AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHODS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,*** was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group B>50 years(n=126) .Mean patient follow-up was 15 mo. RESULTS:The median age was 60 years.A significantly higher proportion of patients in group B presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .Recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency *** multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CONCLUSION:Based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.

Hybrid simulation has been shown to be a cost-effective approach for assessing the seismic performance of structures. In hybrid simulation,critical parts of a structure are physically tested,while the remaining portions of the system are concurrently simulated computationally,typically using a finite element model. This combination is realized through a numerical time-integration scheme,which allows for investigation of full system-level responses of a structure in a cost-effective manner. However,conducting hybrid simulation of complex structures within large-scale testing facilities presents significant challenges. For example,the chosen modeling scheme may create numerical inaccuracies or even result in unstable simulations; the displacement and force capacity of the experimental system can be exceeded; and a hybrid test may be terminated due to poor communication between modules(e.g.,loading controllers,data acquisition systems,simulation coordinator). These problems can cause the simulation to stop suddenly,and in some cases can even result in damage to the experimental specimens; the end result can be failure of the entire experiment. This study proposes a phased approach to hybrid simulation that can validate all of the hybrid simulation components and ensure the integrity largescale hybrid simulation. In this approach,a series of hybrid simulations employing numerical components and small-scale experimental components are examined to establish this preparedness for the large-scale experiment. This validation program is incorporated into an existing,mature hybrid simulation framework,which is currently utilized in the Multi-Axial Full-Scale Sub-Structuring Testing and Simulation(MUST-SIM) facility of the George E. Brown Network for Earthquake Engineering Simulation(NEES) equipment site at the University of Illinois at Urbana-Champaign. Photovoltaic modules (PV) are tested in the industry in standard conditions and their characteristics are given in the datasheet. After installation on the site, its performance is not evaluated separately of the other parts of the PV generator. Nevertheless some discrepancies of the expected generation and the measured ones are verified. To deal with these discrepancies it is worth characterizing the module on the site and acquire data in an easy and inexpensive way. This paper presents a proposed characterization system as well as its application. A module test was performed on a site located in Brazil. At these latitudes it is common to have high amount of ultraviolet components. The characterization system measures separately the ultraviolet component, as well as the visible and global irradiations. The results obtained are analyzed and the effect of the UV variation is observed, proving the importance of doing this characterization on site after implementation of the PV system.

The recent introduction of glassy carbon(GC)microstructures supported on flexible polymeric substrates has motivated the adoption of GC in a variety of implantable and wearable *** probes such as electrocorticography and penetrating shanks with GC microelectrode arrays used for neural signal recording and electrical stimulation are among the first beneficiaries of this *** the expected proliferation of these neural probes and potential clinical adoption,the magnetic resonance imaging(MRI)compatibility of GC microstructures needs to be established to help validate this potential in clinical ***,we present GC microelectrodes and microstructures—fabricated through the carbon micro-electro-mechanical systems process and supported on flexible polymeric substrates—and carry out experimental measurements of induced vibrations,eddy currents,and *** induced vibration,induced voltage,and MRI experiments and finite element modeling,we compared the performances of these GC microelectrodes against those of conventional thin-film platinum(Pt)microelectrodes and established that GC microelectrodes demonstrate superior magnetic resonance compatibility over standard metal thin-film ***,we demonstrated that GC microelectrodes experienced no considerable vibration deflection amplitudes and minimal induced currents,while Pt microelectrodes had significantly larger *** also showed that because of their low magnetic susceptibility and lower conductivity,the GC microelectrodes caused almost no susceptibility shift artifacts and no eddy-current-induced artifacts compared to Pt *** together,the experimental,theoretical,and finite element modeling establish that GC microelectrodes exhibit significant MRI compatibility,hence demonstrating clear clinical advantages over current conventional thin-film materials,further opening avenues for wider adoption of GC microelectrodes in chronic clinical app

Anesthesia and surgery have an impact on inflammatory responses, which influences perioperative homeostasis. Inhalational and intravenous anesthesia can alter immune-system homeostasis through multiple processes that include activation of immune cells(such as monocytes, neutrophils, and specific tissue macrophages) with release of pro-or anti-inflammatory interleukins, upregulation of cell adhesion molecules, and overproduction of oxidative radicals. The response depends on the timing of anesthesia, anesthetic agents used, and mechanisms involved in the development of inflammation or immunosuppression. Obese patients are at increased risk for chronic diseases and may have the metabolic syndrome, which features insulin resistance and chronic low-grade inflammation. Evidence has shown that obesity has adverse impacts on surgical outcome, and that immune cells play an important role in this process. Understanding the effects of anesthetics on immune-system cells in obese patients is important to support proper selection of anesthetic agents, which may affect postoperative outcomes. This review article aims to integrate current knowledge regarding the effects of commonly used anesthetic agents on the lungs and immune response with the underlying immunology of obesity. Additionally, it identifies knowledge gaps for future research to guide optimal selection of anesthetic agents for obese patients from an immunomodulatory standpoint.

Model Study: The genus Acinetobacter is composed of opportunistic pathogens that mainly affects immunocompromised and patients during a prolonged hospitalization period. A. baumannii can survive in the hospital setting at various locations such as mechanical ventilators, dialysis machines, ventilation systems, water sources, skin and mucous membranes of health professionals and patients, drug preparations and disinfectants among others. Antimicrobials are essential for the treatment of infections, so their use has become common, and often inappropriate and abusive. This pathogen has presented multi-resistance to several classes of antimicrobials and has become a major cause of death in hospitals. Objective: To evaluate the occurrence of Acinetobacter baumannii in clinical samples from the Laboratory of Clinical Analyzes, HCSL of Pouso Alegre, MG. Methods: All the samples referring to the patients of the Samuel Libanio Clinical Hospital including most of the specialties were evaluated. From the positive samples for Acinetobacter baumannii the medical records of hospitalized patients were evaluated. From the medical records, data were obtained as gender and age of the patient, place of hospitalization, together with month and year of the laboratory procedure. Result: A total of 14,859 samples of diverse materials were evaluated, being 80 (0.5%) samples positive for Acinetobacter baumannii. Of these positive samples, the largest occurrences were identified in male patients 59 (73.7%), aged over 60 years 37 (46.3%). The most affected hospital sector was Adult ICU with 44 (55.0%) of the cases. In May 2015, there were 5 (6.3%) cases registered and in May 2016, 16 (20.0%) cases. The clinical material with the highest occurrence of Acinetobacter baumannii was Mini Bal 29 (36.3%).
