[背景]人类在生命早期的发育期间可能特别容易受到环境暴露的影响。有关这一主题的人体研究一般集中在单一暴露与健康效应之间的关系。而"暴露组"的概念涵盖了从受孕开始的的全部暴露,完善了基因组。[目的]人类生命早期暴露组(HELIX)项目是一个新的合作研究项目,目的是为了采用新的暴露评估和生物标志分析方法来描述生命早期多种环境因素的暴露,并将这些与生物标志组学和儿童健康结局相关联,从而刻画"生命早期暴露组"。本文描述了该项目的总体设计。[方法]HELIX将利用欧洲现有的6个出生队列研究来估计产前、产后暴露的一系列化学和物理暴露。建立全部队列中总共32 000对母亲和儿童的暴露模型,并在一个包含1 200对母亲和儿童的子队列中测量生物标志。嵌套重复采样的定组研究(n=150)将收集生物标志的变化数据,利用智能手机来评估流动性和体力活动,并监测个体暴露。采用组学技术确定与暴露相关的分子学特征(代谢组、蛋白质组、转录组和表观基因组)。对于多次暴露,采用统计方法估计胎儿和儿童的成长、肥胖、神经发育和呼吸系统结局中的暴露-效应关系。项目还将进行一项健康效应评估测试,以评价组合暴露的风险和收益。[结论]HELIX是描述欧洲人群生命早期暴露组并解开它与组学标志物和儿童健康之间关联的首次尝试之一。作为对暴露组学这一概念的验证,该项目向生命过程中暴露组迈出了重要的第一步。

背景有症状颅内动脉粥样硬化性疾病（intraeranial atherosclerotic disease，ICAD）是全世界范围内最常见的卒中原因之一。研究显示，严重ICAD患者的复发性卒中风险很高，亟需有效的预防手段。药物治疗是治疗ICAD的基础，对所有患者都至关重要。血管内治疗也是一种潜在的治疗选择，但在最佳的技术和患者选择方面仍需进一步探索。本指南旨在为ICAD患者提供诊断和血管内治疗的推荐意见。方法进行文献回顾，以选#2000年至2011年期间发表的有关ICAD的文献。根据美国心脏协会（AmericanHeartAssociation，AHA）庄荚国卒中协会标准对证据进行评估和分类。在神经介入外科学学会标准委员会对现有证据进行评价的基础上制定推荐意见。按照美国神经病学学会（AmericanAcademyofNeurology，AAN）、AHA卒中委员会以及牛津大学循证医学中心（CentreforEvidencebasedMedicine，CEbM）建议的循证医学指南进行评估。结果总共选定59篇文献。支架置人术与强化药物治疗预防颅内动脉狭窄患者复发性卒中（StentingversusAggressiveMedicalManagementforPreventingRecurrentStrokeinIntracranial＆enosis。SAMMPRIS）研究是现有的唯一一项前瞻性随机对照试验，文献的证据级别被归类为AHA b级、AANⅡ级和CEbM1b级。有症状椎动脉或颅内动脉粥样硬化性疾病支架置人术（StentingofSymptomaticAtheroscleroticLesionsintheVertebralorIntracranialarteries，SSYLVtA）试验是由独立的神经科医生进行转归评价的一项前瞻性非随机研究，文献的证据级别被归类为AHAb级、AANⅢ级和CEbM2级。其他研究均为非对照性或未进行客观的转归评价，因此证据级别被归类为AHAC级、AAHIV级和CEbM4级。结论对于有症状ICAD患者，阿司匹林＋氯毗格雷联合治疗3个月和强化控制危险因素是一线治疗方案。对于经过选择的有症状ICAD患者，球囊血管成形／支架置入术是一种可供选择的治疗方案。需要进一步研究，以确定合适的患者选择标准以及针对各种亚组患者的最佳治疗方案。

1引言图1为位于宽度为b的河流或人工渠中的水轮机。我们分两种不同情况讨论:一种是装在有坝的河床式电站的水轮机(见文中第2部分),而且坝拦截了整个河道(阻塞比σ=1);另一种是自由流动环境中仅利用水流动能的水轮机(见文中第3部分),会有分流存在(σ<1)。这两种水轮机都受到了类似拓扑结构的限制。因为这类水轮机放置在任何浅河道中,流量可以显著改变,尤其高阻塞比下流量变化更大。

AIM: To evaluate daclatasvir vs telaprevir, each combined with peginterferon alfa-2a/ribavirin(peg IFN/RbV), in treatment-naive hepatitis C virus(HCV) genotype(GT) 1-infected ***: In this phase 3, randomized, open-label, noninferiority study, 602 patients were randomly assigned(2:1) to daclatasvir vs telaprevir, stratified by IL28 b rs12979860 host genotype(CC vs non-CC), cirrhosis status(compensated cirrhosis vs no cirrhosis), and HCV GT1 subtype(GT1a vs GT1b). Patients were selected by study inclusion criteria from a total of 793 enrolled patients. Patients received daclatasvir 60 mg once daily or telaprevir 750 mg 3 times daily plus peg IFN/RbV. Daclatasvir recipients received 24 wk of daclatasvir plus peg IFN/RbV; those without an extended rapid virologic response(e RVR; undetectable HCV-RNA at weeks 4 and 12) received an additional 24 wk of peg IFN/RbV. Telaprevir-treated patients received 12 wk of telaprevir plus peg IFN/RbV followed by 12(with e RVR) or 36(no e RVR) wk of peg IFN/RbV. The primary objective was to compare for noninferiority of sustained virologic response rates at posttreatment week 12(SVR12) in GT1b-infected patients. Key secondary objectives were to demonstrate that the rates of anemia(hemoglobin bV than with telaprevir + peg IFN/RbV among GT1b-infected patients. Resistance testing was performed using population-based sequencing of the NS5 A region for all patients at baseline, and for patients with virologic failure or relapse and HCV-RNA ≥ 1000 IU/m L, to investigate any link between NS5 A polymorphisms associated with daclatasvir resistance and virologic outcome. RESULTS: Patient demographics and disease characteristics were generally balanced across treatment arms; however, there was a higher proportion of black/African Americans in the daclatasvir groups(6.0% and 8.2% in the GT1 b and GT1 a groups, respectively) than in the telaprevi

本文报告 195 9～ 1998年青海高原人与动物多房棘球绦虫感染情况的研究结果。共手术治疗泡型包虫病人111例 ;男性 73例 ,女性 38例 ;其中肝泡球蚴病 10 0例 ,肺泡球蚴病 2例 ,脑泡球蚴病 7例 ,脾泡球蚴病 2例。年龄17～ 74岁 ,成年人最为常见。牧民和农民最多见 ,其他职业少见。普查 370 2人 ,b超诊断为肝泡球蚴病者 2 3人( 0 .6 2 % ) ,EM1 8血清学阳性者 30人 ( 0 .81% ) ,两种方法均符合者 13人 ( 0 .35 % )。调查中间宿主 7种 ,在家养动物牦牛肝脏和肺脏及藏绵羊肝脏发现泡球蚴感染 ,感染率分别为 4.0 9% ( 18/ 384)和 5 .36 % ( 31/ 5 78) ;野生动物黑唇鼠兔的肝脏和肺脏及灰尾兔的肺脏发现泡球蚴的感染 ,其感染率分别为 3.45 % ( 11/ 319)和 12 .5 0 % ( 1/ 8)。调查终宿主 5种 ,在藏犬和藏狐肠内均证实多房棘球绦虫感染 ,其感染率分别为 5 .0 8% ( 3/ 5 9)和 33.33% ( 4/ 12 )。提示青海高原存在多房棘球绦虫的藏犬、藏狐 /黑唇鼠兔、灰尾兔和藏犬、藏狐 /牦牛。

Dothideomycetes comprise a highly diverse range of fungi characterized mainly by asci with two wall layers(bitunicate asci)and often with fissitunicate *** species are saprobes,with many asexual states comprising important plant *** are also endophytes,epiphytes,fungicolous,lichenized,or lichenicolous *** occur in terrestrial,freshwater and marine habitats in almost every part of the *** accept 105 families in Dothideomycetes with the new families Anteagloniaceae,bambusicolaceae,biatriosporaceae,Lichenoconiaceae,Muyocopronaceae,Paranectriellaceae,Roussoellaceae,Salsugineaceae,Seynesiopeltidaceae and Thyridariaceae introduced in this *** family is provided with a description and notes,including asexual and asexual states,and if more than one genus is included,the type genus is also *** family is provided with at least one figure-plate,usually illustrating the type genus,a list of accepted genera,including asexual genera,and a key to these genera.A phylogenetic tree based on four gene combined analysis add support for 64 of the families and 22 orders,including the novel orders,Dyfrolomycetales,Lichenoconiales,Lichenotheliales,Monoblastiales,Natipusillales,Phaeotrichales and *** paper is expected to provide a working document on Dothideomycetes which can be modified as new data comes to *** is hoped that by illustrating types we provide stimulation and interest so that more work is carried out in this remarkable group of fungi.

Targets for high repetition rate laser facilities:needs,challenges and perspectives

A number of laser facilities coming online all over the world promise the capability of high-power laser experiments with shot repetition rates between 1 and 10 Hz. Target availability and technical issues related to the interaction environment could become a bottleneck for the exploitation of such facilities. In this paper, we report on target needs for three different classes of experiments: dynamic compression physics, electron transport and isochoric heating, and laser-driven particle and radiation sources. We also review some of the most challenging issues in target fabrication and high repetition rate operation. Finally, we discuss current target supply strategies and future perspectives to establish a sustainable target provision infrastructure for advanced laser facilities.
