Role of Genetic Ancestry in Oropharyngeal Squamous-Cell Carcinoma: A Cross-Sectional Study in Brazil

Background: HPV infection represents an important etiologic factor for Oropharyngeal Squamous Cell Carcinoma (OPSCC). The different ethnic backgrounds could be related to different susceptibility to Human Papillomavirus (HPV). The aim of our study was to assess the whole of genetic ancestry in HPV status in OPSCC patients. Methods: We conducted a cross-sectional study on patients with OPSCC admitted to the Barretos Cancer Hospital, Brazil from 2014 to 2019. Of these, DNA extraction was performed on 40 patients and genetic ancestry was assessed using a specific panel of 46 informative ancestry markers. Results: We observed a predominance of European ancestry (63%), followed by African (18%), Amerindian (9%) and Asian (8%) both in the OPSCC HPV-positive and HPV-negative group. We did not find any statistically significant differences between the HPV-positive and HPV-negative OPSCC groups in relation to European (p = 0.499), African (p = 0.448), Asian (p = 0.275) or Amerindian (p = 0.836) ancestry. Conclusions: We found a predominance of European ancestry, both in the HPV-positive and HPV-negative groups. In our study, we did not find statistically significant differences between HPV-positive or HPV-negative groups in relation to ancestry.

Because the aetiology of chronic pelvic pain is complex, studies of the condition involve extensive investigation but provide few conclusions. Numerous studies have addressed the experiences of women with chronic pelvic pain, as well as the interaction between those women and their health care providers. Our objective was to investigate how physicians at a specialised clinic perceive the medical care provided to such women. This was a qualitative study employing semi-structured interviews and content analysis. We interviewed seven physicians at the Chronic Pelvic Pain Outpatient Clinic of the University Hospital, Faculty of Medicine of Ribeir?o Preto, University of S?o paulo, Brazil. Medical training and practice constituted the central theme of the study, which was subdivided into categories addressing the influence that the current medical training has on the type of medical care provided to women with chronic pelvic pain. Medical practice has been characterised by a reductionist approach to health and illness, as well as by the fragmentation of health care. These characteristics are, to a certain extent, the result of the biomedical model of education, which has been predominant, ignoring social, cultural, psychological and emotional aspects. There is a need to shift the medical paradigms toward a humanistic model of health care. We hope that we have provided a critical view of current medical training and practice, as well as of their effects in various health care settings, particularly in the provision of care to women with chronic pelvic pain.

During the 2014-2016 water shortage crisis, the Metropolitan Area of Sao paulo (MASP) water supply system extracted pumping volume from the Cantareira System. Before the crisis, between 1984 and 2013, the reservoir’s average water extraction flow was 29.6 m3·s-1. During the period of pumping volume usage, the average extraction flow was 16.2 m3·s-1. Following the crisis, two new mitigation policies were implemented: a water extraction Resolution (in 2017) and a Resolution for water reallocation from another basin (in 2018). This study provides a novel investigation of the Cantareira System water crisis by assessing the mitigation policies impacts on storage level dynamics. The system storage level was evaluated using the reservoir simulation module of PDM-Cemaden hydrological model, assuming that the new policies had already been implemented prior to the crisis. A control simulation was run with observed in- and out-flow and operationally-practiced extraction flow. The storage level dynamics impacts were evaluated under 4 water mitigation policies scenarios varying the policies implementation starting date, the extraction flow range and including the water reallocation variable. Results showed that pumping volume would only need extraction during a short period (Scenarios I, III and IV), and considering the water reallocation, pumping volume extraction would not have been necessary (Scenario II). Although the pumping volume would still have been extracted during a short period, water shortage impact would have been lessened, had the policies been already implemented before the crisis. The water mitigation policies implementation supports the reservoirs storage management but does not guarantee that MASP water demand is fully met. Therefore, in order to effectively improve water security, further policies and practices to reduce water demand and enhance supply should be considered.

After 4K, 8K UHD resolution can be considered the new challenge for film, visualization and television. Due to the dimension of 8K image, the remote micro and macro images are considered appropriate for visualization with scientific and educational purposes. This article describes the implementation of the 8K video plug-in to play UHD videos into the SAGE, a collaborative environment for the advancement of scientific research and education developed by EVL (Electronic Visualization Laboratory), upgrading its current resolution for video playing. It also presents the challenges of producing 8K content, the development of the code and introduces the features to view 8K ultra high definition videos in multiple tiles through the use of low cost computer clusters. The SAGE 8K video plug-in implementation was made possible because of the previous development of the Fogo Player, a system to run 4K and 8K content. The results were successfully presented at the CineGrid International Workshop 2013 in San Diego.

Role of Cognitive Behavioral Therapy in Fibromyalgia: A Systematic Review

A literature search of articles from 2002-2019 was performed using Medline, Embase, Cochrane, LILACS, IBECS, CRD, and Epistemonikos databases, to analyze the effects of Cognitive-Behavioral Therapy (CBT) in the treatment of fibromyalgia. Twenty-seven articles were selected in which CBT was performed exclusively by specialist physicians, associated or not with conventional pharmacological treatment and/or physical exercise. In most articles, CBT worked with self-knowledge and cognitive restructuring, attempting to reduce pain perception, and it showed a general improvement in daily activities by decreasing patient’s limitations, such as morning stiffness. The literature showed significant correlations of CBT in pain processing over time. When CBT was compared to conventional pharmacological therapy, a certain superiority of CBT could be observed concerning the quality of life, catastrophizing, and acceptance of pain. However, when they were simultaneously applied, this improvement in quality of life was not observed.

Oral PTCTS (Particulated Transialidase) Removes Serum Microparticles and Decreases Inflammation in Atherosclerotic Plaques of Rabbits

Background: Previous studies showed that atherosclerotic plaque vulnerability was related with microparticles (MPs)-vesicles larger than 100 nm, which released MMP9 collagenase. In our previous study, intramuscular injection of a new drug (PTCTS) normalized oxidized LDL serum levels and reduced rabbit atherosclerosis. Now, we studied administration of oral PTCTS in order to clarify anti-atherosclerotic mechanism of action, analyzing if the treatment removed MPs containing ox-LDL and Mycoplasma pneumoniae antigens and improved the immune response. Methods: We compared two groups of rabbits. Control group (CG, n = 6)—1% cholesterol enriched diet for 12 weeks;Treated group (TG, n = 8)—1% cholesterol enriched diet for 12 weeks with administration of PTCTS (400 μl/day) during the last 6 weeks of diet. The animals had their blood collected, in three different phases of the protocol before being fed with hypercholesterolemic diet, before being treated with water or PTCTS and at the moment of sacrifice. The serum was submitted to immunofluorescence technique to evaluate the quantity of microparticles marked with antibodies against Mycoplasma pneumoniae and ox-LDL. A fragment of aorta was submitted to immunohistochemical detection of antigens from MMP9, ox-LDL, NF-κB and IL-1β. Results: PTCTS showed significant reduction in MMP-9 (P = 0.001) and a tendency of reducing IL-1β (P = 0.09) in the aortic plaques compared with CG. In the serum, PTCTS was able to remove microparticles containing antigen of ox-LDL (P = 0.004) and Mycoplasma pneumoniae (P β and mycoplasma, as well as a better stabilization of the atheromatous plaque by reducing levels of MMP-9, avoiding plaque rupture, without causing mortality or toxicity.

COVID-19: Feelings Experienced by Nurses, Physiotherapists, and Medical Doctors in a Brazilian Field Hospital

Facing critical situations such as the ones triggered by COVID-19 may take healthcare professionals to confront their psychological resources, which may induce a higher stress level. We aimed to understand the feelings experienced by nurses, physiotherapists, and medical doctors in a field hospital dedicated to patients with COVID-19 in the biggest city of Brazil. Methods: We performed a qualitative study in a state field hospital in S?o paulo city, Brazil, in a sample of 25 volunteer healthcare professionals. Participants answered to a sociodemographic electronic survey and were subjected to an individual semi-structured on-site interview. Results: Participants were mostly female (68%), comprised eight nurses, eight physiotherapists, and nine medical doctors, had an average age of 36.8 years, and were professionally experienced. Speech analysis revealed two thematic categories (related to work and assistance in the pandemic) and 11 subcategories: adaptation to the scenario, stressing experience, psychological support, difficulties with workwear, safety with workwear, non-verbal communication, empathy, patient comfort, outstanding experiences, major challenges, and teamwork. Conclusion: Nurses, physiotherapists, and medical doctors’ experiences while working in the field hospital were permeated by both positive and negative feelings. Those feelings originated directly from the fear of the unknown when dealing with a new disease without well-established scientific evidence, the use of medical workwear for long periods, and communication and environmental difficulties. In contrast, positive feelings, related to teamwork and empathy, arose and overlapped with the difficulties.
