background This study evaluates the validity of Alvarado Score in predicting disease severity and the development of postoperative complications in pediatric *** This is a retrospective,bicentered study on pediatric patients with emergency appendicectomy performed from 2006 to *** relationship of Alvarado Score(low:1-4,medium:5-6,high:7-10)and operative findings/complications was *** A total of 316 patients were included and the median age on admission was 10.8 *** overall median score was 8.***.13.3%,20.2%,and 66.5%of patients had low,medium,and high risk scores,respectively.36.1%of patients had complicated appendicitis and the median score was comparable with that of the uncomplicated cases(7.0 vs 7.4,p=0.21).More complicated cases were found in the medium-risk group(high vs medium vs low=29.7%vs 61.4%vs 31.0%,p=0.01).Rebound tenderness had the highest positive predictive value(65%)for complicated *** complications were found in 16.5%of patients with a higher median score(7.87 vs 5.8,p=0.01).Conclusion Alvarado Score does not predict disease severity but postoperative *** with medium risk score should also be treated promptly for the risk of having complicated *** tenderness may be a signal for complicated appendicitis and should be properly examined.

Liposome, albumin and polymer polyethylene glycol are nanovector formulations successfully developed for anti-cancer drug delivery. There are significant differences in pharmacokinetics, efficacy and toxicity between pre- and post-nanovector modification. The alteration in clinical pharmacology is instrumental for the future development of nanovector-based anticancer therapeutics. We have reviewed the results of clinical studies and translational research in nanovectorbased anti-cancer therapeutics in advanced pancreatic adenocarcinoma, including nanoparticle albumin-bound paclitaxel and nanoliposomal irinotecan. Furthermore, we have appraised the ongoing studies incorporating novel agents with nanomedicines in the treatment of pancreatic adenocarcinoma.

A pot experiment was conducted in a plastic film house to evaluate the translocation and uptake of heavy metals(Pb,Cd,Cu,and Zn) into brown rice(Oryza sativa L.) and the heavy metals residues in soils which had previously been irrigated with domestic wastewater for a long time(3 years).The range of Pb,Cd,Cu,and Zn was 5.10 ± 0.01,0.105 ± 0.017,5.76 ± 0.42,and 23.56 ± 1.40 mg kg-1,respectively in the domestic wastewater-irrigated soil,and 0.370 ± 0.006,0.011 ± 0.001,0.340 ± 0.04,and 2.05 ± 0.18 mg kg-1,respectively,in the domestic wastewater-irrigated brown *** results indicated that application of domestic wastewater to arable land slightly increased the levels of Pb,Cd,Cu,and Zn in soil and brown rice(P he concentrations of heavy metals in brown rice were lower than the recommended tolerable levels proposed by the Joint FAO/WhO Expert Committee on Food ***,the continuous monitoring and pollution control of hazardous materials from domestic wastewater are needed in order to prevent excessive build-up of heavy metals in the food chain.

A new spherical torus called VEST （Versatile Experiment Spherical Torus） is designed, constructed and successfully commissioned at Seoul National University. A unique design feature of the VEST is two partial solenoid coils installed at both vertical ends of a center stack, which can provide sufficient magnetic fluxes to initiate tokamak plasmas while keeping a low aspect ratio configuration in the central region. According to initial double null merging start-up scenario using the partial solenoid coils, apprdpriate p6wer supplies for driving a toroidal field coil, outer poloidal field coils, and the partial solenoid Coils are fabricated and successfully commissioned. For reliable start-up, a pre-ionization system with two cost-effective homemade magnetron power supplies is also prepared. In addition, magnetic and spectroscopic diagnostics with appropriate data acquisition and control systems are well prepared for initial operation of the device. The VEST is ready for tokamak plasma operation by completing and commissioning most of the designed components.

heterostructured eutectic high-entropy alloys(EhEAs)have attracted significant attention owing to their novel properties,such as balanced combinations of strength and fracture ***,the toughening/strengthening mechanisms of these EhEAs have not been thoroughly *** this study,we developed a series of dual-phase Al_((18–2x))Co_(30)Cr_((11+x))Fe_((11+x))Ni_(3)0(x=-1,0,1)eutectic and neareutectic hEAs containing face-centered cubic(FCC)and body-centered cubic(BCC)*** the high amount of BCC,which is referred to as the brittle phase,newly developed EhEAs exhibited superior fracture ***,we discovered that a fully eutectic hEA exhibited further improvements in both yield stress and fracture toughness,outperforming our off-eutectic and other previously reported *** combining experiments and theoretical models,we demonstrated that the synergistic increase in both strength and toughness in our fully eutectic hEA was derived from the high hetero-deformationinduced(hDI)strengthening/toughening associated with a high misorientation angle at the grain/phase boundaries.

In dental clinics with an open floor plan,the risk of patient-to-patient transmission of respiratory disease is a *** dental procedures large amounts of bioaerosol are produced and patients cannot wear personal protective *** paper examines how to effectively deploy air cleaner to reduce the infection risk in dental clinics with an open floor *** locations of air cleaners at various clean air delivery rates(CADRs)were *** dispersion of bioaerosol was studied through numerical simulations,and risk assessment was performed by a dose-response *** findings indicated that dental patients downstream of the background ventilation have a higher infection risk than those to the left and right of an infected patient(i.e.,the source).The lowest infection risks for the adjacent patients were found when the air cleaner was place opposite to the dentists,i.e.,on the floor at low CADR levels of 2.2 m^(3)/min or on the bench at CADR levels of 4.4 m^(3)/min or *** results of this study indicated that air cleaner can mitigate the risk of patient-to-patient transmission of SARS-CoV-2 in dental clinics with an open floor *** CADR levels determine the optimal placement of air cleaners.

A concrete numerical example of Z6-equivariant planar perturbed hamiltonian polynomial vector fields of degree 5 having at least 24 limit cycles and the configurations of compound eyes are given by using the bifurcation theory of planar dynamical systems and the method of detection functions. There is reason to conjecture that the hilbert number h(2k + 1) ? (2k + I)2 - 1 for the perturbed hamiltonian systems.

We demonstrate the transmission of directly modulated 10-Gb/s WDM signals over 320 km of negative dispersion fiber (dispersion: -2.5 ps/km/nm @1550 nm) without dispersion compensation. The results indicate that a regional metro WDM network could be implemented cost-effectively by using the proposed negative dispersion fiber and direct modulated lasers.

hemoperitoneum is a rare and potentially life-threatening complication of GIST. We reported a 54- year-old man who developed disseminated intra-abdominal recurrence from a previously resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically 8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery in the management of GIST. In our patient, disease progression after second line targeted therapy and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy and aggressive surgical treatment produced reasonably good survival outcome.
