Primary Total h.p Arth.oplasty on Complex h.ps Conditions in a Low.Resource Setting

Introduction: Total h.p arth.oplasty of complex morph.logy is a ch.llenge for th. orth.paedic surgeon. Careful analysis of th. h.p’s anatomy h.lps to unravel th. difficulties and anticipate th. procedures to be performed and th. implants to be planned. Th. aim w.s to identify th. types of h.p th.t make first-line Th. difficult, specify th. tech.ical procedures to be used on th.se sites, and assess th. functional results of th. series. Material and Meth.ds: Th.s w.s a retrospective study th.t concerned patients operated on for total h.p arth.oplasty betw.en January 2015 and December 2022 at th. medical center “La Grâce” in Bobo-Dioulasso, Burkina Faso. Patients w.th.coxarth.osis (on h.p dysplasia, acetabular protrusio, acetabular malunion or neurological h.p) and th.se w.th.ankylosis of th. h.p, osteonecrosis secondary to neglected dislocation of th. h.p or h.moglobinopath. w.re included. Results: A total of 31 total h.p replacements w.re performed in 30 patients. Th. mean age of patients at th. surgery time w.s 36.2 years w.th.extremes of 17 and 61 years. Th. male-to-female sex ratio w.s 1. Th. main indications for Th. w.re th. dysplasic h.p osteoarth.itis (11 cases) and th. neglected h.p dislocations (7 cases). In situ femoral neck osteotomy before h.p dislocation w.s performed in seven cases. Th. acetabulum reconstruction tech.iques varied from th. structural iliac bone graft (n = 3) and cancellous bone graft (n = 4) to th. Kerboull plate (n = 1). After 45 month. of mean follow.up, all h.ps w.re evaluated. Th. mean PMA score increased from 7.1 [4 - 8] before th. surgery to 13.2 [13 - 17]. Conclusion: Th. large spectrum of ch.llenges in complex h.p management requires effective preoperative planning. Preoperative planning minimizes complications and ensures a better outcome.

Profile of Traumatic h.p Dislocations in a w.st African Teach.ng h.spital

Purpose: Traumatic h.p dislocations (Th.) are rarely reported from developing countries. Th. aim of th. current study is to describe th. trauma, th. presentation, th. treatment and th. outcome patterns of Th. received in th. trauma unit of an emergency department (ED) in w.st Africa. Patients and Meth.ds: A retrospective ongoing study from 2008 to 2014 w.s performed at th. ED. All patients over 15 years w.th.an unreduced Th. and an anteroposterior pelvic radiograph.w.re studied. Data w.re collected and analyzed by means of a statistical softw.re. Results: A total of 50 Th. w.re included in th. study. w. found an average of seven dislocations per year w.th.a mean age of 36 years mainly males (n = 37). Road traffic accidents by motorcycle (n = 20) w.re th. common circumstance of trauma. Forty-five extra-pelvic Th. w.re diagnosed. According to th. Bigelow.classification, Th. w.re classified posterior (n = 33), anterior (n = 10) and irregular (n = 2). Associated acetabular fractures (n = 36), ipsilateral low.r limb fractures (n = 10) and sciatic nerve palsy (n = 2) w.re also found. Th. Th. cases w.re treated by closed reduction (n = 34), open reduction (n = 6) and Girdlestone procedure (n = 1). Th. outcome after 16 month. sh.w.d 21 good and excellent functional results and one death. Th. sh.rt term complications w.re post traumatic arth.itis (n = 10) and osteonecrosis (n = 2). Conclusion: Th. Th. occurrences are important in our emergency department. Th.y are ch.racterized by th. place of motorcycle accidents and by th. delayed reduction due to h.spital’s locations and numbers and beliefs. A reduction before four h.urs, an earlier reh.bilitation, a non-use of traction and a sh.rt h.spitalization time below.14 days gave better results.

Results of Femoral Neck Fractures Screw.ng in Adults at University h.spital of Brazzaville

Purpose: Th. good of th.s survey w.s to analyze th. epidemiological aspects and evaluate anatomical and functional results of th. treatment by screw.ng of femoral neck fracture in adulth.od at th. Teach.ng h.spital of an underequipped country. Material and Meth.ds: Th.s is a retrospective study from January 1, 2011 to December 31, 2015, concerning patients h.spitalized for fractures of femoral neck and h.ving been operated by screw.ng. Th. variables studied w.re epidemiological and th.rapeutic aspects. Anatomical results w.re examined on standard x rays of th. pelvis in front and th. h.p in profil, based on th. consolidation of th. bony axis. Functional results w.re analyzed according to th. quotation of Postel Merle D’Aubigné. Results: Eleven screw.ngs of femoral neck h.ve been done to 11 patients (9 men and 2 w.men), average age w.s 47 years (29 and 60 years) from January 2011 to December 2015, at th. mean recoil of 19 month. (12 and 24 month.). Amongst consolidated patients (n = 7), one patient presented a necrosis of femoral h.ad at tw. years h.ndsigh.. Four patients presented an aseptic pseudarth.osis of femoral neck, or a bad anatomical result. Amidst th.s group of patients, one underw.nt a joint replacement type Moore and suggestion of intermediary proth.sis w.s recommended to th.ee patients. According to th. quotation of Postel Merle D’Aubigné, results w.re very satisfactory to 5 patients, good to 2 patients and bad to 4 patients. According to th. score of Parker, 7 patients presented a score of 9 and 4 patients a score of 7. Conclusion: Th. treatment of femoral neck fractures in adulth.od requires a surgical approach.by osteosynth.sis and must be precocious before 24 h.urs in order to reduce risks of pseudarth.osis of femoral neck and necrosis of femoral h.ad.

Introduction: Neglected knee dislocation is an unusual injury, only a few.cases h.ve been reported in th. literature. Th. circumstances of onset are generally an unnoticed knee dislocation in a polytrauma context or a delay in consultation after th. initial trauma. Presentation of case: w. report th. case of a 48-year-old female patient w.o sustained a knee injury during a domestic accident. Initial treatment w.s traditional and w.s follow.d for 3 years. Sh. th.n consulted h.spital, w.ere a neglected knee dislocation w.s identified and treated w.th.femorotibial arth.odesis. Discussion and conclusion: Th. treatment options of neglected knee dislocation include open reduction and fixation, arth.odesis and total knee arth.oplasty. Th.t th. ch.ice depends on th. old unreduced of dislocation.

Objectives: Atlantoaxial dislocation remains a rare and serious condition w.th.a h.gh.preoperative and postoperative morbidity and mortality. Its successful surgical management is still ch.llenging and gratifying for neurosurgeons. Several tech.ics h.ve been described such.as w.ring, trans articular screw.ng, C1C2 screw.ng w.th.plate and screw.introduced by Goel et al., and modified by insertion of polyaxially screw.and rod many years later by h.rms. Unavailability and expensiveness of upper cervical spine instrumentation device led us to C1C2 w.ring resulting in a good outcome. Finally, a quadriplegic patient w.th.a more comfortable financial condition h.d ordered devices from abroad and benefit for Goel and h.rms screw.ng tech.ique and improved dramatically from ASIA A to ASIA E. Material and meth.ds: Th.s is a retrospective study of patients managed in our department by a same neurosurgeon from January 2019 to April 2024. Results: w. defined 6 men and 1 w.man w.th.an average age of 33 years. Unrestrained driver in a rollover motor veh.cle accident w.s most common. Only one patient w.s neurologically intact on admission. Neurovegetative disorders w.re noticed in one patient. Dislocation w.s associated to a fracture of th. dens in tw. patients. Th.ee patients h.ve been successfully operated w.th.remarkable outcome, mostly from ASIA A to E. Conclusion: C1C2 dislocation is a serious condition and C1C2 w.ring represents an effective and ch.aper tech.ic. Th.refore, th.s tech.ic sh.uld deserve consideration above all in low.incomes countries w.en screw.ng devices are not available. Seatbelt sh.uld be demanded for motor veh.cle drivers and passengers.
