AIM:To determine the prevalence of a family history suggestive of Lynch syndrome (LS) among patients with colorectal cancer (cRc) followed in a coloproctology outpatient clinic in Southern Brazil.METHODS:A consecutive sample of patients with cRc were interviewed regarding personal and family histories of cancer.clinical data and pathology features of the tumor were obtained from chart review.RESULTS:Of the 212 cRc patients recruited,61 (29%) reported a family history of cRc,45 (21.2%) were diagnosed under age 50 years and 11 (5.2%) had more than one primary cRc.Family histories consistent with Amsterdam and revised Bethesda criteria for LS were identified in 22 (10.4%) and 100 (47.2%) patients,respectively.Twenty percent of the colorectal tumors had features of the high microsatellite instability phenotype,which was associated with younger age at cRc diagnosis and with Bethesda criteria (P cRc screening and only 4% of patients with suspected LS were referred for genetic risk assessment.cONcLUSION:A significant proportion of patients with cRc were at high risk for LS.Education and training of health care professionals are essential to ensure proper management.

Efficient in vivo delivery of anti-inflammatory proteins to modulate the microenvironment of an injured spinal cord and promote neuroprotection and functional recovery is a great challenge.Nucleoside-modified messenger RNA(mRNA)has become a promising new modality that can be utilized for the safe and efficient delivery of therapeutic proteins.Here,we used lipid nanoparticle(LNP)-encapsulated human interleukin-10(hIL-10)-encoding nucleoside-modified mRNA to induce neuroprotection and functional recovery following rat spinal cord contusion injury.Intralesional administration of hIL-10 mRNA-LNP to rats led to a remarkable reduction of the microglia/macrophage reaction in the injured spinal segment and induced significant functional recovery compared to controls.Furthermore,hIL-10 mRNA treatment induced increased expression in tissue inhibitor of matrix metalloproteinase 1 and ciliary neurotrophic factor levels in the affected spinal segment indicating a time-delayed secondary effect of IL-105 d after injection.Our results suggest that treatment with nucleoside-modified mRNAs encoding neuroprotective factors is an effective strategy for spinal cord injury repair.

Inductive Model Generation for Text classification Using a Bipartite Heterogeneous Network

Algorithms for numeric data classification have been applied for text classification. Usually the vector space model is used to represent text collections. The characteristics of this representation such as sparsity and high dimensionality sometimes impair the quality of general-purpose classifiers. Networks can be used to represent text collections, avoiding the high sparsity and allowing to model relationships among different objects that compose a text collection. Such network- based representations can improve the quality of the classification results. One of the simplest ways to represent textual collections by a network is through a bipartite heterogeneous network, which is composed of objects that represent the documents connected to objects that represent the terms. Heterogeneous bipartite networks do not require computation of similarities or relations among the objects and can be used to model any type of text collection. Due to the advantages of representing text collections through bipartite heterogeneous networks, in this article we present a text classifier which builds a classification model using the structure of a bipartite heterogeneous network. Such an algorithm, referred to as IMBHN （Inductive Model Based on Bipartite Heterogeneous Network）, induces a classification model assigning weights to objects that represent the terms for each class of the text collection. An empirical evaluation using a large amount of text collections from different domains shows that the proposed IMBHN algorithm produces significantly better results than k-NN, c4.5, SVM, and Naive Bayes algorithms.

An Unusual case of Submandibular Sialolithiasis in Early childhood: A case Report

Sialolithiasis is the most common condition of the salivary glands and it is characterized by the development of a calcified structure within a salivary duct. This condition can lead to inflammation, bacterial infection, and abscess. The different forms of management range from the direct massage of the duct to the complete removal of the gland. This report describes an unusual case of submandibular sialolithiasis in a four-year-old girl. The main complaints were pain when eating and swelling in the floor of the mouth. The diagnosis was sialolithiasis in the duct of the submandibular gland. Since the salivary stone was palpable and located in the anterior portion of the duct, a minimally invasive procedure was planned. An excisional biopsy was performed and a unilateral salivary stone was removed. The histopathological analysis confirmed the diagnosis. Although rare in children, understanding the diagnostic process and the different treatment modalities leads to a favorable prognosis.

pRB expression in esophageal mucosa of individuals at high risk for squamous cell carcinoma of the esophagus

AIM: To investigate the pRb expression in a large group of patients with history of chronic exposure to the main risk factors for development of squamous cell carcinoma of the esophagus. METHODS: One hundred and seventy asymptomatic individuals at high risk for esophageal squamous cell carcinoma (consumption of more than 80 g of ethanol and 10 cigarettes/d for at least 10 years) underwent upper gastrointestinal endoscopy with biopsies of the esophageal mucosa. As a control group, specimens of esophageal mucosa obtained from 20 healthy subjects were also studied. Immunohistochemical assessment of the tissues was performed using a monoclonal antibody anti-pRB protein. RESULTS: Absence of the pRB staining, indicating loss of RB function, was observed in 33 (19.4%) of the individuals at risk for esophageal cancer, but in none of the healthy controls (P creased in a stepwise fashion according to the severity of the histological findings (P cosa (11/97 or 11.3%), chronic esophagitis (17/60 or 28.3%), low-grade dysplasia (3/10 or 30%), high-grade dysplasia 1/2 or 50%) and squamous cell carcinoma (1/1 or 100%). cONcLUSION: Our findings suggest that abnormal expression of the pRB protein may be implicated in the process of esophageal carcinogenesis. Additional studies are warranted to define the role of the pRBprotein as a biomarker for development of esophageal squamous cell carcinoma in individuals at high risk for this malignancy.

The management of hepatocellular carcinoma(Hcc)is challenging because most patients have underlying cirrhosis,and the treatment provides,historically,a limited impact on the natural history of patients with advanced-stage disease.Additionally,recurrence rates are high for those patients who receive local and locoregional modalities,such as surgical(resection and transplantation)or imageguided(ablation and intra-arterial)therapies.Translational research has led to new concepts that are reshaping the current clinical practice.Substantial advancements were achieved in the understanding of the hallmarks that drive hepatocarcinogenesis.This has primed a successful incorporation of novel agents with different targets,such as anti-angiogenic drugs,targeted-therapies,and immune-checkpoint inhibitors.Although clinical trials have proven efficacy of systemic agents in advanced stage disease,there is no conclusive evidence to support their use in combination with loco-regional therapy.While novel local modalities are being incorporated(e.g.,radioembolization,microwave ablation,and irreversible electroporation),emerging data indicate that locoregional treatments may induce tumor microenvironment changes,such as hyperexpression of growth factors,release of tumor antigens,infiltration of cytotoxic lymphocytes,and modulation of adaptative and innate immune response.Past trials that evaluated the use of antiangiogenic drugs in the adjuvant setting after ablation or chemoembolization fail to demonstrate a substantial improvement.current efforts are directed to investigate the role of immunotherapy-based regimens in this context.The present review aims to describe the current landscape of systemic and locoregional treatments for Hcc,present evidence to support combination approaches,and address future perspectives.

Mitomycin c in pterygium treatment

Pterygium is a benign lesion usually growing from the nasal side of the conjunctiva onto the cornea.Most cases of pterygium does not cause problem or requires specific treatment.The exact cause of pterygium is not clear yet,but some factors are pointed as causes,being the most important the long-term ultraviolet ray exposure.Pterygium surgery is usually considered when there are symptoms that do not respond to conservative treatment.Recurrence is the main complication of the surgery,and much has been done to avoid it.Mitomycin c（MMc） has been used as a fibroblast proliferation inhibitor during the surgery to reduce the chance of recurrence of the pterygium.This review describes the use of MMc as an adjunctive,the optimal dosage,the duration of administration of MMc and possible complications,when used during,after and before the surgery.Most studies suggest that increased exposure（dose or duration） of MMc is associated with a lower recurrence,but with higher risks of complications.

Saline Intrusion Response to Sea Level Rise and Its Implications on Water and coastal Management: A case Study in Brazil

Global temperature is predicted to increase in the end of the century and one of the primary consequences of this warming is the sea level rise. considering the vulnerabilities on coastal systems and water resources, it is important to evaluate the potential effects of this rising in coastal areas, since the saline intrusion on rivers would be intensified, leading to problems related to water quality. In this context, the present work aimed to verify saline intrusion changes along an important river, São Francisco canal, located in Rio de Janeiro State, Brazil. For this purpose, a hydrodynamic modeling was performed using SisBaHiA, considering different sea levels and tide conditions. According to the results, it was verified the intensification on saline intrusion and higher salinity values due to a sea level rise of 0.5 m. These results show that new licenses for water withdrawals must be carefully analyzed as the fluvial flow plays an important role to contain the saltwater intrusion on the studied river. Accordingly, it is recommended the evaluation of climate change effects in order to choose best strategies to reduce coastal vulnerability, and the use of this theme on environmental licensing and territorial planning, integrating water planning with coastal management.

Radiofrequency ablation for treatment of hypersplenism: A feasible therapeutic option

We present a case of a patient with hypersplenism secondary to portal hypertension due to hepato-splenic schistosomiasis, which was accompanied by severe and refractory thrombocytopenia. We performed spleen ablation and measured the total spleen and ablated volumes with contrast-enhanced computed tomography and volumetry. No major complications occurred, thrombocytopenia was resolved, and platelet levels remained stable, which allowed for early treatment of the patient's underlying disease. Previous work has shown that splenic radiofrequency ablation is an attractive alternative treatment for hypersplenism induced by liver cirrhosis. We aimed to contribute to the currently sparse literature evaluating the role of radiofrequency ablation(RFA) in the management of hypersplenism. We conclude that splenic RFA appears to be a viable and promising option for the treatment of hypersplenism.
