Objective： Primary uterine leiomyosarcomas （ULMS） are rare, and the optimal treatment is controversial. We aimed to assess the outcome and prognostic factors in a multicenter population of women treated for primary ULMS. Methods： We retrospectively collected data of 110 women treated in 19 institutions of the Rare Cancer Network （RCN）. Inclusion criteria consisted of a pathology report confirming the diagnosis of ULMS, aged 18-80 years, complete International Federation of Gynecology and Obstetrics （FIGO） stage information, complete information on treatment, and a minimum follow-up of 6 months. Local control （LC） and locoregional control （LRC）, overall survival （OS） and disease-free survival （DFS） rates were computed using the Kaplan-Meier method. Univariate analysis was implemented using the log rank test, and multivariate analysis using the Cox model. Results： All patients underwent surgery. Seventy-five patients （68%） received adjuvant radiotherapy （RT）, including brachytherapy in 18 （I 6%）. Seventeen patients （15 %） received adjuvant chemotherapy. Median follow-up was 58 （range, 6-240） months. Five-year OS and DFS rates were 50% and 34%, and LC and LRC rates were 88% and 72%, respectively. On multivariate analysis, independent favorable prognostic factors were younger age, FIGO stage I, small tumor size, previous uterine disease, and no vascular invasion for OS and DFS. FIGO stage was the only favorable factor influencing LRC. Adjuvant local or systemic treatments did not improve the outcomes. Eight patients treated with RT presented a grade 3 acute toxicity, and only one patient with grade 3 late toxicity. Conclusions： In this large population of primary ULMS patients, we found good results in terms of LC and LRC. Nevertheless, OS remains poor, mainly due to the occurrence of distant metastases. Background: Overall, the use of antibiotics is increasing. We noticed different practices amongst different hospitals in their prophylactic use of antibiotics during operative hysteroscopy. This review investigates whether there is sufficient evidence to recommend antibiotic prophylaxis in operative hysteroscopy. Methods: We performed a systematic search in “Pubmed” using “hysteroscopy” and “antibiotic” and “prophylaxis” as search items. We found 16 hits of which 4 were suitable to include in our review. Findings: In an included Cochrane review no conclusion was drawn since no articles could be included. A prospective study by Nappi?et al.?[1]?support the recommendation of the American College of Obstetricians and Gynaecologists not to prescribe routinely antibiotic prophylaxis in cases of hysteroscopic surgery. Bhattacharya?et al. conducted a prospective study that was ended prematurely. They conclude that there is no convincing evidence that prophylactic antibiotics are of value in hysteroscopy. A review by Morrill?et al. yielded no new data. Conclusion: We did not find a significant increase in clinical relevant infectious symptoms after hysteroscopy in women treated with or without antibiotic prophylaxis, although data are insufficient to draw a definite conclusion. Therefore, we suggest a prospective study is mandatory on this topic.

Objective Flock fibres are proposed as a tracer in cases where secondary transfer of the tracer is needed. Methods The research was carried out following a case in which people were suspected to commit large numbers of burglaries. Fibres were applied to the vehicle of the suspects and collected from later crime scenes. A device was developed to assist in the appli- cation of fibres and a number of experiments on secondary transfer were carried out. Results The device can be used to apply many fibres homogeneously in a short time. A single contact will not fully deplete the first surface from flock fibres. 1%~15% of the fibres are transferred to the 3rd surface representing the crime scene. The actual number of transferred fibres is dependent on variables such as the choice of surfaces. Conclusion The results indicate that, given the selected parameters, it is realistic to expect that relatively large numbers of fibres are transferred to a crime scene and can be collected.

Downhill varices are located in the upper part of the esophagus and are usually related to superior vena cava obstruction. Bleeding from these varices is extremely rare. We describe a 77-year-old patient with hematemesis due to downhill varices as a result of recurrent goiter. A right lobe thyroidectomy was carried out with disappearance of the varices.

This review article critically compares two widely used types of catalysis,chemo-and biocatalysis,and provides insights on their greenness according to specified parameters.A comparative analysis of the environmental impact of chemo-and biocatalytic oxyfunctionalisation reactions based on published experimental data reveals that both methods produce comparable amounts of waste,with the majority stemming from the solvent ***,it is emphasised that the synthesis of the catalysts themselves,including biocatalysts,should also be considered when assessing their environmental *** review underscores the complexity of assessing the environmental impact of catalytic oxyfunctionalisation *** article also discusses the relationship between solvent properties and the energy demands for chemical transformations and downstream processing,underlining that the choice of solvent can significantly influence the environmental impact of a catalytic ***,the review highlights the importance of considering the recyclability of reagents and the secondary CO_(2)emissions caused by the energy requirements of the reaction when evaluating the environmental impact of a catalytic *** chemo-and biocatalysis produce a certain environmental impact,the greenness of either method is dependent on several factors,including the type of waste generated,the recyclability of reagents,and secondary CO_(2)*** review therefore recommends using consistent metrics and a comprehensive life cycle assessment approach to evaluate this environmental impact,and highlights the importance of considering the synthesis of the catalysts themselves.

采用离心机物理试验模拟方法研究认识当地震发生时第四纪地层中断层同震错动行为特征的工作中,若采用单一简化的边界条件难以取得合理可信的结果.本文首次采用了对比约束最大(刚性边界)和约束最小(随动边界)两种边界约束试验的方法,来控制性认识应当介于这两种情况之间的自然界中不同边界约束情况下的有关问题.试验取得了合理且与野外地震现场情况基本相符的试验结果,明确给出了建设工程安全避让距离.这项工作不但进行了对边界条件处理的基础科研方法探索而且对工程建设防震减灾有实际应用意义,相关研究成果已经应用于《中华人民共和国国家标准GB 50011—2010,建筑抗震设计规范》.如果这种方法用于数值模拟研究同类问题中也可行的话,将可以从新的角度来考虑数值模拟工作中的边界约束条件难以处理的问题.
