Surgical management of facial dog bite trauma:A contemporary perspective and review

Objective:To perform a review evaluating management of and complications stemming from dog bite trauma sustained to the head and neck over the past decade.Data Sources:PubMed and cochrane Library.Methods:The authors searched the PubMed and cochrane Library databases for relevant published literature.A total of 12 peer-reviewed canine-exclusive series inclusive of 1384 patient cases describing facial dog bite trauma met inclusion criteria.Wounds including fractures,lacerations,contusions,and other soft-tissue injuries were evaluated.Demographics related to clinical course and management,operating room requirements,and antibiotic usage were compiled and analyzed.Initial trauma and surgical management complications were also assessed.Results:75.5%of patients sustaining dog bites required surgical intervention.Of these patients,7.8%suffered from postsurgical complications,including hypertrophic scarring(4.3%),postoperative infection(0.8%),or nerve deficits and persistent paresthesias(0.8%).Prophylactic antibiotics were administered to 44.3%of patients treated for facial dog bites and the overall infection rate was 5.6%.concomitant fracture was present in 1.0%of patients.conclusion:Primary closure,often in the OR may be necessary,with few cases requiring grafts or flaps.Surgeons should be aware that the most common complication is hypertrophic scarring.Further research is needed to elucidate the role of prophylactic antibiotics.

Sand wave migration near the southeastern corner of Martha's Vineyard, Massachusetts, USA

Sand waves of approximately 2 m in height were observed to migrate nearly 40 m with counterclockwise rotation between two bathymetric surveys performed three months apart near the southeastern corner of Martha’s Vineyard, Massachusetts. The region is characterized by strong tidal currents, intermittent energetic surface wave events, and shallow water with local depth ranging from 2 to 7 m. This study uses the process-based model, Delft3D, with a three-dimensional approach to examine the sand wave dynamics by incorporating surface waves, winds, currents, and bathymetric observations. The model successfully simulates sand wave migration in comparisons to observations. Model sensitivity analyses show that the sand wave migration reduces by 65% with the absence of the surface waves. The modeled sand wave migration speed is correlated with the tidal current Shields parameter, and sharp increases in migration speed occur when the wave-driven Shields parameter increases in response to energetic surface wave events. The combined effect of tides, surface waves, and bathymetry is the origin of the rotational aspect of the sand wave, using the Shields parameter as an indicator of tidal currents and surface wave influence on sand wave dynamics.

AN AccURATE TWO-DIMENSIONAL THEORY OF VIBRATIONS OF ISOTROPIc,ELASTIc PLATES

An infinite system of two-dimensional equations of motion of isotropic elastic plates with edge and corner conditions are deduced from the three-dimensional equations of elasticity by expansion of displacements in a series of trigonometrical functions and a linear function of the thickness coordinate of the plate. The linear term in the expansion is to accommodate the in-plane displacements induced by the rotation of the plate normal in low-frequency flexural motions. A system of first-order equations of flexural motions and accompanying boundary conditions are extracted from the infinite system. It is shown that the present system of equations is equivalent to the Mindlin＇s first-order equations, and the dispersion relation of straight-crested waves of the present theory is identical to that of the Mindlin＇s without introducing any corrections. Reduction of present equations and boundary conditions to those of classical plate theories of flexural motions is also presented.

There is a huge gap between(1)the state of workflow technology on the one hand and the practices in the many labs working with data driven methods on the other and(2)the awareness of the FAIR principles and the lack of changes in practices during the last 5 years.The cWFR concept has been defined which is meant to combine these two intentions,increasing the use of workflow technology and improving FAIR compliance.In the study described in this paper we indicate how this could be applied to machine learning which is now used by almost all research disciplines with the well-known effects of a huge lack of repeatability and reproducibility.Researchers will only change practices if they can work efficiently and are not loaded with additional tasks.A comprehensive cWFR framework would be an umbrella for all steps that need to be carried out to do machine learning on selected data collections and immediately create a comprehensive and FAIR compliant documentation.The researcher is guided by such a framework and information once entered can easily be shared and reused.The many iterations normally required in machine learning can be dealt with efficiently using cWFR methods.Libraries of components that can be easily orchestrated using FAIR Digital Objects as a common entity to document all actions and to exchange information between steps without the researcher needing to understand anything about PIDs and FDO details is probably the way to increase efficiency in repeating research workflows.As the Galaxy project indicates,the availability of supporting tools will be important to let researchers use these methods.Other as the Galaxy framework suggests,however,it would be necessary to include allsteps necessary for doing amachine learning task including those that require human interaction andtodocument all phases with the help of structured FDOs.

Investigating the Physiological Mechanisms between Resistance Training and Pain Relief in the cancer Population: A Literature Review

This literature review examines the mechanisms of how exercise, specifically in the form of resistance training, may lead to pain relief in the cancer population. Primary data from three different cancer populations: breast, prostate, and lung, will be examined. A number of experimental studies have been conducted to confirm the effectiveness of resistance training on pain relief as well as the biochemical pathways that relate to this process. In this review, we will examine 5 randomized controlled trials. For the purposes of this review, pain is defined as physical suffering or discomfort associated with illness. Pain is the body’s natural signal, bringing attention to damage that has been sustained by tissues. However, chronic pain is common in the cancer population, and often serves no good purpose but instead will negatively impact both physical and mental health. The three types of pain: nociceptive, neuropathic, and inflammatory pathways have been investigated, and the knowledge of pain mechanisms allows for the understanding of how it is associated with pain. The purpose of this exploratory literature review is to give insight on how to maximize pain-relieving effects of resistance training. Research has indicated that resistance training modulates pain pathways by upregulating the release of pain-relieving substances including beta-endorphins, anti-inflammatory cytokines, and endocannabinoids. Understanding of the benefits of resistance training may be useful in relieving cancer pain, and reproducing effects of pain-relieving strategies while minimizing the symptoms related to cancer and its treatment.

In semi-arid lands,vegetation is distributed in shrub patches immersed in a less vegetated interpatch matrix.Grazing affects perennial grass seed bank through a decrease in seed rain and an increase in seed predation and soil compaction.Nevertheless,some species with anchorage mechanisms in their seeds might overcome this,such as Nassella tenuis(Phil.)Barkworth.This is an important species in grazing paddocks because it has an intermediate palatability and its relatively tolerant to grazing.These characteristics allow N.tenuis to increase its abundance in grazed sites.Our objective was to assess how grazing affects the key palatable species from seeds to seedlings:i.e.,seed rain,soil seed bank,and seedling recruitment in different microsites along a windward-leeward transect across shrub canopy.We hypothesized that:(1)the negative effects of grazing on N.tenuis fructification are reflected in its seed rain,soil seed bank,and seedling recruitment,especially in interpatches;(2)Nassella tenuis seed rain reduction,soil compaction by cattle in grazed sites,and removal of seeds by wind decrease its soil seed bank,especially in microsites exposed to the predominant wind;and(3)the decrease in N.tenuis soil seed bank and cover increase in annual species in grazed sites have negative effects on its seedling recruitment,especially in microsites exposed to predominant wind.We placed seed traps,collected soil samples,and monitored seedling recruitment in different locations around shrub canopy to address our hypotheses.Also,we established a manipulative experiment in which we sow N.tenuis seeds and followed its recruitment in different microsites.We compared the seed rain,soil seed bank,natural seedling recruitment,and sown seeds recruitment of N.tenuis between grazed and ungrazed sites.We analyzed differences between microsites along a windward-leeward transect across shrubs patches.Seed rain and soil seed bank had the same density in patches and interpatches both in ungrazed and grazed sites.But seed rain was higher,and soi

Small bowel MRI enteroclysis or follow through:Which is optimal?

AIM:To determine if a nasojejunal tube(NJT) is required for optimal examination of enteroclysis and if patients can be examined only in the supine position.METHODS:Data were collected from all patients undergoing small bowel(SB) magnetic resonance imaging(MRI) examination over a 32-mo period.Patients either underwent a magnetic resonance(MR) follow-through(MRFT) or a MR enteroclysis(MRE) in the supine position.The quality of proximal and distal SB distension as well as the presence of motion artefact and image quality were assessed by 2 radiologists.RESULTS:One hundred and fourteen MR studies were undertaken(MRFT-49,MRE-65) in 108 patients in the supine position only.Image artefact was more frequent in MRE than in MRFT(29.2% vs 18.4%) ,but was not statistically significant(P = 0.30) .Adequate distension of the distal SB was obtained in 97.8% of MRFT examinations and in 95.4% of MRE examinations,respectively.Proximal SB distension was,however,less frequently optimal in MRFT than in MRE(P = 0.0036) ,particularly in patients over the age of 50 years(P = 0.0099) .Image quality was good in all examinations.cONcLUSION:All patients could be successfully imaged in the supine position.MRE and MRFT are equivalent for distal SB distension and artefact effects.Proximal SB distension is frequently less optimal in MRFT than in MRE.MRE is,therefore,the preferred MR examination method of the SB.

Personalized medicine in non-small cell lung cancer: a review from a pharmacogenomics perspective

Non-small cell lung cancer is a prevalent and rapidly-expanding challenge to modern medicine. While generalized medicine with traditional chemotherapy yielded comparatively poor response rates and treatment results, the cornerstone of personalized medicine using genetic profiling to direct treatment has exalted the successes seen in the field and raised the standard for patient treatment in lung and other cancers. Here, we discuss the current state and advances in the field of personalized medicine for lung cancer, reviewing several of the mutation-targeting strategies that are approved for clinical use and how they are guided by patient genetic information. These classes include inhibitors of tyrosine kinase(TKI), anaplastic lymphoma kinase(ALK), and monoclonal antibodies. Selecting from these treatment plans and determining the optimal dosage requires in-depth genetic guidance with consideration towards not only the underlying target genes but also other factors such as individual metabolic capability and presence of resistance-conferring mutations both directly on the target gene and along its cascade(s).Finally, we provide our viewpoints on the future of personalized medicine in lung cancer, including target-based drug combination, mutation-guided drug design and the necessity for data of population genetics, to provide rough guidance on treating patients who are unable to get genetic testing.

Disagreement between symptom-reflux association analysis parameters in pediatric gastroesophageal reflux disease investigation

AIM: To assess the agreement within 3 commonly used symptom-reflux association analysis (SAA) parameters investigating gastroesophageal reflux disease (GERD) in infants. METHODS: Twenty three infants with suspected GERD were included in this study. Symptom index (SI), Symptom sensitivity index (SSI) and symptom association probability (SAP) related to cough and irritability were calculated after 24 h combined pH/multiple intraluminal impedance (MII) monitoring. Through defined cutoff values, SI, SSI and SAP values are differentiated in normal and abnormal, whereas abnormal values point towards gastroesophageal reflux (GER) as the origin of symptoms. We analyzed the correlation and the concordance of the diagnostic classification of these 3 SAA parameters.RESULTS: Evaluating the GER-irritability association, SI, SSI and SAP showed non-identical classification of normal and abnormal cases in 39.2% of the infants. When irritability was taken as a symptom, there was only a poor inter-parameter association between SI and SSI, and between SI and SAP (Kendall’s tau b = 0.37, P ctively). Evaluating the GER-cough association, SI, SSI and SAP showed non-identical classification of normal and abnormal cases in 52.2% of the patients. When cough was taken as a symptom, only SI and SSI showed a poor inter-parameter association (Kendall’s tau b = 0.33, P cONcLUSION: In infants investigated for suspected GERD with pH/MII-monitoring, SI, SSI and SAP showed a poor inter-parameter association and important dis-agreements in diagnostic classification. These limitations must be taken into consideration when interpreting the results of SAA in infants.

climate change is making it more challenging to meet the food demands of a growing global population.Increased food produc-tion relies on continual crop improvements to generate higher and more stable yields,especially with increasingly unpredictable environments and less arable land.The improvement of traits that promote climate resilience and resource utilization,for example,greater photosynthetic capacity,increased nitrogen use efficiency,and optimized root and shoot architecture,repre-sents a promising avenue for engineering crops to yield more with less(Evans and Lawson,2020).A key challenge for crop engineering is optimizing performance in specific environments.
