Transcatheter versus surgical aortic valve replacement in severe, symptomatic aortic stenosis

Aortic stenosis （AS） is the most common type of valvular heart disease in the elderly. Surgical aortic valve replacement （SAVR） has been the standard practice for treating severe, symptomatic AS, but recently new treatment options have emerged, Transcatheter aortic valve replacement （TAVR） is now an established treatment option in patients at high surgical risk. In this review, we focus on recent developments and compare the two treatment methods in specific populations in terms of efficacy and safety （e.g., in patients with history of prior thoracic surgery, type of anesthesia employed, access site used or need for permanent pacing）. The impact of comorbidities （pulmonary hypertension, arterial hypertension and obesity paradox）, the cost-effectiveness of TAVR vs. SAVR and advances in transcatheter valve technology as well as issues that merit further investigation are further discussed. Moreover, outcomes and complications of TAVR in patients of different risk category （extremely high, high, intermediate and low risk） are analyzed. We strongly believe that during the following years, TAVR may evolve as the treatment of choice in a broader group of patients with symptomatic AS and beyond those with intermediate and high-risk features.

1 Introduction Atrial fibrillation （AF） constitutes the most frequent cardiac arrhythmia with an increasing prevalence and incidence in the general population, demonstrating a significant impact on cardiovascular morbidity and mortality. AF has been correlated with an increased risk of stroke, systemic embolism and long term morbidity compared to individuals with sinus rhythm. Extending the decision-making process during yellow phase from human drivers to autonomous vehicles:A microsimulation study with safety considerations

One of the main factors affecting the safety of signalised intersections is the stop/go behaviour during the yellow *** previous research has exhaustively examined drivers’stop/go decision-making,the expected autonomous vehicles’(AVs’)stop/go behaviour has not yet been thoroughly *** a series of simulation experiments developed for conventional and autonomous vehicles using different carfollowing,lane-changing,lateral placement and stop/go model parameter values,we examine here whether the default VISSIM stop/go parameter values can adequately replicate the observed drivers’behaviour at the considered intersection and assess the suitability of using the currently available options,albeit referring to human drivers,to simulate the expected stop/go behaviour of *** also propose a policy framework for determining the desired behaviour of AVs in yellow interval,which is integrated into an AVs logic and achieved in the last simulation to explore the effect of automation on the stop/go outcome and,hence,on the safety level of signalised *** data analysis and modeling techniques were used for the formulation of certain scenarios,including binary choice *** default stop/go parameter values were found unfit to replicate the observed stop/go behaviour and subjected to *** to the currently available options,the proposed AVs logic proved to produce the most accurate results,in terms of the stop/go simulation *** the impact of automation on the stop/go outcome,the simulation experiments showed that AVs preferred a more conservative behaviour in favor of road safety,as indicated by the significant reduction(≈15%)in the number of vehicles crossing the stop line during the yellow light and zero instances of red light ***,compared to the conservative drivers represented by the default stop/go parameter values,AVs preferred a more rational behaviour in favor of intersection capacit

This work deals with the experimental study of a surface dielectric-barrier discharge,as a part of the ongoing interest in the control of plasma induced electro-fluid dynamic effects(*** actuators).The discharge is generated using a plasma reactor consisting of a fused silica plate which is sandwiched between two printed circuit boards where the electrodes are *** reactor is driven by narrow high voltage square pulses of asymmetric rising(25 ns)and falling(2.5μs)parts,while the discharge evolution is considered in a temporarily and spatially resolved manner over these *** is,conventional electrical and optical emission analyzes are combined with high resolution optical emission spectroscopy and ns-resolved imaging,unveiling main characteristics of the discharge with a special focus on its propagation along the dielectric-barrier *** voltage rising part leads to cathode-directed ionization waves,which propagate with a speed up to 105m s~(-1).The voltage falling part leads to cathode sheath formation on the driven electrode.Τhe polarization of the dielectric barrier appears critical for the discharge dynamics.

Scalable Distributed Sensor Fault Diagnosis for Smart Buildings

The enormous energy use of the building sector and the requirements for indoor living quality that aim to improve occupants' productivity and health, prioritize Smart Buildings as an emerging technology. The Heating, Ventilation and Air-Conditioning(HVAC) system is considered one of the most critical and essential parts in buildings since it consumes the largest amount of energy and is responsible for humans comfort. Due to the intermittent operation of HVAC systems, faults are more likely to occur, possibly increasing eventually building's energy consumption and/or downgrading indoor living quality. The complexity and large scale nature of HVAC systems complicate the diagnosis of faults in a centralized framework. This paper presents a distributed intelligent fault diagnosis algorithm for detecting and isolating multiple sensor faults in large-scale HVAC *** the HVAC system as a network of interconnected subsystems allows the design of a set of distributed sensor fault diagnosis agents capable of isolating multiple sensor faults by applying a combinatorial decision logic and diagnostic reasoning. The performance of the proposed method is investigated with respect to robustness, fault detectability and scalability. Simulations are used to illustrate the effectiveness of the proposed method in the presence of multiple sensor faults applied to a 83-zone HVAC system and to evaluate the sensitivity of the method with respect to sensor noise variance.
