Intestinal neuronal dysplasia type B:A still little known diagnosis for organic causes of intestinal chronic constipation

Intestinal neuronal dysplasia type B(IND-B) is a controversial entity among the gastrointestinal neuromuscular disorders. It may occur alone or associated with other neuropathies, such as Hirschsprung's disease(HD). Chronic constipation is the most common clinical manifestation of patients. IND-B primarily affects young children and mimics HD, but has its own histopathologic features characterized mainly by hyperplasia of the submucosal nerve plexus. Thus, IND-B should be included in the differential diagnoses of organic causes of constipation. In recent years, an increasing number of cases of IND-B in adults have also been described, some presenting severe constipation since childhood and others with the onset of symptoms at adulthood. despite the intense scientific research in the last decades, there are still knowledge gaps regarding definition, pathogenesis, diagnostic criteria and therapeutic possibilities for IND-B. However, in medical practice, we continue to encounter patients with severe constipation or intestinal obstruction who undergo to diagnostic investigation for HD and their rectal biopsies present hyperganglionosis in the submucosal nerve plexus and other features, consistent with the diagnosis of IND-B. This review critically discusses aspects related to the disease definitions, pathophysiology and genetics, epidemiology distribution, clinical presentation, diagnostic criteria and therapeutic possibilities of this still little-known organic cause of intestinal chronic constipation.

Efficient honey production requires knowledge about the behavior of the workers and the parameters that influence the strength of the colony. In this study, the objective was to analyze the interaction between the foraging behavior of worker honeybees and pollen storage levels in Africanized honeybees colonies. Colonies with low pollen storage increased pollen intake rates, but this value was 15% lower than colonies with high pollen storage, demonstrating a direct relationship between the pollen storage levels and foraging activity. The difference in pollen intake rates varied according to the number of foraging honeybees and pollen load collected by each individual. Under both high and low pollen storage, colonies returned pollen storage to initial level within 16 days, suggesting that honeybees regulate pollen storage levels around a homeostatic set point. Relationship between pollen storage levels and colony brood production was also found, indicating how alterations in the behavior of each individual can affect the strength of the colony.

多孔界质中界面的平衡与稳定性是调整流体分布的主要因素。在本文中,我们对双锥形孔隙段中具有轴对称界面,即头状弯月面与颈状弯月面的平衡与稳定性进行了测定。对于稳定界面,我们还得出了毛管压力P_c与汞体积V_(Hg)之间的关系。这些结果在我们为体积控制汞孔隙度测量所作的蒙特卡罗模拟中得到了应用。

Based on Russian and the Middle East corticosteroids trials in MERS-CoV, we performed methylprednisolone pulse therapy (MPT), resulting in a clinical trial still without result. Our previous cohort (not compared n = 18) showed 76% of MPT patients did not progress to orotracheal intubation as MTP blocked the cytokine storm, a lower result compared to Tehran’s study explained by performing MPT in any lung phase. The Middle East study had been carried out during the initial lung phase. We are in an international emergency. Considering previous protocols and clinical practice, we understand that MPT must be used in COVID-19, and the indication to avoid going to the hospital when the first symptoms appear should be changed urgently for the population with inflammatory comorbidities. This article aims to: 1) show the Iranian protocol to reduce deaths and intubations by COVID-19;2) present a possible approach to the patient COVID-19 with methylprednisolone pulse and strict criteria for orotracheal intubation to avoid hypoxemia;3) highlight that there is already a protocol that can be an international guideline-based on the Iranian work for the treatment of CoVID-19;and 4) argue that corticosteroids are not controversial, but their use in a period outside the best timing period makes it controversial;and 5) emphasise the urgency of modifying the current protocol that postpones the visit of patients to the hospital in case of symptoms, since late hospital evaluation has been catastrophic for a world population.

Evaluating oblique Shock Waves Characteristics on a Double-Wedge Airfoil

Numerical and experimental study to evaluate aerodynamic characteristics in supersonic ow over a double wedge airfoil is carried out using Fluent software and a supersonic wind tunnel, respectively. The Schlieren visualization method was also used to develop the experimental step of this study. The supersonic wind tunnel reached a proximately a Mach number of 1.8. The result got showed oblique shock waves visualization on double-wedge airfoil and the numerical simulation, the flow behavior as function of Mach number, pressure, temperature and density in the flow field on the computational model. The simulation allowed to observe the shock wave and the expansion fan in the leading and tailing edge of double-wedge airfoil. From the numerical and experimental comparison, an agreement at the shock wave angle and Mach number was observed, with a difference about 1.17% from the experimental results.

A relation between heat transferred and energy loss, for turbulent flow. In different tube arrangements, is made. The conditions are determined which decide the dimensions and velocities for a heat exchanger. Also, a reference to the economic dimensioning of heat exchangers is presented. In this study, the conditions which a heat exchanger must satisfy represent the best balance between the amounts of material employed. The investigation is restricted to the case of turbulent flow.

In this paper the delaware Method published in 1963 is analyzed and upgraded with using correction factors which take into account the undesirable currents of the mean flow. However, this method presents graphically these correction factors which imply an impediment to fulfill the software calculations. Thus, the equations corresponding to the correction factor equations and a Fortran 77 numerical program were established. This system is given to explore different design alternatives in order to find the optimal solution to each proposed problem. The results of this work was a simple software that can perform calculations with the introduction of parameters depending only on the geometry of the heat exchanger, i.e., geometry, temperature and fluid characteristics eliminating the human errors and increasing the calculations speed and accuracy.

Background: Brain metastases from papillary thyroid carcinoma are infrequent occurring in 0.15% to 1.3% of patients, and they are generally accompanied by synchronous metastases in other sites and in isolation are even less frequent;the information about their management is limited and it has been based mainly on retrospective studies, experiences, case reports, and management guidelines of brain metastasis of other cancers. Aim: We report this case with the objective of describing how unpredictable the behavior of thyroid cancer can be despite being classified as low risk of recurrence. Case Presentation: A female patient with a diagnosis of papillary thyroid carcinoma classified as low risk who after more than a year of surveillance and without alterations in laboratory and imaging studies, developed a single brain metastasis susceptible to treatment with radioactive iodine (RAI) therapy. Conclusion: This case emphasizes the importance of a close follow-up of patients and not to minimize any symptom, no matter how simple it may seem, since cancer has no rules in its evolution.
