State of telemedicine use in pediatric surgery in the USA-where we stand and what we can gain from the cOVID-19 pandemic: a scoping review

Background Within the last decade,the use of telemedicine within in primary care in the USA has greatly expanded;however,use remains uncommon in surgical specialties.The spread of coronavirus disease 2019(cOVID-19)prompted healthcare institutions to limit in-person contact,resulting in an increase in telemedicine across all specialties,including pediatric surgery.The aims of this review were to evaluate potential barriers that limited the use of telemedicine in pediatric surgery prior to the cOVID-19 period and to define how best to incorporate its use into a pediatric surgical practice going forward.Methods A scoping review was performed to identify gaps in the literature pertaining to the use of telemedicine within general pediatric surgery in the USA prior to the outbreak of cOVID-19.Next,a focused evaluation of the legislative and organizational policies on telemedicine was performed.Lastly,findings were summarized and recommendations for future research developed in the context of understanding and overcoming barriers that have plagued widespread adoption in the past.Results Despite evidence of telemedicine being safe and well received by adult surgical patients,a total of only three studies representing original research on the use of telemedicine within pediatric surgery were identified.Legislative and organizational policies regarding telemedicine have been altered in response to cOVID-19,likely resulting in an increase in the use of telemedicine among pediatric surgeons.conclusions Telemedicine offers a safe and effective option for patients seeking an alternative to the in-person clinic appointment.The increased utilization of telemedicine during the cOVID-19 pandemic will provide an opportunity to learn how best to leverage the technology to decrease disparities and to overcome previous barriers.

Pediatric dog bite injuries in the USA: a systematic review

Introduction Dog bites are one of the leading causes of non-fatal emergency room visits in children.These injuries not only cause physical harm but can lead to long-term psychological stress.This study evaluated the current literature related to pediatric dog bite injuries to identify research gaps which should be prioritized to improve a major public health concern.Methods We performed a keyword search of PubMed,Scopus,and OVID Medline databases(January 1980-March 2020)for all published studies focused on dog bite injuries in the pediatric population(≤18 years of age)using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses.Results Out of 1859 abstracts screened,43 studies involving 86880 patients were included.Twenty-nine studies were retrospective chart reviews characterizing the epidemiology of dog bites and their associated treatment outcomes;six were prospective cohort studies;two were cross-sectional studies;and six were experimental studies.Synthesized results demonstrate that childrenchildrenck,and face.conclusion Studies analyzing the prevention or psychosocial consequences of dog bites injuries are needed.

Effect of restricted feeding on hen performance,egg quality and organ characteristics of individual laying hens

This study was to assess the impact of permanent or temporary restricted feeding on laying hen production traits,physiology,and egg quality.Two hundred and forty individually housed ISA Brown hens were monitored across 2 phases,assigned to 3 treatments:ad libitum feeding(ALF),temporary restricted feeding(TRF)and permanent restricted feeding(PRF),n=80 hens per treatment.In Phase 1(P1),22 to40 weeks,the TRF and PRF hens were offered 115 g of feed daily.In Phase 2(P2),41 to 46 weeks,the TRF hens were transitioned to ALF status while the ALF and PRF hens remained as in P1.From 35 to 40 weeks,eggs were collected once weekly from 15 hens per treatment and assessed for differences in albumen,yolk,and shell variables.At 45 weeks,10 hens each from the ALF and PRF groups were euthanized and differences in organ characteristics were assessed.In P1,feed intake,feed to egg conversion ratio and body weight(BW)change were lower(Pcompared to hens allocated the ALF treatment.In P2,TRF and ALF hens had a higher egg production and egg mass than PRF(Pchange in P2 was higher in TRF and similar in both ALF and PRF,while feed intake and feed conversion ratio were higher in TRF followed by ALF and least in the PRF treatment group(Pc syndrome lesion score compared to PRF.Restricting hens to 115 g of feed per day from point of lay restrained BW,improved feed conversion ratio and albumen quality and reduced abdominal fat pad deposition and clinical signs of fatty liver haemorrhagic syndrome in individually housed laying hens.

Quasi-variable Discount Rates and Market Volatility: An Empirical Study

This paper proposes an assumption of quasi-variable discount rates to explain the excess volatility puzzle of stock market. Under the assumption, the ARMAX model is derived based on the ccAPM model and cRRA utility function to describe the linear relationship between the discount rate and the consumption growth rate. We conducted empirical research on this model using historical data of the US stock market. The results confirm a significantly negative relationship between consumption growth rate and discount rate. Subsequently, the results of Monte carlo simulation show that given the risk preference coefficient and dividend sequence, the rational expectations price fluctuation obtained under the assumption of quasivariable discount rate is the largest.

Background:Esports players,like traditional athletes,practice for long hours and,thus,are vulnerable to the negative health effects of prolonged sitting.There is a lack of research on the physical activity and the health ramifications of prolonged sitting by competitive players.The purpose of this study was to investigate activity levels,body mass index(BMI),and body composition in collegiate esports players as compared to agematched controls.Methods:Twenty-four male collegiate esports players and non-esports players between 18 and 25 years of age signed a written consent to participate.Physical activity was examined using daily activity(step count)with a wrist-worn activity tracker.A questionnaire assessing physical activity was also administered.Secondary outcomes included body-fat percentage,lean-body mass,BMI,and bone mineral content measured using dual X-ray absorptiometry.Results:The step count in the esports players was significantly lower than the age-matched controls(6040.2±3028.6 vs.12843.8±5661.1;p=0.004).Esports players exhibited greater body-fat percentage(p=0.05),less lean body mass(p=0.003),and less bone mineral content(p=0.03),despite no difference in BMI between the esports and non-esports players.conclusion:As compared to non-esports players,collegiate esports players were significantly less active and had a higher body-fat percentage,with lower lean body mass and bone mineral content.The BMIs showed no difference between the 2 groups.Esports athletes displayed significantly less activity and poor body composition,which are all correlated with potential health issues and risk of injury.BMI did not capture this difference and should not be considered as an accurate measure of health in competitive esports players.

Does a shift in shade tolerance as suggested by seedling morphology explain differences in regeneration success of northern red oak in native and introduced ranges?

Across North America, forests dominated by Quercus rubra L. (northern red oak), a moderately shade-tolerant tree species, are undergoing successional replacement by shade-tolerant competitors. Under closed canopies, Q. rubra seedlings are unable to compete with these shade-tolerant species and do not recruit to upper forest strata. In Europe, natural regeneration of introduced Q. rubra is often successful despite the absence of fire, which promotes regeneration in the native range. considering that understorey light availability is a major factor affecting recruitment of seedlings, we hypothesized that Q. rubra seedlings are more shade tolerant in the introduced range than in the native range. Morphological traits and biomass allocation patterns of seedlings indicative of shade tolerance were compared for Q. rubra and three co-occurring native species in two closed-canopy forests in the native range (Ontario, canada) and introduced range (Baden-Württemburg, Germany). In the native range, Q. rubra allocated a greater proportion of biomass to roots, while in the introduced range, growth and allocation patterns favored the development of leaves. Q. rubra seedlings had greater annual increases in height, diameter and biomass in the introduced range. Q. rubra seedlings in the introduced range were also younger;however, they had a mean area per leaf and a total leaf area per seedling that were five times greater than seedlings in the native range. Such differences in morphological traits and allocation patterns support the hypothesis that Q. rubra expresses greater shade tolerance in the introduced range, and that natural regeneration of Q. rubra is not as limited by shade as in the native range. The ability of Q. rubra seedlings to grow faster under closed canopies in Europe may explain the discrepancy in regeneration success of this species in native and introduced ranges. Future research should confirm findings of this study over a greater geographical range in native and introduced ecosystems, and examine the genet

A 20-Year Follow-up after the Fontan Operation in a Population with Hypoplastic Left Heart Syndrome

Background:Thromboembolic events are a cause of significant morbidity and mortality in the Fontan population.We previously reported on coagulation profile changes in a cohort of patients with hypoplastic left heart syndrome(HLHS)from Stage I through Fontan completion.In this report,we examine their clinical status,anticoagulation and incidence of thromboembolic events up to 20 years post Fontan.Methods:A retrospective chart review was conducted for twenty(20)surviving patients,from 1998 through December 2020.Patients who underwent orthotopic heart transplantation(OTx)were followed until their transplant.Patients who were found in the original study to have a factor VIII activity level>160%,were examined separately.Results:Most patients had follow-up within the last two years(2018–2020).Two patients underwent OTx and two patients died.Anticoagulation strategy was variable.Most patients were on aspirin monotherapy.There was a total of twelve thrombotic events(63.2%).These included six cerebrovascular accidents(two of which were fatal).Three out of the seven patients with elevated factor VIII activity from the original study had thromboembolic events(42.9%).Fontan complications were variable.Some degree of Fontan Associated Liver Disease was universal.conclusions:This retrospective review of a group of single-ventricle patients post Fontan,illustrates the variability in anticoagulation therapy that exists in this population.A large proportion of patients suffered a significant thromboembolic event,including the patients with elevated factor VIII.Further investigation into the patients with elevated factor VIII may help determine whether a different antithrombotic strategy post Fontan would be beneficial.
