Uterine Fibroids Operated in the Obstetric Gynecology Department of the Mother-Child Department at the University Hospital of Tengandogo: About 109 Cases

Background: Uterine fibroid is the most common benign gynecological tumor in women of childbearing age and is common in our context. It can be discovered incidentally or by metrorrhagia. Objective: To study the epidemiological, clinical, therapeutic and prognostic aspects of uterine fibroids operated in the gynaecology and obstetrics department of the CHU-T. Patients and Method: This was a descriptive cross-sectional study including all patients operated on for uterine fibromyoma in the gynaecology-obstetrics department. The collection mode was retrospective, over a 5-year period from January 1, 2017 to December 31, 2021 in the mother-child department of the CHU-T. Data entry and analysis were carried out on a microcomputer using Epi info 7.2.5 software. Results: We collected 109 cases of uterine fibroids which represented 42.5% of the surgical activities of the gynaecology department. The average age of the patients was 38.9 years ± 7.8. Married women represented 77.1%. Salaried women accounted for 65.1%. The main reasons for consultation were uterine haemorrhage (53.1%), pelvic pain (40.4%) and hypofertility (31.2%). Ultrasound was performed in all patients to help map the fibroid nuclei. The main indication for surgery was haemorrhagic myoma (43.1%). The surgical treatment was conservative (myomectomy) in 58.7% of cases and radical (hysterectomy) in 27.4% of cases. The most frequent postoperative complication was vulvar haemorrhage and the average hospital stay was 4 days ± 1.4. Anatomical pathological examination of the surgical excision specimen carried out on 30 operated patients concluded that uterine leiomyoma was diagnosed in 100% of cases. Conclusion: Patient education for early detection, universal health insurance and cost subsidies could improve the management of this condition.

External Hemorrhoidal Thrombosis in Recent Births at the Maternity Ward of Saint Camille Hospital in Ouagadougou

Introduction: External hemorrhoidal thrombosis is a clinical is the most common postpartum proctologic accident. The aim of this work was to study hemorrhoidal thrombosis in recent mothers, in order to help improve its management. Patients and Methods: This was a descriptive and analytical cross-sectional study that was carried out at Saint Camille Hospital in Ouagadougou, Burkina Faso. Data collection was prospective and took place over a period of three months from September 1 to November 30, 2019. The study population consisted of recent births. The interview with the patients and their physical examination enabled the collection of data. Results: During the study period, 384 women were examined and 56 (14.6%) of whom presented with external hemorrhoidal thrombosis. The average age of patients who presented with external hemorrhoidal thrombosis was 29 years ± 5.2. The external hemorrhoidal thrombosis appeared in the form of a painful swelling at the level of the anal margin. In multivariate analysis, dyschezia and personal history of hemorrhoidal disease were risk factors for external hemorrhoidal thrombosis in the postpartum period (OR = 37.4 [6.8 - 205.7];OR = 23.9 [3.3 - 174.2]. Consumption of fruit and foods rich in dietary fiber were protective factors against the occurrence of this risk (OR = 0.02 [0.002 - 0.3]). Conclusion: External hemorrhoidal thrombosis remains a complication to be taken into account during the monitoring of postpartum women. It is important to encourage pregnant women to consume foods and fruits rich in fiber seem to reduce the risk of its occurrence.

Anemia in Parturients within the Mother-Child Department of Tengandogo Teaching Hospital, Burkina Faso

Objective: Study the epidemiological, clinical, para-clinical and therapeutic aspects as well as the maternal and perinatal prognosis of anemia in parturients. Background: Anemia is a public health problem in Burkina Faso. It mainly affects pregnant women. Patients and Methods: This was a descriptive and analytical cross-sectional study with prospective data collected from January 1st to September 30th, 2021. The study population included all parturients received within the department. Results: We included 378 parturients. We included in our study, all the parturients who underwent blood count upon admission and who had a hemoglobin rate below 11 g/dl. The frequency of anemia was 35.9%. The average age of anemic parturients was 27 ± 6.3 years. Compliance with anemia preventive treatment was good in 63% of cases. Out of the 378 anemic parturients, 55.8% were delivered by cesarean section. At delivery, 21 stillbirths were recorded. The mean hemoglobin level on admission was 9.27 g/dl ± 1.6. Normochromic microcytic anemia was present in 50.7% of cases. We recorded 1 case of maternal death (0.2%). The perinatal mortality rate was 7.4%. Conclusion: Despite the government’s efforts, anemia in pregnant women remains a public health problem in our country. This reality calls on decision-makers to lay emphasis on measures to prevent anemia in pregnant women.

Contribution of Simulation to the Skills Laboratory for the Learning of Obstetrical Examination in an African Setting with Limited Resources

Objective: To evaluate the interest of the simulation in the learning of the obstetric examination of parturient by medical students. Materials and methods: It was an analytical cross-sectional study that took place from 1 to 28 February 2020 at the Yalgado Ouedraogo Teaching Hospital in Ouagadougou, Burkina Faso. The study population was made up of medical students at the Doctorate II level. The previous experience of the students in the obstetric examination of the parturient, the contribution of the simulation on their feelings and on their skills towards this examination were studied. The Mc Nemar Chi2 test was used to compare percentages on paired samples. Results: The level of difficulty in performing the obstetrical examination in the parturient evolved downwards after the practice of the simulation. The stress level to perform the obstetric exam has been significantly reduced. On the other hand, confidence has increased. The students’ ability to appreciate each of the elements of the obstetric examination studied had improved after the practice of simulation. Conclusion: The simulation in the skills laboratory was a great contribution and the acquisition of models of better fidelity would help to improve the contribution to the confidence building and the acquisition of skills by learners.

Preventive Exchange Blood Transfusion in Pregnant Women with Sickle Cell Disease: Maternal and Perinatal Prognosis in a Country with Limited Resources, Burkina Faso

Context: In pregnant women with sickle cell disease, the management with exchange transfusion could be useful in improving the prognosis of mother and child by reducing the level of hemoglobin S less than 40%. Objective: To analyze the maternal and perinatal outcome during the program of the exchange transfusion in pregnant women with sickle cell disease. Patients and Methods: We conducted a prospective study over a period of 18 months. Pregnant women with a major form of sickle cell disease were included. A manual blood exchange transfusion was performed monthly. We monitored the occurrence of maternal and perinatal morbidity during the follow-up. Results: A total of 42 pregnant women with sickle cell disease were monitored. The frequency of infectious episodes and vaso-occlusive crisis was significantly reduced, respectively from 47.6% and 83.3% before the beginning of the blood exchanges transfusion to 11.9% and 16.7% during blood exchanges transfusion program. All newborns were alive at birth with an Apgar score higher or equal to 7 at the 5th minute. The rate of admission of the newborns at neonatal intensive care unit was 9.3%. Maternal mortality was estimated at 7.1% and there was no early neonatal mortality. Conclusion: Prophylactic exchange transfusion reduces infections and the reoccurrence of vaso-occlusive crisis, which has an impact on perinatal prognosis.

Experience of Patients Who Underwent Scheduled Gynaecological Surgery at the Bogodogo University Hospital, Burkina Faso

Introduction: Surgery can cause anxiety and worry, such as waking up in pain, being physically harmed or dying. This study aimed to assess the experience of patients undergoing planned gynaecological surgery in the Obstetrics Gynaecology and Reproductive Medicine department of the University Hospital Centre of Bogodogo. Patients and methods: This was a cross-sectional study with prospective data collection during the period from June 1 to August 31, 2018. The study population consisted of patients who received scheduled gynaecological surgery during the period. Results: Seventy-six (76) patients underwent scheduled surgery and 62 were included. During discussion with the gynaecologist, 58 patients (93.55%) were satisfied. When the surgery was announced, 41 patients (66.13%) were anxious. At the consultation with the anaesthetist 59 patients (95.16%) were satisfied with the discussions. Fifty-five patients (88.7%) were anxious on admission to the operating theatre. The quality of care was satisfactory for 61 patients (98.39%) and all patients were satisfied with the care team. Conclusion: Periodic evaluation of patients’ experiences should therefore be incorporated into care evaluation policies in health facilities in order to contribute to continuous improvement in the quality of medical and surgical care.

Objective: The objective of our study was to study the surgical activities carried out in the gynecology-obstetrics department of the CHU-YO (Teaching Hospital Yalgado Ouédraogo) from January 1st 2015 to December 31st, 2015. Patients and Methods: This dealt with a cross-cutting descriptive and analytic study with retrospective data collection. Our study has included the female patients who underwent surgery in the operating room and whose medical records were usable. Results: 45% of female patients admitted in the concerned department underwent a surgery. The average age of patients was estimated at 28.02 years ±7 years with extremes of 13 years and 80 years. Obstetrical surgery has involved 89.9% of cases. Female patients have received a loco-regional anesthesia in 92.7% of cases. Emergency surgical operations accounted for 88.8% and caesarean section was the main surgical operation carried out in 87.1% of cases. The mortality rate of the overall surgical operations was 1.04%. Conclusion: A better availability of labile blood products is more likely to reduce the mortality rate of surgical operations under the threshold of 1%.

Low Birth Weight at Term: Risk Factors and Perinatal Prognosis in the Teaching Hospital Yalgado Ouédraogo, Burkina Faso

Objective: The objective of our study was to study the risk factors of low birth weight at term in the Teaching Hospital Yalgado (CHU-YO) Ouédraogo. Patients and Methods: This dealt with a comparative and analytical control case study. The group of cases was made up of female patients who gave birth to newborns with low birth weight at term and that of control cases included female patients who delivered a normal-weighted newborn at term. Results: The frequency of low birth weight at term was therefore estimated at 4.4%. The average age of the parturients was 25 ± 6.36 years. Female patients living in a marital setting accounted for 93.1% of cases and 64.4% of them had no income-generating activities. A maternal underweight, a height below 155 cm, passive smoking, and malaria during pregnancy have been identified as the factors associated with a low birth weight. Conclusion: Quality prenatal care could reduce the incidence of low birth weight at term.

Vaginal Bleeding in the Nonpregnant Patient Received in Emergency at Yalgado Ouedraogo University Hospital of Ouagadougou, Burkina Faso

Objective: To describe the epidemiological, clinical and therapeutic aspects of cases of vaginal bleeding in the nonpregnant patient received in emergency at Yalgado OUEDRAOGO University Hospital. Materials and Methods: It was a retrospective and descriptive study that involved the clinical records of 326 patients collected from January 01, 2009 to December 31, 2013. Results: The average age of women was 47 years old with extremes at 12 and 82 years old. Women of childbearing age accounted for 61.6% and postmenopausal women 18.7%. 70.7% of women were pauciparous or nulliparous. Menorrhagia and pelvic pain were the main signs associated. This symptomatology required hospitalization in 85.2% of cases. The main aetiologies were uterine myomas 49.69%, cervical cancer 23% and functional metrorrhagia 11.04%. Progestin was the most used drug in 67.1% of cases. Total abdominal hysterectomy and abdominal myomectomy were the most commonly used surgical methods with 22.3%, 44.4% of cases. Eleven death cases were observed. Conclusion: Gynecological metrorrhagia is more common in women of childbearing age than in menopausal women. The main causes are fibroma, cervical cancer and functional metrorrhagia.

Sexual Experience of Women after Gynecological Hysterectomy in the Department of Obstetrics and Gynecology of Yalgado Ouedraogo Teaching Hospital, Burkina Faso

Objective: To study the sexual experience of women after gynecological hysterectomy in the obstetrics and gynecology department of Yalgado Ouedraogo University Teaching Hospital in Burkina Faso. Patient and Method: We conducted a cross-sectional study from October 1, 2016 to March 30, 2017. It included any patient who had a gynecological indication for a hysterectomy in the Department of Obstetrics and Gynecology of Yalgado Ouedraogo University Teaching Hospital (CHU-YO) during the study period. A total of 85 patients were included. We collected information from the consultation records, clinical records, and operating room registries and by surveying the patients themselves. Result: The frequency of hysterectomy in the gynecological period was 3.9%. The mean age of the patients was 51.71 (extremes: 35 and 66 years) and the mean parity was 4.8. Married women were the majority. Fibromyoma and genital prolapse accounted for 56.47% and 23.53% respectively of operative indications. Regarding postintervention sexuality, 29.41% of women declared no longer having sexual desire. Also, they complained of vaginal dryness (31.8%), decreased frequency of orgasm (42.3%), dyspareunia (37.65). Sexual dysfunction was present in 82.35% of women. There was a statistically significant difference between global sexual functioning and geographic origin, educational attainment, socio-economic level, surgical approach, preintervention sexual counseling, women’s psychological profile and their emotional relationship with their spouses. Conclusion: The psychosexual soundness of hysterectomy is important. A diagnosis of sexual disorders prior to the intervention and psychological follow-up throughout the therapeutic process of the pathology indicative of a hysterectomy is necessary for successful postoperative sexuality.
