Effect of glaucoma on identification of bottle cap color in ophthalmic medications

A prospective, nonrandomized, observational trial of 60 glaucoma patients to correlate visual acuity and visual field with ability to distinguish bottle cap color of commonly used ophthalmic medications was conducted. A total of 103 eyes from 60 patients(30 women) were evaluated. The mean logMAR acuity was 0.34±0.54(approximately 20/45 Snellen acuity), average Humphrey Visual Field(HVF) mean deviation was-8.58±8.69 db, mean Ishihara plates(out of 14) were 11.78±4.15, and bottle cap color score(out of 10) was 8.56±2.51. Multiple linear regression analysis revealed an independent correlation of visual acuity(P=0.0137) and Ishihara score(Pbut no significant effect with visual field mean deviation(P>0.05). Glaucoma patients with poor visual acuity, but not necessarily advanced visual field loss, are likely to have difficultly identifying the color of their bottle caps. Physicians should be cognizant of this potential issue when reviewing medications with patients.

bACKGROUND basal joint arthritis is a common disease which is especially present in postmenopausal women. A variety of procedures have been described once conservative treatment has failed. The purpose of our study is to present a novel surgical technique based on a clinical case with three-year follow-up which is highly effective, even after failure of previous procedures. CASE SUMMARY We present a case of a 39 year-old-woman who suffered from recurring, persistent, sharp left thumb pain for several years After two operative procedures, she presented again with intolerable, sharp pain over the trapeziometacarpal joint, and wished to proceed with a pain relieving procedure or arthroplasty. Intraoperatively, the articular surfaces appeared well, without any bare areas of bone or eburnation, resulting in the decision to perform a “subtraction hemiarthroplasty”. A shortening osteotomy of the os trapezium was performed by removing a slice of 8-9 mm bone, without damaging the joint cartilages. The osteotomy was stabilized using two screws, the ligament and capsular tissue was suture imbricated, and a cut down on the translation and a dorsal radial ligament reconstruction were performed. Three years after the final procedure, a long-term follow-up demonstrates excellent results, pain free with full range-of-motion. CONCLUSION Subtraction hemiarthroplasty with ligament tightening imbrication and reconstruction of the radial ligament led to excellent results in motion and pain.

Variations of the harmonic components of the X-ray pulse profile of PSR b1509–58

We used the Fourier decomposition technique to investigate the stability of the X-ray pulse profile of a young pulsar PSR b1509–58 by studying the relative amplitudes and phase differences of its harmonic components with respect to the fundamental using data from the Rossi X-Ray Timing Explorer. Like most young rotation powered pulsars, PSR b1509–58 has a high spin down rate. It also has less timing noise, allowing accurate measurement of higher order frequency derivatives which in turn helps in the study of the physics of pulsar spin down. Detailed investigation of pulse profiles over the years will help us establish any possible connection between the timing characteristics and the high energy emission characteristics for this ***, the study of pulse profiles of short period X-ray pulsars can also be useful when used as a means of interplanetary navigation. The X-ray pulse profile of this source has been analyzed for 15 yr（1996–2011）. The long term average amplitudes of the first, second and third harmonics（and their standard deviation for individual measurements） compared to the fundamental are 36.9%（1.7%）, 13.4%（1.9%） and 9.4%（1.8%） respectively. Similarly, the phases of the three harmonics（and standard deviations） with respect to the fundamental are 0.36（0.06）, 1.5（0.2） and 2.5（0.3） radian respectively. We do not find any significant variation of the harmonic components of the pulse profile in comparison to the fundamental.

Deep-water facies sections have advantages of recording complete information across the Permian-Triassic boundary(PTb). Here we present a detailed study on the conodont biostratigraphy and carbon isotope profile ranges from the Wuchiapingian-Changhsingian boundary(WCb) to the PTb of two deep-water facies sections at Zhuqiao and Shiligou in the Middle Yangtze region, western Hubei, South China. Fifteen species and three genera are identified. Eight conodont zones are recognized which in ascending order are the Clarkina orientalis, C. wangi, C. subcarinata, C. changxingensis, C. yini, C. meishanensis, Hindeodus parvus and Isarcicella isarcica zones. The onset of deposition of the deep-water siliceous strata of Dalong Formation in western Hubei began in the Late Wuchiapingian and persisted to the Late Changhsingian. Carbon isotope negative excursions occur near both the WCb and PTb in both sections. The WCb δ13 Ccarb negative excursion is in the C. orientalis and C. wangi zones. The PTb δ13 Ccarb negative excursion began in the C. yini Zone and extended to the I. isarcica Zone. The absence of several Changhsingian zones may indicate the difficulty of extracting conodonts from siliceous strata or the presence of an intra-Changhsingian hiatus.

Growing evidence suggests the implication of the gut microbiota in various facets of health and disease. In this review, the focus is put on microbiota-host molecular cross-talk at the gut epithelial level with special emphasis on two defense systems： intestinal alkaline phosphatase（IAP） and inducible heat shock proteins（iHSPs）. both IAP and iHSPs are induced by various microbial structural components（e.g. lipopolysaccharide, flagellin, CpG DNA motifs）,metabolites（e.g. n-butyrate） or secreted signal molecules（e.g., toxins, various peptides, polyphosphate）. IAP is produced in the small intestine and secreted into the lumen and in the interior milieu. It detoxifies microbial components by dephosphorylation and, therefore, down-regulates microbe-induced inflammation mainly by inhibiting NF-κb pro-inflammatory pathway in enterocytes. IAP gene expression and enzyme activity are influenced by the gut microbiota. Conversely, IAP controls gut microbiota composition both directly, and indirectly though the detoxification of pro-inflammatory free luminal adenosine triphosphate and inflammation inhibition. Inducible HSPs are expressed by gut epithelial cells in proportion to the microbial load along the gastro-intestinal tract. They are also induced by various microbial components, metabolites and secreted molecules. Whether iHSPs contribute to shape the gut microbiota is presently unknown. both systems display strong anti-inflammatory and anti-oxidant properties that are protective to the gut and the host. Importantly, epithelial gene expressions and protein concentrations of IAP and iHSPs can be stimulated by probiotics, prebiotics and a large variety of dietary components, including macronutrients（protein and amino acids, especially L-glutamine, fat, fiber）, and specific minerals（e.g. calcium）and vitamins（e.g. vitamins K1 and K2）.
