The muscle protein myosin binding protein C (MyBPC) is a large multi-domain protein whose role in the sarcomere is complex and not yet fully understood. Mutations in MyBPC are strongly associated with the heart disease familial hypertrophic cardiomyopathy (FHC) and these experiments of nature have provided some insight into the intricate workings of this protein in the heart. While some regions of the MyBPC molecule have been assigned a function in the regulation of muscle contraction, the interaction of other regions with various parts of the myosin molecule and the sarcomeric proteins, actin and titin, remain obscure. In addition, several intra-domain interactions between adjacent MyBPC molecules have been identified. Although the basic structure of the molecule (a series of immunoglobulin and fibronectin domains) has been elucidated, the assembly of MyBPC in the sarcomere is a topic for debate. Large Painful Bursae at Abeche Hospital: Epidemiological, Clinical, Therapeutic and Evolutionary Aspects

Introduction: Acute large bursae (ALB) are a frequent reason for emergency consultations. The aim of this study was to investigate the prevalence, clinical course, treatment and evolution of ALB at Abeche University Hospital. Patients and Method: This was a 45-month cross-sectional study from January 2020 to September 2023. Male patients of any age who had been admitted to and treated for acute large bursae at the Abeche University Hospital were included. Sociodemographic, clinical and therapeutic variables were studied. Results: Acute large bursae accounted for 7.92% of emergency admissions. The average age was 39.40 years. 60.27% of patients came from rural areas. The average consultation time was 4 days, ranging from a few hours to 18 days. The main reason for consultation was pain. Strangulated inguino-scrotal hernia was the most common, followed by acute orchi-epididymitis, accounting for 41.8% and 26% of cases respectively. Traditional treatment prior to consultation was attempted in 13.7% of cases. All patients were treated as emergencies, 41 of whom had received medical treatment. Of the patients treated surgically, orchidopexy was performed in all. Parietal suppuration and anaemia occurred in 6.2% and 4.8% of cases respectively. Conclusion: A accounts for a significant proportion of our emergency care activity. However, patients are seen with a delay, which jeopardises the functional prognosis of the testicle and intestine.

Background Atrial fibrillation （AF） causes a continuum of atrial anatomical remodeling. Methods Using a library of perfusion-fixed human hearts, specimens with AF were compared to controls. during this preliminary assessment study, direct measurements were taken of atrial volume, pulmonary vein （PV） circumference, and left atrial （LA） wall thicknesses. Results Hearts with AF typically had larger atrial volumes, as well as a much larger variation in volume compared to controls （range of 59.6-227.1 mL in AF hearts compared to 65.1-115.9 mL in controls）. For all hearts, right PVs were larger than left PVs （mean： 171.4 ± 84.6 mm^2 for right and 1182 ± 50.1 mm^2 for left, P 〈 0.005）. LA wall thicknesses ranged from 0.7 mm to 3.1 mm for both AF and control hearts. Conclusions Hearts with AF had a large range of sizes which is consistent with the progression of atrial remodeling during AF. The large range of thicknesses will influence the amount of energy needed to create transmural lesions during ablation procedures.

In this note, we provide a consistant thin layer theory for power law and Bingham incompressible fluids flowing down an inclined plane under the effect of gravity. The derivation of such equations is based on formal asymptotic expansions of solutions of Cauchy momentum equations in the shallow water scaling and in the neighbourhood of steady solutions so that we can close the average equations on the fluid height h and the total discharge rate q.

AIM: To study adherence to the widely accepted surveillance guidelines for patients with long-standing colitis in the Netherlands. METHOdS: A questionnaire was sent to all 244 gastroenterologists in the Netherlands. RESULTS: The response rate was 63%. Of all gastroenterologists, 95% performed endoscopic surveillance in ulcerative colitis (UC) patients and 65% in patients with Crohn's colitis. The American Gastroenterological Association (AGA) guidelines were followed by 27%, while 27% and 46% followed their local hospital protocol or no specific protocol, respectively. The surveillance was correctly initiated in cases of pancolitis by 53%, and in cases of left-sided colitis by 44% of the gastroenterologists. Although guidelines recommend 4 biopsies every 10 cm, less than 30 biopsies per colonoscopy were taken by 73% of the responders. Only 31%, 68% and 58% of the gastroenterologists referred patients for colectomy when low-grade dysplasia, high-grade dysplasia (HGd) or dysplasia Associated Lesion or Mass (dALM) was present, respectively. CONCLUSION: Most dutch gastroenterologists performendoscopic surveillance without following international recommended guidelines. This practice potentially leads to a decreased sensitivity for dysplasia, rendering screening for colorectal cancer in this population highly ineffective.

Knowledge and Attitudes of Nursing Staff towards Obstetric Fistula at the Abeche University Hospital, Chad

Introduction: Prevention of obstetric fistula (OF) remains a challenge in Chad where its incidence is 464 cases/year. The present study aims to determine the knowledge and attitudes of nursing staff towards obstetric fistula. Patients and Method: This was a cross-sectional, descriptive and analytical study including nursing staff at the Abéché University Hospital. The survey took place from March to May 2023 and the sampling was exhaustive. data collection was done using a form including a questionnaire on sociodemographic parameters, knowledge and attitudes. Participation in the study was voluntary and individual. Results: Participation in the study was 76.11%. Emergency department staff were the most represented, followed by gynecology-obstetrics staff with 34.4% and 20.91% of cases respectively. Nurses represented 53.17% of participants followed by doctors (23.52%). An exact definition of obstetric fistula was reported by 7.84% of participants and it was partial in 80.39%. The level of knowledge of risk factors was considered good in 12.41%. Exact knowledge of clinical signs was reported by 74.5% of cases. Among the participants, 1.96% reported that the treatment of obstetric fistula is traditional. Knowledge about means of prevention was considered good by 13.72% (n = 21), and attitudes by 26.79%? Obstetric fistula knowledge was influenced by profession (doctor, p = 0.011) and attitudes by service (p = 0.004) and profession (doctor, p = 0.001). Conclusion: Obstetric fistula is a curable disease whose prevention remains possible and requires good knowledge of the disease and the promotion of safe motherhood. This study should serve as a basis for the establishment of the obstetric fistula module in the curriculum of healthcare personnel and the promotion of continuing training for its eradication.

The Helotiales are an ecologically and morphologically highly diverse group of ascomycetes that also includes lichen-inhabiting(lichenicolous)*** generated sequence data of three rdNA regions(nuSSU,nuLSU,5.8S of ITS)from 28 lichenicolous specimens representing nine genera in order to determine their phylogenetic *** on the most complete dataset of helotialean fungi to date,the analyses were performed using Maximum Likelihood(ML)and Bayesian *** results suggest that 1)the lichen-inhabiting life-style in Helotiales was subjected to gains or losses at least three times;2)Thamnogalla,previously tentatively included in Ostropales,is shown to belong to Helotiales;3)ascomata found intermixed with pycnidia of the asexual diplolaeviopsis ranula and possessing the same pigments are tentatively considered as the sexual morph of this taxon,and are shown to belong to Helotiales;4)the lichenicolous species of ***,Llimoniella,Rhymbocarpus,Skyttea,Thamnogalla and Unguiculariopsis form a well-supported clade together with non-lichenicolous encoelioid fungi from the genera Ionomidotis,Cordierites and Encoelia;5)Geltingia associata forms a highly supported clade with the fungicolous asexual fungus Eleutheromyces subulatus,whereas Pezizella epithallina clusters with an aquatic asexual fungus Tetracladium sp.;6)Phaeopyxis punctum belongs to Ostropomycetidae(Lecanoromycetes),but its deep relationships to other groups remain unresolved based on rdNA sequences.

Tunable filter Raman spectroscopy is used to efficiently produce Raman excitation maps of unpurified and type-purified single walled carbon nanotubes (SWCNTs). Maps with fine excitation resolution (1 nm) are created over a wide wavelength range (727 to 980 nm), extending from metallic to semiconducting resonances. At a given wavelength, the wide bandwidth (>3,000 cm–1) allows the comparison of the G band with the radial breathing mode (RBM), and shows the 2d band and other less prominent bands. Materials examined included unsorted powders, aqueous sorted semiconductors, aqueous sorted metals, and polyfluorene sorted semiconductors in toluene. The Raman excitation profiles of the G band are broad, relative to the RBM bands. The maps offer evidence of minority species contamination, except in the case of the polyfluorene sorted semiconductors. Tunable Raman spectroscopy data help validate the simpler fixed wavelength Raman spectroscopy approaches to purity assessment. CNB-001 reduces paraplegia in rabbits following spinal cord ischemia

Spinal cord ischemia associated with trauma and surgical procedures including thoraco-abdominal aortic aneurysm repair and thoracic endovascular aortic repair results in devastating clinical deficits in patients. Because spinal cord ischemia is inadequately treated, we studied the effects of [4-((1 E)-2-(5-(4-hydroxy-3-methoxystyryl-)-1-phenyl-1 H-pyrazoyl-3-yl) vinyl)-2-methoxy-phenol)](CNB-001), a novel curcumin-based compound, in a rabbit SCI model. CNB-001 is known to inhibit human 5-lipoxygenase and 15-lipoxygenase and reduce the ischemia-induced inflammatory response. Moreover, CNB-001 can reduce the level of oxidative stress markers and potentiate brain-derived neurotrophic factor and brain-derived neurotrophic factor receptor signaling. The Tarlov scale and quantal analysis technique results revealed that CNB-001 administered as an intravenous dose(bolus) 30 minutes prior to spinal cord ischemia improved the behaviors of female New Zealand White rabbits. The improvements were similar to those produced by the uncompetitive N-methyl-d-aspartate receptor antagonist memantine. At 48 hours after aortic occlusion, there was a 42.7% increase(P d ischemia duration(n = 14) for rabbits treated with CNB-001(n = 16), and a 72.3% increase for rabbits treated with the positive control memantine(P d to vehicle-treated ischemic rabbits(n = 22). CNB-001 is a potential important novel treatment for spinal cord ischemia induced by aortic occlusion. All experiments were approved by the CSMC Institutional Animal Care and Use Committee(IACUC #4311) on November 1,2012.
