BACKGROUND Achromobacter xylosoxidans is a Gram-negative opportunistic aerobe,usually causing nosocomial infections in immunocompromised patients with manifestations including bacteremia,pneumonia,and catheter-related ***,*** have not yet been reported to cause biliary system *** SUMMARY A 72-year-old woman presented to the outpatient department of our hospital with a chief complaint of *** tomography of her abdomen revealed the presence of a mass of approximately 2.4 cm in the hilar portion of the common hepatic duct,consistent with hilar *** performed endoscopic retrograde cholangiopancreatography(ERCP)to decompress the obstructed left and right intrahepatic ducts(IHDs)and placed 10 cm and 11 cm biliary stents in the left and right IHDs,***,the day after the procedure,the patient developed post-ERCP cholangitis as the length of the right IHD stent was insufficient for proper bile *** blood culture of the patient tested positive for *** measures included the replacement of the right IHD stent(11 cm)with a longer one(12 cm)and administering culturedirected antibiotic therapy,solving the cholangitis-related *** the cholangitis had resolved,the patient underwent surgery for hilar cholangiocarcinoma and survived for 912 d without *** ***-induced biliary system infections are extremely *** awareness of physicians and endoscopists is required as this rare pathogen might cause infection after endoscopic procedures.

BACKGROUND The role of smoking in the incidence of colorectal cancer(CRC)or gastric cancer(GC)in populations undergoing cholecystectomy has not been *** To evaluate the effect of smoking on CRC or GC development in cholecystectomy *** A total of 174874 patients who underwent cholecystectomy between January 1,2010 and December 31,2017 were identified using the korean National Health Insurance Service claims *** patients were matched 1:1 with mem-bers of a healthy population according to age and *** or GC risk after cholecystectomy and the association between smoking and CRC or GC risk in cholecystectomy patients were evaluated using adjusted hazard ratios(HRs)and 95%*** The risks of CRC(adjusted HR:1.15;95%CI:1.06-1.25;P=0.0013)and GC(adjusted HR:1.11;95%CI:1.01-1.22;P=0.0027)were significantly higher in cholecystectomy *** the population who underwent cholecystectomy,both CRC and GC risk were higher in those who had smoked compared to those who had never *** both cancers,the risk tended to increase in the order of non-smokers,ex-smokers,and current *** addition,a positive correlation was observed between the amount of smoking and the risks of both CRC and *** Careful follow-up and screening should be performed,focusing on the increased risk of gastrointestinal cancer in the cholecystectomy group,particularly considering the individual smoking habits.

目的:对运动中st段下移程度不同的心梗后患者(PMIP)的心血管反应进行探讨。方法:46名男性PMIP在跑台上进行递增负荷运动实验,其间测量每级负荷时的血压、心率并连续监测12导联心电图(ECG)。根据跑台第Ⅱ级负荷时的st段下移程度将其分为两组,第一组st段下移st段下移>1.0mm。结果:定量负荷工作时st段下移程度大的患者其心率-血压乘积(RPP)也高。结论:当不便进行ECG监测时,RPP可能是评价患者对运动的临床反应的特效指标。

It has been a challenge for a long time to predict the energy consumption and its relationship with other parameters such as GDP in a country China has become the world's second largest oil and gas consumer since 2003. About 100 million tons of oil was imported in 2003, which was about 38% of the total oil consumption in China. The increase in oil import in China was about one third of the total increase in the world. The crude oil price soared in recent years. Energy crisis may happen if right energy policy was not made. In this study, problems related to the economic development of China were analyzed. Different models were used to predict the energy consumption (including oil, natural gas, coal, etc.) in China and the world. The results from different models were analyzed. The trend of energy consumption in China was compared with those in other developing and developed countries. Possible solutions to the energy problem (maybe crisis) in China were proposed Renewable energy such as geothermal, hydrogen, wind and solar energy will play a more and more important role in the energy supply and the economic development of China. Also the advantages and disadvantages of different renewable energy were analyzed, which demonstrated the clues to make better decisions on the development and application of renewable energy.

目的:探讨12周运动心脏康复锻炼对心梗后患者(PMI患者)心脏机能的影响。方法:112名男性PMI患者(均进行了12周运动心脏康复锻炼)进行一次递增负荷运动实验(Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ级负荷跑速分别为1.7、1.7、1.7、2.5、3.5mph;坡度分别为0.5 %、10 %、12 %、14 %,每一级负荷运动3分钟) ,其间记录每级负荷时的VO2、HR、血压和自我用力感觉(Rpe) ,计算心率血压乘积(RPP) ,并持续监测12导联心电图(ECG) ,然后对上述指标进行相关分析并建立了用HR和Rpe推测VO2和RPP的回归方程。结果:(1)相邻两级负荷间的VO2、HR、RPP、Rpe具有显著差异(Pst段水平(st)和Rpe的峰值分别为26.4±7.1ml·kg-1·min-1、126.8±20.3beats·min-1、80.4±12.9 %、209.0±46.3beats·mmHg·100-1、-1.0±0.7mm和14.9±2.1;(3)运动中VO2、HR、%HRmax、RPP、Rpe呈高度正相关,它们与st呈高度负相关(Ppe推测VO2和RPP的回归方程。结论:(1)康复锻炼后利用改良Bruce跑台方案进行机能测试有效;(2)12周康复锻炼后,PMI患者进行运动时其强度不宜超过80 %HRmax或Rpe不超过15 ;(3)利用本研究建立的预测方程,可在PMI患者康复活动中,根据其心率或Rpe变化间接得知其呼吸循环机能的反应和心肌的耗氧状况。
