Current aspects of therapeutic reduction mammaplasty for immediate early breast cancer management: An update

Breast-conservation surgery(BCS) is established as a safe surgical treatment for most patients with early breast cancer. Recently, advances in oncoplastic techniques are capable of preserving the breast form and quality of life. Although most BCS defects can be managed with primary closure, the aesthetic outcome may be unpredictable. Among technical options, therapeutic reduction mammaplasty(TRM) remains a useful procedure since the BCS defect can be repaired and the preoperative appearance can be improved, resulting in more proportional breasts. As a consequence of rich breast tissue vascularization, the greater part of reduction techniques have based their planning on preserving the pedicle of the nipple-areola complex after tumor removal. Reliable circulation and improvement of a conical shape to the breast are commonly described in TRM reconstructions. With an immediate approach, the surgical process is smooth since both procedures can be carried out in one operative setting. Additionally,it permits wider excision of the tumor, with a superior mean volume of the specimen and potentially reduces the incidence of margin involvement. Regardless of the fact that there is no consensus concerning the best TRM technique, the criteria is determined by the surgeon's experience, the extent/location of glandular tissue resection and the size of the defect in relation to the size of the remaining breast. The main advantages of the technique utilized should include reproducibility, low interference with the oncological treatment and long-term results. The success of the procedure depends on patient selection, coordinated planning and careful intra-operative management.

Transcatheter aortic valve replacement in elderly patients

Aortic stenosis is the most common native valve disease, affecting up to 5% of the elderly population. Surgical aortic valve replacement reduces symptoms and improves survival, and is the definitive therapy in patients with symptomatic severe aortic stenosis. However, despite the good results of classic surgery, risk is markedly increased in elderly patients with co-morbidities. Transcatheter aortic valve replacement （TAVR） allows implantation of a prosthetic heart valve within the diseased native aortic valve without the need for open heart surgery and cardiopulmonary bypass, offering a new therapeutic option to elderly patients considered at high surgical risk or with contraindications to surgery. To date, several multicenter registries and a randomized trial have confirmed the safety and efficacy of TAVR in those patients. In this chapter, we review the background and clinical applications of TAVR in elderly patients.

Post-poliomyelitis syndrome (PPS) is a disorder in individuals who have had poliomyelitis, characterized by new muscle weakness and often associated with other symptoms, including cold intolerance (CI). Qigong is a Traditional Chinese Medicine technique to adjust energy and blood circulation. Objective: To verify the effects and late repercussions of Qigong on CI complaints in PPS patients. Methods: PPS patients (n = 22, 14 females, 8 males;ages 35 - 60) performed Qigong exercises in 40-minute sessions, three times per week, for three consecutive months. They were evaluated at baseline, the end of treatment and every three months for a year using a visual analogue scale adapted for CI (VAS-cold). Results: The systemic VAS-cold scores exhibited significant differences between the baseline, the end of treatment and throughout 12 months of follow-up. Conclusion: The CI scores were low and bearable at the end of intervention and for the following 12 months without activity.

Soil boron fertilization:The role of nutrient sources and rootstocks in citrus production

Boron （B） is a key element for citrus production, especially in tropical regions, where the nutrient availability is commonly low in the soil. In addition, information about doses, fertilizer sources, methods of application, and particularly, differential nutrient demand of scion/rootstock combinations are required for efficient fertilization of commercial groves. In a non-irri- gated sweet orange orchard （cv. Natal）, grafted onto Rangpur lime, Swingle citrumelo or Sunki mandarin, we studied the application of two sources of B： boric acid （17% B, soluble in water） and ulexite （12% B, partiaJly soluble in water） at four levels of supply （control without B, and soil application of 2, 4 and 6 kg ha-1 yr-1 of B）. The experiment was carried out for three years （2004-2006）. Boron availability in the soil and concentration in the leaves, as well as the fruit yield and quality of trees were evaluated. Soil B extracted with hot water and total leaf B positively correlated with doses of the nutrient applied to the trees. Levels of B in the soil and in the leaves did not vary with fertilizer sources. Fruit yield of trees grafted onto Rangpur lime and Swingle citrumelo was more responsive to B doses than those grafted onto Sunki mandarin. The maximum fruit yield of trees grafted onto Swingle was obtained with 3.2 kg ha-1 yr-1 of B, and leaf B level of 280 mg kg-1 that point out to a highest demand for B when this combination was compared with other rootstocks. Furthermore, fertilization with B did not affect the quality of fruits, but correlated with B and potassium （K） concentrations in the leaves. These results also support that the current recommendations for levels of B in leaves should be revisited.

Thermal and salinity effects on locomotor performance of Thoropa taophora tadpoles(Anura,Cycloramphidae)

It is well known that environmental temperature influences several biological functions of ectotherms,notably in *** high permeability of anuran skin,associated with the effect of elevated environmental temperature,potentiates the dehydration process and this combination may restrict locomotor ***pa taophora is an endemic species from the Atlantic Rainforest whose tadpoles are semiterrestrial and predominantly diurnal,and are found in rocky seashores where they are exposed to sea spray and high *** this study we investigated how temperature and salinity conditions affect the locomotor performance in Thoropa taophora *** also assessed how different osmotic concentrations affect the activity of the metabolic pathways that support muscle *** measured the sprint speed of tadpoles of various sizes at different temperatures and salinities in the *** also measured the activity of the enzymes pyruvate kinase(PK),lactate dehydrogenase(LDH)and citrate synthase(CS)in different temperatures and osmotic concentrations,and calculated the thermal sensitivity and the activity constants for each *** results showed that,in general,sprint speed decreased with increasing temperature and ***,whereas the effect of increased salinity was similar in smaller and larger tadpoles,increased temperature had a higher negative impact on sprint speed of larger tadpoles,thus indicating low thermal sensitivity of small *** and LDH thermal sensitivities and LDH constant of activity decreased as the osmolality *** conclusion,the locomotor capacity of tadpoles was decreased by temperature and salinity,which may be related to a decrease in anaerobic metabolism both in terms of sensitivity and total energy turnover through enzymatic *** discuss the ecological consequences,including the potential impacts on predator escape behavior promoted by changes in metabolism and locomotor performance

Variability of Air-Sea CO_(2) Fluxes and Dissolved Inorganic Carbon Distribution in the Atlantic Basin:A Coupled Model Analysis

The biogeochemical dynamics of carbon in the ocean is a subject of fundamental interest to environmental studies. In this context, we have implemented a ten year run of the Brazilian Earth System Coupled Ocean-Atmosphere Model (BESM-OA2.3) integrated with TOPAZ biogeochemical model for the Atlantic basin. The modeled ΔpCO2 for the tropical Atlantic shows very clearly a high dominance of positive fluxes, that is, the CO2 fluxes are sea-to-air throughout the tropical region and for both winter and summer periods. In the mid-latitudes regions negatives fluxes (air-to-sea) were observed for both seasons. An exception to this pattern is an extensive negative tongue on the latitude 10N. The occurrence of this negative ΔpCO2 tongue region in the Tropical Atlantic is highly correlated to negative Evaporation-Precipitation values during this season. In the northern hemisphere (NH) summer the negative values of ΔpCO2 in the tropical Atlantic region are concentrated in the adjacent zone of the Amazon river mouth due to the North Equatorial Counter Current intensification. This process favors the formation of a carbon sink in the adjacent region of the Amazon river mouth. Model results show lowest values of dissolved inorganic carbon (DIC) in a surface layer (100 - 150 m). Highest DIC values are observed in deeper layers and concentrated in an equatorial band. The chlorophyll bloom in equatorial zones was well represented by the model. These blooms are the result of equatorial upwelling that brings the high concentration tongues of DIC present in the equatorial band towards the euphotic zone. This is the first published paper about the BESM-OA2.3 integrated with TOPAZ. The presented results suggest that this modeling system is able to reproduce the main regional carbon dynamics features of the mid-latitude/tropical Atlantic.

Is autoimmune hepatitis a frequent finding among HCV patients with intense interface hepatitis?

AIM:To evaluate the overlap of autoimmune hepatitis in hepatitis C virus(HCV)-infected patients with intense interface ***:Among 1759 patients with hepatitis C submitted to liver biopsy,92(5.2%) presented intense interface *** patients were evaluated regarding the presence of antinuclear antibody(ANA),anti-smooth muscle antibody(SMA) and anti-liver/kidney microsomal antibody(LKM-1),levels of γ-globulin and histological findings related to autoimmune hepatitis(plasma cell infiltrate and presence of rosettes).RESULTS:Among patients with hepatitis C and intense interface hepatitis there was a low prevalence of autoantibodies(ANA=12%,SMA=5%,LKM-1=0%) and the median γ-globulin level was within the normal *** histological findings of autoimmune disease were observed in only two cases(2%).After applying the score for diagnosis of autoimmune hepatitis,only one patient was classified with a definitive diagnosis of autoimmune *** overlap with autoimmune hepatitis was not the explanation for the intense necroinflammatory activity in patients with chronic hepatitis C we sought to identify the variables associated with this *** presence of intense interface hepatitis was associated with more advanced age,both at the time of infection and at the time of the biopsy,and higher prevalence of blood transfusion and alcohol ***:Although possible,overlap with autoimmune hepatitis is a very rare association in HCV-infected patients with intense interface hepatitis,an unusual presentation which seems to be related to other host variables.

Biochar and sugar cane filter cake interaction on physical and hydrological soil properties under tropical field conditions

Biochar is known for its effects on carbon sequestration and soil ***,there is a lack of information about its effects on soil physical and hydraulic properties for tropical *** assessed the effects of biochar(BC)plus sugar cane filter cake(FC)rate,and time of interaction on soil physical and hydraulic properties under humid tropical *** this purpose,a field experiment was installed at a loamy sandy soil with five treatments and four replicates:control(only soil),25 Mg ha^(−1) sugar cane filter cake,and 25 Mg ha^(−1) sugar cane filter cake plus 6.25,12.5,and 25 Mg ha^(−1) Miscanthus biochar,respectively,two soil depths(0-10 and 10-20 cm)and two times of interaction(9 and 18 months).Physical prop-erties(aggregate stability,bulk density,total porosity,pores size distribution)and hydraulic properties(soil water holding capacity,hydraulic conductivity,plant-available water holding capacity)were measured after nine and eighteen *** bulk density decreased slightly,and the porosity increased after nine months,for the biochar plus sugar cane filter cake(both 25 Mg ha^(−1)).After 18 months,biochar plus filter cake interaction increase micropores,aggregate stability,and plant-available water *** hydraulic conductivity was not influenced by sugar cane filter ***,biochar significantly reduced saturated hydraulic conductivity when combined with sugar cane filter cake after 18 *** concluded that sugar cane filter cake in combination with biochar modified the pore size distribution,slightly increased plant-available water holding capacity,and significantly decreased saturated hydraulic conductivity.
