Traps and trumps from adjacent-to-tumor samples in gastric cancer research

The search for cancer biomarkers is frequently based on comparisons between tumors and adjacent-to-tumor samples. However, even after histological confirmation of been free of cancer cells, these adjacent-to-tumor samples might harbor molecular alterations which are not sufficient to cause them to look like cancer, but can differentiate these cells from normal cells. When comparing them, potential biomarkers are missed, and mainly the opportunity of finding initial aberrations presents in both tumors and adjacent samples, but not in true normal samples from non-cancer patients, resulting in misinterpretations about the carcinogenic process. Nevertheless, collecting adjacent-to-tumor samples brings trumps to be explored. The addition of samples from non-cancer patients opens an opportunity to increase the finds of the molecular cascade of events in the carcinogenic process. Differences between normal samples and adjacent samples might represent the first steps of the carcinogenic process. Adding samples of non-cancer patients to the analysis of molecular alterations relevant to the carcinogenic process opens a new window of opporttmides to the discovery of cancer biomarkers and molecular targets.

The leaf has a vital role in the functions of the plant, being responsible for photosynthesis and gas exchange. Thus, the objective of this study was to fit a mathematical equation model to estimate the leaf area of Maytenus obtusifolia Mart. through the linear dimensions of the leaves. For that, six hundred and fifteen healthy leaves were collected from plants belonging to the Federal University of Espírito Santo, São Mateus Campus, in the municipality of São Mateus, located in the north of the State of Espírito Santo, Brazil. All leaves were digitized and the images processed using the ImageJ®software, obtaining the measurements of the maximum length of the main midrib (L), the maximum width of the leaf blade (W) and the real leaf area (RLA) of each sheet. Subsequently, the product of length and width multiplication (LW) was also obtained. 500 sheets were randomly separated for the generation of models of mathematical equations and their respective coefficient of determination (R2), where RLA was used as dependent variable as function of L, W or LW as independent variable. Based on the models generated, a 115 leaf sample was used for validation, where the L, W and LW values of this sample were replaced in the adjusted equations, thus obtaining the estimated leaf area (ELA). A comparison of the means of RLA and ELA was performed by Student’s t test at 5% probability. We also calculated the mean absolute error (MAE), the root mean square error (RMSE) and the Willmott index (d). The best equation was defined by the following criteria: non-significant values of RLA and ELA averages, R2 and index d closest to unit, and MAE and RMSE values with greater proximity to zero. The quadratic model equation represented by ELA=0.18122798+0.72847767(LW)+0.00002789(LW)2 generated by multiplying the length with the width (LW) is the most suitable for the estimation of the leaf area of Maytenus obtusifolia Mart., in a fast, safe and non-destructive way.

Bredemeyera floribunda Wild (Polygalaceae), also known as pau-de-ovo is a medicinal plant used to treat snakebites, as tonic and fortifiers to treat fatigue and exhaustion. The aim of the present study was to document the knowledge of herbal sellers in the preparation of the so-called “B. floribunda fortifier” and to compare the plant root content with the recommended nutritional values and the permitted limit for medicinal plants. The study revealed that, unlike other regions of Brazil, in the Midwest region, the root of the plant is used as food to treat fatigue and exhaustion. The contents in B. floribunda root decreased in the following order: Ca > K > P > Na > Fe > Mg > Si > Mn > Zn > Cu. Daily intake of 30 grams of B. floribunda roots are considered as an excellent source of Cr, Cu, Mn and Se for people 19 to 50 years of age according to the RDA. In addition, Cu and Fe showed content above the established limit for medicinal plants stipulated by other countries. The root of the plant has low K, Mg, P, Na and Zn content. When the contents in roots of the plant are below the oral dose of reference, consumption of this plant can be considered safe and without risk to human health. On the other hand, concentration of elements such as Cu, Fe, Ni, Al, Cr and Se exceeding the allowable limit may cause serious health problems due to prolonged ingestion and large amounts consumed daily. The Brazilian legislation does not establish a permissible limit of macro- and micro-elements in medicinal plants;thus, the results presented in this study will contribute to the Brazilian public health policy on medicinal plants.

After Enzyme Replacement Therapy (ERT) using recombinant human acid alpha-glucosidase (rhGAA), survival of Infantile Pompe Disease (IPD) patients through the first 18 months of age has been documented and acquisitions of motor development are an important outcome, but description of its course is scarce. Objective: To describe the motor development in an IPD patient and its correlation with clinical conditions during the first 18 months on ERT with rhGAA. Methods: By longitudinal observational study of an IPD case at early stage. Clinical and biochemical characteristics were obtained from patient records. Echocardiogram assessed cardiac indexes and the urinary biomarker—glucose tetrasaccharide (Glc4)—was obtained by HPLC/UV, following sample derivatization with butyl 4-amino benzoate and analysis on a C18 stationary phase column. Motor skills were evaluated with Alberta Infant of Motor Scale (AIMS) and motor delay was considered as motor percentile (p) below 10. Descriptive statistical analysis was carried out and t-test was used to calculate the differences among means, with significance level defined as p value < 0.05. Results: After ERT beginning amelioration of the cardiomyopathy with reduced left ventricle mass index (LVMI) from the 2nd month onwards was observed, but above the upper normal limit for healthy children and CRIM-positive IPD patients. Although GAA antibodies level remained above the recommended titers and fluctuating elevation of Glc4 quantified, motor development analysis showed an ascendant curve expected for age within achievement of independent ambulation. Motor delay after pneumonia and maintenance of hypotonia were noted. Variation of Glc4 appeared long after a transitory intercurrence. Conclusion: In an IPD case, motor development can have normal evolution despite hypotonia. Motor analysis seems to be sensitive to follow-up clinical intercurrences. Hypertension is a complex disease that constitutes an important public health problem and demands many studies in order to understand the molecular mechanisms involving his pathophysiology. Therefore, an increasing number of studies have been conducted and new therapies are continually being discovered. In this context, exercise training has emerged as an important non-pharmacological therapy to treat hypertensive patients, minimizing the side effects of pharmacological therapies and frequently contributing to allow pharmacotherapy to be suspended. Several mechanisms have been associated with the pathogenesis of hypertension, such as hyperactivity of the sympathetic nervous system and renin-angiotensin aldosterone system,impaired endothelial nitric oxide production, increased oxygen-reactive species, vascular thickening and stiffening, cardiac hypertrophy, impaired angiogenesis, and sometimes genetic predisposition. With the advent of microRNAs(miRNAs), new insights have been added to the perspectives for the treatment of this disease, and exercise training has been shown to be able to modulate the miRNAs associated with it. Elucidation of the relationship between exercise training and miRNAs in the pathogenesis of hypertension is fundamental in order to understand how exercise modulates the cardiovascular system at genetic level. This can be promising even for the development of new drugs. This article is a review of how exercise training acts on hypertension by means of specific miRNAs in the heart, vascular system, and skeletal muscle.

Objective: To evaluate the association of socio-demographic and clinical characteristics, and risk factors with occurrence of pressure ulcers in institutionalized elderly people. Method: This cross-sectional, analytical study, in quantitative approach, performed from the database analysis linked to the project “Pressure Ulcer in institutionalized elderly people: Association of incidence with the risk factors to functional and nutritional assessment” developed in six long-stay institutions for the elderly people in João Pessoa. Data were analyzed using the Statistical Package for Social Sciences (SPSS) version 20.0. Associations were made through chi square test and Odds Ratio. Results: The clinical conditions of significant associations with the occurrence of pressure ulcers were the variables neurological disorders (p = 0.011) and visual impairment (p = 0.005). As for risk factors, the most important was fecal incontinence (p Conclusion: The analysis of the associations pointed out problems that require intervention in order to prevent health implications of the elderly people and to minimize the risk and occurrence of this injury.

Pompe disease (PD) is a rare inborn error of metabolism due to an abnormal acid alpha-glucosidase (GAA) activity that comprises glycogen breakdown mainly in the lysosomes. Since the introduction of enzyme replacement therapy (ERT), with recombinant human GAA for the early onset PD patient, a relevant field of clinical research due to the benefits regarding survival rate has been widely documented worldwide. Objective: To describe the clinical characteristics and the ERT effects in a series of Brazilian patients with infantile onset PD (IOPD) under ERT. Methods: Brazilian patients diagnosed with IOPD under ERT were recruited through their physicians participating in the International Pompe Disease Registry from 2009 to 2017. Data were collected by an online survey. Results: 10 IOPD patients were identified through the survey with a death rate of 30% and technology dependency rate reported as 80% (motor, respiratory or nutritional fields) of the patients. After the third year of ERT, motor disabilities were lost in 50% of ambulated patients. The overall characteristics were similar to international studies. Conclusion: Despite ERT benefits in cardiac involvement, motor disabilities seem to be much more compromised in IOPD patients, with high technology dependence, especially after three years of age.

New health care demands directed at children with chronic conditions have been demanded mainly after the Zika virus epidemic. Given this, the Brazilian states have produced plans to cope with this congenital infection. In the state of Rio de Janeiro, the elaborated plan had as one of its actions a training proposal for physiotherapists, speech therapists and occupational therapists, focusing on early stimulation and focused on the health care network, especially primary care and specialized rehabilitation centers. Thus the purpose of this article is to describe the impact of a response strategy to Zika virus congenital infection, based on the multi-professional training proposal in earlier stimulation in the public health attention network of Rio de Janeiro State. Descriptive study, observational from the earlier stimulation training course to the public health attention network of Rio de Janeiro State as a coping action to the health care of children with Zika virus congenital infection. At the time of writing, partial results are available after 11 classes were performed. Based on the analysis of the evaluation form of the 11 courses taught by professionals of a referral hospital in Rio de Janeiro and student form, using the Excel program, 299 health network professionals were qualified, from 56 different municipalities of RJ, mostly females (n = 259;86.82%). 175 physiotherapists (59%), 88 speech therapists (29%), 32 occupational therapists (11%) and 4 academics or other professional backgrounds (1%). The articulation between the different levels of public assistance, specifically: primary, secondary and tertiary, in this context assumes a fundamental action for good practices being undertaken considering the population’s health demands.
