Effect of propranolol on the splanchnic and peripheral renin angiotensin system in cirrhotic patients

AIM:To evaluate the effect of β-blockade on angiotensins in the splanchnic and peripheral circulation of cirrhotic patients and also to compare hemodynamic parameters during liver transplantation according to propranolol pre-treatment or not. METHODS:Patients were allocated into two groups:outpatients with advanced liver disease(LD) and during liver transplantation(LT). Both groups were subdivided according to treatment with propranolol or not. Plasma was collected through peripheral venipuncture to determine plasma renin activity(PRA),Angiotensin(Ang) Ⅰ,Ang Ⅱ,and Ang-(1-7) levels by radioimmunoassay in LD group. During liver transplantation,hemodynamic parameters were determined and blood samples were obtained from the portal vein to measure renin angiotensin system(RAS) ***:PRA,Ang Ⅰ,Ang Ⅱ and Ang-(1-7) were signifi cantly lower in the portal vein and periphery in all subgroups treated with propranolol as compared to non-treated. The relationships between Ang-(1-7) and Ang Ⅰ levels and between Ang Ⅱ and Ang Ⅰ were significantly increased in LD group receiving propranolol. The ratio between Ang-(1-7) and Ang Ⅱ remained unchanged in splanchnic and peripheral circulation in patients under β-blockade,whereas the relationship between Ang Ⅱ and Ang Ⅰ was significantly increased in splanchnic circulation of LT patients treated with propranolol. During liver transplantation,cardiac output and index as well systemic vascular resistance and index were reduced in propranolol-treated subgroup. CONCLUSION:In LD group,propranolol treatment reduced RAS mediators,but did not change the ratio between Ang-(1-7) and Ang Ⅱ in splanchnic and peripheral circulation. Furthermore,the modification of hemodynamic parameters in propranolol treated patients was not associated with changes in the angiotensin ratio.

Keloid of Ears: Recurrence and Its Complications

In this case report, we present a male patient who arrived at the hospital for the first time at the age of 8 with giant keloids in both ears and diagnosed by the dermatology and Plastic Surgery as having the “Disease of Jorge Lobo”, which is a fungal infection due to the story of the endemic characteristics. After the first surgical intervention, he received guidance from the team for keloids and left the hospital using elastic mesh and a request to change the place where he lived to reduce his contact with the fungi. Fourteen years later, the patient returned to the Plastic Surgery Service having even larger keloids in both ears, twice the size than the first time. We did the second surgical intervention to remove it, but with the patient’s commitment that he would correctly comply with the guidelines determined by the surgical team. After finishing all the steps and a post-operative for the case, we observed the satisfaction and the increase of the patient’s mood, happier and without the embarrassment of that physical defect.

Behavioral Risk Factors for Chronic Non-Communicable Diseases in Adults and Elderly People in the City of Ribeirão Preto—SP

Aims: to analyze the presence and simultaneity of behavioral risk factors for chronic non-communicable disease (NCD) in adults and the elderly. Methods: quantitative, cross-sectional and observational study. Healthy and unhealthy eating are classified according to the frequency of food consumption. Smoking is expressed by the percentage of smokers. Harmful alcohol consumption by the percentage of individuals who consumed alcoholic beverages at least once in the last 30 days. Classification of the level of physical inactivity required to practice at least 150 minutes of physical activity. Results: the sample constituted 719 people, 535 adults and 184 elderly. The behavioral risk factor reported by most adults 499 (93.3%) and elderly 156 (84.8%) was unhealthy eating. Frequency 72% higher of smoking, four times higher of harmful alcohol consumption and 10% higher of unhealthy food among adults when compared to the elderly. The simultaneity between risk factors was 39% greater in adults than in the elderly. Discussion: risk factors evaluated in this study are key aspects of the development of NCDs. Adults have 2.43 times the chance of presenting two risk factors and 7.73 times the chance of presenting three in relation to the elderly. Conclusion: To achieve more effective and differentiated results, knowing and directing measures to control behavioral risk factors, whether isolated or concurrent, requires specific knowledge.

This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.

Characteristics of Mesenchymal Stem Cells under Hypoxia

Mesenchymal stem cells (MSC) are considered non-hematopoietic multipotent stem cells with self-renewal properties and the ability to differentiate into a variety of mesenchymal tissues. Optimal conditions for the culture of these cells have been the subject of investigation for several years. In particular, ideal oxygen tension levels have not been established in the literature. In physiological environments, oxygen tension may vary from 12% in peripheral blood to 1% in the deep zone of cartilage regions. In any case, oxygen tension is considerably lower in vivo when compared with the normal atmosphere of standard cell culture conditions (21%). The objective of this study was to review the literature available on MSC characteristics (cell cycle, survival, proliferation, differentiation, morphology, immunophenotype, cytogenetics) when cultured under hypoxic conditions. Our focus on optimal culture conditions is justified by the key role currently played by these cells in regenerative medicine.

Risk of Surgical Site Infection in Patients Undergoing Total Knee Arthroplasty

The aim of this study was to determine the participation of risk factors in the infection in patients who underwent total knee arthroplasty. This is a prospective longitudinal study, descriptive and analytical, that was carried out between January 2013 and december 2015. Eighty-one procedures were performed in 78 individuals, 16 males and 62 females. Three subjects underwent the procedure on both knees. The following predictive factors of infection were identified: Age, gender, comorbidities, evaluation of functional physical status using the American Society of Anesthesiologists Physical Status Classification System (ASAPS), length of hospital stay, use of prophylactic antibiotics and duration of surgery. The data was tabulated in a database using a Microsoft Excel spreadsheet. For statistical analysis, the chi-square test, chi-square test for independence and Fischer’s exact test were applied, all at a significance level of 5% (0.05). Surgical site infection was more prevalent in males (23.5%), age over 70 years (25.0%), for left gonarthrosis (17.9%), in the presence of comorbidities (17.9%), when hospital stay was 6 or more days (27.8%), in total arthroplasties of left knee (17,9%), in surgical times of up to 2 hours (20%), in the use of prophylactic antibiotics (16.7%) and in level II classification in the ASAPS (18.8%). The independence test was not significant in any of the evaluated factors (p-value greater than 0.05). The most prevalent bacteria identified in cases of infection was Staphylococcus aureus (33.2%). The risk factors studied were not statistically significant.

Background:Leishmaniasis causes alterations and lesions in the genital system,which leads to azoospermia and testicular atrophy in animals during the chronic phase of the *** aim of this study was to reveal the kinetics of Leishmania chagasi infection in the genital system of male golden hamsters(Mesocricetus auratus).Methods:Animals were intraperitoneally inoculated with amastigotes from *** different time points animals were euthanized and genital organs processed for histo-pathological,qPCR,cytokines and testosterone detection ***:Our results showed a high parasite load in testis,followed by an increase of pro-inflammatory cytokines IL1-β,TNF-αand IFN-γ,and ***,IL-4 expression was upregulated and basal parasite persistence in testis was observed using the experimental ***:Extracellular amastigotes migrated to the epididymis posing as a potential major factor of parasite persistence and venereal transmission of *** infection in hamsters.

Aquatic physical therapy for Parkinson’s disease

One of the predominant effects of PD is alteration of motor skills, which leads to compromised posture and physical balance. Aquatic physical therapy, performed in a heated pool, can serve as one of intervention alternatives for PD carriers. Aim: To analyze the effects of the Halliwick Concept hydrotherapy in acquisition of motor skills in Parkinson’s disease (PD) patients. Methods: A clinical, qualitative method was used to evaluate 7 volunteers (59.85 ± 7.92 years of age, both male and female), who were in II and III stages of PD according to the Hoehn & Yar Scale. The participants were evaluated in the start and end of the interventions phase, and then the evaluation was conducted interventions in a heated pool (32?C - 34?C) twice a week, each session lasting approximately 30 minutes, totaling 10 sessions. The Halliwick Principles’ 3-phase 10-point methods were used for acquisition of aquatic motor skills. Results: They indicate an improvement in their ability to float in prone and supine positions (P = 0.04*) and longitudinal rotation in the bipedal position. Conclusion: The activation of motor control improved the motor skills of the participants.

Each Epiphyte on Its Branch: A Comparative Study between Different Phorophytes

Epiphytes are plants that use the mechanical support provided by a host tree, called phorophytes, without emitting haustorial structures. Constituting an important component of the flora, playing an important role in the maintenance of ecosystems, such as water and nutrient cycling, in addition to providing resources, sometimes unique, such as food and shelter, for the canopy fauna. However, the epiphytic flora may show high beta diversity, even when compared with different phorophytes, possibly as a response to the dendromorphological characteristics of the phorophyte. Thus, this work aims to: 1) compare whether the species richness of epiphytes differs in trees with different morphological characteristics, and 2) compare species composition among three tree species. For this, 10 individuals of three tree species were selected: Guaria guidonia, Ficus sp., and Roystonea oleracea. In each individual, the number of species (richness) of vascular epiphytes was recorded and obtained from the species composition. To test differences in richness values between the three trees, we used analysis of variance (ANOVA) followed by Tukey’s test. To understand whether the epiphyte community formed separate groups according to the tree species used, we used the available non-metric multidimensional scaling method (NMDS). Thirty-four species of epiphytes were found in the 30 phorophytes surveyed. The three tree species showed significantly different richness, with the highest found in Guarea guidonea and the lowest in Roystonea oleraceae. The NMDS showed that epiphytes group the three tree species into distinct groups, although Ficus sp individuals present a transitional community between the two other species. Thus, the results suggest that the differences in the richness and composition of the epiphytic flora of the different phorophytes are influenced by their distinct morphological characteristics.

What Do Seedlings Like? The Relationship between Seedling Richness and Abundance with Abiotic Factors

Natural regeneration is the interaction of natural processes to restore the forest ecosystem. Its dynamics are influenced by the intensity and extent of a series of abiotic and biotic factors, which may be intrinsic or extrinsic. Knowing the importance of establishing natural regeneration within forest ecosystems, this work aimed to evaluate whether litter depth and trail distance influenced seedling abundance and richness in a forest fragment undergoing natural restoration. The hypothesis tested in this research was that abiotic factors influence the natural regeneration of this forest since they are factors that are directly linked to seed germination and seedling establishment. 30 plots of 1 m2 were randomly analyzed within the forest located on the brown trail. A millimeter ruler was used to diagnose the litter depth and a measuring tape to measure the distance from the plot to the edge. In each plot the seedlings were morpho-specified and each morphospecies had the number of individuals counted. Linear regression tests were performed to assess the relationships between species richness and trail distance and litter depth. The same was done for species abundance. All results showed that there is no relationship pattern between any of the variables. Other factors also influence the regeneration of a forest, such as luminosity and seed bank. Furthermore, litter depth is related to the successional stage of the forest. It was concluded that the abiotic factors tested do not influence the regeneration of the study area.
