Background Lower exercise tolerance is an important component of asthma and the possible effects of non-invasive ven-tilation on exercise capacity in individuals with severe therapy-resistant asthma(STRA)are *** study aimed to evaluate the immediate effect of continuous positive airway pressure(CPAP)on exercise tolerance in children with *** We performed a controlled,randomized,crossover clinical trial including subjects aged 6 to 18 years old diagnosed with ***,anthropometric and lung function data were *** participants in the intervention group(IG)used CPAP(PEEP 10cmH2O and FiO20.21)for a period of 40 *** in the control group(CG)used CPAP with mini-mum PEEP at 1 cmH20 also for 40 ***,subjects from both groups underwent cardiopulmonary exercise testing(CPET).After a 15-day washout period,on a subsequent visit,subjects participated in the opposite group to the initial *** Thirteen subjects with a mean age of 12.30±1.7 years were *** variables of peak expiratory flow(PEF)and forced expiratory volume in the first second(FEV1)before using CPAP and after performing CPET did not show sig-nificant *** CPET results,there was no significant difference(P=0.59)between groups at peak exercise for oxygen consumption—VO2(CG:33.4±6.3 and IG:34.5±5.9,mL kg^(-1)min^(-1)).However,the IG(12.4±2.1)presented a total test time(min)significantly(P=0.01)longer than the CG(11.5±1.3).Conclusion The results suggest that the use of CPAP before physical exercise increases exercise duration in children and adolescents with STRA.

BACKGROUND:Upper gastrointestinal(GI)bleeding is a common complication of portal hypertension in cirrhotic patients,and hepatocellular carcinoma(HCC)is the most common tumor in cirrhotic *** from tumor erosion into the GI tract is very rare.A patient with HCC and gastric tumor invasion was described and the previously reported cases were reviewed. METHOD:A patient with upper GI bleeding was treated in a tertiary hospital. RESULTS:A cirrhotic patient with a HCC invading the stomach leading to upper GI bleeding was treated by left lateral segmentectomy and sub-total *** bleeding was controlled and a good surgical outcome was achieved. CONCLUSIONS:HCC with gastric invasion should be differentially diagnosed from upper GI bleeding in cirrhotic *** can be controlled and symptomatic relief marked in selected cases.

In this editorial,we comment on the paper by Muthu et al published in the recent issue of the *** editorial review focusses on the use of adipose-derived stem cells(ADSCs)in knee osteoarthritis *** discuss the differences between the stromal vascular fraction and microfragmented adipose tissue and highlight the results of clinical studies comparing both treatments and the use of hyaluronic acid,platelet-rich plasma,and bone marrow aspirate *** use of expanded ADSCs is also discussed;moreover,concerns regarding treatment with ADSCs,particularly the heterogeneity of published studies and the need to standardize protocols to explore clinical potential is explored.

Oil palm (Elaeis guineensis Jacq.) is one of the agricultural crops with the greatest potentialfor vegetable oil production in Brazil. However, a disease of unknown etiology popularlyknown as Fatal Yellowin (FY) has caused damage to Brazilian farmers particularly in theeastern region of the Amazon. So, the objective of this study was to evaluate the spatialdependence of FY on three oil palm genotypes, grown for many years in an organic produc-tion system in the Amazon region. The study area had 4104 ha, divided into 139 plots. Ineach plot, the monthly incidence of disease was monitored forming a database. The num-ber of diseased plants per year, number of accumulated diseased plants, number of dis-eased plants per hectare, growth rate of diseased plants and incidence of accumulateddisease were evaluated. The results indicated spatial distribution of the variables adjustedto the gaussian, spherical and exponential models, with predominance of the first *** increases the purpose that FY is caused by biotic factors. The highest range achievedin the study was 2929 m indicating the susceptibility of the tested genotypes. Some plotsclose to the river had the highest incidence of the disease on the study, probably associatedwith higher soil moisture.

BACKGROUND： Fibrolamellar hepatocellular carcinoma （FLHCC） is a rare disease with an indolent behavior. Its prognosis is better than that of patients with hepatocellular carcinoma. The authors present their experience with resection of FLHCC. METHODS： Twenty-one patients with FLHCC were treated at our institution between 1990 and 2012. Of these patients, 14 were subjected to resection of the tumor. Patient demographics, medical history, results of imaging studies and laboratory tests, surgical data, and pathologic findings were evaluated. RESULTS： The median age of the patients at the diagnosis of the tumor was 20 years and 14 patients were female. None of the patients had tumor-associated chronic liver disease or cirrhosis. The mean tumor size was 12.8 cm （range 6-19） and 18 patients had a single liver nodule. Fourteen patients were subjected to hepatectomy and six of them had lymph node metastases resected. Pathologic evaluation revealed that 5 （35.7%） patients had major vascular invasion. Tumor recurrence was seen in 8 patients （66.7%）, during a follow-up. The median survival time for patients who were subjected to resection was 36 months. The 5-year overall survival rate and disease free survival rate were 28.0% and 8.5%, respectively. Univariate analysis showed that vascular invasion was the only variable associated with the disease free survival ***： Despite an aggressive treatment, patients with FLHCC presented unexpected low survival rates. It seems that an underestimated malignant behavior is attributed to this disease, and that the forms of adjuvant treatment should be urgently evaluated.

Let M and N be topological spaces,let G be a group,and letτ:G×M→M be a proper free action of *** this paper,we define a Borsuk-Ulam-type property for homotopy classes of maps from M to N with respect to the pair(G,τ)that generalises the classical antipodal Borsuk-Ulam theorem of maps from the n-sphere S^(n) to R^(n).In the cases where M is a finite pathwise-connected CWcomplex,G is a finite,non-trivial Abelian group,τis a proper free cellular action,and N is either R^(2) or a compact surface without boundary different from S^(2) and RP^(2),we give an algebraic criterion involving braid groups to decide whether a free homotopy class β∈[M,N]has the Borsuk-Ulam *** an application of this criterion,we consider the case where M is a compact surface without boundary equipped with a free actionτof the finite cyclic group *** terms of the orient ability of the orbit space Mof M by the actionτ,the value of n modulo 4 and a certain algebraic condition involving the first homology group of M,we are able to determine if the single homotopy class of maps from M to R^(2) possesses the Borsuk-Ulam property with respect to(Z_(n),τ).Finally,we give some examples of surfaces on which the symmetric group acts,and for these cases,we obtain some partial results regarding the Borsuk-Ulam property for maps whose target is R^(2).
