mediates the activation of mitogenic Ras activation pathway through binding to a range of tyrosine-phosphorylated motifs on growth factor receptors such as the epidermal growth factor receptor （EGFR） and erbB2. An aberrant Grb2-depedent Ras activation pathway has been shown to be essential for cellular transformation in a subset of human tumors. Overexpression of Grb2 protein is also related to liver tumorigenesis in mice and human breast cancer cells. Therefore, molecules that can selectively inhibit the linkage of Grb2-SH2 domain to phosphorylated receptors are expected to be useful for the treatment and/or prevention of hyperproliferative diseases such as leukemias, breast and overian cancers. Based upon global modifications, a highly potent peptide ligand 8 was discovered with a Kd=0.36 μM, evaluated by Biacore binding assay.

Modified rives-stoppa repair for abdominal incisional hernias

Incisional hernias are a prevalent problem in abdominal surgery and occur in 11% of patients who undergo laparotomy. Primary suture clo-sure of incisional hernias results in a 31%-58% chance of recurrence. The addition of a pros-thetic mesh implant decreases recurrence rates to 8%-10%. Popularized in Europe by Rives and Stoppa, the sublay technique has proven to be very effective, with low recurrence rates (0%-23%) and minimal complications. The pur-pose of the study was to evaluate the experi-ence of a single surgeon at a large tertiary care center performing a modified Rives-Stoppa re-pair for abdominal incisional hernias. To do this, the records of all patients undergoing a modi-fied Rives-Stoppa incisional hernia repair be-tween January 2000 and August 2003 were ret-rospectively reviewed. Outpatient clinic notes, discharge summaries, operative reports, and laboratory data were reviewed for patient demographics, surgical data and postoperative complications. Univariate analysis was per-formed in order to identify predictors for recur-rence. During the study period, 83 patients un-derwent a modified Rives-Stoppa incisional hernia repair. Nineteen patients were excluded due to incomplete medical records. No patients required postoperative exploration for an in-tra-abdominal catastrophe. Twenty-five percent (n=16) of patients had a complication as a result of the hernia repair. Only two patients (3.1%) developed recurrent incisional hernias. History of diabetes (p=0.007) and benign prostatic hy-perplasia (p=0.000) were the only significant predictors for recurrence. The results presented here confirm that the modified Rives-Stoppa retromuscular repair is an effective method for the repair of incisional hernias. The complica-tion and recurrence rates compare favorably to results for currently popular alternative tech-niques.

The evolution of sex is one of the most important and controversial problems in evolutionary *** sex is almost universal in higher animals and plants,its inherent costs have made its maintenance difficult to ex*** most famous of these is the twofold cost of males,which can greatly reduce the fecundity of a sexual population,compared to a population of asexual *** the past century,multiple hypotheses,along with experimental evidence to support these,have been put forward to explain widespread costly *** this review,we outline some of the most prominent theories,along with the experimental and observational evidence supporting ***,there have been 4 classes of theories:the ability of sex to fix multiple novel advantageous mutants(fisher-Muller hypothesis);sex as a mechanism to stop the build-up of deleterious mutations in finite populations(Muller’s ratchet);recombination creating novel genotypes that can resist infection by parasites(Red Queen hypothesis);and the ability of sex to purge bad genomes if deleterious mutations act synergistically(mutational deterministic hypothesis).Current theoretical and experimental evidence seems to favor the hypothesis that sex breaks down selection interference between new mutants,or it acts as a mechanism to shuffle genotypes in order to repel parasitic ***,there is still a need to collect more data from natural populations and experimental studies,which can be used to test different hypotheses.
