Impact of conditioning regimen on peripheral blood hematopoietic cell transplant

AIM To investigate infused hematopoietic cell doses and their interaction with conditioning regimen intensity +/-total body irradiation(TBI) on outcomes after peripheral blood hematopoietic cell transplant(PBHCT).METHODS Our retrospective cohort included 247 patients receiving a first, T-replete, human leukocyte antigen-matched allogeneic PBHCT and treated between 2001 and2012. Correlations were calculated using the Pearson product-moment correlation coefficient. Overall survival and progression free survival curves were generated using the Kaplan-Meier method and compared using the log-rank *** Neutrophil engraftment was significantly faster after reduced intensity TBI based conditioning [reduced intensity conditioning(RIC) + TBI] and > 4 × 10~6 CD34+cells/kg infused. A higher total nucleated cell dose led to a higher incidence of grade II-IV acute graft-versus-host disease in the myeloablative + TBI regimen group(P = 0.03), but no significant difference in grade III-IV graft-versus-host disease. A higher total nucleated cell dose was also associated with increased incidence of moderate/severe chronic graft-versus-host disease, regardless ofconditioning regimen. Overall and progression-free survival were significantly better in patients with a RIC + TBI regimen and total nucleated cell dose > 8 ×10~8/kg(3 years, overall survival: 70% vs 38%, P = 0.02, 3 years, progression free survival: 64% vs 38%, P = 0.02).CONCLUSION TBI and conditioning intensity may alter the relationship between infused cell doses and outcomes after PBHCT. Immune cell subsets may predict improved survival after unmanipulated PBHCT.

The Oak Ridge National Laboratory (ORNL) is the largest and most diverse energy, research, and development institution within the Department of Energy (DOE) system in the United States. As such, the site endures constant land development that creates rigorous growing conditions for urban vegetation. Natural resource managers at ORNL recognize that trees are an integral component of the landscape and are interested in characterizing the urban forest and their associated ecosystem services benefits. We evaluated the urban forest structure, quantified ecosystem services and benefits, and estimated economic value of resources using i-Tree Eco at ORNL. While this assessment captured over 1100 landscape trees, the ORNL Natural Resources Management for landscape vegetation can be expanded to include unmanaged landscapes, e.g. riparian areas, greenspace, and other vegetative attributes to increase ecosystem services benefits. Assigning a monetary value to urban forest benefits help to inform decisions about urban forest management, ideally on cost-benefit analysis.

BACKGROUND Management of single small hepatocellular carcinoma(HCC)is straightforward with curative outcomes achieved by locoregional therapy or *** transplantation is often considered for multiple small or single large *** of two small HCC whether presenting synchronously or sequentially is less *** To define the outcomes of patients presenting with two small *** Retrospective review of HCC databases from multiple institutions of patients with either two synchronous or sequential HCC≤3 cm between January 2000 and March *** outcomes were overall survival(OS)and transplant-free survival(TFS).RESULTS 104 patients were identified(male n=89).Median age was 63 years(interquartile range 58-67.75)and the most common aetiology of liver disease was hepatitis C(40.4%).59(56.7%)had synchronous HCC and 45(43.3%)had sequential.36 patients died(34.6%)and 25 were transplanted(24.0%).1,3 and 5-year OS was 93.0%,66.1% and 62.3% and 5-year post-transplant survival was 95.8%.1,3 and 5-year TFS was 82.1%,45.85% and 37.8%.When synchronous and sequential groups were compared,OS(1,3 and 5 year synchronous 91.3%,63.8%,61.1%,sequential 95.3%,69.5%,64.6%,P=0.41)was similar but TFS was higher in the sequential group(1,3 and 5 year synchronous 68.5%,37.3% and 29.7%,sequential 93.2%,56.6%,48.5%,P=0.02)though this difference did not remain during multivariate *** TFS in patients presenting with two HCC≤3 cm is poor regardless of the timing of the second *** patients presenting with two small HCC should be considered for transplantation.

There has been a dramatic increase in medical complications related to synthetic cannabinoid (SC) use either by water pipe or vaping. The legalization of marijuana in an increasing number of states has also resulted in an increase in a number of complications related not just to marijuana, but in particular, to SC. As a result, there have been recent increased reports of acute pulmonary injury related to inhaled SC products. We describe that rarely endotracheal intubation with mechanical ventilation has been required to treat the acute respiratory distress syndrome (ARDS) and the diffuse alveolar hemorrhage (DAH) associated with the acute toxicity of SC inhalation. We describe the second reported case of successful utilization of mechanical ventilation and extracorporeal membrane oxygenation (ECMO) in order to treat acute pulmonary toxicity caused by SC inhalation by a water pipe. While the exact pathophysiology of these interesting and recent pulmonary complications is unknown, the recent increase in exposure to SC via water pipe systems and vaping suggests that there will be many more cases of patients that will require ECMO as a form of life-saving therapy.

[背景]短期臭氧暴露与日死亡率增加相关。浓度-反应关系的形状对于估算公众健康影响至关重要,特别是假设存在阈值时。[目的]研究1993—2006年间,英国5个城市及5个乡村地区每日臭氧暴露浓度与死亡率之间的浓度-反应关系。[方法]采用泊松回归,控制季节性、温度和流感等变量,分析每日最大8h臭氧浓度与每日全死因死亡率之间的相关性,分别为全年及特定季节假设了线性、线性阈值和样条模型。作者还检测了灵敏度以及替代温度指标,前者用以调整对颗粒物的测定(仅限市区)。[结果]在所有年份的分析中,在伦敦发现臭氧与全死因死亡率的浓度-反应关系中存在明确的阈值,为65μg/m(395%CI:58～83);但是,在其他城市或乡村地区存在阈值的证据却很少。综合线性效应估计城市和乡村地区的全死因死亡率具有可比性:臭氧浓度每增加10μg/m3,全死因死亡率分别增加0.48%(95%CI:0.35～0.60)和0.58%(95%CI:0.36～0.81)。季节性分析表明,夏季的月份中在城市和乡村地区都存在臭氧影响的阈值。[结论]应该采用阈值和非阈值模型评价整个环境范围内臭氧对健康的影响,并且根据季节进行分层。在伦敦存在阈值效应,但在其他研究区域却未发现,这一点需要进一步研究探讨。乡村地区臭氧暴露对公众健康的影响不容忽视。

这个报告评述了开放远程教育在日本当前的地位。开放远程教育在日本被用于不同层次的非义务教育——在高中、大专、大学和研究生院均有。研究结果显示,个别化学习和小组合作学习(cooperativelearning)不同于小组协作学习(collaborativelearning),日本开放远程教育现在参与跨国联盟中,为了保证远程教育的质量并得到国内与国际的认证,需要介绍引进协作学习的方法。多媒体教学模式是最近开发并确认有效的教学模式,对教师与学生都提供了必要的支持。为了提升日本的开放远程教育,在受关注的领域里,本文提出了七个策略:①课程大纲的设计——提高透明度与信任;②互相认证与知识产权保护;③教与学的研究;④开放远程教育联盟——提高公众的接受程度;⑤电子图书馆和开放源码知识。⑥教师专业发展——提高教师应有的责任感。⑦社团发展。需要在未来的研究中对这七个策略进行探索。本研究广泛地参考了日本开放远程教育所有网址提供的内容,希望这个报告成为未来研究者有用的资源。

Degenerative disc disease(DDD) is associated with intervertebral disc degeneration of spinal instability. Here, we report that the cilia of nucleus pulposus(NP) cells mediate mechanotransduction to maintain anabolic activity in the discs. We found that mechanical stress promotes transport of parathyroid hormone 1 receptor(PTH1 R) to the cilia and enhances parathyroid hormone(PTH) signaling in NP cells. PTH induces transcription of integrin α_vβ_6 to activate the transforming growth factor(TGF)-β-connective tissue growth factor(CCN2)-matrix proteins signaling cascade. Intermittent injection of PTH(iPTH) effectively attenuates disc degeneration of aged mice by direct signaling through NP cells, specifically improving intervertebral disc height and volume by increasing levels of TGF-β activity, CCN2, and aggrecan. PTH1 R is expressed in both mouse and human NP cells. Importantly,knockout PTH1 R or cilia in the NP cells results in significant disc degeneration and blunts the effect of PTH on attenuation of aged discs. Thus, mechanical stress-induced transport of PTH1 R to the cilia enhances PTH signaling, which helps maintain intervertebral disc homeostasis, particularly during aging, indicating therapeutic potential of iPTH for DDD.
