Oncologic aspects of the decision-making process for surgical approach for colorectal liver metastases progressing during chemotherapy

colorectal cancer represents the third most diagnosed malignancy in the *** liver is the main site of metastatic disease,affected in 30%of patients with newly diagnosed *** resection is considered the only potentially curative treatment for colorectal liver metastasis(cRLM),with a 5-year survival rate ranging from 35%to 58%.However,up to 80%of patients have initially unresectable disease,due to extrahepatic disease or bilobar multiple liver *** availability of increasingly effective systemic chemotherapy has contributed to converting patients with initially unresectable liver metastases to resectable disease,improving long-term outcomes,and accessing tumor *** recent years,response to preoperative systemic chemotherapy before liver resection has been established as a major prognostic *** studies have demonstrated that patients with regression of hepatic metastases while on chemotherapy have improved outcomes when compared to patients with stabilization or progression of the *** if disease progression during chemotherapy represents an independent negative prognostic factor,some patients may still benefit from surgery,given the role of this modality as the main treatment with curative intent for patients with *** selected cases,based on size,the number of lesions,and tumor markers,surgery may be offered despite the less favorable prognosis and as an option for non-chemo responders.

Pestalotiopsis is a taxonomically confused,pathogenic and chemically creative genus requiring a critical reexamination using a multi-gene phylogeny based on ex-type and ex-epitype *** this study 40 isolates of Pestalotiopsis,comprised of 28 strains collected from living and dead plant material of various host plants from china were studied by means of morphology and analysis of ITS,β–tubulin and tef1 gene sequence *** on molecular and morphological data we describe 14 new species(Pestalotiopsis asiatica,***,***,***,***,***,***,***,***,***,***,***,*** and ***)and three species are epitypified(***,*** and ***).Of the 10 gene regions(AcT,β-tubulin,cAL,GPDH,GS,ITS,LSU,RPB 1,SSU and tef1)utilized to resolve cryptic Pestalotiopsis species,ITS,β–tubulin and tef1 proved to be the better *** other gene regions were less useful due to poor success in PcR amplification and/or in their ability to resolve species *** a single gene tef1 met the requirements for an ideal candidate and functions well for species delimitation due to its better species resolution and PcR ***β-tubulin showed fairly good differences among species,a combination of ITS,β-tubulin and tef1 gene data gave the best resolution as compared to single gene *** work provides a backbone tree for 22 ex-type/epitypified species of Pestalotiopsis and can be used in future studies of the genus.

A sample preparation method based on ultrasound assisted-extraction （UAE） of ca, Mg and P from swine feed has been described. The experiment was performed to cover the variables influencing the sonication process and, the method validation using standard reference material. Final solutions obtained upon sonication were analyzed by flame atomic absorption spectrometry （for ca and Mg） and by UV-vis spectrophotometry （for P）. The best conditions for metal extraction were as follows： sample mass： 100 mg in 20 mL 0.10 mol/L HcI, a particle size：〈60 lam, sonication time： 5 cycles of 10 s and ultrasound power： 102 w. The UAE method was applied in digestibility assays in different piglet feeds and their results showed that it is highly comparable （P 〉 0.05） to the other methods used for such purposes, as block digestion, and offered a ca, Mg and P method of quantification limit of 10.6, 12.4 and 14 mg/kg, respectively. The major advantages of the UAE method compared to other methods are the high treatment rate, low reagent usage in the extracts and, it does not generate toxic residues that might negatively affect human health and the environment, accompanied by good precision and accuracy.

The hydraulic fracturing is a nonlinear,fluid-solid coupling and transient problem,in most cases it is always time-consuming to simulate this process *** recent years,although many numerical methods were proposed to settle this problem,most of them still require a large amount of computer *** it is a high demand to develop more efficient numerical approaches to achieve the real-time monitoring of the fracture geometry during the hydraulic fracturing *** this study,a reduced order modeling technique namely Proper Generalized Decomposition(PGD),is applied to accelerate the simulations of the transient,non-linear coupled system of hydraulic fracturing problem,to match this extremely tight response time *** separability of the solution in space and time dimensions is studied for a simplified model *** solid and fluid equations are coupled explicitly by inverting the solid discrete problem,and a simple iterative procedure to handle the non-linear characteristic of the hydraulic fracturing problem is proposed in this *** validation illustrates that the results of PGD match well with these of standard finite element method in terms o f fracture opening and fluid pressure in the ***,after the off-line calculations,the numerical results can be obtained in real time.

Despite international efforts to limit worker exposure to coal dust,it continues to impact the health of thousands of miners across *** coal dust has been studied to improve risk models and its control to protect *** size distribution analyses shows that using spraying systems to suppress airborne dusts can reduce particulate matter concentrations and that coals with higher ash yields produce finer *** are marked chemical differences between parent coals and relatively coarse deposited dusts(up to _(500)μm,DD_(500)).Enrichments in ca,K,Ba,Se,Pb,cr,Mo,Ni and especially As,Sn,cu,Zn and Sb in the finest respirable dust fractions could originate from:(i)mechanical machinery wear;(ii)variations in coal mineralogy;(iii)coal fly ash used in shotcrete,and carbonates used to reduce the risk of *** enrichments in ca in mine dusts are attributed to the use of such concrete,and elevated K to raised levels of phyllosilicate mineral *** concentrations are higher in the parent coal than in the DD_(500),probably due to relatively lower levels of organic *** concentrations of all elements observed in this study remained below occupational exposure limits.

colorectal cancer represents the third most common cancer and about 20%are diagnosed with synchronous metastatic *** a historical point of view,surgery remains the mainstream treatment for resectable colorectal liver metastases(cRLM).Furthermore,disease outcomes are improving due significant advances in systemic treatments and diagnostic ***,the optimal timing for neoadjuvant chemotherapy or upfront surgery for cRLM has not yet been established and remains an open ***,patient selection combining image workouts,time of recurrence,positive lymph nodes,and molecular biomarkers can improve the decision-making ***,molecular profiling is rising as a promising field to be incorporated in the multimodal approach and guide patient selection and sequencing of *** biomakers,genetic profiling,and circulating tumor DNA have been used to offer as much personalized treatment as possible,based on the precision oncology concept of tailored care rather than a guideline-based *** review article discusses the role of molecular pathology and biomarkers as prognostic and predictor factors in the diagnosis and treatment of resectable cRLM.

Phospholipase c zeta(PLcζ)is a sperm-specific protein that triggers oocyte *** analysis of PLcζexpression in human spermatozoa can be used as a diagnostic marker for oocyte activation *** laboratory has previously optimized a standard"in-house"assay to determine PLcζexpression in human ***,one study has suggested that an antigen unmasking method(AUM)would be more efficient in visualizing PLcζin human *** study aimed to compare our established assay and AUM(involving Hcl,acidic Tyrode's solution[AT],and heat).The mean relative fluorescence(RF)intensity of PLcζin frozen-thawed spermatozoa from fourteen fertile donors stained with the in-house method was significantly higher than three other AUM groups(in-house[mean±standard error of mean]:18.87±2.39 arbitrary units[a.u.]vs non-AUM:11.44±1.61 a.u.,AT-AUM:12.38±1.89 a.u.,and Hcl-AUM:12.51±2.16 a.u.,Pway analysis of variance).The mean RF intensity of PLcζin AT-and Hcl-treated spermatozoa from 12 infertile males was not significantly different from that of the non-AUM ***,the in-house method resulted in the highest RF intensity(12.11±1.36 a.u.,Pcificity testing of antibody-antigen binding indicated that the in-house method showed more specific binding than spermatozoa treated by the *** conclusion,our in-house method showed superior visualization and reliability than the AUM,thus supporting the continued use of our in-house assay for clinical research screening.
