Purpose:To investigate whether blood flow restriction(BFR) without concomitant exercise mitigated strength reduction and atrophy of thigh muscles in subjects under immobilization for lower ***:The following databases were searched:PubMed,CINAHL,pedro,Web of Science,Central,and ***:The search identified 3 eligible studies,and the total sample in the identified studies consisted of 38 *** and isometric torque of the knee flexors and extensors was examined in 2 ***-sectional area of thigh muscles was evaluated in 1 study,and thigh girth was measured in 2 *** BFR protocol was 5 sets of 5 min of occlusion and 3 min of free flow,twice daily for approximately 2 *** a whole,the included studies indicate that BFR without exercise is able to minimize strength reduction and muscular atrophy after *** is crucial to emphasize,however,that the included studies showed a high risk of bias,especially regarding allocation concealment,blinding of outcome assessment,intention-to-treat analyses,and group similarity at ***:Although potentially useful,the high risk of bias presented by original stodies limits the indication of BFR without concomitant exercise as an effective countermeasure against strength reduction and atrophy mediated by immobilization.

Brazilian biomes are home to a significant portion of the world’s biodiversity,with a total of 14%of existing species and still concentrate 20%of the world’s water ***,changes in biomes have a direct impact on rainfall patterns and water *** on this,the objective was to evaluate the variability of rainfall in the four existing biomes in the Northeast Brazil(NEB)and their interaction with the ENSO climate variability mode and regional scale meteorological systems via CHELSA *** this,monthly rainfall data were used from 1979 to 2013,with a spatial resolution of 1 km×1 km of the CHELSA product,and seasonal and annual rainfall patterns were extracted via *** was found that the rainy season in the Amazon,Caatinga and Cerrado biomes occurred between January and April,with varying intensities,except for the Atlantic *** seasonality patterns are associated with the NEB meteorological systems,with emphasis on ITCZ(all Biomes),UTCV(Amazon,Caatinga and Cerrado),Frontal Systems(extreme south of Caatinga,Cerrado and Atlantic Forest)and EWD/TWD in the(Atlantic Forest).In the inter-annual scale,the remarkable influence of ENSO was verified,mainly in the years 1983,1985,1989,1993,1998,2009 and *** is noteworthy that 1985 was the wettest year of the period,with a surplus in all biomes,while the driest year differs between the Amazon(1983),Atlantic Forest and Caatinga(1993)and Cerrado(2012)*** study via orbital product in NEB showed that anthropogenic processes and natural variability interfere with the forms of rain interception in the biomes and hence in rainfall patterns and water recycling in NEB.

Introduction: Stroke incidence in young patients is about 10 cases in 100.000, according to several European studies. In this age group arterial dissection is one of the main pathological mechanisms involved. The internal carotid’s artery (ICA) main supraclinoid branch is the anterior choroidal artery (AChA). The occurrence of infarction in its territory due to internal carotid dissection is considered to be a rare event and may have different clinical presentations due to anatomical variability. Clinical case: A 31-year-old male patient, without any known cardiovascular risk factors or chronic medication, presented with acute onset of stabbing right sided headache while practicing football. Visual disturbances and hemiparesis with hypesthesia of his left arm were also mentioned. On admission left homonymous hemianopsia, left hemiparesis and left extensor plantar reflex were present. Brain magnetic resonance showed hyperintensity of T2 and FLAIR signals and restricted diffusion pattern suggested acute/subacute infarctions in the thalamic and subcapsular area, corpus callosum, splenium and subcortical parietal right region. Magnetic resonance angiography (MRA) of the brain showed reduction of the right ICA’s caliber, mainly of its supraclinoid segment in which a marked irregular stenosis was visualized, suggestive of arterial dissection. This stenotic segment included the origin of the AChA and of the posterior communicating cerebral artery with an exchange in their territories. Lumbar puncture results were normal as were analytical investigations which included CBC, sedimentation rate, syphilis serology and immunologic and prothrombotic screen. There were no phenotype characteristics suggestive of connective tissue disease. Conclusion: Trauma seems to be the most probable lesion mechanism for the occurrence of intracranial carotid’s dissection in this particular case, as the patient was practicing vigorous sports at time of onset. Brazil’s continental dimensions bring huge challenges for the Federal Administration,particularly in relation to low income *** this sense,in 2009,Federal Government has launched a huge habitational program entitled“Minha Casa Minha Vida”(in English MHMLP-the“My House,My Life”Program)which has been,so far,the biggest social housing Federal program,and has produced millions of SIH(social interest housing)*** program was discontinued in *** the program has been huge,there were few units built in urban centers,increasing the urban *** feasibility of SIH enterprises in the context of the MHMLP leads to the production towards the outskirts,where the land cost better fits the *** cost analysis,however,does not take into consideration future costs of an inefficient infrastructure,poor social services and public safety issues,with impact on the cities’operational *** paper compares the total building cost of SIH in peripheral and central areas,emphasizing the benefits of production in central areas when considering indirect operational and infrastructure *** research takes into account the infrastructure already available in central areas and the opportunity of appropriate abandoned or underutilized buildings and urban empty *** this sense,a case study has been conducted in two projects in the Metropolitan Area of Rio de Janeiro(RJ)comparing the estimated budget of SIH built far from urban centers,to the probable budget occupying underutilized *** indicate the necessity to review the parameters adopted when discussing the production of low income housing considering the cities’development as an integrated system.

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). This study also showed that health professionals are using practice management to deal with their behavior to protect their health.

Objective: As isolates of Cryptococcus are frequently kept collection stocks in institutions, sometimes without proper characterization, we sought to determine the genotype profiles, protease and phospholipase activities “in vitro” and the susceptibility testing for azoles and amphotericin B. Methodology: 84 isolates from several regions of Brazil (40 samples from clinical origin and 44 isolates from environmental origin) were maintained at the microorganism’s bank of the Biomedical Science Institute (ICB-USP) of the São Paulo University, in São Paulo, Brazil. This isolates was submitted fungal strains determination, DNA extraction and purification, determination of genotype by URA5 gene RFLP of CGB-positive isolates, protease and phospholipase activity and susceptibility to antifungals. Results: Of six CGB positive isolates tested by RFLP-PCR, only four presented a genomic profile consistent C. gattii species (VGII), while two other were C. neoformans (VNI and VNIII), indicating the existence of canavanine-resistante C. neoformans isolates in the culture collections. The clinical isolates secreted higher levels of phospholipase and environmental isolates but no differences were observed for the protease levels. Almost all isolates were sensible to azoles and amphotericin B. Conclusion: We point out in this research the existence of C. neoformans strains resistant to canavanine and intrinsic characteristic of C. gatti. These results demonstrate the importance to perform a detailed characterization of isolates kept in culture collections.

Acute respiratory distress syndrome(ARDS) represents a serious problem in critically ill patients and is associated with in-hospital mortality rates of 33%-52%. Recruitment maneuvers(RMs) are a simple, low-cost, feasible intervention that can be performed at the bedside in patients with ARDS. RMs are characterized by the application of airway pressure to increase transpulmonary pressure transiently. Once non-aerated lung units are reopened, improvements are observed in respiratory system mechanics, alveolar reaeration on computed tomography, and improvements in gas exchange(functional recruitment). However, the reopening process could lead to vascular compression, which can be associated with overinflation, and gas exchange may not improve as expected(anatomical recruitment). The purpose of this review was to discuss the effects of different RM strategies- sustained inflation, intermittent sighs, and stepwise increases of positive end-expiratory pressure(PEEP) and/or airway inspiratory pressure- on the following parameters: hemodynamics, oxygenation, barotrauma episodes, and lung recruitability through physiological variables and imaging techniques. RMs and PEEP titration are interdependent events for the success of ventilatory management. PEEP should be adjusted on the basis of respiratory system mechanics and oxygenation. Recent systematic reviews and meta-analyses suggest that RMs are associated with lower mortality in patients with ARDS. However, the optimal RM method(i.e., that providing the best balance of benefit and harm) and the effects of RMs on clinical outcome are still under discussion, and further evidence is needed.

Amyloidosis is the result of fibrous and insoluble amyloid protein deposition in extracellular spaces of tissues and organs, including the skin. Through the analysis of medical records, clinical condition and laboratory tests, this article proposes a rare case report of an adult patient with symptoms and diagnosis of polymyositis, histopathology of nodular amyloidosis with skin lesions suggestive of lupus. The patient is still being followed at the dermatology clinic of Valença Medical School, in order to improve the prognosis and prevent the progression of existing symptoms.
