New therapeutic strategy with extracorporeal membrane oxygenation for refractory hepatopulmonary syndrome after liver transplant: A case report

BACKGROUND Due to the lack of published literature about treatment of refractory hepatopulmonary syndrome(hPS)after liver transplant(LT),this case adds information and experience on this issue along with a treatment with positive outcomes.hPS is a complication of end-stage liver disease,with a 10%-30%incidence in cirrhotic patients.LT can reverse the physiopathology of this process and restore normal oxygenation.however,in some cases,refractory hypoxemia persists,and extracorporeal membrane oxygenation(ECMO)can be used as a rescue therapy with good results.CASE SUMMARY A 59-year-old patient with alcohol-related liver cirrhosis and portal hypertension was included in the LT waiting list for hPS.he had good liver function(Model for End-Stage Liver Disease score 12,Child-Pugh class B7).he had pulmonary fibrosis and a mild restrictive respiratory pattern with a basal oxygen saturation of 82%.The macroaggregated albumin test result was>30.Spirometry demonstrated a forced expiratory volume in one second(FEV1)of 78%,forced vital capacity(FVC)of 74%,FEV1/FVC ratio of 81%,diffusion capacity for carbon monoxide of 42%,and carbon monoxide transfer coefficient of 57%.he required domiciliary oxygen at 2 L/min(16 h/d).The patient was admitted to the intensive care unit(ICU)and extubated in the first 24 h,needing high-flow therapy and non-invasive ventilation and inhaled nitric oxide afterwards.Reintubation was needed after 72 h.Due to the non-response to supportive therapies,installation of ECMO was decided with progressive recovery after 9 d.Extubation was possible on the tenth day,maintaining a high-flow nasal cannula and de-escalating to conventional oxygen therapy after 48 h.he was discharged from ICU on postoperative day(POD)20 with a 90%-92%oxygen saturation.Steroid recycling was needed twice for acute rejection.The patient was discharged from hospital on POD 27 with no symptoms,with an 89%-90%oxygen saturation.CONCLUSION Due to the favorable results observed,ECMO could become the central axis of treatment of hPS and ref

We set out to model the oven-drying kinetics of a legume known as pigeon pea, harvested in the Bouenza department in the south-west of the Republic of Congo. The drying kinetics of pigeon peas was carried out in an oven under experimental conditions using temperatures of: 50°C, 60°C and 70°C. Seven mathematical models were used to describe pigeon pea drying. During drying, water loss was faster and shorter at 70°C [10.446 g/25 g wet weight (wwb) for 320 min (5.3 h)] compared to 50°C [10.996 g/25 g wet weight (wwb) for 520 min (8.6 h)] and 60°C [10.616 g/25 g wet weight (wwb) for 420 min (7.0 h)] where it was slower and longer. With regard to modeling, and based on the principle of choosing the right model focusing on the high value of R2 and low values of χ2 and RMSE, two models were selected, the Midili model for temperatures of 50°C and 60°C and the henderson and Pabis model modified for temperature of 70°C showed better results. The R2, χ2 and RMSE values calculated for pigeon pea are 0.99985, 3.93404E-5 and 0.00627;0.9997, 9.245E-5 and 0.00962;0.99996, 1.56332E-5 and 0.00395 respectively at 50°C, 60°C and 70°C.

The current trend of replacing a percentage of gasoline with ethanol has promoted the development of new processes for its production from lignocellulosic biomass. This work reports the production of ethanol from the Camalote grass (Paspalum fasciculatum Willd). The lignocellulosic biomass was subjected to acid hydrolysis at 125C and 15 psi with h2SO4 concentrations at 5%, 10%, and 20%, obtaining an average of reducing sugars (pentoses and hexoses) from the hydrolyzed juice with 12.3%, 10%, and 17% Brix, respectively. The sugars were fermented using yeast of the Saccharomyces cerevisiae at 30C for 48 hours. Finally, the ethanol was distilled at 78C, and the average yields were obtained through analysis of variance with a 95% confidence level. The values indicate that there is a significant difference (p > 0.05), the Tukey study shows that all the % v/v averages are different from each other. For h2SO4 concentration at 5% (10.33 ± 2), h2SO4 at 10% (9.33 ± 1.8), and h2SO4 at 20% (6.33 ± 2). The acidity analysis for the ethanol obtained from each treatment gave a value of 1.8 mg/L of acetic acid in all cases.

While electrocatalyzed alkyne annulations of arenes represent a powerful strategy for the assembly of heteroaromatic motives,electrochemical C(sp2)−h activations of alkenes remain scarce.herein,a strategy for the rhodaelectro-catalyzed function-alization of enamides is presented for the efficient construction of pyrroles using electricity as a sustainable oxidant.Moreover,the tuning of the solvent system allowed a fascinating switch in chemoselectivity,which is not possible with traditionally used chemical oxidants,giving rise to lactone architectures.The rhoda-electrocatalysis features a broad scope as well as high regio-and chemoselectivities.

BACKGROUND： An emergency department （ED） visit may be distressing and anxiety-provoking for older adults （age 〉 65 years）. No studies have specifically evaluated the effect of music listening on anxiety in older adults in the ED. OBJECTIVE： The objective of this pilot study was to evaluate the effect of music listening on anxiety levels in older ED patients. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This was a randomized pilot study in the geriatric ED of an urban academic tertiary medical center. This was a sample of English-speaking adults （age 〉 65 years） who were not deaf （n = 35）. Subjects consented to participate and were randomized to receive up to 60 min of music listening with routine care, while the control group received routine care with no music. Subjects in the music treatment group received headphones and an electronic tablet with pre-downloaded music, and were allowed to choose from 5 selections. MAIN OUTCOME MEASURES： The primary outcome was change in anxiety levels, measured by the state- trait anxiety inventory （STAI）, at enrollment and 1 h later. RESULTS： A total of 35 participants were enrolled： 74% were female, 40% were white, and 40% were black; of these, 32 subjects completed the study protocol. When comparing control （n = 18） against intervention subjects （n = 17）, there were no significant differences in enrollment STAI scores （43.00 ± 15.00 vs. 40.30± 12.80, P = 0.57）. STAI scores 1 hour after enrollment （after the music intervention） were significantly reduced in the intervention subjects compared to the control subjects （with reduction of 10.00± 12.29 vs. 1.88 ± 7.97, P = 0.03）. CONCLUSION： These pilot results suggest that music listening may be an effective tool for reducing anxiety among older adults in the ED.

Overexpression of neurotrophic factors in nigral dopamine neurons is a promising approach to reverse neurodegeneration of the nigrostriatal dopamine system,a hallmark in Parkinson's disease.The human cerebral dopamine neurotrophic factor(h CDNF)has recently emerged as a strong candidate for Parkinson's disease therapy.This study shows that h CDNF expression in dopamine neurons using the neurotensinpolyplex nanoparticle system reverses 6-hydroxydopamine-induced morphological,biochemical,and behavioral alterations.Three independent electron microscopy techniques showed that the neurotensin-polyplex nanoparticles containing the h CDNF gene,ranging in size from 20 to 150 nm,enabled the expression of a secretable h CDNF in vitro.Their injection in the substantia nigra compacta on day 21 after the 6-hydroxydopamine lesion resulted in detectable h CDNF in dopamine neurons,whose levels remained constant throughout the study in the substantia nigra compacta and striatum.Compared with the lesioned group,tyrosine hydroxylase-positive(Th^(+))nigral cell population and Th+fiber density rose in the substantia nigra compacta and striatum after h CDNF transfection.An increase inβIII-tubulin and growth-associated protein 43 phospho-S41(GAP43 p)followed Th^(+)cell recovery,as well as dopamine and its catabolite levels.Partial reversal(80%)of drugactivated circling behavior and full recovery of spontaneous motor and non-motor behavior were achieved.Brain-derived neurotrophic factor recovery in dopamine neurons that also occurred suggests its participation in the neurotrophic effects.These findings support the potential of nanoparticle-mediated h CDNF gene delivery to develop a disease-modifying treatment against Parkinson's disease.The Institutional Animal Care and Use Committee of Centro de Investigación y de Estudios Avanzados approved our experimental procedures for animal use(authorization No.162-15)on June 9,2019.

The sediment deposited behind a dam(DS) and its inorganic materials(IM-DS) were characterized and used for the removal of praseodymium(Pr) from aqueous solutions. Quartz, albite, and kaolinite were found in the sediment by X-ray diffraction. Kinetic data fit well to a pseudo second-order model, the equilibrium time for DS and IM-DS was 2 h and 3 min, respectively, and the adsorption capacity was higher for Pr/DS(qe= 4.91 mg/g) than for Pr/IM-DS(qe= 3.01 mg/g) in these conditions. Pr/DS and Pr/IM-DS data isotherms were in agreement with the Langmuir-Freundlich model and the maximum adsorption capacities were 4.95 and 4.01 mg/g, respectively. Both processes are exothermic(enthalpy change, Δh0=-22.8 and-21.8 k J/mol for DS and IM-DS, respectively), the increase of the randomness is small, and the feasibility and spontaneity are low. Praseodymium was desorbed by ethylenediaminetetraacetic acid disodium(EDTA-Na2), sodium chloride, nitric, citric, and humic acids solutions, and one hundred percent of praseodymium was desorbed by the EDTA-Na2solution. The Pr2(EDTA)3(praseodymium ethylenediaminetetraacetate) complex was completely desorbed from both materials(DS and IM-DS). Distribution coefficients(KD) depend upon praseodymium concentration,temperature, p h, and the kind of adsorbent. Chemical reactions of praseodymium with kaolinite and iron minerals could explain the adsorption.

We introduce the concept of Canonical Workflow Building Blocks(CWBB),a methodology of describing and wrapping computational tools,in order for them to be utilised in a reproducible manner from multiple workflow languages and execution platforms.The concept is implemented and demonstrated with the BioExcel Building Blocks library(BioBB),a collection of tool wrappers in the field of computational biomolecular simulation.Interoperability across different workflow languages is showcased through a protein Molecular Dynamics setup transversal workflow,built using this library and run with 5 different Workflow Manager Systems(WfMS).We argue such practice is a necessary requirement for FAIR Computational Workflows and an element of Canonical Workflow Frameworks for Research(CWFR)in order to improve widespread adoption and reuse of computational methods across workflow language barriers.

CD74 is a protein whose initial role in antigen presentation was recognized two decades ago. Recent studies have revealed that it has additional functions as a receptor for macrophage migration inhibitory factor and as a receptor for an important human pathogen, helicobacter pylori (h pylori). The role of CD74 as a receptor is important because after binding of migration inhibitory factor or h pylori, NF-κB and Erk1/2 activation occurs, along with the induction of proinflammatory cytokine secretion. This review provides an up-to-date account of the functions of CD74 and how it might be involved in inflammation and cancer within the gastrointestinal tract.
