background Although the Head Positioning in acute Stroke Trial(HeadPoST)showed no effect of the flat head position(FP;vs sitting up head position(SUP))on functional outcome,we hypothesised that it could still offer benefits if commenced early in those with acute ischaemic stroke(AIS)of at least moderate *** Subgroup analysis of HeadPoST in participants with National Institutes of Health Stroke Scale(NIHSS)scores≥7,≥10 and≥14,randomised to FP or SUP<4.5 hours of AIS onset on functional outcomes defined by a shift in scores on the modified Rankin scale(mRS)and death/disability(mRS scores 3-6),and any cardiovascular serious adverse *** regression analyses were undertaken adjusted for study design and baseline risk *** There was no significant differential treatment effect in patient subgroups defined by increasing baseline NIHSS scores:adjusted OR and 95%CI for ordinal shift and binary(3-6)mRS scores:for NIHSS≥7(n=867)0.92(0.67 to 1.25)and 0.74(0.52 to 1.04);NIHSS≥10(n=606)0.80(0.58 to 1.10)and 0.77(0.49 to 1.19);NIHSS≥14(n=378)0.82(0.54 to 1.24)and 1.22(0.69 to 2.14).Conclusions Early FP had no significant effect in patients with moderate-severe AIS.

The objective of this study was to analyze the profitability of production Parica reforestation in the municipality of Paragominas-PA. The study was conducted in the municipality of Paragominas Messoregiao located in the northeast of Para. The basic data used in the economic evaluation of production of planting parica （costs and revenues） were derived from primary source through raising production cost of parica with farmers from the municipality, analyzing the period of production horizon of seven years. The discount rate chosen for economic evaluation was 10% per year. NPV （net present value） calculation IRR （internal rate of return）, BCR （benefit-cost ratio） and EP （break-even point）. To check the economic feasibility of producing parica the following criteria were used for economic analysis. The results for NPV, IRR and BC ratio were respectively of $495,970, 28.83% and 2.43. For EP （break-even point） the value of 41.08% was found, which demonstrates the minimum amount of production, so that revenue equals costs. Based on the results obtained, it was found that the reforestation model with parica proposed in this paper presents economically viable for deployment.

This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.

Punica granatum *** known antimicrobial and anti-inflammatory *** of different parts of this plant have been commonly used in the folk medicine to prevent and treat infections;and studies have supported the use of *** extracts for this ***,little is known of the effects of ***-derived fractions in severe infectious diseases such as ***,we investigated the actions of the hexane fraction(HF),rich in lipophilic compounds,obtained from the leaves of this plant,in a murine model of polymicrobial *** pre-treatment with HF increased mortality in septic *** same animals presented with lower levels of interleukin-6,nitric oxide and hydrogen peroxide in their peritoneal lavage samples than vehicle *** also increased polymorphonuclear cell accumulation in to the peritoneum of mice with ***-derived nitric oxide upon lipopolysaccharide stimuli was markedly reduced by incubation with ***,triterpenes,phytosterols,vitamin E and ascorbyl palmitate were detected in the *** results indicate that *** leaf lipophilic fractions may worsen sepsis *** effect may be associated with its bioactive compounds which act synergistically or not,increasing sepsis mortality in vivo.
