Background: The use of nutritional supplements(NS) among athletes is widespread. However, little is known about the relationship between nutritional adequacy and NS usage. The aims of this study were to evaluate the NS usage and to compare the nutritional intake from food and prevalence of micronutrient inadequacy(PMI) between NS users and ***: Portuguese athletes from 13 sports completed an NS usage questionnaire and a semi-quantitative food-frequency questionnaire assessing information over the previous 12 months. The estimated average requirement cut-point method was used to calculate PMI. General linear models were used to compare nutritional intake and NS usage. Chi-squared tests and logistic regression were performed to study, respectively,relationships and associations between PMI and NS ***: From the 244 athletes(66% males, 13–37 years), 64% reported NS usage. After adjustment, NS users showed a higher intake from food(p < 0.05), for at least 1 gender, for energy, and for 7 of the 17 studied nutrients. The highest PMI were seen for vitamins D and E, calcium, folate,and magnesium. After adjustment, NS users, irrespective of gender, reported lower PMI for calcium(OR = 0.28, 95%CI: 0.12–0.65), and female users for magnesium(OR = 0.06, 95%CI: 0.00–0.98).Conclusion: Athletes using NS reported a higher nutritional intake from food, and a lower PMI for several nutrients. Perhaps, those who were taking NS were probably the ones who would least benefit from it.

We report a case of a 56-year-old woman with a history of allogenic bone marrow transplantation for two years,complaining with dysphagia and weight loss. Upper endoscopy revealed esophageal stenosis and extensive mucosa sloughing. Biopsies confirmed the diagnosis of graft-vs-host disease(GVHD). Balloon dilation,corticosteroids and cyclosporin resulted in marked clinical improvement. Gastrointestinal tract is involved in the majority of patients with chronic GVHD. Esophageal manifestations are rare and include vesiculobullous disease,ulceration,esophageal webs,casts or strictures. Sloughing esophagitis along with severe stenosis requiring endoscopic dilation has never been reported in this context.

Purpose: To investigate the prognostic impacts of a combined clopidogrel-metabolizing genotypes CYP 2C19 (+ or -)*2/(+ or -)*17 on patients with acute coronary syndrome (ACS). Population and methods: Prospective, longitudinal study of 95 patients admitted for ACS to a single coronary care unit. Patients less than 75 years of age, who survived hospitalization and to whom clopidogrel was prescribed, were included. CYP2C19 genotyping was performed at dis- charge. For analysis, the patients were grouped as follows: Group A ([+]*2/[+]*17) n = 8;Group B ([+]*2/[-]*17) n = 18;Group C ([-]*2/[+]*17) n = 27;and Group D ([-]*2/[-]*17) n = 42. Platelet function was assessed by an ADP platelet aggregation test using a commercially available kit. The primary end-point was a composite of mortality or readmission for ACS. The median time of follow-up was 136.0 (79.0 - 188.0) days. Results: The mean age of the study patients was 59.9 ± 10.7 years, and 83.2% were male. The allele frequencies of CYP2C19*2 and CYP2C19 *17 were 14.2% and 20%, respectively. Both allele frequencies were in Hardy Weinberg equilibrium. The patient groups were homogenous for demographic data, cardiovascular risk factors, GRACE and CRUSADE bleeding scores, left ventricular ejection fraction, and coronary anatomy. ADP platelet aggre-gation was similar for all groups (respective rates for groups A, B, C, D were 17.5 U (10.3 – 18.7) vs 20.0 U (17.3 – 26.8) vs 16 U (12 – 19) vs 12 U (8 – 22), p = 0.4). Event-free survival was significantly lower for group B (respective rates for Groups A, B, C, D were 87.5% vs 68.8% vs 96.3% vs 92.5%;p = 0.02). By multivariate Cox regression analysis, the CYP2C19 (+)*2/(-)*17 diplotype was an independent predictor of outcome, conferring a 5.2-fold higher adjusted risk for the composite endpoint than the others diplotypes. Objective: To evaluate intraobserver and interobserver reliability of the measurement of the mechanical axis of the lower limb by means of the panoramic radiograph of lower limbs by physicians with different levels of medical practice. Method: A retrospective cross-sectional study with preoperative and postoperative radiographic analysis of a group of 100 patients submitted to total knee arthroplasty. Results: It was observed that there is a highly significant intraobserver and interobserver agreement (p Conclusion: The panoramic radiograph of the lower limbs is a reliable method for measuring the mechanical axis of the lower limb in patients submitted to total knee arthroplasty, regardless of the level of medical practice of the evaluator.

Patient-Reported Outcomes in Prostate Cancer: Prospective Changes Analysis for Prognosis Prediction

Prostate cancer is affecting a higher proportion of male population. Health Related Quality of Life assessment can guide the development of an interdisciplinary and patient-centered care intervention. This study is aimed to assess Health Related Quality of Life in prostate cancer patients. Relationships between socio-demographic, clinical characteristics and patient-reported outcomes have been considered. Consecutive outpatients with prostate cancer, admitted at the Urology Clinic of the Instituto Português de Oncologia do Porto, were studied (n = 300). Health Related Quality of Life was assessed as part of the routine practice. The European Organisation for Research and Treatment of Cancer general questionnaire, QLQ-C30, and its specific module for prostate cancer patients, QLQ-PR25, were used. Evolution along time (elapsed since diagnosis, and up to 5 years) was considered in order to search for a prognosis prediction in prostate cancer patients. This study confirms the feasibility of a systematic Health Related Quality of Life assessment. Global Health Related Quality of Life was found to be higher 6 months after diagnosis, decreasing then until the second year after diagnosis and improving thereafter. A peak with better scores was identified at the fifth year after diagnosis. Social and physical dimensions revealed a similar pattern. Clinical significance was found 6 months and 5 years after diagnosis. The prospective analysis of Health Related Quality of Life changes is able to explore the patients’ outcomes in order to find patterns and relationships for prognosis prediction along the disease course. Such approach might promote patient confidence and thus a better cancer experience.

Chronic low back pain has a huge impact on daily living and a negative economic and professional effect. It is a matter of debate and concern for all health professionals involved, particularly spine surgeons. Recent discoveries on the innervation and biochemical properties of the intervertebral disc clarify the role of this structure as a possible cause of chronic low back pain. However, multiple causes may be present in the same patient making the diagnosis a challenging process. Discogenic pain is defined as a chronic low back pain induced by a degenerative disc disease. There are no specific characteristics of discogenic pain, although it has a higher incidence in younger age, it is usually localized medially in the back, worsens with axial loading and improves with recumbence. In the last decades we have assisted the emergence of multiple treatment techniques. However, neither the conservative treatment nor the interventional management has strong evidence in treating discogenic pain. Randomized control clinical trials are sought to improve patient outcome. Meanwhile, we believe each patient should be approached on an individual base. Discogenic pain: we care.
