Turnaround times for molecular testing of pediatric viral cerebrospinal fluid samples in United Kingdom laboratories

BACKGROUND Rapid molecular testing has revolutionized the management of suspected viral meningitis and encephalitis by providing an etiological diagnosis in<90 min with potential to improve outcomes and shorten inpatient ***,use of molecular assays can vary *** To evaluate current practice for molecular testing of pediatric cerebrospinal fluid(CSF)samples across the United Kingdom using a structured *** A structured telephone questionnaire survey was conducted between July and August *** was collected on the availability of viral CSF nucleic acid amplification testing(NAAT),criteria used for testing and turnaround times including the impact of the coronavirus disease 2019 *** Of 196/212(92%)microbiology laboratories responded;63/196(32%)were excluded from final analysis as they had no on-site microbiology laboratory and outsourced their *** 133 Laboratories included in the study,47/133(35%)had onsite facilities for viral CSF *** currently undertaking onsite NAAT(n=47)had much faster turnaround times with 39 centers(83%)providing results in≤24 h as compared to those referring samples to neighboring laboratories(5/86;6%).CONCLUSION Onsite/near-patient rapid NAAT(including polymerase chain reaction)is recommended wherever possible to optimize patient management in the acute setting.

Gastroenterology training in a resource-limited setting: Zambia, Southern Africa

AIM: To evaluate need for and efficacy of a structured gastroenterology didactic session in expanding awareness and understanding of digestive disorders. METHODS: A four-day symposium was developed with didactic sessions (days 1, 2) and practical endoscopy (days 3, 4). Didactic sessions included case presentations highlighting pathophysiology and management. One nurse and four practicing gastroenterologists from the United Kingdom led lectures and supervised work-shops with audience participation. Practical endoscopy focused on diagnostic and therapeutic procedures and their application to diagnosis and treatment of ailments of the gastrointestinal tract. Preand post-workshop questionnaires were distributed to participants during didactic sessions. A pre-workshop questionnaire gauged expectations and identified objectives to be met at thesymposium. Post-workshop questionnaires were administered to assess efficacy of each session. Participants graded sessions from 1 (poor) to 5 (excellent) on quality of case presentations, knowledge, clarity and mode of presentation. We assessed if time allotted to each topic was sufficient, value of sessions, impact on practice and interest in future symposiums. RESULTS: There were 46 attendees on day 1: 41% undergraduates, 41% residents, 11% consultants and 4% unspecified. Day 2 (a Saturday) had 24 participants: 17% undergraduates, 71% residents, 9% consultants, 4% unspecified. Primary pre-workshop symposium expectation was to gain knowledge in: general gastroenterology (55.5%), practical endoscopy (13.8%), pediatric gastroenterology (5%), epidemiology of gastrointestinal disorders specific to Zambia (6%), and interaction with international speakers (6%). The post-symposium questionnaire was answered by 19 participants, of whom 95% felt specific aims were met; all would attend future conferences and recommend to others. NanoHDA： A nanoparticle-assisted isothermal amplification technique for genotyping assays

Isothermal methods, such as helicase-dependent amplification （HDA）, have an advantage over polymerase chain reaction for DNA amplification owing to their ease of operation. Here, we developed a new HDA method that is nanoparticle-assisted, termed nanoHDA. This method uses gold nanoparticles （AuNPs） to improve the sensitivity and specificity of the isothermal method. In HDA, the denaturation of DNA templates is mediated by helicases, but this method is limited by the low denaturation efficiency of helicases. In this report, AuNPs with preferential affinity for single-stranded DNA （ssDNA） were utilized to improve the denaturation efficiency of helicases. The same affinity property of nanoparticles can also enhance specificity by suppressing primer-dimer formation. This nanoHDA method was employed to genotype the KRAS gene in genomic DNA samples from colorectal cancer patients, as achieved by the hybridization of nanoHDA amplicons using the NanoBioArray chip.

Koena, a Novel Compound That Dampens Inflammation in the Skin

Inflammatory skin diseases are characterized by the activation of the innate and adaptive immune system via the production of pro-inflammatory cytokines. The main proinflammatory cytokines responsible for this development include TNF-alpha, Interleukin 1, 6 and 17, also the transcription factor NF-kappa B. Small molecule anti-inflammatory compounds such as those detailed in the paper, show much promise in dealing with these skin disorders. Four nutraceutical molecules that are well characterized by their capacity to interact with many of the pro-inflammatory cytokines, importantly IL-17, are found in an Australian product, Koena. Koena has been shown to not only decrease these cytokines but also help increase the production of Interleukin 10 and important anti-inflammatory cytokines. This paper reports clinical benefits of Koena in a variety of circumstances;eczema and psoriasis;insect bites and sting;solar damaged skin and keratosis. An analysis of patient/purchaser reviews via the website *** to ascertain what condition they purchased the product for, if any, and the satisfaction rating after use. More than 35% of reviewers reported purchasing Koena to help with inflamed skin. Those reporting being satisfied with the result was 95% for this use. This paper reports on the benefit of small molecule anti-inflammatories in modulating and not blocking an immune response. It also illustrates the benefits of multitherapy when dealing with complex inflammatory responses as opposed to monotherapy.

The Effects of a Unilateral Strength and Power Training Intervention on Inter-Limb Asymmetry and Physical Performance in Male Amateur Soccer Players

The aims of this study were(a)to investigate the effects of a unilateral training program in reducing inter-limb asymmetry in male soccer players;(b)to explore such effects on measures of physical performance and unilateral inter-limb ***-four soccer players,randomly assigned to a 6-week unilateral strength and power training(UNI)(n=12)or a control group(CON)(n=12),performed single countermovement jump(SLCMJ),single leg broad jump(SLBJ),single leg drop jump(SLDJ),10-m sprint,and 505 change of direction(COD)speed *** jump scores revealed small to large improve-ments in SLCMJ,SLBJ,and SLDJ reactive strength index(RSI)(g=0.46 to 1.66)in the UNI group,whereas negligible changes were found in the CON group(g=−0.31 to 0.33).Asymmetry indexes showed a moderate significant reduction in the SLDJ(RSI)and in the SLDJ stiffness(K)(g=1.00 to 1.11)in the UNI *** between-group comparison indicated a significant change in the SLDJ(RSI)and in the SLDJ(K)(g=1.01 to 1.07)in favour of the UNI ***,a unilateral training program seems to be able to reduce between-limb imbalances and foster improvements in jump performance,without any detrimental effects on linear or COD speed *** the importance of these physical characteristics for soccer,it is suggested that unilateral strength and power training are incorporated into strength training routines for players of all levels.

Jasmonic acid-isoleucine formation in grapevine(Vitis vinifera L.) by two enzymes with distinct transcription profiles

The plant hormone jasmonic acid （JA） is essential for stress responses and the formation of reproductive organs, but its role in fruit development and ripening is unclear. Conjugation of JA to isoleucine is a crucial step in the JA signaling pathway since only JA-Ile is recognized by the jasmonate receptor. The conjugation reaction is catalyzed by JA-amido synthetases, belonging to the family of Gretchen Hagen3 （GH3） proteins. Here, in vitro studies of two grapevine （Vitis vinifera L. cv Shiraz） GH3 enzymes, VvGH3-7 and VvGH3-9, demonstrated JA-conjugating activ- ities with an overlapping range of amino acid substrates, including isoleucine. Expression studies of the correspond- ing genes in grape berries combined with JA and JA-lle measurements suggested a primary role for JA signaling in fruit set and cell division and did not support an involvement of JA in the ripening process. In response to methyl JA （MeJA） treatment, and in wounded and unwounded （distal）leaves, VvGH3-9 transcripts accumulated, indicating a participation in the JA response. In contrast, VvGH3-7 was unresponsive to MeJA and local wounding, demonstrating a differential transcriptional regulation of VvGH3-7 and VvGH3- 9. The transient induction of VvGH3-7 in unwounded, distal leaves was suggestive of the involvement of an unknown mobile wound signal.

Assessment of compatibility of rhlGF-1/rhlGFBP-3 with neonatal intravenous medications
