Physical Accessibility for Disabled People: Analysis of Toilet Facilities in Primary Health Care Units

Background: Accessibility enables the effective participation of disabled persons in public and private areas and the use of urban equipment and street furniture. Objective: This study aimed to analyze the physical accessibility of toilet facilities in urban and rural primary health care units. Methods: It was a quantitative descriptive study conducted in 157 Primary Health Care Units of 16 municipalities in the Baturité Massif region, Ceará, Brazil. The study took place from August 2014 to May 2015, with a checklist type form, designed from the Technical Standard 9050 of the Brazilian National Standards Organization, specifying the ideal access conditions established by law to allow mobility of persons with physical disabilities. Data were processed through the Statistical Package for the Social Sciences software and organized into tables. Results: Toilets designed for physically disabled people were accessible as the location and signs (59.9%), identified with symbols for males and females (57.3%);however, the doorway width was smaller than needed to accommodate a wheelchair (77.7%). Inside the bathroom, only the forward approach was possible (59.9%). Grab bars positioned on the side and rear walls were inadequate or nonexistent (67.6%);toilet seats (91.1%) and toilet paper dispensers (96.2%) were mostly in inaccessible heights;flush controls in appropriate height (59.2%) and activated by light pressure (58%). Sinks without pedestal (51%), but higher than recommended (80.3%) and without single handle faucets (95.6%). It was verified that the toilets of basic health units located in urban areas had better accessibility conditions compared to those in rural areas. Conclusion: Results showed that the analyzed units presented physical inaccessibility in some toilet facilities, making it difficult or even impossible the accessibility for the disabled. Objective: To validate the educational board game “Drugs: playing fair” for visually impaired people in Brazil and Portugal. Methods: Study of apparent validation carried out in two associations for visually impaired people in Fortaleza, Brazil, and in Porto, Portugal. Thirty-six visually impaired people, 18 from each country, participated in the study. An evaluation tool with 23 items on specifications, content and motivation of the game was applied. Results: The scores awarded in both countries were excellent, with means varying in Brazil from 9.0 to 9.6 and in Portugal, from 8.4 to 9.2. As for the categories and subcategories, the best means in Brazil were: content (9.5);theoretical and methodological consistency (9.6) and concepts/information (9.5). In Portugal, the best means were concepts/information (9.2) and curiosity (9.2). Only two items showed a significant difference: “it allows interaction” (p = 0.024) and “compatible degree of difficulty” (p = 0.012). Conclusion: The educational game on drugs was validated in Brazil and Portugal.

A quantitative descriptive study aimed to evaluate the physical structure accessibility of gynecological nursing clinics and their bathrooms in basic health units for users with motor disabilities. A checklist tool/instrument was applied for analyzing 16 items in the bathroom and four in the nursing clinic. 157 basic health units were evaluated. In the nursing clinic, counters and tables (79.6%) were accessible, but stretchers (93%), seats (93.6%) and doors (87.3%) were inaccessible;moreover, 17.8% of the units examined proved totally inaccessible. In the bathroom, the items with the best evaluation were the height of the toilet flusher (47%) with operation by slight pressure (44.6%) and the wall-mounted sink (42.7%). Bathroom doors and turning area/space for wheelchairs were inaccessible. No bathroom reached full accessibility, and 31.2% were entirely inaccessible. Non-compliance was evident of the standard 9050:2004 of the Brazilian Association of Technical Standards. Raising the awareness of health and engineering professionals is recommended to learn technical accessibility standards and implement them.

Doxycycline （DOX） exhibits anti-inflammatory and MMP inhibitory properties. The objectives of this study were to evaluate the effects of DOX on alveolar bone repair. Controls （CTL） and DOX-treated （10 and 25 mg. kg- 1） molars were extracted, and rats were killed 7 or 14 days later. The maxillae were processed and subjected to histological and immunohistochemical assays. Hematoxylin-eosin staining （7th day） revealed inflammation in the CTL group that was partly reversed after DOX treatment. On the 14th day, the CTL group exhibited bone neoformation, conjunctive tissue, re-epithelization and the absence of inflammatory infiltrate. DOX-treated groups exhibited complete re-epithelization, tissue remodelling and almost no inflammation. Picrosirius red staining in the DOXlO group （7th and 14th days） revealed an increased percentage of type I and III collagen fibres compared with the CTL and DOX25 groups. The DOXlO and DOX25 groups exhibited increases in osteoblasts on the 7th and 14th days. However, there were fewer osteoclasts in the DOXlO and DOX25 groups on the 7th and 14th days. Wnt-lOb- immunopositive cells increased by 130% and 150% on the 7th and 14th days, respectively, in DOX-treated groups compared with the CTL group. On the 7th day, Dickkopf （Dkk）-I immunostaining was decreased by 63% and 46% in the DOXlO and DOX25 groups, respectively. On the 14th day, 69% and 42% decreases in immunopositive cells were observed in the DOXlO and DOX25 groups, respectively, compared with the CTL group. By increasing osteoblasts, decreasing osteoclasts, activating Wnt lOb and neutralising Dkk, DOX is a potential candidate for bone repair in periodontal diseases.

This study aimed to show that the physicochemical proprieties obtained by Fourier transform infrared spectroscopy （FTIR）, thermogravimetry （TG）, and scanning electronic microscopy （SEM） can be useful tools for evaluating the quality of active pharmaceutical ingredients （APIs） and pharmaceutical products. In addition, a simple, sensitive, and efficient method employing HPLC-DAD was developed for simulta- neous determination of lidocaine （LID）, ciprofloxacin （CFX） and enrofloxacin （EFX） in raw materials and in veterinary pharmaceutical formulations. Compounds were separated using a Gemini Cm （250 mm×4.6 mm, 5μm） Phenomenex column, at a temperature of 25℃, with a mobile phase containing 10 mM of phosphoric acid （pH 3.29）： acetonitrile （85.7：14.3, v/v） and a flow rate of 1.5 mL/min. Physicochemical characterization by TG, FTIR, and SEM of raw materials of LID, CFX, and EFX provided information useful for the evaluation, differentiation, and qualification of raw materials. Finally, the HPLC method was proved to be useful for evaluation of raw material and finished products, besides satisfying the need for an analytical method that allows simultaneous determination of EFX, CFX, and LID, which can also be extended to other matrices and applications.

Background: This study evaluated the effects of Eimeria spp. challenge and dietary supplementation with free methionine or methionine dipeptide on animal performance;expression of genes associated with the immune system, antioxidant system, and amino acid transport in the jejunum;and redox status of the jejunum of broiler ***: A randomized, 2 × 3 factorial design was used, in which Eimeria spp. challenge was the first factor(Eimeria-challenged, EC, or unchallenged, UC, broilers) and methionine supplementation was the second factor(non-supplemented, NS;free DL-methionine, DL-Met;and methionine dipeptide, DL-methionyl-DL-methionine, DLMMet). At 14 days of age, chickens were inoculated orally with sporulated oocysts of Eimeria acervulina, Eimeria praecox, Eimeria maxima, and Eimeria mitis. Birds were killed by cervical dislocation 144 h post-inoculation(PI), and the jejunum was collected for biochemical and molecular ***: EC broilers had a 13% lower feed intake(FI), 37% lower body weight gain(BWG), and 39% higher feed conversion ratio(FCR) than UC broilers. Chickens fed the DL-Met diet had higher BWG(about 12% higher) and better FCR(about 12% lower) than chickens fed the NS diet. EC chickens had lower relative weight of the bursa of Fabricius(51.8%) and higher relative weights of the spleen and whole intestine(53.6% and 26.3%, respectively) than UC chickens. Eimeria spp. challenge led to an increase in the levels of oxidative substances, such as nitrite and thiobarbituric acid reactive substances(TBARS), in the jejunum of chickens 144 h PI. Among UC chickens, those fed the DL-Met diet had higher total antioxidant capacity(TAC) and lower catalase(CAT) and superoxide dismutase(SOD) activities. EC chickens that received the NS diet had higher carbonylated protein content(CP). This result was associated with their lower TAC and catalase activity.
