Tumor-stroma ratio as prognostic factor for survival in rectal adenocarcinoma: A retrospective cohort study

AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal *** TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma without prior neoadjuvant treatment in the period 1996-2006 by two observers to assessreproducibility. Patients were categorized into three categories: TSR-high [carcinoma percentage(CP) ≥ 70%], TSR-intermediate(CP 40%, 50% and 60%) and TSR-low(CP ≤ 30%). The relation between categorized TSR and survival was analyzed using Cox proportional hazards *** Thirty-six(23.4%) patients were scored as TSR-low, 70(45.4%) as TSR-intermediate and 48(31.2%) as TSRhigh. TSR had a good interobserver agreement(κ = 0.724, concordance 82.5%). Overall survival(OS) and disease free survival(DFS) were significantly better for patients with a high TSR(P = 0.01 and P = 0.02, respectively). A similar association existed for disease specific survival(P = 0.06). In multivariate analysis, patients without lymph node metastasis and an intermediate TSR had a higher risk of dying from rectal cancer(HR = 5.27, 95%CI: 1.54-18.10), compared to lymph node metastasis negative patients with a high TSR. This group also had a worse DFS(HR = 6.41, 95%CI: 1.84-22.28). An identical association was seen for OS. These relations were not seen in lymph node metastasis positive patients. CONCLUSION The TSR has potential as a prognostic factor for survival in surgically treated rectal cancer patients, especially in lymph node negative cases.

Retinal nerve fiber layer changes based on historic CD4 nadir among HIV positive patients undergoing glaucoma evaluation

AIM: To determine relationships between retinal nerve fiber layer(RNFL) thickness and nadir CD4 cell count in human immunodeficiency virus(HIV) positive patients evaluated for glaucoma ***: Data were reviewed for 329 HIV positive patients evaluated for glaucoma suspicion. High-definition optical coherence tomography(OCT) RNFL measurements were obtained at least 6 mo apart. Analyses were performed to identify relationships between nadir CD4 count and RNFL ***: Totally 110 eyes of 55 patients met inclusion criteria, of which 46 eyes were from subjects with nadir CD4<200 cells/mm^3 and 64 had nadir CD4≥200 cells/mm^3. Patients with nadir CD4<200 cells/mm^3 had significantly thicker superior(119.7±18.6 μm) and temporal(63.8±11.7 μm) quadrants at time of initial OCT compared to the superior(112.8±16.8 μm, P=0.048) and temporal(57.1±11.9 μm, P=0.004) quadrants of patients with higher nadir CD4. This trend toward thicker RNFL among subjects with lower nadir CD4 cell counts persisted at the time of follow up OCT where participants with nadir CD4<200 cells/mm^3 showed average RNFL thickness in the superior and temporal quadrants of 117.9±18.3 μm and 63.8±12.8 μm, respectively, compared to a superior thickness of 110.5±16.9 μm(P=0.034) and temporal thickness of 57.3±11.6 μm(P=0.007) among those with higher nadir CD4. CONCLUSION: Patients with lower nadir CD4 cell counts have thicker RNFL in the superior and temporal quadrants compared to those with higher nadir CD4 counts. RNFL thickness in HIV positive patients may be affected by historic HIV disease control and should be considered when evaluating HIV positive patients for glaucoma.

More than twenty species of lichenicolous fungi have been described in Phoma,a large anamorphic genus of primarily plant-associated pathogens with broad geographic *** obtained nuclear and mitochondrial rDNA sequences from 19 fungal cultures isolated from specimens representing four described and two undescribed lichenicolous species in the *** multilocus phylogeny indicates that lichenicolous Phoma species represent at least two phylogenetically distinct clades in the Phaeosphaeriaceae,one including a new species,Phoma puncteliae,isolated from a specimen of Punctelia rudecta collected inMaryland,USA,and another group of primarily lichenicolous *** latter group includes four described lichenicolous Phoma species,an unidentified melanized rock fungus,and a new lichenicolous Phoma species isolated from Xanthomendoza species collected in Canada that we are naming *** specimens in this clade collected from different lichen genera and species were found to be very similar genetically,which calls into question the recent practice of recognizing lichenicolous Phoma species mainly by differences in host preference.

Dear Editor,Burn scar contracture is a common problem in healing burn wounds of the neck. It can cause both pain and dysfunction if not treated adequately (1)The treatment of such wounds often involves a combination of surgery and splinting therapy (2)A variety of splints, including the thermoplastic static neck splint [3], the Watusi collar [4], manually fabricated splints, and pre-fabricated splints such as the Philadelphia collar have been used for the management of scar contractures. However, each type of splint has its own advantages and disadvantages, and none of these splints seem to reduce the need for skin reconstruction nor delays the time until surgical re-construction [5].

Treatment of severe steroid refractory ulcerative colitis

Although systemic steroids are highly efficacious in ulcerative colitis (UC), failure to respond to steroids still poses an important challenge to the surgeon and physician alike. Even if the life time risk of a fulminant UC flare is only 20%, this condition is potentially life threatening and should be managed in hospital. If patients fail 3 to 5 d of intravenous corticosteroids and optimal supportive care, they should be considered for any of three options: intravenous cyclosporine (2 mg/kg for 7 d, and serum level controlled), infliximab (5 mg/kg Ⅳ, 0-2-6 wk) or total colectomy. The choice between these three options is a medical- surgical decision based on clinical signs, radiological and endoscopic findings and blood analysis (CRP, serum albumin). Between 65 and 85% of patients will initially respond to cyclosporine and avoid colectomy on the short term. Over 5 years only 50% of initial responders avoid colectomy and outcomes are better in patients naive to azathioprine (bridging strategy). The data on infliximab as a medical rescue in fulminant colitis are more limited although the efficacy of this anti tumor necrosis factor (TNF) monoclonal antibody has been demonstrated in a controlled trial. Controlled data on the comparative efficacy of cyclosporine and infliximab are not available at this moment. Both drugs are immunosuppressants and are used in combination with steroids and azathioprine, which infers a risk of serious, even fatal, opportunistic infections. Therefore, patients not responding to these agents within 5-7 d should be considered for colectomy and responders should be closely monitored for infections.

Virally-Mediated Versus Grazer-Induced Mortality Rates in a Warm-Temperate Inverse Estuary (Spencer Gulf, South Australia)

We investigated the seasonal dynamics of flow cytometrically-defined populations of viruses, heterotrophic bacteria, and the picoeukaryotic and prokaryotic phytoplankton at three sites in the temperate oligotrophic inverse estuary of Spencer Gulf (South Australia). We consistently identified two sub-populations of viruses, three sub-populations of heterotrophic bacteria, one population of picoeukaryotic phytoplankton and two populations of prokaryotic phytoplankton (cyanobacteria Prochlorococcus and Synechococcus). Both the cytometric community composition and the abundance of viruses, heterotrophic bacteria and both prokaryotic (Synechococcus and Prochlorococcus) and eukaryotic picophytoplankton were consistent with previous observations conducted in South Australian continental shelf waters. Noticeably LDNA bacteria (i.e. inactive or dormant cells) were consistently significantly the most abundant group of heterotrophic bacteria (totaling from 29% to 68% of total bacterial abundance) and were up to 10-fold more abundant than that previously reported in South Australian continental shelf waters, including the nearby Saint Vincent Gulf. These results suggest an overall low activity of the microbial community, and are consistent with previous evidence that LDNA cells may play a greater role in heterotrophic processes than HDNA cells in oligotrophic waters. In an attempt to further assess the qualitative and quantitative nature of the mortality of these organisms, we used a specific dilution assay to assess the relative contribution of viruses and microzooplankton grazers to the mortality rates of heterotrophic bacteria, and picoeukaryotic and prokaryotic phytoplankton. We consistently reported site-specific, population specific and sea-son-specific viral lysis and grazing rates of heterotrophic bacteria and the picoeukaryotic and prokaryotic (cyanobacteria Prochlorococcus and Synechococcus) phytoplankton across sites and seasons.
