Pitfalls and optimal approaches to diagnose melioidosis

Melioidosis is a severe and fatal infectious disease in the tropics and subtropics. It presents as a febrile illness with protean manifestation ranging from chronic localized infection to acute fulminant septicemia with dissemination of infection to multiple organs characterized by abscesses. Pneumonia is the most common clinical presentation. Because of the wide range of clinical presentations, physicians may often misdiagnose and mistreat the disease for tuberculosis, pneumonia or other pyogenic infections. The purpose of this paper is to present common pitfalls in diagnosis and provide optimal approaches to enable early diagnosis and prompt treatment of melidoidosis. Melioidosis may occur beyond the boundaries of endemic areas. There is no pathognomonic feature specific to a diagnosis of melioidosis. In endemic areas, physicians need to expand the diagnostic work-up to include melioidosis when confronted with clinical scenarios of pyrexia of unknown origin, progressive pneumonia or sepsis. Radiological imaging is an integral part of the diagnostic workup. Knowledge of the modes of transmission and risk factors will add support in clinically suspected cases to initiate therapy. In situations of clinically highly probable or possible cases where laboratory bacteriological confirmation is not possible, applying evidence-based criteria and empirical treatment with antimicrobials is recommended. It is of prime importance that patients undergo the full course of antimicrobial therapy to avoid relapse and recurrence. Early diagnosis and appropriate management is crucial in reducing serious complications leading to high mortality, and in preventing recurrences of the disease. Thus, there is a crucial need for promoting awareness among physicians at all levels and for improved diagnostic microbiology services. Further, the need for making the disease notifiable and/or initiating melioidosis registries in endemic countries appears to be compelling.

Melioidosis in India and Bangladesh: A review of case reports

Objective: To conduct an epidemiological and clinical review of published case reports of melioidosis from India and Bangladesh. Methods: Data from published case reports were abstracted and summarized. We further compared the clinical epidemiology of the melioidosis cases in India with case series from highly endemic areas in Northern Australia and Southeast Asia to elucidate any differences in presentations and risk factors between the regions.Results: We identified a total of 99 cases published between 1953 and June 2016, originating from India(n=85) or Bangladesh(n=14). Cases were predominantly male and ranged in age from 1 month to 90 years. Diabetes mellitus was the most common risk factor reported(58%). About 28% of the cases had history of exposure via high-risk occupations or exposure to contaminated water. The overall case fatality rate(CFR) was 26%. Factors influencing mortality included the occurrence of septic shock(CFR, 80%), environmental exposure(CFR,39%), primary presentation of pneumonia(CFR, 38%), misdiagnosed and/or mistreated cases(CFR, 33%) or the presence of a risk factor(CFR, 29%). Because of the small number of cases in Bangladesh, pattern of clinical epidemiology is limited to India. Soft tissue abscess(37%)was the most common clinical presentation reported from India followed by pneumonia(24%)and osteomyelitis/septic arthritis(18%). Neurological melioidosis(n=10, 12%) presented as pyemic lesions of the brain or meninges. A few cases of prostatic abscess(n=4) in men and parotid abscess(n=4) were also noted. The above patterns were consistent with case series from Southeast Asia and Northern Australia for the most part, in terms of risk factors associated with infection and factors influencing mortality. Differences included clinical presentation of pneumonia which was notably lower than that reported in Southeast Asia and Northern Australia; a higher proportion of neurological and parotid abscess presentation; and a lower CFR compared to that reported in case series in Southeast Asia. Soil quality estimation using environmental covariates and predictive models:an example from tropical soils of Nigeria

Background:Information addressing soil quality in developing countries often depends on results from small experimental plots,which are later extrapolated to vast areas of agricultural land.This approach often results in misin-formation to end-users of land for sustainable soil nutrient management.The objective of this study was to estimate the spatial variability of soil quality index(SQI)at regional scale with predictive models using soil–environmental covariates.Methods:A total of 110 composite soil samples(0–30 cm depth)were collected by stratified random sampling schemes at 2–5 km intervals across the Cross River State,Nigeria,and selected soil physical and chemical properties were determined.We employed environmental covariates derived from a digital elevation model(DEM)and Senti-nel-2 imageries for our modelling regime.We measured soil quality using two approaches[total data set(TDS)and minimum data set(MDS)].Two scoring functions were also applied,linear(L)and non-linear(NL),yielding four indices(MDS_L,MDS_NL,TDS_L,and TDS_NL).Eleven soil quality indicators were used as TDS and were further screened for MDS using principal component analysis(PCA).Random forest(RF),support vector regression(SVR),regression kriging(RK),Cubist regression,and geographically weighted regression(GWR)were applied to predict SQI in unsampled locations.Results:The computed SQI via MDS_L was classified into five classes:≤0.38,0.38–0.48,0.48–0.58,0.58–0.68,and≥0.68,representing very low(classⅤ),low(classⅣ),moderate(classⅢ),high(classⅡ)and very high(classⅠ)soil quality,respectively.GWR model was robust in predicting soil quality(R^(2)=0.21,CCC=0.39,RMSE=0.15),while RF was a model with inferior performance(R^(2)=0.02,CCC=0.32,RMSE=0.15).Soil quality was high in the southern region and low in the northern region.High soil quality class(>49%)and moderate soil quality class(>14%)dominate the study area in all predicted models used.Conclusions:Structural stability index,sand content,soil oganic carbon content,and mean weight diameter of a

This study was performed to examine the separate and simultaneous influence of predictive models’choice alongside sample ratios selection in soil organic matter(SOM).The research was carried out in northern Morocco,characterized by relatively cold weather and diverse geological conditions.The dataset herein used accounted for 1591 soil samples,which were randomly split into the following ratios:10%(∼150 sample ratio),20%(∼250 sample ratio),35%(∼450 sample ratio),50%(∼600 sample ratio)and 95%(∼1200 sample ratio).Models herein involved were ordinary kriging(OK),regression kriging(RK),multiple linear regression(MLR),random forest(RF),quantile regression forest(QRF),Gaussian process regression(GPR)and an ensemble model.The findings in the study showed that the accuracy of SOM prediction is sensitive to both predictive models and sample ratios.OK combined with 95%sample ratio performed equally to RF in conjunction with all the sample ratios,as the latter did not show much sensitivity to sample ratios.ANOVA results revealed that RF with a∼10%sample ratio could also be optimum for predicting SOM in the study area.In conclusion,the findings herein reported could be instrumental for producing cost-effective detailed and accurate spatial estimation of SOM in other sites.Furthermore,they could serve as a baseline study for future research in the region or elsewhere.Therefore,we recommend conducting series of simulation of all possible combinations between various predictive models and sample ratios as a preliminary step in soil organic matter prediction.

Liver transplantation for hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is the commonest primary malignancy of the liver. It usually occurs in the setting of chronic liver disease and has a poor prognosis if untreated. Orthotopic liver transplantation (OLT) is a suitable therapeutic option for early,unresectable HCC particularly in the setting of chronic liver disease. Following on from disappointing initial results,the seminal study by Mazzaferro et al in 1996 established OLT as a viable treatment for HCC. In this study,the "Milan criteria" were applied achieving a 4-year survival rate similar to OLT for benign disease. Since then various groups have attempted to expand these criteria whilst maintaining long term survival rates. The technique of living donor liver transplantation has evolved over the past decade,particularly in Asia,and published outcome data is comparable to that of OLT. This article will review the evidence,indications,and the future direction of liver transplantation for liver cancer.

Hepatocellular carcinoma with child Pugh-A Cirrhosis treated with stereotactic body radiotherapy

AIM To evaluate the control, survival, and hepatic function for Child Pugh(CP)-A patients after Stereotactic body radiotherapy(SBRT) in hepatocellular carcinoma(HCC).METHODS From 2009 to 2016, 40 patients with Barcelona Liver Clinic(BCLC) stages 0-B HCC and CP-A cirrhosis completed liver SBRT. The mean prescription dose was 45 Gy(40 to 50 Gy in 4-5 fractions). Local relapse, defined as recurrence within the planning target volume was assessed with intravenous multiphase contrast computed tomography or magnetic resonance imaging every 4-6 mo after completion of SBRT. Progression of cirrhosis was evaluated by CP and Model for End Stage Liver Disease scores every 3-4 mo. Toxicities were graded per the Common Terminology Criteria for Adverse Events(v4.03). Median follow-up was 24 mo.RESULTS Forty-nine HCC lesions among 40 patients were analyzed in this IRB approved retrospective study. Median tumor diameter was 3.5 cm(1.5-8.9 cm). Six patients with tumors ≥ 5 cm completed planned selected transarterial chemoembolization(TACE) in combination with SBRT. Eight patients underwent orthotropic live transplant(OLT) with SBRT as a bridging treatment(median time to transplant was 12 mo, range 5 to 23 mo). The Pathologic complete response(PCR) rate in this group was 62.5%. The 2-year in-field local control was 98%(1 failure). Intrahepatic control was 82% and 62% at 1 and 2 years, respectively. Overall survival(OS) was 92% and 60% at 1 and 2 years, with a median survival of 41 mo per Kaplan Meier analysis. At 1 and 2 years, 71% and 61% of patients retained CPA status. Of the patients with intrahepatic failures, 58% developed progressive cirrhosis, compared to 27% with controlled disease(P = 0.06). Survival specific to hepatic failure was 92%, 81%, and 69% at 12, 18, and 24 mo. There was no grade 3 or higher toxicity. On univariate analysis, gross tumor volume(GTV) < 23 cc was associated with freedom from CP progression(P = 0.05), hepatic failure-specific survival(P = 0.02), and trended with OS(P = 0.10).CONCLUSION SBRT is safe

Attitude, Practice, and Factors Affecting Contraceptive Use among Women Attending Postnatal Care in a Tertiary Health Facility in Jos North LGA, Plateau State, Nigeria

The high population growth rate has been associated with the increased level of poverty, morbidity, mortality, and decreased life expectancy. Modern contraception is an important factor in controlling fertility through prevention of unintended and unwanted pregnancies. Contraception (birth control) prevents pregnancy by interfering with the normal process of ovulation, fertilization, and implantation. Contraceptive use is still very low in Nigeria and Sub-Saharan Africa (SSA), where the levels of fertility and unmet need for family planning are high. The attitude of women toward contraception will usually affect the practice and utilization of contraceptives. The objective of this study is to assess the attitude, prevalence, and factors affecting contraceptive use among women attending Postnatal care in a tertiary health facility in Jos North L. G. A Plateau State, Nigeria. There was a descriptive cross-sectional study done between September to October 2019, with a sample size of 250 women. Respondents were determined using a multi-stage sampling method. Data collection was done using an interviewer-administered, pre-tested, structured questionnaire. The data was collected, entered, and analyzed using IBM Statistical package for social sciences (SPSS) version 20.0. Most women have a positive attitude 162 (64.8%) toward contraception while 88 (35.2%) had negative attitude toward contraception. Three quarters (75.2%) of women are willing to use contraception. The prevalence of contraceptive use among respondents was 35.6% while the prevalence of previous use of contraceptives among the respondents was 71.2%. Fifty (27.9%) used condom (barrier method), 40 (22.9%) used withdrawal method. Majority of the other respondents were spread across OCPs, injectables and implants. Major factors affecting the use of contraceptives were majorly attributed to informed choice with 60.1%, Partner’s decision (44.1%) and mild or no adverse reaction (12.8%). Background: Contraceptive acceptance and utilization in society has been a huge challenge for communities in developing countries. There is a need to support the increase in the utilization of contraception and family planning services. In all this, good knowledge and practice of contraception services will enable women to select the best methods. The study seeks to qualitatively assess the knowledge, attitude, and practice of contraceptive use among women attending postnatal care in a health facility in Jos, Plateau State, Nigeria. Methods: This qualitative study was conducted in Bingham University Teaching Hospital, Jos, Plateau State in September 2019. This study utilized Focus Group Discussions among 36 women recruited using the purposive sampling technique. There were four (4) sessions involving nine (9) women per session. Findings: Of the 36 women, a majority (88.9%) had positive perception regarding contraception, all agreed that contraception and family planning is beneficial to families and communities. A majority felt that communities are yet to accept contraception. Most (83.3%) of the women had good knowledge of contraception. Three-quarters (77.8%) of the women have used contraception in the past, and about half (41.7%) are using it currently. Attitude towards adoption of contraceptives after current pregnancy was generally good. About half (52.7%) of the participants stated they required spousal approval before they adopt a method of contraception. Conclusion: There is a need for health agencies, development partners, and government to continue the health education, community sensitization, and support towards making contraception and family planning commodities available and accessible.

Determination of Lanthanides, Thorium, Uranium and Plutonium in Irradiated (Th, Pu)O_(2) by Liquid Chromatography Using α-Hydroxyiso Butyric Acid (α-HIBA)

An HPLC method is presented for the separation and determination of lanthanides (Lns), thorium (Th), uranium (U) and plutonium (Pu) from irradiated (Th, Pu)O2. Individual separation of Lns, Th, U and Pu is a challenging task becauseof 1) lanthanideshavingsimilar physical and chemical properties, 2) presence of complex matrix like irradiated fuel and 3) the co-existence of multiple oxidation states of Pu. Different procedures were developed for separation of individual lanthanides and actinides.The individual lanthanides were separated on a dynamically modified reversed phase (RP) column using n-octane sulfonic acid as an ion interaction reagentand employingdual gradient(pH and concentration) of α-hydroxyisobutyric acid (HIBA). In order to improve the precision on the determination of Lns, terbium (Tb) was used as an internal standard. The method was validated employing simulated high level liquid waste. Concentrations of lanthanides viz. lanthanum (La) and neodymium (Nd) in the dissolver solution were determined based on their peak areas. Th, U and Pu were separated on a RP column using mobile phase containing HIBA and methanol. Since Pu is prone to exist in multiple oxidation states, all the oxidation states were converted into Pu (IV) using H2O2 in 3M HNO3. Under the optimized conditions, Pu(IV) eluted first followed by Th and U. The concentrations of Th, U and Pu were determined by standard addition method andwere found to be 1.10 ± 0.02 mg/g, 5.3 ± 0.3 μg/g and 27 ± 1 μg/g, respectively, in the dissolver solution of irradiated fuel. These values were in good agreement with the concentration of Th determined by biamperometry and those of U and Puby isotope dilution thermal ionization mass spectrometry.
