Acceptability of an acupuncture intervention for geriatric chronic pain: an open pilot study

OBJECTIVE： This study investigated the acceptability and effectiveness of acupuncture for persistent musculoskeletat pain in the elderly and assessed the conditions for a future controlled trial. METHODS： A total of 60 patients, hospitalized in a geriatric hospital were enrolled. The intervention consisted of eight acupuncture sessions. The main outcome was the patient＇s participation rate. Regarding pain, the evaluation was based on pre- and post-treatment variations. As a high proportion of the patients had cognitive impairment, the behavioral pain scale DOLOPLUS-2 was chosen although self evaluation was used wherever possible. RESULTS： The mean age of the patients was 83 years. The acceptance rate was very high （89.6%） and 90% of the patients completed the entire course of treatment. After five weeks, the mean DOLOPLUS score had decreased significantly （P〈0.01）. The patients reported improved sleep quality and a reduction in their anxiety symptoms. Furthermore, caregivers noticed a decrease in patient aggressiveness making care easier. CONCLUSION： Our results suggest that acupuncture is highly acceptable and could be very useful in the management of chronic pain when performed in very old frail people with chronic physical and mental disability. Tuberous sclerosis complex(TSC)is a rare multisystem,autosomal dominant neurocutaneous syndrome in which epilepsy is the most common of several neurological and psychiatric *** two thirds of patients develop drug-resistant epilepsy for whom surgical resection of epileptogenic foci is indicated when seizures remain inadequately controlled following trial of two antiseizure *** challenge with presurgical and surgical approaches with patients with TSC is overcoming the complexity from the number of tubers and the multiplex epileptogenic network forming the epileptogenic *** suggest that seizure freedom is achieved by 55%-60%of patients,but predictive factors for success have remained elusive,which makes for unconfident selection of surgical *** article presents three different cases as illustrations of the potential challenges faced when assessing the suitability of TSC patients for epilepsy surgery.

An Analytic Study on Internal Linkages Between Human Development and Globalization

Globalization is often understood as increasing global economic integration, global forms of governance, and globally inter-linked social and environmental developments. The target of this paper1 is to demonstrate the connection among human development, globalization, and institutional quality. The second part provides the methodology of measuring the overall globalization with an emphasis on the 2011 Konjunkturforschungsstelle (KOF) Globalization Index. The KOF Globalization Index includes economic, social, and political contexts. The third part shortly introduces one of the parameters of institutional quality?Human Development Index (HDI) (or rather Inequality-adjusted Human Development Index (IHDI) as the real indicator of the level of human development) and its methodology and results. For the analysis, two out of three dimensions of IHDI were used (long and healthy life and access to education) and two dimensions of Globalization Index were employed (social and political globalization). The third part compares indices and scores together, analyzes them, and assesses the relationships between the HDI and the Globalization Index. It is possible to conclude from the results achieved in the study that the social globalization has stronger linkages with human development than with the political globalization, yet, spurring growth rates and reduced poverty in countries with poor institutions cannot be achieved simply by globalizing their economies.

The biosynthesis of isoprenoids in plant cells occurs from precursors produced in the cytosol by the mevalonate （MVA） pathway and in the plastid by the methylerythritol 4-phosphate （MEP） pathway, but little is known about the mechanisms coordinating both pathways. Evidence of the importance of sugar signaling for such coordination in Arabi- dopsis thaliana is provided here by the characterization of a mutant showing an increased accumulation of MEP-derived isoprenoid products （chlorophylls and carotenoids） without changes in the levels of relevant MEP pathway transcripts, proteins, or enzyme activities. This mutant was found to be a new loss-of-function allele of PRL1 （Pleiotropic Regulatory Locus 1）, a gene encoding a conserved WD-protein that functions as a global regulator of sugar, stress, and hormone responses, in part by inhibition of SNFl-related protein kinases （SnRK1）. Consistent with the reported role of SnRK1 kinases in the phosphorylation and inactivation of the main regulatory enzyme of the MVA pathway （hydroxymethylglutaryl coenzyme-A reductase）, its activity but not transcript or protein levels was reduced in prll seedlings. However, the accumulation of MVA-derived end products （sterols） was unaltered in mutant seedlings. Sucrose supplementation to wild- type seedlings phenocopied the prll mutation in terms of isoprenoid metabolism, suggesting that the observed isoprenoid phenotypes result from the increased sugar accumulation in the prll mutant. In summary, PRL1 appears to coordinate isoprenoid metabolism with sugar, hormone, and stress responses.
